This study was performed to investigate the survival rate of single implant used in maxillary anterior region during follow up periods. 231 patients whose single missing tooth in maxillary region had been replaced with 237 implant at the periodontal dept. of Yonsei University Hospital between February 1993 and December 2004. The following results are compiled from 231 patients who received single implant surgery. 1. The major cause of single tooth loss in maxillary anterior region is trauma, followed by periodontal disease and congenital missing. 2. The total survival rate of single implant placed in maxillary anterior region is 94.5%. 3. The survival rate of single implant placed in type II and type IV was 100% and in type III was 92.7%. As for the bone quantity, the survival rate in type A(100%) was most, followed by type B(97.3%) and type C, D (93.5%). 4. The survival rate of implant placement combined with advanced surgery was 94.4%. The results showed that the placement of single implant is considered as a reliable treatment option for a single missing tooth in maxillary anterior region except in certain extreme conditions especially with poor bone quality and quantity.
Statement of Problem: To conduct a successful function of implant prosthesis in oral cavity for a long time, it is important that not only structure materials must have the biocompatibility, but also the prosthesis must be designed for the stress, which is occurred in occlusion, to scatter adequately within the limitation of alveolar bone around implant and bio-capacity of load support. Now implant which is used in clinical part has a very various shapes, recently the fixture that has tapered form of internal connection is often selected. However the stress analysis of fixtures still requires more studies. Purpose: The purpose of this study is to stress analysis of the implant prosthesis according to the different implant systems using finite element method. Material and methods: This study we make the finite element models that three type implant fixture ; $Br{\aa}nemark$, Camlog, Frialit-2 were placed in the area of mandibular first premolar and prosthesis fabricated, which we compared with stress distribution using the finite element analysis under two loading condition. Conclusion: The conclusions were as follows: 1. In all implant system, oblique loading of maximum Von mises stress of implant, alveolar bone and crown is higher than vertical loading of those. 2. Regardless of loading conditions and the type of system. cortical bone which contacts with implant fixture top area has high stress, and cancellous bone has a little stress. 3. Under the vertical loading, maximum Von mises stress of $Br{\aa}nemark$ system with external connection type and tapered form is lower than Camlog and Frialit-2 system with internal connection type and tapered form, but under oblique loading Camlog and Frialit-2 system is lower than $Br{\aa}nemark$ system.

Statement of problem. Accurate fit between the implant components is important because the misfit of the implant components results in frequent screw loosening, irreversible screw fracture, plaque accumulation, poor soft tissue reaction, and destruction of osseointegration. Purpose. This study is to evaluate the machining accuracy and consistency of the implant fixture/ abutment/screw interfaces of the internal connection system by using a Stereoscopic Zoom microscope and FE-SEM(field emission scanning electron microscope) Materials and methods. The implant systems selected in this study were internal connection type implants from AVANA(Osstem^{\circledR}), Bioplant(Cowell-Medi^{\circledR}), Dio(DIO^{\circledR}), Neoplant(Neobiotech ), Implantium(Dentium)systems. Each group was acquired 2 fixtures at random. Two piece type abutment and one piece type abutment for use with each implant system were acquired. Screw were respectively used to hold a two piece type abutment to a implant fixture. The implant fixtures were perpendiculary mounted in acrylic resin block. Each two piece abutment was secured to the implant fixture by screw and one piece abutment also secured to the implant fixture. Abutment/fixture assembly were mounted in liquid unsaturated polyester. All samples were cross-sectioned with grinder-polisher unit. Finally all specimens were analysed the fit between implant fixture/abutment/screw interfaces Results and conclusions. 1. Implant fixture/abutment/screw connection interfaces of internal connection systems made in Korea were in good condition. 2. The results of the above study showed that materials and mechanical properties and quality of milling differed depending on their manufacturing companies.

PURPOSE. To determine the change in stability of single-stage, three different design of implant systems in humans utilizing resonance frequency analysis for early healing period(24 weeks), without loading. MATERIAL AND METHODS. Twenty-five patients were included into this study. A total of 45 implants, three different design of implant systems(group A,C,R) were placed in the posterior maxilla or mandible. The specific transducer for each implant system was used. ISQ(implant stability quotient) reading were obtained for each implant at the time of surgery, 3, 6, 8, 10, 12, 24 weeks postoperatively. Data were analyzed for different implant type, bone type, healing time, anatomical locations. RESULTS. For each implant system, a two-factor mixed-model ANOVA demonstrated that a significant effect on ISQ values(group A=0.0022, C=0.017, R=0.0018). For each implant system, in a two-factor mixed model ANOVA, and two-sample t-test, the main effect of jaw position(P > .005) on ISQ values were not significant. CONCLUSIONS. All the implant groups A, C and R, the change patterns of ISQ over time differed by bone type. Implant stability increased greatly between week 0 and week six and showed slow increase between week six and six months(plateau effect).
Purpose: Various studies have investigated 3-dimensional (3D)-printed implants using Ti6Al-4V powder; however, multi-root 3D-printed implants have not been fully investigated. The purpose of this study was to explore the stability of multirooted 3D-printed implants with lattice and solid structures. The secondary outcomes were comparisons between the 2 types of 3D-printed implants in micro-computed tomographic and histological analyses. Methods: Lattice- and solid-type 3D-printed implants for the left and right mandibular third premolars in beagle dogs were fabricated. Four implants in each group were placed immediately following tooth extraction. Implant stability measurement and periapical X-rays were performed every 2 weeks for 12 weeks. Peri-implant bone volume/tissue volume (BV/TV) and bone mineral density (BMD) were measured by micro-computed tomography. Bone-to-implant contact (BIC) and bone area fraction occupancy (BAFO) were measured in histomorphometric analyses. Results: All 4 lattice-type 3D-printed implants survived. Three solid-type 3D-printed implants were removed before the planned sacrifice date due to implant mobility. A slight, gradual increase in implant stability values from implant surgery to 4 weeks after surgery was observed in the lattice-type 3D-printed implants. The marginal bone change of the surviving solid-type 3D-printed implant was approximately 5 mm, whereas the value was approximately 2 mm in the lattice-type 3D-printed implants. BV/TV and BMD in the lattice type 3D-printed implants were similar to those in the surviving solid-type implant. However, BIC and BAFO were lower in the surviving solid-type 3D-printed implant than in the lattice-type 3D-printed implants. Conclusions: Within the limits of this preclinical study, 3D-printed implants of double-rooted teeth showed high primary stability. However, 3D-printed implants with interlocking structures such as lattices might provide high secondary stability and successful osseointegration.
PURPOSE. The aim of this study was to compare the fracture of implant component behavior of external and internal type of implants to suggest directions for successful implant treatment. MATERIALS AND METHODS. Data were collected from the clinical records of all patients who received WARANTEC implants at Seoul National University Dental Hospital from February 2002 to January 2014 for 12 years. Total number of implants was 1,289 and an average of 3.2 implants was installed per patient. Information about abutment connection type, implant locations, platform sizes was collected with presence of implant component fractures and their managements. SPSS statistics software (version 24.0, IBM) was used for the statistical analysis. RESULTS. Overall fracture was significantly more frequent in internal type. The most frequently fractured component was abutment in internal type implants, and screw fracture occurred most frequently in external type. Analyzing by fractured components, screw fracture was the most frequent in the maxillary anterior region and the most abutment fracture occurred in the maxillary posterior region and screw fractures occurred more frequently in NP (narrow platform) and abutment fractures occurred more frequently in RP (regular platform). CONCLUSION. In external type, screw fracture occurred most frequently, especially in the maxillary anterior region, and in internal type, abutment fracture occurred frequently in the posterior region. placement of an external type implant rather than an internal type is recommended for the posterior region where abutment fractures frequently occur.
PURPOSE. To measure axial displacement of different implant-abutment connection types and materials during screw tightening at the recommended torque by using a contact scanner for two-dimensional (2D) and three-dimensional (3D) analyses. MATERIALS AND METHODS. Twenty models of missing mandibular left second premolars were 3D-printed and implant fixtures were placed at the same position by using a surgical guide. External and internal fixtures were used. Three implant-abutment internal connection (INT) types and one implant-abutment external connection (EXT) type were prepared. Two of the INT types used titanium abutment and zirconia abutment; the other INT type was a customized abutment, fabricated by using a computer-controlled milling machine. The EXT type used titanium abutment. Screws were tightened at $10N{\cdot}cm$, simulating hand tightening, and then at the manufacturers' recommended torque ($30N{\cdot}cm$) 10 min later. Abutments and adjacent teeth were subsequently scanned with a contact scanner for 2D and 3D analyses using a 3D inspection software. RESULTS. Significant differences were observed in axial displacement according to the type of implant-abutment connection (P<.001). Vertical displacement of abutments was greater than overall displacement, and significant differences in vertical and overall displacement were observed among the four connection types (P<.05). CONCLUSION. Displacement according to connection type and material should be considered in choosing an implant abutment. When adjusting a prosthesis, tightening the screw at the manufacturers' recommended torque is advisable, rather than the level of hand tightening.
Purpose : The purpose of this study was to evaluate the effect of amount of cantilever in intra-crown according to implant fixture position on mechanical strength of internal conical joint type implant. Materials and Methods : Internal conical joint type implant fixture, abutment screw, abutment was connected and gold alloy prostheses were fabricated and cemented on abutment. For fatigue fracture test, the specimens were loaded to the 350 N, 2,000,000 cycle on 3, 4, 5, and 6 mm off-center of gold alloy prostheses. The fracture pattern of implant component was observed. Results : No fatigue fracture found on 3 and 4 mm group. But initial crack pattern found on 3 specimens of 4 mm group. Fatigue fracture found on all specimens of 5 mm group. But complete fracture was not observed. One specimen of 6 mm group fracture completely. Implant fixture fracture wax not observed. Conclusion : The mechanical failure of implant prostheses increased with the loading area farther from center of implant fixture. To reduce mechanical problem of internal joint type implant, surgical and prosthetic consideration is needed.
Considerable controversy surrounds the choice of the best abutment type for implant prosthetics. The two most common structures are hex and non-hex abutments. The non-hex abutment typically furnishes a larger contact area between itself and the implant than that provided by a hex structure. However, when a hex abutment is loaded, the position of its contact area may be deeper than that of a non-hex abutment. Hence, the purpose of this study is to determine the different biomechanical behaviors of an internal bone-level implant based on the abutment type-hex or non-hex-and clinical crown length under static and cyclic loadings using finite element analysis (FEA). The hex structure was found to increase the implant and abutment stability more than the nonhex structure among several criteria. The use of the hex structure resulted in a smaller volume of bone tissues being at risk of hypertrophy and fatigue failure. It also reduced micromovement (separation) between the implant components, which is significantly related to the pumping effect and possible inflammation. Both static and fatigue analyses, used to examine short- and long-term stability, demonstrated the advantages of the hex abutment over the non-hex type for the stability of the implant components. Moreover, although its impact was not as significant as that of the abutment type, a large crown-implant ratio (CIR) increased bone strain and stress in the implant components, particularly under oblique loading.

Objectives: In this study, we determined the incidence and pattern of screw loosening in patients who received dental implants. Materials and Methods: Patients who received implants between January 2008 and October 2013 and completed their prosthetic rehabilitation were evaluated for the incidence, frequency, and onset of screw loosening using dental charts and radiographs. The association between each factor and screw loosening was analyzed using the chi-square test and a multivariate analysis with binary logistic regression models (P<0.05). Results: Total 1,928 implants were placed in 837 patients (448 males, 389 females), whose follow-up period after loading varied from 0.25 to 70 months (mean period, 31.5 months). Screw loosening occurred in 7.2% of implants. Most cases occurred less than six months after loading. Among those, 22.3% experienced recurrent screw loosening. Screw loosening was most common in the molar region (8.5%) and frequently associated with an implant diameter of ≥5 mm (14.2%). External implant-abutment connections (8.9%) and screw-retained implant prostheses (10.1%) showed higher incidence of problems than internal implant-abutment connections and cement-retained implants, respectively. Screw loosening was most common in implant prostheses with single crowns (14.0%). Conclusion: Within the limits of the current study, we conclude that the incidence of screw loosening differs significantly according to the position of implant placement, the type of implant and manufacturer, implant diameter, the type of implant-abutment connection, the type of retention in the implant prosthesis, and the type of implant prosthesis.
