The effect of Er:YAG laser irradiation on the surface microstructure and roughness of $TiO_2$ implant (Er:YAG 레이저 조사가 산화 티타늄 블라스팅 임플란트 표면 미세 구조 및 거칠기에 미치는 영향)

  • Purpose: The aim of this study was to evaluate the effect of Er:YAG laser on microstructure and roughness of $TiO_2$ blasting implant surface. Materials and Methods: Ten $TiO_2$ blasting implant were used in this experiment. One implant was control group, and nine $TiO_2$ blasting implant surfaces were irradiated with Er:YAG laser under 100 mJ/pulse, 140 mJ/pulse, and 180 mJ/pulse condition for 1 min, 1.5 min, and 2 min respectively. Optical interferometer and scanning electron microscopy was utilized to measure roughness and microstructure of specimens. Results: The surface roughness was decreased after Er:YAG laser irradiation in all groups, but there was no significant difference. 100 mJ/pulse and 140 mJ/pulse group did not alter the $TiO_2$ blasting implant surface in SEM study while 180 mJ/pulse group altered the $TiO_2$ blasting implant surface. Implant surfaces showed melting, microfracture and smooth surface in 180 mJ/pulse group. Conclusion: Detoxification of implant surface using Er:YAG laser must be irradiated with proper energy output and irradiation time to prevent implant surface alteration.

  • The success of an implant is determined by its integration into the tissue surrounding the biomaterial. Surface roughness is considered to influence the behavior of adherent cells. The aim of this in vitro study was to determine the effect of surface roughness on Saos-2 osteoblast-like cells. Titanium disks blasted with 75 ${/mu}m$ aluminum oxide particles and machined titanium disks were prepared. Saos-2 were plated on the disks at a density of 50,000 cells per well in 48-well dishes. After 1 hour, 1 day, 6 days cell numbers were counted. One day, 6 days after plating, alkaline phosphatase(ALPase) activity was determined. Compared to experimental group, the number of cells was significantly higher on control group. The stimulatory effect of surface roughness on ALPase was more pronounced on the experimental group than on control group. These results demonstrate that surface roughness alters proliferation and differentiation of osteoblasts. The results also suggest that implant surface roughness may play a role in determining phenotypic expression of cells.

Bone-to-Implant Contact according to the Surface Roughness of the Implants (임플란트의 표면조도가 골융합에 미치는 영향)

  • This study was performed to evaluate the difference of the bone-to-implant contact according to the surface roughness of the implants. Two beagles were used in the experiment. Extraction of the all premolars was performed in the mandible. In 3 months of healing, screw-shaped pure titanium machined surface implants (Implantium(R), Dentium Co. Korea), implants blasted with 45${\mu}m$ $TiO_2$ particles, (Implantium(R), Dentium Co. Korea) and implants blasted with 100${\mu}m$ $TiO_2$ particles (Implantium(R), Dentium Co. Korea) in diameter 3.4mm and length 6mm were installed in the edentulous mandible. Each dog was sacrificed at 4, 12 weeks after placement and then nondecalcified specimens were prepared for histologic analysis. The results of this study were as follows. 1. At 4 , 12 weeks after the surgery, bone-to-implant contact in the surface blasted with $TiO_2$ particles was higher than that in the pure titanium machined surface respectively. 2. Osseointegration in the surface blasted with 45${\mu}m$ $TiO_2$ particles was more increased than that in the surface blasted with 100${\mu}m$ $TiO_2$ particles. 3. Bone-to-implant contact was increased with time independent of surface roughness. 4. Bone formation was in the outfolded area more than inside the threads independent of surface roughness. From the above results, we were able t o find the most bone-to-implant contact in 45${\mu}m$ $TiO_2$ blasted implant.

  • Purpose: One of the most frequent complications related to dental implants is peri-implantitis, and the characteristics of implant surfaces are closely related to the progression and resolution of inflammation. Therefore, a technical modality that can effectively detoxify the implant surface without modification to the surface is needed. The purpose of this study was to evaluate the effect of erbium-doped: yttrium, aluminium and garnet (Er:YAG) laser irradiation on the microstructural changes in double acid-etched implant surfaces according to the laser energy and the application duration. Methods: The implant surface was irradiated using an Er:YAG laser with different application energy levels (100 mJ/pulse, 140 mJ/pulse, and 180 mJ/pulse) and time periods (1 minute, 1.5 minutes, and 2 minutes). We then examined the change in surface roughness value and microstructure. Results: In a scanning electron microscopy evaluation, the double acid-etched implant surface was not altered by Er:YAG laser irradiation under the condition of 100 mJ/pulse at 10 Hz for any of the irradiation times. However, we investigated the reduced sharpness of the specific ridge microstructure that resulted under the 140 mJ/pulse and 180 mJ/pulse conditions. The reduction in sharpness became more severe as laser energy and application duration increased. In the roughness measurement, the double acid-etched implants showed a low roughness value on the valley area before the laser irradiation. Under all experimental conditions, Er:YAG laser irradiation led to a minor decrease in surface roughness, which was not statistically significant. Conclusions: The recommended application settings for Er:YAG laser irradiation on double acid-etched implant surface is less than a 100 mJ/pulse at 10 Hz, and for less than two minutes in order to detoxify the implant surface without causing surface modification.

Analysis of surface form change after performing prophylaxis procedure on implant surface using various oral hygiene instruments (다양한 구강위생기구를 이용하여 임플란트 표면의 Prophylaxis 시행시 표면형태의 변화분석)

  • It is improtant that performing prophylaxis procedure on an infected implant surface in order to treat peri-implantitis should not change the surface roughness and composition, so that the surface can be recovered to almost same condition as initial implant surface. This thesis, therefore, studied an effect of various oral hygiene instrument on implant surface. A surface roughness measurement instrument and an infection electron microscope were used to observe a change on surface. The purpose of this study was to obtain a clinical guidelines during implant care and peri-implantitis treatment. The result were as follows 1. Ra values (surface roughness value) at experimental group 1, group 2, and group 5 were increased significantly as compared with comparison group(p<0.05). 2. When compared experimental group 1 with each experimental groups at which prohylaxis procedure was performed, mean values of Ra at experimental group 2, group 3, group 6, and group 7 were decreased significantly(p<0.05). 3. Mean value of Ra was lowest at experimental group 2, and highest at experimental group 2, and highest at experimental group 5. 4. Analysis of SEM showed that was significant surface change at experimental group 2, group 3, group 4, group 5, and group 6 as compared with comparison group(X1000). 5. Analysis fo EDX showed that a quantity of Ti on surface for experimental group 6 was very similar to that for comparison group. In conclusion, air-powder abrasive and citric acid, plastic instrument are safe methods to use for performing prophylaxis procedure on implant care or for cleaning and sterilization process on treatment of peri-implantitis, based on the result that those method did not affect implant surface roughness and Ti composition.

Micromorphometric change of Ti$O_2$ blast implant surface conditioned with tetracycline-HCI (산화 티타늄 블라스팅 임프란트 표면구조에 대한 염산 테트라싸이클린의 영향)

  • The present study was performed to evaluate the surface roughness and effect of Tetracycline-HCI on the change of implant surface microstructure according to application time. Ti$O_2$ surface Implant was utilized. Implant surface was rubbed with 50mg/ml Tetracycline-HCI solution for 0.5min, 1min, 1.5min, 2min, 2.5min and 3min respectively in the Tetracycline-HCI group. Then, specimens were measured surface roughness and processed for scanning electron microscopic observation. The results of this study were as follows. 1. Ti$O_2$ blast implant surface showed increased surface roughness 1.5 minute after treatment with Tetracycline-HCI. But, there were not significant differences in saline group after treatment. 2. Tetracycline-HCI group showed changed surface micro-morphology in SEM after 1.5 minute. There were not significant differences in saline group after treatment. 3. Between Tetracycline-HCI group and saline group, there were difference in surface roughness change and SEM micro-morphology. Tetracycline-HCI have influence on Ti$O_2$ blast implant surface. In conclusion, the detoxification with 50mg/ml Tetracycline-HCI must be applied respectively with different time according to various implant surfaces.

  • The present study was performed to evaluate the effect of Tetracycline-HCI and Saline on the change of implant surface microstructure and surface roughness according to application time. Implants with resorbable blasting media surface were utilized. Before test all 13 implants were measured surface roughness. Among them, 6 implants were rubbed with 50mg/ml Tetracycline-HCl solution and other 6 implants with saline for $\frac{1}{2}$min., 1min., $1\frac{1}{2}$min., 2min., $2\frac{1}{2}$min and 3min. Then, specimens were processed for scanning electron microscopic observation and surface roughness after test. The results of this study were as follows. 1. Control group showed a few irregular, rough, uneven surface with crater-like depression. 2. The test group with Tetracycline-HCl conditioning showed an altered surface when Tetracycline-HCl was applied for 30secs, and showed a various surface alteration as application times go on. 3. The test group with Saline conditioning showed no significant surface differences and surface roughness. 4. The significant increase of Ra value was showed when Tetracycline-HCl was applied for 30secs. In conclusion, the 50mg/ml Tetracycline-HCl must not be applied for the RBM surface implant for surface treatment.

ON THE BONE TISSUE REACTION TO IMPLANTS WITH DIFFERENT SURFACE TREATMENT METHODS (임플랜트 표면 처리 방법에 따른 골조직 반응에 대한 연구)

  • Statement of problem: Implant surface characteristics plays an important role in clinical success and many studies have been made for improvement of success by changing surface roughness. Purpose: Appropriate increase of surface roughness increases the activity of osteoblast and enhance contact and retention between bone and implant. Material and method- Machined, SLA and RBM surface implants, which are the most commonly used implants were implanted into the tibia of rabbits and after 1 week, 4 weeks, 8 weeks and 12 weeks there were histologic and histomorphometric analysis and study for bone gradient and change of Ca/P ratio using EDS(Energy Dispersive X-ray Spectroscope). Results: Comparison of bone-implant contact showed no significant difference among each implant. In comparison of bone area rates, SLA showed higher value with significant difference at 1 week and 4 weeks, and SLA and RBM at 8 weeks than Machined implant (p<0.05). In analysis of bone constituents with EDS, titanium was specifically detected in new bones and the rates were constant by surface treatment method or period. In case of Ca/P ratio, according to surface treatment method, each group showed significant difference. Lots of old bone fragments produced during implantation remained on the rough surface of RBM implant surface and each group showed histological finding with active synthesis of collagen fibers until 12 weeks. In transmission electronic microscopic examination of sample slice after elapse of twelve weeks, tens nm of borderline (lamina limitans like dense line)was seen to contact the bone, on the interface between bone and implant. Conclusion: SLA and RBM implant with rough surface shows better histomorphometrical result and the trend of prolonged bone formation and maturation in comparison with Machined implant. In addition, implant with rough surface seems to be helpful in early stage bone formation due to remaining of old bone fragments produced in implantation. From the results above, it is considered to be better to use implant with rough surface in implantation.

Wettability of titanium implants depending upon surface properties (타이타늄 표면 처리 특성에 따른 젖음성에 관한 연구)

  • Statement of problem: When an implant is fixed, a fixture comes into contact with a tissue fluid. Adhesion of a tissue fluid to a surface of implant is various case by case. Purpose: The ultimate goal of this work is to analyze a correlation between a surface roughness and wettability of implant specimens. A measurement for wettability is performed considering 4 types of specimen implant with surface treatments different from each other to investigate the change of wettability with the elapse of time. Material and methods: Firstly, 20 specimens of titanium were prepared. The specimen were made of a commercial Titanium Grade IV with the diameter of 10 mm and the thickness of 1 mm. According to the method of surface treatment, the specimens were classified into 4 groups of 5 specimens per group. Group A: Machined Surface Group B: Anodized surface Group C: RBM (HA blasting) surface Group D: CMP (calcium methaphosphate) coating surface. Surface roughness of specimen was measured using SV-3000S4 (Mituyoto, Japan). The measurement was based on the standard of JIS1994. Sessile drop method was used to measure the wettability, which measures contact angle between implant disc and saline with the time interval of 5, 10, and 15 seconds. SPSS 11.0 was used to analyze the collected data. In order to analyze the difference of wettability and surface roughness according to implant surface treatment method. The statistical significance was tested with the confidence level of 95%. Pearson's correlation coefficient was used to evaluate the correlation of surface roughness and wettability. Results: The difference of surface roughness was statistically significant in the order of Group C ($1.69{\pm}0.26$), Group D ($1.58{\pm}0.16$), Group B ($0.78{\pm}0.14$) Group A ($0.18{\pm}0.05$). The wettability has also a statistically significant difference, which was in the order of group B ($17.70{\pm}2.66$), Group C ($27.86{\pm}4.52$), Group D ($66.28{\pm}3.70$) Group A ($70.52{\pm}8.00$). There was no difference in wettability with the passage of time. Conclusions: 1. The surface roughness was high in the order of RBM, CMP, Anodized, Machined group (P<.05). 2. The wettability was high in the order of Anodized, RBM, CMP, Machined group (P<.05). 3. There was no statistical significance in the correlation of surface roughness and wettability.

EFFECTS OF SURFACE COATING ON THE SCREW RELEASE OF DENTAL IMPLANT SCREW (치과용 임플란트 나사의 풀림에 미치는 표면코팅 효과)

  • Statement of problem: Implant screw loosening has been remained problem in restorative practices. Surface treatment of screw plays a role of preventing screw from loosening in implant screw mechanism. Purpose : The purpose of this study was to investigate surface characteristics of TiN and ZrN film ion plated screw with titanium and gold alloy screw and to evaluate wear resistance, surface roughness, and film adhesion on screw surface using various instruments. Material and methods : GoldTite screws and titanium screws provided by 3i (Implant Innovation, USA) and TorqTite screws or titanium screws by Steri-Oss (Nobel Biocare, USA) and gold screws and titanium screws by AVANA (Osstem Implant, korea) were selected. Ion plating which is much superior to other surface modification techniques was carried out for gold screws and titanium screws using Ti and Zr coating materials with nitrogen gas. Ion nitrided surface of each abutment screw was observed with field emission scanning electron microscopy (FE-SEM, micro-diamond scratch tester, vickers hardness tester, and surface roughness tester. Results : 1) The surface of gold screw and GoldTite is more smooth than ones of other kinds of non coated screw. 2) The ZrN and TiN coated surface is the more smooth than ones of other kinds of screw. 3) The hardness of TiN and ZrN coated surface showed higher than that of non coated surface. 4) The TiN coated titanium screw and ZrN coated gold screw have a good wear resistance and adhesion on the surface. 5) The surface of ZrN coated screw showed low surface roughness compared with the surface of TiN coated screw. Conclusion : It is considered that the TiN and ZrN coated screw which would prevent a screw from loosening can be applicable to implant system and confirmed that TiN and ZrN film act as lubricant on surface of screw due to decrease of friction for recycled tightening and loosening.
