Short-term improvement of masticatory function after implant restoration

  • Purpose: Dental implants present several advantages over other tooth replacement options. However, there has been little research on masticatory function in relation to implant treatment. Therefore, the aim of the present study was to evaluate the improvement of masticatory function two weeks after implant restoration. Methods: Masticatory ability was evaluated with the subjective food intake ability (FIA) and objective mixing ability index (MAI) methods. Fifty-four subjects with first and second missing molars completed the study. The subjects were asked to complete a self-reported questionnaire about 30 different food items, and to chew wax samples 10 times both before and two weeks after implant restoration. A total of 108 waxes were analyzed with an image analysis program. Results: Dental implant restoration for lost molar teeth on one side increased the FIA score by 9.0% (P<0.0001). The MAI score also increased, by 14.3% after implant restoration (P<0.0001). Comparison between the good and poor mastication groups, which were subdivided based on the median MAI score before implant restoration, showed that the FIA score of the poor group was enhanced 1.1-fold while its MAI score was enhanced 2.0-fold two weeks after an implant surgery. Conclusions: Using the FIA and MAI assessment methods, this study showed that masticatory function was improved two weeks after implant restoration. In particular, the enhancement of masticatory function by implant restoration was greater in patients with relatively poor initial mastication than in those with good initial mastication.

  • Purpose: The aim of this study was to obtain objective and standardized information on masticatory function and patient atisfaction following second molar single implant therap. Methods: Twenty adult patient, who had restored second molar single implants more than 1 month before the study, were enrolled in this stud. All patients received a chewing test using peanuts before and after insertion of the implant prosthesi, with a questionnaire and visual analogue scale (VAS) to evaluate the effect of second molar single implant therap. Results: his study obtained standardized information on the masticatory function objectively (e.g., P, R, $X_{50}$) before (Pre-insertion) and after insertion (Post-insertion) of the implant prosthesis. Masticatory performance (P) after insertion of the implant prosthesis significantly increased from $67.8{\pm}9.9$ to $84.3{\pm}8.5$% (P<0.0001). With the implant prosthesis, the P value increased by 24%. The masticatory efficiency index (R) of Post-insertion is higher than that of Pre-insertion (P<0.0001). With the implant prosthesis, the R value increased by 29%. The median particle size ($X_{50}$) of Post-insertion is lower than that of Preinsertion (P<0.0001). More than 90% of the patients were satisfied with the second molar single implant therapy from a functional point of view. Conclusions: These findings indicate that a second molar single implant can increase masticatory function.

  • STATEMENT OF PROBLEM: The improvement in oral function and comfort from the dental implant appears to depend on the particular type of implant support used with the denture. The number and positioning of implants have an influence on the force transfer and subsequent stress distribution around implants. Nevertheless, a quantitative comparison has not been made between the types of implant prosthesis used with different materials compared to conventional complete denture. PURPOSE: The objective of this study is to assess the masticatory performance, bite force and impact of two different type of implant supported prostheses on oral health-related quality of life compared to conventional complete denture with GOHAI, validated oral-specific health status measures, the sieving method, and the Prescale Dental System. MATERIAL AND METHODS: From the years 1999 to 2006, a total of 30 completely edentulous patients in a single arch were selected from the Yonsei University Dental Hospital, Department of Prosthodontics and Implant Clinic in Seoul, S. Korea. Patients were divided into 3 groups of 10 each. Group HR was restored with fixed-detachable hybrid prostheses with resin teeth. Group FP had fixed dentures with porcelain teeth while Group CD had a complete denture. The masticatory performance was compared between 3 groups. RESULTS: The results showed a significant improvement in oral health-related quality of life with dental implants compared to a conventional denture in GOHAI comparison. Overall, implant prostheses showed a higher masticatory performance ($S_{50}$) and maximum bite force compared with conventional dentures (P < .05) but no differences between different implant supported prostheses (P > .05). CONCLUSION: Within the limitation of this study, the numbers of implant and material of implant prostheses does not appear to impact patient satisfaction, masticatory performance or bite force.

치과 임플란트 시술 전·후 만족도 변화 비교 (Comparison of Satisfaction Before and After Dental Implant)

  • 본 연구는 치과 임플란트 시술 환자의 만족도에 미치는 요인을 확인하고 시술 후 만족도 향상을 위해 시행된 연구이다. 일개 치과병원에 내원한 임플란트 환자 178명을 대상으로 조사한 결과, 임플란트 시술 전·후의 저작 기능은 1.07점, 사회적 기능 만족도 점수는 0.7점, 심리적 기능 만족도 점수 0.97점 높게 나타났다. 따라서, 임플란트 시술 만족도 향상을 위해서는 임플란트 시술 환자를 대상으로 하는 장기간 추적관찰 연구가 진행되어야 하고 환자의 만족도 변화를 분석하고 평가하기 위한 지속적인 후속연구의 필요성을 제안하며, 임플란트 최종 보철물 장착 후 구강보건교육과 상담 등 지속적인 관리가 필요할 것이다.

  • 임플란트 보철물의 저작능력과 관련되어 다양한 요인들이 존재하며 여기에 관해 다양한 연구가 이루어지고 있지만 대부분의 후향적 연구에서는 임플란트 저작력이 다른 유형의 보철물과 비교해서도 우수한 것으로 결론 내리고 있다. 그러나 임상적으로 임플란트 보철치료 후에 저작능력에 대해 만족하지 못하는 환자들이 간혹 있으며 이러한 저작의 불만족을 유발할 가능성이 있는 여러 가지 원인들 중 신경학적 원인에 대해 고찰해 보고자 한다. Pubmed database에서 Implant chewing ability, masticatory ability 등의 임플란트 저작과 관련된 검색어를 사용하여 검색 후 임플란트 보철물의 저작능력에 관련된 요인들 및 임상과 관련된 논문들을 선택하고 고찰하였다. 저작능력에 관한 정의, 저작능력에 관련된 요인들에 대해 고려하였으며 이러한 요인들 중 신경학적 원인과 관련된 내용을 분석 평가하였다. 치주인대(Periodontal Ligament: PDL)의 기계적감각수용기(Mechanoreceptor)는 저작운동 시 치아로부터 얻은 정보를 뇌간으로 전달하여 저작운동의 조절에 관여한다. 임플란트의 경우 치주인대의 부재로 인해 저작운동 시 저작운동 적응 능력이 떨어지며 특히 딱딱한 음식을 저작 시 이러한 현상이 두드러진다. 저작근, TMD의 mechanoreceptor 또한 저작운동에 관여하기 때문에 치주인대의 기계적감각수용기 부재를 보상할 가능성도 있으며 임플란트 주변 조직에 있는 nerve fiber가 감각능력에 관여할 수 있을 가능성에 대한 보고도 있으니 이에 대한 추가 연구가 더 필요할 것으로 사료된다.

임플란트 보철의 기능별 환자 만족도 연구 (Patient satisfaction study by function of implant prosthesis)

  • Purpose: This study aimed to provide the basic data for improving patient satisfaction and dental services by examining the satisfaction of implant patients who have become important customers of dental treatment due to the increased demand for implants and identifying the factors affecting satisfaction. Methods: Out of 145 questionnaires, excluding 15 questionnaires that were unfaithfully responded to or difficult to analyze, 130 questionnaires were used for the analysis of the satisfaction of the dental patients with implant treatment in 19 dental hospitals and clinics in Gyeonggi-do, South Korea. Results: Implant satisfaction was as high as 3.20 (±0.56) for masticatory function satisfaction, 3.19 for psychological function satisfaction (±0.59), and 3.01 (±0.58) for social function satisfaction. Conclusion: Consistent patient care is important because an implant is not a one-shot cure but ends over a period of several months to several years. It is meaningful in that this study measured the satisfaction of implant prostheses evaluated from a patient's point of view and examined various factors and needs affecting patient satisfaction to provide the basic data for research that can contribute to improving the satisfaction of implant patients.

임플란트 시술에 대한 환자의 기능별 만족도 (Patients' Functional Satisfaction of Dental Implant Treatment)

  • 본 연구는 임플란트 시술에 대한 환자의 기능별 만족도를 파악하고 이와 관련된 요인을 분석함으로써 환자만족도 향상에 도움이 될 수 있는 기초자료를 제공하고자 시행되었다. 2007년 1월 3일부터 9월 30일 까지 서울 인천 경기도에 소재한 치과 의료기관에서 임플란트를 식립한 환자를 대상으로 설문조사를 하였으며, 최종 분석은 120부(92.3%)를 사용하였다. 수집한 자료는 t-test와 ANOVA 분석, Pearson 상관분석을 수행하였으며, 다음과 같은 결과를 얻었다. 1. 일반적 특성에 따른 각 기능별 만족도는 통계적으로 유의한 차이를 보이지 않았다. 2. 자가 구강위생관리에 따른 각 기능별 만족도의 차이를 분석한 결과 정기적으로 치석을 제거하고 일일잇솔질 횟수가 4회 이상인 집단에서 사회적 기능 및 심미적 기능에 대한 만족도가 가장 높은 것으로 나타났다(p < .05). 3. 저작기능에 대한 만족도는 소구치 부위에 식립하였을 때 가장 높았고, 사회적 심미적 기능에 대한 만족도는 전치 부위에서 가장 높았으나 통계적인 유의성은 없었다. 4. 사회적 기능과 심미적 기능에 대한 만족도는 치과병원에서 가장 높았으며 다음은 치과의원, 종합병원순으로 나타났다(p < .05).5. 저작기능과 사회적 기능, 심미적 기능 및 총만족도는 서로 양(+)의 상관관계를 보였고, 특히 총만족도와 사회적 기능의 만족도간에 상관계수가 가장 높은 것으로 나타났다(p < .01).

의치 장착 노인과 임플란트 장착 노인의 저작능력 및 삶의 질 비교 연구 (A study of comparative the mastication capability and life quality of elderly people using dentures or implants)

  • Objectives : The purpose of this study was to determine the effects of oral health on the quality of life of elderly patients with dentures or implants. It is our intention that through this analysis we can aid seniors in making the choice between dentures or implants, considering future masticatory function and the likely effects of this decision on the patients quality of life. Methods : This survey was conducted between November 1, 2010 and November 25, 2010, in Kyunggi-Do, whasung city. The research was carried out at one public health center, among elderly patients. 105 seniors were surveyed using the direct interviewing method, oral health related quality of life was measured by the Philadelphia Geriatric Center(PGC) Morale Scale. Collected data was examined using the SPSS 13.0 program, using frequency, mean, and standard deviation analysis, T-test and $x^2$ test. Results : The number of residual teeth for patients who have implants was $20.45{\pm}5.85$, while among those patients with dentures the number was considerably lower, $8.11{\pm}7.66$(p<0.05). The results of patients masticatory function, was $28.13{\pm}2.40$ for those with implants, and $25.35{\pm}4.15$ with dentures. The results were better for those with implants. Among implant patients overall satisfaction was rated $25.21{\pm}3.63$, higher than for those with dentures $20.20{\pm}6.79$(p<0.05). Quality of life was scored higher for patients with implants($12.76{\pm}2.61$) compared to those with dentures($10.47{\pm}3.52$)(p<0.05). The subjects masticatory function was highly related to the number of remaining teeth and greater masticatory function had a positive effect on patients quality of life and life satisfaction. Conclusion : Dental treatments such as dentures and implants will have a great influence on patients quality of life and life satisfaction. Implants can improve the oral health related quality of life and satisfaction and the general quality of life more than denture therapy.

국산 Avana implant의 임상결과에 관한 연구

  • Osseointegrated dental implants have now become and accepted form of treatment for patients with a fully or partially missing dentition. The purpose of this study was to evaluate the performance of AVANA implant-Korea. A total of 323 AVANA implants were placed at 9 centers in the edentulous and partially edentulous jaws of 88 consecutive patients ranging in age form 18 to 79 years. So we get the clinical results. 1. Both patients (94.4%) & doctors (94%) were satisfied with the results, in patient's aspect 'most satisfaction' case were more common(52%) 2. The length of fixture which was frequently used was 13mm (168/319,44.5%) followed by 15mm and 10mm, and 3.75mm width implant (239/319,74.9%) was the most popularly used. 3. Mandibutar posterior was the most common installation site (140/317.43%) 4. A mean implant survival rate was 96.2%. That showed little difference which reported other paper. Cause of failure consisted of inflammation $ infection-36%, fall of osseointegration-18%, paln during masticatory function-9%. improper prosthetic function $ procedure-9%. 5. The most popularly used types of abutment was the UCLA abutment(168/305,55.5%)

Diagnosis and Clinical Management of Retrograde Peri-Implantitis Associated with Adjacent Apical Periodontitis: a Case Report

  • Peri-apical implant lesion, also known as 'retrograde peri-implantitis' can occur with multifactorial etiological factors. The purpose of this case report is to demonstrate resolution of periapical implant lesion by removal of causative factors and saving implant by regenerative therapy. A 54-year old male patient with mild dull pain around implant on the right mandibular second premolar area due to persistent peri-apical infection of the adjacent first premolar was treated. Extraction of tooth with symptomatic apical periodontitis and regenerative therapy on the buccal fenestration area of the implant and extraction site were performed. After 6-month reentry, notable regenerated bone tissue around implant was found, and implant placement on the previous extraction site was performed. After 14-month follow-up from the regenerative therapy, neither biological nor mechanical complication could be found around the implant, evidenced by high implant stability, normal clinical probing depth, and absence of discomfort spontaneously and during masticatory function. In conclusion, surgical intervention including regenerative therapy using bone graft and barrier membrane on periapical implant lesion can be suggested as one of the treatment options considering the extent of periapical lesion.
