Influence of the connection design and titanium grades of the implant complex on resistance under static loading

  • PURPOSE. The purpose of this study was to evaluate the resistance to deformation under static overloading by measuring yield and fracture strength, and to analyze the failure characteristics of implant assemblies made of different titanium grades and connections. MATERIALS AND METHODS. Six groups of implant assemblies were fabricated according to ISO 14801 (n=10). These consisted of the combinations of 3 platform connections (external, internal, and morse tapered) and 2 materials (titanium grade 2 and titanium grade 4). Yield strength and fracture strength were evaluated with a computer-controlled Universal Testing Machine, and failed implant assemblies were classified and analyzed by optical microscopy. The data were analyzed using the One-way analysis of variance (ANOVA) and Student's t-test with the level of significance at P=.05. RESULTS. The group $IT4_S$ had the significantly highest values and group IT2 the lowest, for both yield strength and fracture strength. Groups $IT4_N$ and ET4 had similar yield and fracture strengths despite having different connection designs. Group MT2 and group IT2 had significant differences in yield and fracture strength although they were made by the same material as titanium grade 2. The implant system of the similar fixture-abutment interfaces and the same materials showed the similar characteristics of deformation. CONCLUSION. A longer internal connection and titanium grade 4 of the implant system is advantageous for static overloading condition. However, it is not only the connection design that affects the stability. The strength of the titanium grade as material is also important since it affects the implant stability. When using the implant system made of titanium grade 2, a larger diameter fixture should be selected in order to provide enough strength to withstand overloading.

  • Purpose: The purpose of this study was to evaluate the pattern and the magnitude of stress distribution in the supporting tissues surrounding three different types of implants(ITI, 3i. and Bicon implant system) Material and method: Photoelastic models were made with PL-2 resin(Measurements Group, Raleigh, USA) and three implants of each kind were placed in the mandibular posterior edentulous area distal to the canine. For non-splinted restorations, individual crowns were fabricated on three titanium abutments. For splinted restorations, 3-unit axed partial dentures were fabricated. Photoelastic stress analyses were carried out to measure the fringe order around the implant supporting structure under simulated loaded conditions(15 lb. 30 lb). Conclusion: The results were as follows; 1 Regardless of the implant design, stresses were increased in the apex region of loaded implant when non-splinted restorations were loaded. While relatively even stress distribution occurred with splinted restorations. Splinting was effective in the second implant. 2. Strain around Bicon implant were lower than those of other implants, which confirmed the splinting effect. The higher the load, the more the stress occurred in supporting tissue, which was most obvious in the Bicon system. 3. Stress distribution in the supporting tissue was favorable in the ITI system. while the other side of 3i system tended to concentrate the stress in some parts.

  • Statement of problem : Since the concept of osseointegrated dental implant by $Br{\aa}nemark$ et al was first applied to mandibular full edentulous patients. Recently it is considerated the first treatment option on missing teeth. A common problem associated with dental implant restorations is loosening of screws that retain the prosthesis to the abutment and the abutment to the implant fixture. Purpose : This study is to examine the influence on screw loosening of implant-abutment designs. Material and methods : External hex, cone screw, beveled hex, cam cylinder, cylinder hex by means of evaluating the loosening torques, with respect to a range of tightening torques after repeated loading. Result : 1. Cone screw, beveled hex groups are the highest initial tightening rate and cylinder hex, external hex groups are the lowest initial tightening rate (p < 0.05). 2. Cone screw groups are the highest after repeated loading tightening rate and cylinder hex groups are lowest after repeated loading tightening rate(p < 0.05). 3. Cone screw groups have the highest initial stability and anal stability. 4. All groups are decreased tightening rate after repeated loading.

임플랜트 Overdenture의 Bar설계에 따른 하악지지조직의 광탄성학적 응력분석 (PHOTOELASTIC STRESS ANALYSIS ON THE MANDIBLE CAUSED BY IMPLANT OVERDENTURE)

  • This study was performed to evaluate the effects of number and alignment of implant fixture and various bar designs on the retention of denture and the stress distribution. Six kinds of photoelastic mandibular models and nine kinds of overdenture specimens were designed. A unilateral vertical load was gradually applied on the right first molar to calculate the maximal dislodgement load of each specimen. A unilateral vertical load of 17 Kgf was applied on the right first molar and a vertical load of 10 Kgf was applied on the interincisal edge region. The stress pattern which developed in each photoelastic model was analyzed by the reflection polariscope. The results obtained were as follows: 1. The maximal dislodgement load reversely increased with the distance from the loading point to the implant fixture, while it linearly increased with that from the most posterior implant fixture to the mesial clip. The maximal dislodgement load also increased with the use of a cantilever bar. 2. Under the posterior vertical load, the stress to the supporting tissue of the denture base increased with the distance from the loading point to the implant future. The stress concentration on the apical area of the implant future reversely increased with the distance from the loading point to the implant future. 3. In the overdentures supported by two implant fixtures under the posterior vertical load. the specimen implanted on lateral incisor areas with a cantilever bar exhibited more favorable stress distribution than that without a cantilever bar. The specimen implanted on the canine areas without a cantilever bar, however, exhibited more favorable stress distribution. 4. In the overdentures supported by three implant fixtures. the specimen implanted ell the midline and canine areas exhibited more favorable stress distribution than that implanted oil the midline and the first premolar areas. 5. In the overdentures supported by four implant fixtures. the specimen implanted with two adjacent implant fixtures exhibited more favorable stress distribution than that implanted at equal distance under the posterior vertical load. 6. Under the anterior vertical load, the overdentures supported by three implant fixtures exhibited stress concentration on the supporting structure of the middle implant future. In overdentures supported by two or four implant futures, no significant difference was noted in stress distribution between the types of bars. These results indicate that the greater the number of implant fixtures, the better the stress distribution is. A favorable stress distribution may be obtained in the overdentures supported by two or three implant fixtures, if the location and the design of the bar are appropriate.

  • The osseointegration of titanium implants has been examined over the past 50 years. Many implant systems have been introduced and have become popular to the implant dentistry. The designs of the connection between implant fixture and abutment are divided into external vs internal connection. From beginning, the $Br{\aa}nemark$ system was characterized by an external hexagon. Internal connection has been developed to reduce stress transferred to the bone. These differences may have impact on the clinical procedures and protocols, laboratory and components costs, and incidence of complications. Therefore, the clinician has to know the different biomechanical features and understand their implications to produce successful implant-supported prosthesis with an external or an internal connection system.

임플란트-지대주-나사의 적합에 관한 연구 (A STUDY ON THE FIT OF THE IMPLANT-ABUTMENT-SCREW INTERFACE)

  • Statement of problem : There have been previous studies about considerable variations in machining accuracy and consistency in the implant-abutment-screw interfaces. Purpose : The purpose of this study was to evaluate the machining accuracy and consistency of implant/abutment/screw combinations on two randomly selected implants from each of four manufactures. Material and methods : In this study, screws were respectively used to secure a cemented abutment, to a hexlock implant fixture ; teflon coated titanium alloy screw(Torq-Tite) and titanium alloy screw in Steri--Oss system, gold-plated gold-palladium alloy screw(Gold-Tite) and titanium alloy screw in 3i system gild screw ana titanium screw in AVANA Dental Implant system, and titanium screws in Paragon System. The implants were perpendicularly mounted in polymethyl methacrylate autopolymerizing acrylic resin block(Orthodontic resin, Densply International Inc. USA) by use of dental surveyer. Each abutment screw was secured to the implant with recommended torque value using a digital torque controller. Each screw was again tightened after 10 minutes. All samples were cross sectioned with grinder-polisher unit(Omnilap 2000 SBT Inc) after embeded in liquid unsaturated polyester (Epovia, Cray Valley Inc) Results : There were the largest gaps in the neck areas of screws in hexagonal extension implants which were examined in this study. The leading edge of the abutment screw thread (superior surface) was in contact with the implant body thread, and the majority of the contacting surfaces were localized to the middle portion of the mating threads. Considerable variation in the contacting surfaces was noted in the samples evaluated. Amounts of contact in the abutment screw thread were larger for assemblies with Gold-Tite screw, gold alloy screw. Torq-Tite screw than those with titanium screws. The findings of intimate contact between the screw and screw seat were seen in all samples, regardless of manufacturers. However, microgap between the head and lateral neck surface of the screw and the abutment could be dectected in all samples. The findings of intimate contact between the platform of the implant and the bottom of the abutment were consistent in all samples, regardless of manufacturers. However, microgaps between the lateral surface of external hex of the fixture and the abutment could be dectected in all samples. Conclusion : Considerable variations in machining accuracy and consistency were noted in the samples and the implant-abutment-screw interfaces were incomplete. From the results of this study, further development of the system will be required, including improvements in pattern design.

  • The osseointegrated implant conducts the stress directly to the bone due to lack of cushoning effect of periodontal ligament. So, the design and material quality of superstructure plays an important role in resolution and diffusion of stress. Recently, the various superstructures have been developed to improve esthetics and resolve various complicated conditions. The purpose of this study was to evaluate the stress induced by various system on the osseointegrated implant using UCLA abutment, EsthetiCone abutment, Anatomic abutment as well as Branemark conventional abutment. The stress distribution was evaluated by the photoelastic method which can simultaneously observe all around stress distribution. The superstructures embedded in epoxy resin specimen were loaded at various angle with a force of 15Kg to analyse the stress distribution of the fixture. The results of this study were obtained as follows : 1. Under vertical loading, the large and broad stress was distributed below the fixture in all systems. 2. The fringe order of the stress was increased in proportion to tillting the specimen. The largest stress was shown in 25 angled degree tilting case. 3. The Branemark conventional abutment showed the lowest value, and EsthetiCone abutment, Anatomic abutment and UCLA abutment showed the stress value in accending order.

임플란트 CT에서 방향 표시자의 방향과 잔존골을 고려한 임플란트 식립 방향의 관계에 관한 연구 (Study of the relationship between the indication rod of stent on implant CT and the real path of implant fixture insertion considering residual ridge)

  • Purpose : To assess the relationship between the direction of the indicating rod of the radiographic stent for ideal prosthetic design and the actual possible path of implant fixture placement when residual ridge resorption is considered. Materials and Methods: The study materials consisted of 326 implant sites (male 214 cases and female 112 cases) from a total of 106 patients (male 65 patients and female 41 patients) who desired implant prostheses. Computed tomography of patients were taken and reformatted using ToothPix/sup (R)/ software. Bony defects, bony sclerosis, the change of the direction of indicating rod, and root proximity of the adjacent teeth were examined on the CT-derived images. Results: The rate of the irregular crestal cortex was relatively high on premolar and molar area of maxilla. Mandibular molar area showed relatively high rate of focal sclerosis on the area of implant fixture insertion. The position of the indicating rods were relatively acceptable on the molar areas of both jaws. However, the position of the indicating rods should be shifted to buccal side with lingual rotation of the apical end on maxillary anterior teeth and premolar area. Conclusion: Clinically determined rod direction and position of the indicating rod for implant placement was not always acceptable for insertion according to the reformatted CT images. The pre-operative treatment plan for implant should be determined carefully, considering the state of the alveolar bone using the reformatted CT images.

내측 연결형 임플란트 지대주의 체결부 길이 변화에 따른 비선형 유한요소법적 응력분석 (The non-linear FEM analysis of different connection lengths of internal connection abutment)

  • 목적: 본 연구는 임플란트 지대주의 체결부 길이와 골정에 대한 고정체 상단의 위치에 따라서, 하중 적용 시 고정체, 지대주, 나사 및 골에서의 응력 분포에 어떤 변화가 나타나는지 평가해 보고자 하였다. 재료 및 방법: 원추형 사면과 육각 기둥 형태의 회전저항구조를 갖는 내측 연결형 임플란트에서 지대주의 체결부 길이가 2.5 mm, 3.5 mm, 4.5 mm인 경우와 각 경우에서 고정체 상단이 골정과 같거나 골정보다 2 mm 상방에 위치할 때를 상정하여, 유한요소분석을 위한 모델을 총 6개 형성하였다. 각 경우에서 170 N의 30도 경사하중을 적용하였다. 결과: 6개의 유한요소분석 모델 모두에서, 임플란트는 고정체와 지대주의 체결 상단 부위에서, 주위 골에서는 골정 부근에서 최대 응력 값을 나타냈다. 지대주의 체결부 길이가 길어질수록, 주어진 하중에서 발생되는 고정체, 지대주, 나사 및 골에서의 최대 응력 값은 줄어들었다. 고정체 상단이 골정과 동일한 높이에 있는 경우보다 골정 2 mm 상방에 위치하는 경우에서 지대주의 체결부 길이가 길어질수록 모든 부위에서 더 큰 비율로 최대 응력 값의 감소를 나타내었다. 결론: 지대주의 체결부 길이가 길어질수록 모든 부위에서 주어진 하중에 따른 최대 응력 값이 줄어드는 것을 알 수 있었고, 임플란트의 기계적 및 생물학적 합병증 예방에 도움이 될 수 있을 것으로 판단된다.

A short-term clinical study of marginal bone level change around microthreaded and platform-switched implants

  • Purpose: The marginal bone levels around implants following restoration are used as a reference for evaluating implant success and survival. Two design concepts that can reduce crestal bone resorption are the microthread and platform-switching concepts. The aims of this study were to analyze the placement of microthreaded and platform-switched implants and their short-term survival rate, as well as the level of bone around the implants. Methods: The subjects of this study were 27 patients (79 implants) undergoing treatment with microthreaded and platform-switched implants between October 2008 and July 2009 in the Dental Hospital of Yonsei University Department of Periodon-tology. The patients received follow-up care more than 6 months after the final setting of the prosthesis, at which time periapical radiographs were taken. The marginal bone level was measured from the reference point to the lowest observed point of contact between the marginal bone and the fixture. Comparisons were made between radiographs taken at the time of fixture installation and those taken at the follow-up visit. Results: During the study period (average of 11.8 months after fixture installation and 7.4 months after the prosthesis delivery), the short-term survival rate of microthreaded and platform-switched implants was 100% and the marginal bone loss around implants was $0.16{\pm}0.08$ mm, the latter of which is lower than the previously reported values. Conclusions: This short-term clinical study has demonstrated the successful survival rates of a microthread and platform-switched implant system, and that this system is associated with reduced marginal bone loss.
