골유착성 임플랜트 보철치료시 자연지대치와의 연결형태에 관한 유한요소적 응력분석 연구 (A STUDY ON THE CONNECTION MODALITY BETWEEN IMPLANT AND TOOTH IN OSSEOINTEGRATED PROSTHETIC TREATMENT USING FINITE ELEMENT ANALYSIS)

  • Tho osseointegrated dental prosthetic treatment has develped for the edentulous patient with severely resorbed alveolar ridge, and has given us a successful clinical results to date. Nowadays the partially edentulism is included among the indications of the osseointegrated prosthetic treatment. The purpose of this study was to analyze the stress distribution at supporting bone according to the types of connection modality between implant and tooth in the superstructure. Two dimensional finite element stress analysis was applied for this study. FEM models were created using software Super SAP for MBM 16bit personal computer. Three modalities of connection were modeled and analyzed under load condition. The results were as follws: 1. The stress develped at tooth and implant in the cancellous bone was lower in the case of rigid connection than in the case of norigid connection, but higher between the two implants in the case of rigid connection than in the case of nonrigid connection. 2. The stress developed at the cortical bone and at the supporting bone interface was lower in the case of rigid connection than in the case of nonrigid connection 3. The stress developed at the supporting tissue interface of the implant nearby the tooth, was lower in the case of rigid connection than in the case of nonrigid connection. 4. The stress developed at the supporting tissue interface of posteriormost implant, was same between the cases of rigid and nonrigid connection. 5. The stress distribution related to the freestanding case was generally similar to the stress distribution pattern of nonrigid connection case. 6. The magnitude of applied load which produces deformation within elastic limit, had influence on the absolute value of stress, but had no influence on the pattern of stress distribution of the same case.

Blossom smart expander technology for tissue expander-based breast reconstruction facilitates shorter duration to full expansion: A pilot study

  • Background This study evaluated the Blossom system, an innovative self-filling, rate-controlled, pressure-responsive saline tissue expander (TE) system. We investigated the feasibility of utilizing this technology to facilitate implant-based and combined flap with implant-based breast reconstruction in comparison to conventional tissue expansion. Methods In this prospective, single-center, single-surgeon pilot study, participants underwent either implant-based breast reconstruction or a combination of autologous flap and implantbased breast reconstruction. Outcome measures included time to full expansion, complications, total expansion volume, and pain scores. Results Fourteen patients (TEs; n=22), were included in this study. The mean time to full expansion was 13.4 days (standard error of the mean [SEM], 1.3 days) in the combination group and 11.7 days (SEM, 1.4 days) in the implant group (P=0.78). The overall major complication rate was 4.5% (n=1). No statistically significant differences were found in the complication rate between the combination group and the implant group. The maximum patient-reported pain scores during the expansion process were low, but were significantly higher in the combination group (mean, 2.00±0.09) than in the implant group (mean, 0.29±0.25; P=0.005). Conclusions The reported average duration for conventional subcutaneous TE expansion is 79.4 days, but this pilot study using the Blossom system achieved an average expansion duration of less than 14 days in both groups. The Blossom system may accommodate single-stage breast reconstruction. The overall complication rate of this study was 4.5%, which is promising compared to the reported complication rates of two-stage breast reconstruction with TEs (20%-45%).

A comparative clinical study on oxidized titanium implants and sandblasted large-grit acid etched implants in soft bone

  • Purpose: The aim of this retrospective study was to compare the survival rate of oxidized titanium implants and sandblasted large-grit acid etched implants in soft bone. Methods: 201 oxidized titanium implants were inserted in 84 patients between May 1999 and May 2004. 120 sandblasted large-grit acid etched implants were inserted in 74 patients between December 2000 and May 2004. The patients were followed-up 0${\sim}$5 years in ITI group or 0${\sim}$6 years in BRA group, respectively. The following information was collected from the patient records: age, gender, systemic disease, implant type, number, length and diameter of the implants, their location in the jaws, bone quantity, the number of failed implants, the causes of failure, and advanced surgery for bone augmentation. Results: In the oxidized titanium implants, 8 implants showed early failure, and 1 implant showed late failure, respectively. The cumulative survival rate was 95.48%. In the sandblasted large-grit acid etched implants, 1 implant showed late failure and cumulative survival rate was 99.10%. The cumulative survival rate and the survival rates in the case of the advanced procedure during the implant placement were not significantly different in both groups. Conclusions: Oxidized titanium implants and sandblasted large-grit acid etched implants can be used successfully in soft bone regardless of the surgical methods used during the implant placement.   • 임플란트 피개의치는 전통적인 총의치에 비해 높은 유지력 및 저작능력, 환자의 만족도를 보인다. 특히 골흡수가 심한 하악 무치악 환자의 치료에서는 우선되는 치료방법으로 고려된다. 임플란트 피개의치를 위한 attachment에는 임플란트간 연결된 bar type과 연결되어있지 않은 solitary type으로 구분할 수 있다. 최근 임상에서 많이 사용하는 solitary type의 경우 임플란트간의 평행관계가 중요하며, 임플란트간의 평행관계가 불량한 경우 attachment의 마모 및 유지력 감소로 유지관리를 내원횟수가 증가할 수 있다. 본 증례는 평행하지 못한 두 개의 하악 임플란트 식립 환자에서 milled bar를 이용하여 제작한 임플란트 피개의치로 만족할 만한 결과를 얻었기에 이를 보고하고자 한다.

The prognosis of splinted restoration of the most-distal implants in the posterior region

  • PURPOSE. The aim of this study was to compare the efficacies of two-implant splinting (2-IS) and single-implant restoration (1-IR) in the first and second molar regions over a mean functional loading period (FLP) of 40 months, and to propose the appropriate clinical considerations for the splinting technique. MATERIALS AND METHODS. The following clinical factors were examined in the 1-IR and 2-IS groups based on the total hospital records of the patients: sex, mean age, implant location, FLP, bone grafting, clinical crown-implant ratio, crown height space, and horizontal distance. The mechanical complications [i.e., screw loosening (SL), screw fracture, crown fracture, and repeated SL] and biological complications [i.e., peri-implant mucositis (PM) and peri-implantitis (PI)] were also evaluated for each patient. In analysis of two groups, the chi-square test and Student's t-test were used to identify the relationship between clinical factors and complication rates. The optimal cutoff value for the FLP based on complications was evaluated using receiver operating characteristics analysis. RESULTS. In total, 234 patients with 408 implants that had been placed during 2005 - 2014 were investigated. The incident rates of SL (P<.001), PM (P=.002), and PI (P=.046) differed significantly between the 1-IR and 2-IS groups. The FLP was the only meaningful clinical factor for mechanical and biological complication rates in 2-IS. CONCLUSION. The mechanical complication rates were lower for 2-IS than for 1-IR, while the biological complication rates were higher for 2-IS. FLP of 39.80 and 46.57 months were the reference follow-up periods for preventing biological and mechanical complications, respectively.

무치악 환자에서 로케이터 부착장치를 이용한 임플란트 피개의치 수복 증례 (Implant overdenture treatment using Locator attachment system on edentulous patient)

  • 심하게 흡수된 치조제는 기능적인 하중을 견디기 어렵고 구강 조직의 유해 변화와 더불어 환자의 의치 적응을 어렵게 한다. 환자의 불편감을 해소하고 의치의 안정성과 유지력을 증가시키기 위해 임플란트를 이용한 피개의치가 치료 방법으로 선택될 수 있다. 로케이터 부착장치는 사용상의 편리성이 높으며 다양한 유지력을 갖는 남자부를 보유함으로써 임상에서 볼 형태 부착장치대용으로 많이 사용되고 있다. 본 증례는 77세 여자환자로서 낮은 수직고경과 심한 치조제 흡수를 보이는 상하악 무치악 상태였다. 이에 하악에 4개의 임플란트를 식립하고, 상악은 총의치, 하악은 로케이터 부착장치를 이용한 임플란트 피개의치로 수복하여 기능 및 심미적으로 만족할 만한 결과를 얻었기에 이를 보고하고자 한다.

Bone-added osteotome sinus floor elevation with simultaneous placement of non-submerged sand blasted with large grit and acid etched implants: a 5-year radiographic evaluation

  • Purpose: Implant survival rates using a bone-added osteotome sinus floor elevation (BAOSFE) procedure with simultaneous placement of a non-submerged sand blasted with large grit and acid etched (SLA) implant are well documented at sites where native bone height is less than 5 mm. This study evaluated the clinical results of non-submerged SLA Straumann implants placed at the time of the BAOSFE procedure at sites where native bone height was less than 4 mm. Changes in graft height after the BAOSFE procedure were also assessed using radiographs for 5 years after the implant procedure. Methods: The BAOSFE procedure was performed on 4 patients with atrophic posterior maxillas with simultaneous placement of 7 non-submerged SLA implants. At least 7 standardized radiographs were obtained from each patient as follows: before surgery, immediately after implant placement, 6 months after surgery, every year for the next 3 years, and after more than 5 years had passed. Clinical and radiographic examinations were performed at every visit. Radiographic changes in graft height were calculated with respect to the implant's known length and the original sinus height. Results : All implants were stable functionally, as well as clinically and radiographically, during the follow-up. Most of the radiographic reduction in the grafted bone height occurred in the first 2 years; reduction after 2 years was slight. Conclusions: The simultaneous placement of non-submerged SLA implants using the BAOSFE procedure is a feasible treatment option for patients with severe atrophic posterior maxillas. However, the grafted bone height is reduced during the healing period, and patients must be selected with care.

  • 임플란트를 보조적으로 이용함으로써 후방 연장 국소의치에 부가적으로 유지와 지지를 얻는 임플란트 보조 국소의치(implant-assisted RPD, IARPD)에 대한 관심이 증가하고 있다. 본 증례는 장기간 후방연장 국소의치의 사용으로 인하여 심하게 기울어진 교합평면을 가진 부분 무치악 환자에서 상악의 고정성 보철 수복과 하악의 지르코니아 교합면을 가지는 임플란트 보조 국소의치를 이용하여 심미적이며 장기적으로 안정적인 교합평면을 가지도록 전악 수복하였으며, 기능적으로 만족스러운 결과를 얻었기에 이를 보고하고자 한다.

Prognosis in case of nerve disturbance after mandibular implant surgery in relation to computed tomography findings and symptoms

  • Purpose: The aim of this study was to evaluate the computed tomography (CT) imaging findings and clinical symptoms of patients who complained of neurosensory disturbances after mandibular implant surgery, and to investigate the relationships of these parameters with the prognosis for recovery. Methods: CT scans were reviewed in 56 patients with nerve disturbance after mandibular implant surgery. Two oral radiologists classified the imaging findings into intrusion, contact, close, and separate groups according to the distance from the inferior border of the implant to the roof of the mandibular canal (MC). The symptoms of 56 patients were classified into 8 groups and the frequency of each group was investigated. Patients were categorized according to symptom improvement into no recovery and recovery groups, and the relationships of recovery with the CT classification and specific symptom groups were analyzed. Results: Thirty-eight of the 56 nerve disturbance cases showed improvement. The close and separate groups in the CT classification had a strong tendency for recovery (90.9% and 81.8%, respectively) (P<0.05). Although the lowest recovery rate was found in the intrusion group, it was non-negligible, at 50%. The 6 patients with a worm crawling feeling all improved, while the 8 cases with a tightening sensation showed the lowest recovery rate, at 12.5%, and the symptom of a tightening sensation occurred only in the intrusion and contact groups. Conclusions: The closer the implant fixture was to the MC on CT images, the less likely the patient was to recover. Regarding paresthesia symptoms, while a worm crawling feeling is thought to be a predictor of recovery, a tightening sensation appeared to be associated with a lower recovery rate.

Sinus floor elevation and simultaneous implant placement in fresh extraction sockets: a systematic review of clinical data

  • Combining different procedures to reduce the number of surgical sessions and patient discomfort in implant placement and sinus floor elevation has been recommended, and evidence supports good outcomes. The aim of this study was to review the results of clinical studies on sinus floor elevation through extraction sockets and simultaneous immediate posterior implant placement. An electronic search was carried out in PubMed, Scopus, and Web of Science to find English articles published in or before August 2020. A manual search was also performed. Titles, abstracts, and the full-text of the retrieved articles were studied. Thirteen studies met our eligibility criteria: 6 retrospective case series, 3 case reports, 2 prospective cohort case-series, 1 prospective case series, and 1 randomized controlled trial. Overall, 306 implants were placed; 2 studies reported implant survival rates of 91.7% and 98.57%. The others either did not report the survival rate or reported 100% survival. Sinus floor elevation through a fresh extraction socket and simultaneous immediate implant placement appears to be a predictable modality with a high success rate. However, proper case selection and the expertise of the clinician play fundamental roles in the success of such complex procedures.
