The impaction of the maxillary permanent incisor is a common clinical problem and is mostly found at the "labial to the alveolar process." Surgical exposure and orthodontic treatment with fixed orthodontic appliances can be considered if normal eruption of the labillay impacted tooth is not expected. Surgical exposure of the impacted tooth, that is usually under the attached gingiva or is surrounded by alveolar bone through gingivectomy and removal of alveolar bone, may give a rise to complications such as diminution in the width of the attached gingiva, inflammation of the gingiva, and the loss of marginal alveolar bone. Therefore, closed eruption technique, which includes surgical exposure and orthodontic treatment with fixed orthodontic appliances followed by repositioning of surgical flap, is preferred. However, apically repositioned flap of the impacted tooth, which is beneath the movable submucosal area or is above the alveolar crestal area, can prevent unwanted exposures and facilitate successful tooth eruption. In this report, we described esthetic results of three patients with unerupted maxillary permanent incisor who were performed with an apically positioned flap without the loss of attached gingiva.

Root resorption of the permanent maxillary incisors can occur due to ectopic eruption of the permanent canines. Severe root resorption threatens the long-term survival of the affected incisors. The aim of the present study was to identify risk factors for root resorption of the maxillary incisors associated with impacted maxillary canines. In the present study, we retrospectively analyzed the Cone-Beam Computed Tomography (CBCT) scans of 65 children and adolescents with ectopically erupting maxillary canines (total of 88 impacted canines). Root resorption of central incisors was significantly associated with the mesiodistal position and root development of the adjacent canine. Root resorption of lateral incisors was significantly associated with sex, age, and the buccolingual and vertical position of the adjacent canine. However, enlargement of the dental follicle was not significantly associated with root resorption of adjacent incisors. Although incisor resorption is difficult to diagnose and predict, our findings suggest that changes in the dental follicles of the erupting maxillary canines do not cause resorption of the adjacent permanent incisors. CBCT should be utilized to ensure early diagnosis of impacted canines and precise evaluation of incisor root resorption.

Autotransplantation is a procedure which transplants teeth from the original position to other positions in the same individual. It is classified surgical reposition by intraalveolar autotransplantation and transalveolar autotransplantation. The prognosis for successful autotransplantation is dependent on a number of factors such as root development, surgical technique, patient's age, endodontic treatment, time and type of splinting, preservation of periodontal ligament and storage medium. The most important factor is preservation of periodontal ligament. The cause of the failure of transplantation include damage of the transplant during removal from deep palatal malposition, poor regeneration of the bone around the transplant and chronic periodontal infection. In case I, Impacted maxillary canine for which surgical exposure and orthodontic treatment was impossisle was transplanted. After 2 weeks, It showed periapical radiolucency and external root resorption. So, endodontic treatment was done. One year later, permanent filling was done with gutta percha. In case II, Transpositioned maxillary central incisor was transplanted after extraction of impacted mesiodens. Pulp vitality was maintained during 5 months without other clinical symptons.

Transposition has been described as an interchange of position of permanent tooth and is a relatively rare dental anomaly. Transposition of teeth may occur both in the maxillary and mandibular arches. but it appears more often in the maxilla of individual teeth, the maxillary canine is the most often involved. A canine transposes most often with a first premolar and less frequently transposes with a lateral incisor. Incomplete transposition is a condition describing an interchange in the position of the crowns of two permanent teeth, while the root apices remain in their relative position. Complete transposition is a situation in which both the crowns and entire root structure are transposed. The etiologic factors of transposition are tooth buds interchange, retained deciduous canines, migration of the erupting canine, trauma to deciduous teeth etc. This report describes a case of a transposition between a maxillary left canine and a lateral incisor and impaction of a maxillary left central incisor due to trauma to deciduous dentition.

Normal eruption of the canine is important for the transition to the permanent dentition. Etiologies, including premature loss or delayed retention of deciduous teeth, neoplasm and abnormality of lateral incisor can cause impaction of the maxillary canine. Untreated canine impaction can result in malocclusion, cyst formation and obstacles in orthodontic treatment. The aim of this study is to evaluate location of the impacted maxillary canine and to identify correlation between location and management of the impaction including complications. Using panoramic radiographs and CBCT scan, images of 89 children diagnosed with impaction of the maxillary canine, location of impacted canines was evaluated. The choice of treatment and complications were investigated to identify correlation. Results show that the most commonly impacted location of the maxillary canine was in the mid-alveolar area, followed by buccal side and palatal side. Orthodontic traction was selected more frequently than the other treatments. As complications, displacement of adjacent tooth was occurred most frequently. Buccally impacted canines showed increased tendency towards displacement. The more buccally the canine was impacted, the less orthodontic traction was chosen as the treatment. The canine impacted mesially to the central incisor showed increased tendency to occur root resorption. Therefore, early diagnosis by periodic examination, appropriate treatment and intervention is required.
A long-term follow-up study of a case of impacted teeth associated with a large dentigerous cyst in the left maxilla is presented. The patient was a 30-year-old man who had a large dentigerous cyst in the premaxilla and left posterior maxilla, which impacted the canine and supernumerary tooth. This is one of the most prevalent types of odontogenic cysts associated with an erupted or developing tooth, particularly the mandibular third molars. The other teeth commonly affected in order of frequency are the maxillary canines, maxillary third molars and rarely, the central incisor. Radiographically, the cyst appears as huge ovoid well-demarcated unilocular radiolucency with a sclerotic border and causes ectopic displacement of the inferior border of the maxillary sinus without destruction. Dentigerous cysts may grow unnoticed to such extensive sizes as to occupy a considerable portion of the maxillary sinus. These cysts appear to be associated with a supernumerary tooth in the maxillary anterior incisors region called the mesiodens and impacted canine. The present case report describes the surgical enucleation of a dentigerous cyst involving the permanent maxillary left canine and mesioden. After surgery, left maxillary sinus recovered their normal size and apposition of bone was observed around the apex of the posterior teeth. During the subsequent years, there was no recurrence of the cystic lesion but the inflammation was evoked in the anterior maxilla after 42 months. This complication appeared to have correlated with bony healing in the enucleation site of the cyst. We report the healing status of a huge dentigerous cyst in the maxilla for 5 years with a review of the relevant literature.
Objective: The aim of this study was to determine the relationship between the status of a unilateral palatally impacted maxillary canine (as seen on the panoramic radiograph) and the orthodontic treatment duration. Methods: A total of 36 subjects were chosen (8 males and 28 females, $13.7{\pm}2.5$ years). All patients had undergone orthodontic traction of the impacted canine after a closed flap surgery. The position of impacted canine on the panoramic radiograph was traced and calculated with regard to the treatment duration. Results: The canine overlap over the lateral incisor (COGr) had significant statistical difference between the short-term and the long-term treatment duration groups (divided by average treatment time of 21 months) (p<0.05). Multiple regression indicated that the variables of significance (with treatment duration as the dependent variable) were the canine vertical height from the occlusal plane $(HCV,\;r^2=0.115;\;p<0.05)$ and the mandibular anterior width $(MnDW,\;r^2=0.142;\;p<0.05)$. The treatment duration, the canine angle to the incisor midline (CA), and HCV decreased from primary dentition (under 12 yrs) to permanent dentition (15 yrs). Howerer, these increased again at the adolescent ages (over 16 yrs). Conclusion: These findings suggested that orthodontic treatment of a palatally impacted canine would show good prognosis at an age of early permanent dentition with the canine showing smaller CA and HCV.

Dilaceration is a developmental disturbance in shape of teeth. It refers to an angulation, or a sharp bend or curve, in the root or crown of a formed tooth. Dilacerations are estimated to occur in 3% of all permanent dentitions. However, the etiology of dilaceration is not yet fully understood. Traumatic injury to the deciduous predecessors and ectopic development of the tooth germ are the two commonly cited causes of this anomaly. The involved tooth is usually the maxillary central incisor and it also shows high prevalence of impaction. At first, We tried to do surgical exposure with orthodontic traction but to no avail. After then surgical repositioning of the dilacerated tooth was performed. Finally, Satisfactory results were obtained at the end of treatment, with improvement in dental esthetics and functional aspects.

Odontomas are the common type of odontogenic tumors and generally are asymptomatic and frequently lead to impaction or delayed eruption of permanent teeth. They are composed of enamel, dentin, cementum and pulp tissue and are divided into compound and complex according to the morphology of the hard tissues. Compound odontomas consist of varying numbers of small toothlike structure and have a predilection for the maxillary anterior regions. Complex odontomas consist of an unorganized mass of odontogenic tissues and comprise approximately 25 percent of all odontomas and have a predilection for the mandibular posterior regions. The etiology of odontomas is uncertain but hypothesized to involve local trauma, infection and genetic factors. Treatment of odontoma is conservative surgical removal and are little probability of recurrence. These two cases were about the patients with delayed eruption of mandibular first molar and mandibular lateral incisor. We surgically removed odontoma, exposed impacted tooth and guided impacted tooth into normal position by orthodontic traction. At the completion of traction, the mandibular first molar and mandibular lateral incisor was positioned fairly within the arch and complications such as root resorption were not observed.

Supernumerary teeth are teeth which result from the continued budding of the enamel organ of the preceding tooth or from excessive proliferation of cells. They are most often found in the maxillary anterior region. And they can be responsible for a variey of irregularities in the primary and transitional dentition. There are two morphological types of supernumerary teeth, supplemental and rudimentary. Supplemental teeth have normal shape and size. In contrast, rudimental teeth have abnormal shape and smaller size. Supplemental supernumerary teeth are most common in permanent lateral incisor area. Its extraction must be decided more carefully with differential diagnosis between normal teeth, because it has normal shape and size. We reports 3 cases of the normal incisor shaped teeth in the maxillary anterior region. In all cases, we used the 3D Dental-CT as well as the conventional plain film such as periapical, occlusal, and panoramic radiograph. Consequently, 3B Dental-CT was valualble to figure out the exact position and morphology of supernumerary teeth, to do more conservative surgery and to reduce surgery stress and time.
