A Case of Henoch-Schonlein Purpura Nephritis Complicating Encephalopathy Accompanied by Hypertension and Cerebral Vasculitis (고혈압과 뇌혈관염이 동반된 상태에서 뇌증을 보인 Henoch-Schonlein Purpura Nephritis 1례)

  • Henoch-Shonlein purpura(HSP) is a systemic small-vessel vasculitis that primarily affects the skin, gastrointestinal tract, joints, and kidneys. The nervous system may be involved, less commonly than other organs. When the central nervous system(CNS) was involved, headache, changes in mental status, seizures, and focal neurologic deficits have been reported. Hypertension, uremic encephalopathy, metabolic abnomalities, electrolyte abnormalities, or cerebral vasculitis were suggested as possible causes of the neurologic manifestation. Diagnosis of vasculitic involvement of CNS is difficult. Magnetic resonance imaging of the brain is the modality of choice for the evaluation of the CNS disease. Steroid or plasmapheresis are used in treatment of cerebral vasculitis. We experienced a case of 9-year-old boy who had presented with Henoch-Schonlein purpura nephritis complicating encephalopathy accompanied by hypertension and cerebral vasculitis. Brain MRI showed multiple small nodular-linear pattern enhancing lesions in whole cerebral hemispheres and focal increased T2 signal in the right basal ganglia. We used intravenous immunoglobulin in treatment of cerebral vasculitis. We report this case with a brief review of related literature.

Immune thrombocytopenia associated with sarcoidosis (면역성혈소판감소증을 동반한 사르코이드증)

  • Sarcoidosis is a systemic disease of unknown cause involving multiple organs and is characterized by noncaseating granuloma. Immune thrombocytopenia (ITP) is an autoimmune disease characterized by increased peripheral platelet destruction due to the presence of an antibody to the platelet and abnormal platelet production. There is no known pathogenesis that occurs concurrently with ITP and sarcoidosis. However, considering together of 2 known pathogenesis, abnormal immune response triggers either ITP or sarcoidosis. The disease that develops first stimulates secondary disease. After development of secondary disease, they stimulate each other. A few cases of ITP associated with sarcoidosis are well documented in English; however, the disease has rarely been reported in Korea. Here, we report on a case of ITP with sarcoidosis in a 29-year-old man. He suffered from easy bruising. The chest X-ray and the contrast-enhanced computed tomography scan showed bihilar lymphadenopathy and reticulonodular infiltrates. Bone marrow study and fluoroscopy-guided percutaneous needle biopsy were performed and the patient was diagnosed with sarcoidosis and ITP. He was put on 400 mg/kg of intravenous immunoglobulin for 5 days and administered oral steroids and further follow-up will be carried out. He has shown a good response without significant bleeding event. However, administration of more oral steroid and additional follow-up is required than for single disease, whether sarcoidosis or ITP.

Outbreaks of mumps: an observational study over two decades in a single hospital in Korea

  • Purpose: The introduction of the mumps vaccine has dramatically reduced the number of mumps cases, but outbreaks have recently occurred among highly vaccinated populations in developed countries. Epidemiological and clinical characteristics of patients with mumps admitted between 1989 and 2012 in a single hospital in Korea are described in the present study. Methods: We retrospectively evaluated inpatients with mumps between 1989 and 2012 and outpatients and inpatients with mumps in 2011-2012. Results: A total of 152 patients with mumps were admitted between 1989 and 2012, and 163 patients were recorded in 2011-2012. The highest number of admitted cases occurred in 1998 and 2012 (35 and 34 cases, respectively). Among the patients admitted in 2011-2012, the highest frequency was observed among people aged 15-19 years, and low frequency was observed in those aged <4 years and >20 years, compatible to the city data and national data. In patients admitted to our department in 1998 (35 cases) and in 2010-2012 (27 cases), there were significant differences in the mean age and the rate of secondary measles-mumps-rubella (MMR) vaccination, but had similar clinical features, including complications, except aseptic meningitis. Antimumps immunoglobulin (Ig) G was positive in 83% and 100%, and IgM was positive in 67% and 41%, respectively, in the two periods. Conclusion: In Korea, recent mumps outbreaks have occurred mainly among secondary school students who received two doses of the MMR vaccine. The vaccinees might have a modified immune reaction to viral insults, manifesting modified epidemiological and clinical features.

Prediction of unresponsiveness to second intravenous immunoglobulin treatment in patients with Kawasaki disease refractory to initial treatment

  • Purpose: This study investigated predictors of unresponsiveness to second-line intravenous immunoglobulin (IVIG) treatment for Kawasaki disease (KD). Methods: This was a single-center analysis of the medical records of 588 patients with KD who had been admitted to Asan Medical Center between 2006 and 2014. Related clinical and laboratory data were analyzed by univariate and multivariate logistic regression analyses. Results: Eighty (13.6%) of the 588 patients with KD were unresponsive to the initial IVIG treatment and received a second dose. For these 80 patients, univariate analysis of the laboratory results obtained before administering the second-line IVIG treatment showed that white blood cell count, neutrophil percent, hemoglobin level, platelet count, serum protein level, albumin level, potassium level, and C-reactive protein level were significant predictors. The addition of methyl prednisolone to the second-line regimen was not associated with treatment response (odds ratio [OR], 0.871; 95% confidence interval [CI], 0.216-3.512; P=0.846). Multivariate analysis revealed serum protein level to be the only predictor of unresponsiveness to the second-line treatment (OR, 0.160; 95% CI, 0.028-0.911; P=0.039). Receiver operating characteristic curve analysis to determine predictors of unresponsiveness to the second dose of IVIG showed a sensitivity of 100% and specificity of 72% at a serum protein cutoff level of <7.15 g/dL. Conclusion: The serum protein level of the patient prior to the second dose of IVIG is a significant predictor of unresponsiveness. The addition of methyl prednisolone to the second-line regimen produces no treatment benefit.

Curcumin Inhibits the Activation of Immunoglobulin E-Mediated Mast Cells and Passive Systemic Anaphylaxis in Mice by Reducing Serum Eicosanoid and Histamine Levels

  • Curcumin is naturally occurring polyphenolic compound found in turmeric and has many pharmacological activities. The present study was undertaken to evaluate anti-allergic inflammatory activity of curcumin, and to investigate its inhibitory mechanisms in immunoglobulin E (IgE)/Ag-induced mouse bone marrow-derived mast cells (BMMCs) and in a mouse model of IgE/Ag-mediated passive systemic anaphylaxis (PSA). Curcumin inhibited cyclooxygenase-2 (COX-2) dependent prostaglandin $D_2$ ($PGD_2$) and 5-lipoxygenase (5-LO) dependent leukotriene $C_4$ ($LTC_4$) generation dose-dependently in BMMCs. To probe the mechanism involved, we assessed the effects of curcumin on the phosphorylation of Syk and its downstream signal molecules. Curcumin inhibited intracellular $Ca^{2+}$ influx via phospholipase $C{\gamma}1$ ($PLC{\gamma}1$) activation and the phosphorylation of mitogen-activated protein kinases (MAPKs) and the nuclear factor-${\kappa}B$ (NF-${\kappa}B$) pathway. Furthermore, the oral administration of curcumin significantly attenuated IgE/Ag-induced PSA, as determined by serum $LTC_4$, $PGD_2$, and histamine levels. Taken together, this study shows that curcumin offers a basis for drug development for the treatment of allergic inflammatory diseases.

Prognostic factors and efficacy of human intravenous immunoglobulin G in dogs with idiopathic immune-mediated hemolytic anemia: a retrospective study

  • This study was conducted to determine the effect of treatment with intravenous human immunoglobulin G (hIVIgG) on outcome in dogs with idiopathic immune-mediated hemolytic anemia (IMHA), and to identify prognostic variables that determine outcome in affected dogs. Thirty-seven dogs that met the inclusion criteria were enrolled in a retrospective study. The dogs were categorized into two groups based on their having received hIVIgG. There was no significant difference in survival between the hIVIgG group and the non-hIVIgG group. Mortality during hospitalization and at 1 month, 1 year, or 2 years after discharge was not significantly different between the hIVIgG and the non-hIVIgG groups. Hemoglobinuria was significantly less prevalent in dogs that lived more than 1 year than in those who lived less than 1 year, and was less prevalent in dogs that lived more than 2 years than in those who lived less than 2 years. However, there was no difference in the presence of hemoglobinuria between dogs that lived less than 1 month and those that lived more than 1 month. Overall, there was no evidence of a beneficial effect of hIVIgG in dogs with idiopathic IMHA.

A Comparative Study of Serologic Methods for Detection of Mumps Antibody in Korean Children (한국 소아의 Mumps 바이러스 항체보유에 관한 혈청학적 진단방법의 비교)

  • Mumps is an extremly common infectious disease affecting predominantly young children hut it is not a severe disease in terms of mortality. One hundred and two sera from infants of 3 different groups which are vaccinated, unvaccinated and unknown were detected to mumps antibody. The tests used were Complement Fixation(CF) test, Single Radial Hemolysis(SRH) test, Hemagglutination Inhibition(HI) test, Enzyme Linked Immunosorbent Immunoglobulin G(ELISA IgG) test, Enzyme Linked Immunosorbent Immunoglobulin M(ELISA IgM) test. 1. The rate of positivity for mumps antibody in 102 sera wera 89.16%(74/83) by Hl test, 68.83%(53/77) by ELISA IgG test, 64.58%(62/96) by SRH test, 63.24%(43/68) by ELISA IgM test and 50.00%(49/98) by CF test. 2. The rate of positivity by 5 tests for 55 sera turned out to be very similar with above results respectively. 3. The correlation coefficients(r) between ELISA IgG test ant H1 test, ELISA IgG test and ELISA IgM test were 0.34(P<0.0l) and 0.31(P<0.02), respectively. 4. The percentage of apparently natural infection of mumps seemed to be 65.15%(43/66) in infants. 5. Seroconversion rate of mumps by vaccination were 90.91%(10/11). 6. Among the 53 infants who were tested with ELISA IgG 15 were below 15 months age of(28.30%) and this percentage may be taken as a suggestion that mumps vaccination should be given earlier than present practice. 7. ELISA IgG test was found very sensitive and recommendable method for large scale screening for the presence of antibody to mumps.

Anti-allergic effect of Lactobacillus rhamnosus IDCC 3201 isolated from breast milk-fed Korean infant (한국인 모유영양아의 분변에서 분리한 Lactobacillus rhamnosus IDCC 3201의 항 알레르기 효과)

  • We investigated 23 lactic acid bacteria isolated from Korean breast milk-fed infant in order to select strains which show superior anti-allergic effect. The candidates were cultivated and then we obtained dried powders of tyndallized cells and supernatant concentrate separately. Screening was carried out with down-regulation of interleukin (IL) 4 and up-regulation of IFN-${\gamma}$ in mouse splenocytes. As a result of the screening, we selected Lactobacillus rhamnosus IDCC 3201 (RH3201) for oral feeding to ovalbumin-sensitized BALB/c mice. Oral administration of RH3201 as dead cell bodies and supernatant concentrate suppressed hyper-production of serum immunoglobulin (Ig) E levels compared to vehicle group. Such anti-allergic effects were achieved by improvement of the balance between cytokines produced from type-1 helper T (Th1) and type-2 helper T (Th2) lymphocytes. Therefore, RH3201 has potential to improve atopic symptoms by immunomodulatory effect.

Effects of Lycii fructus and Astragalus membranaceus Mixed Extracts on Immunomodulators and Prevention of Diabetic Cataract and Retinopathy in Streptozotocin-induced Diabetes Rat Model (Streptozotocin으로 유발한 당뇨병성 쥐에서의 당뇨병성 백내장과 망막병증에 대한 구기자와 황기 혼합 추출물 등의 면역 조절 및 예방 효과)

  • This study was conducted to investigate the effects of Lycii fructus and Astragalus membranaceus mixed extracts on immunomodulators and prevention in a streptozotocin-induced diabetes rat model. A total of 28 male rats were divided into four dietary groups and fed a commercial diet (A), commercial diet plus induced diabetes by a streptozotocin (STZ) injection (B), induced diabetes by STZ plus medicinal crop extracts(I&$H^{(R)}$) diet (C), and medicinal crop extracts (I&$H^{(R)}$) diet (D). Immunoblotting analyses revealed cytokine expression, and ELISA analyses revealed immunoglobulin E and nitric oxide production. As a results, the tumor necrosis factor-$\alpha$ (TNF-$\alpha$) and inducible nitric oxide synthase (iNOS) as a inflammatory cytokine were decreased. Interleukin-6 (IL-6) and signal transducer and activation of transcription 3 (STAT3) cytokine related in diabetes expression through JAK/STAT3 pathway were also decreased. Furthermore, immunoglobulin E and nitric oxide production were decreased in the serum and lens, respectively. These results suggest that Lycii fructus and Astragalus membranaceus mixed extracts provide positive effects on immunomodulators and prevention in diabetes and eye disease complications.

Inhibitory Effect of Immunoglobulin E Production by Poncirus tripoliata (지실(枳實)에 의한 면역(免疫)글로블린 E 생성(生成)의 억제효과(抑制效果))

  • Poncirus trifoliata (L.) Raf (Rutaceae) fruits (PTFE) has been used for the treatment of allergic disease. IgE is normally one of the least abundant immunoglobulin (Ig) isotypes in the serum of both humans and several species of experimental animals: however a number of different stimuli can result in profound increases in IgE levels relative to other isotypes. In rodents, infection with many parasitic helminths can cause approximately 100-fold elevation in IgE within 2 wks. Immunization of mice with small amounts of protein antigens on alum also results in 10-fold to fold increase in total serum IgE, much of it specific for the immunizing antigen. In this experiment, I investigated the effect of an aqueous extract of Poncirus trifoliata (L.) Raf (Rutaceae) fruits (PTFE) on a in vivo and in vitro IgE production. PTFE dose-dependently inhibited the serum levels of IgE induced by antigens. The regulation of IgE synthesis is influenced by T cells and T cell derived factors. IL -4, a T cell-derived cytokine, has been shown to stimulate murine IgE synthesis both in vitro and in vivo. Current evidence suggests that IL-4 induces IgE synthesis in the mouse by stimulating H chain isotype switch. Lipopolysaccharide (LPS) plus IL-4 cause about l00-fold increase in IgE secretion by murine B cells. The effects of PTFE on the IL-4-dependent IgE response by mouse whole spleen cells were studied. Whole spleen cells were cultured for 7 days in the presence of LPS plus IL-4 and PTFE and the supernatants were assayed for IgE. IL-4 dependent IgE production of LPS-stimulated whole spleen cells was inhibited by PTFE. Moreover, in the present study using U266Bl human IgE-bearing B cells, I found that PTFE inhibited the production of IgE activated by LPS plus IL-4. These results indicate that PTFE have antiallergic activity by inhibition of IgE production from B cells.
