Leading Study for the Restoration of Pediatric Immunization Systems in North Korea following Reunification of Korean Peninsula (통일 후 북한지역 영·유아 대상 예방접종체계 수복을 위한 선행연구)

  • Background: 8,000 children in North Korea died before they reached the 1 year after births in 2013. The high mortality rate of children under five years of age is mainly caused by infectious diseases and malnutrition. The need for national pediatric immunization and supply of vaccines will be vital when the abrupt reunification occurs. Objective: The purpose of this study is to scrutinize the pediatric immunization coverage of North Korea. Additionally it is to estimate the amount and the costs needed to vaccinate. Methods: The target population is the children of North Korea. The method is based on a pre-survey and an interview of North Korea defectors. The target interviewees searched for are as follows; doctors, teachers, and others. The interview includes questions on mortality rates and immunization coverage. The analysis is largely based on the statements of the health care providers within the selected group. Results: The interviewees are 8; 7 female and 1 male. The birth years range from 1956 to 1982. 3 out of 8 are former health care providers; a doctor, a pharmacist, and a nurse. The morbidity rate of infectious diseases exceeds the data from WHO. The immunization coverage is nearly 0% after 1980s. In order to ensure the welfare of North Korean children, at least 8,234,000 vaccine doses, requiring over 105 million U.S. dollars, are needed. Conclusion: The morbidity rate of infectious disease in North Korea is conspicuous. The preparation for supply and expenditure of vaccines is vital.

Policy Content Analysis of the Expanded National Immunization Program in the Republic of Korea (국가필수예방접종 보장범위 확대 시범사업의 정책 내용분석 - 국가필수예방접종 공급방식을 중심으로 -)

  • The purpose of this study was to evaluate comparatively the content of the Expanded National Immunization Program according to the provision method between 2005 and 2006 in Korea. We assessed the impact of the mutually exclusive vaccination policy using the result reports of the 2005 and 2006 Demonstration Project and the related references by the content analysis. The public health centers paid vaccination fees to the private clinic and hospital in the 2005 Demonstration Project in Daegu metropolitan city and Gunpo city. But, the public health centers directly supplied free vaccination services to the children in the 2006 Demonstration Project in Gangneung city, Yangsan city, and Yeongi-gun. The total budgets of 2005 and 2006 Demonstration Project were 6.57 billion won and 0.65 billion won, respectively. The computerized registration rates and timeliness rates of administration of each vaccination had improved all in the 5 Demonstration Project regions. However, the computerized registration rates of most vaccination in Gunpo city were higher than those in the 2006 Demonstration Project regions except hepatitis B. Especially, the computerized registration rate of BCG was 48.3%, but the BCG coverage rate by the follow-up telephone survey was 99.8% in Daegu metropolitan city. The community parents in all the regions were satisfied because of expanding financial and geographical access to immunization coverage. In conclusions, from the aspect of the main outcomes, the implementation of two different financial immunization aids appears to be widely accepted among these parents and to have had an impact on vaccination coverage. In the future, the government must try to enact that the national immunization policy including under-immunised or incompletely immunised groups would be achieved by the affordable method of the public-private dynamics.

Up-to-date or Complete Immunization Coverage and Their Related Factors (영유아의 예방접종 및 그 관련요인)

  • Objectives: Understanding the predictor of immunization status in childhood is critical issue to improve National Immunization Program (NIP). The aims of this study were to verify the status of up-to-date or complete immunization coverage and to investigate its related factors. Methods: As of 2005, according to local residence registry data, there were 2,188 children who aged 12 to 35 months in Nonsan city, Korea. We conducted household survey for aged 12 to 35 months children, using questionnaires to obtain data on the status of immunization such as BCG, DTaP (diphtheria, tetanus, and pertussis), Polio, and MMR (mump, measles, rubella). Finally 1,472 participated in the survey. The operating definitions used in this study were following; "Complete immunization rate" refers to the rate of children who received all immunization within recommended age intervals fully "on-time"; "The 4:3:1 series" means status of receiving the fourth diphtheria-tetanus-pertussis (4 DTP), the third Polio (3 Polio), and the first measles-mumps-rubella (1 MMR) doses. Multivariate logistic regression analyses were used to determine factors affecting complete vaccination coverage of children. Results: Immunization rates of vaccine based on the vaccination card were from 92.7% to 96.4% except 4th DTaP (79.3%). Complete immunization rate of Korea NIP was 74.0% and that of the 4:3:1 series was 77.1%. A parent as primary caregiver (OR 0.59, 95% CI 0.39-0.87 at 19-35 months of children's age) and first-born children (OR 1.79, 95% CI 1.05-3.03 at 24-35 months of children's age) were significantly related to complete immunization coverage of Korea NIP. And a parent as primary care giver (OR 0.58, 95% CI 0.38-0.88 at 19-35 months of children's age) and first-born children (OR 1.94, 95% CI 1.21-3.14 at 19-35 months, OR 2.23, 95% CI 1.27-3.91 at 24-35 months of children's age) were significantly related to complete immunization rate of 4:3:1 series. Conclusions: Government should take actions to increase complete immunization rate. In particular, intervention on the secondary caregiver and non-first-born children should be needed.

Prevalence and Determinants of Influenza Vaccination in Koreans (국내 독감백신 투여현황과 투여 결정에 영향을 주는 요인에 관한 연구)

  • This study was aimed to determine influenza vaccination coverage in 2004 in Koreans and investigate the factors associated with vaccination. Documentation of vaccination status and baseline data was conducted by a survey using questionnaire sheets. Baseline data and vaccination status were documented on 1465 people out of whom 60.1% received the influenza vaccine. Forty-seven percent of the responders were male; 4% were aged 1-12, 32% were aged 13-39, 32% were aged 40-64 and 32% were aged 65 or older. Twenty-three percent reported a chronic illness, increasing their risk for complications from influenza. Predictors of influenza vaccination were: older age (OR=11.7, 95% CI 5.1-26.8), the presence of chronic illness (OR=2.3, 95% CI 1.1-4.7), previous vaccination (OR=1.8, 95% CI 1.1-2.8), belief that influenza vaccine is effective in preventing influenza (OR=2.5, 95% CI 1.1-5.7) and education level (OR=1.7, 95% CI 1.0-2.7). Immunization rates were much higher in those who will take immunization again (OR=10.4, 95% CI 5.5-19.6). Factors affecting the decision on immunization were self-determination (43.6%), public relations (24.1%), recommendation from family members or friends (22.4%) and consulting with health professionals (5.8%). The main reason not to take influenza vaccine was the thought that they are healthy (50.1%). Overall, influenza vaccine coverage was high in those aged 65 or older. Immunization against influenza was influenced more by existing medical problem and belief about the vaccine's effectiveness, rather than sex or residence.

Immunization Statuses of Children in Childcare Centers and Sociodemographic Factors Associated with Delayed DTaP4 Immunization

  • Purpose: Immunization is one of the simplest and most effective approaches for protecting the health of children from preventable infectious disease. This study assessed the immunization statuses of children presenting to childcare centers in Korea and compared the sociodemographic factors of children with age-appropriate immunization for DTaP4 with those with delayed immunization for DTaP4. Method: A national mail survey was used to collect data on the children's immunization history and sociodemographic information. A total of 248 mothers returned completed questionnaires for their children. Results: The overall immunization coverage for HepB, a series of DTaP and polio and varicella was above 95%. However, the percentage of age-appropriate immunizations of vaccines ranged from 45.3% to 69.0% depending on the vaccines. Only the age of the child was a significant factor for delayed immunization for DTaP4. Conclusion: To fully protect children from vaccine-preventable disease, childcare providers can playa role in monitoring the immunization statuses of children in their care centers to ensure that they all receive all childhood immunizations on time.

  • National Immunization Program (NIP) in korea is regulated by government for effective prevention of infectious diseases since 1954. But the program is only performed in public health center, so many people had various complaints such as inconvenience to use public center, high cost when vaccinated in private medical organization and lack of speciality and registration etc. In this reason, it was very difficult to raise the vaccination rate to 95 percent, infectious disease preventable rate. With this background national assembly and government try to expand the NIP coverage both public health center and private medical organization with increasing budget from 2009. So in this paper an overview and preparation of Korean pediatrician according to the expanding plan of NIP are described.

  • Promotion of immunization including early recognition of disease symptoms and effective control of communicable disease have been the important role of the school heath teacher in Korea. The school heath teacher as a nurse have been practiced vaccination for students as one of the major method for promotion of immunization in school. Recently it was occured difficulty for school vaccination that is practicing by school health teacher. So it is predicted for reducing of vaccination coverage rates of students. The objectives of this study are, first; to examine current guidlines of government, second, to analysis problem according to current guidline, third, to discuss further direction for school vaccination. There are used study methods as litrature review, analysis of statistical data and case study etc. Analytical framework for increased effects of vaccination in this study was safty, cost -effectiveness and coverage rates of vaccination. Major suggestions are as follows : First, It is different from the role of the public and private sectors in preventive health services. The preventive health services are characterized that the effect of health promotion is large, but the period of input effect is slow. Therefore the leading role of school vaccination should be have school and school health teacher including public sectors. Second, Health management of contemporary people can be effective with cooperative relationship between various health manpower. School health teacher as a medical manpower should be provide vaccination service continiously for health promotion of students. Third, It is necessary to maintain collective vaccination in school because have lots of advantage at safty, cost-effectiveness and coverage rates. In conclusion, school heath teacher should have central and independent role in school vaccination in according to current law. Also it will be assured lawful compensation in prepare to accident in vaccination.

Factors Influencing Vaccination in Korea: Findings From Focus Group Interviews

  • Objectives: Immunization is considered one of the most successful and cost-effective public health interventions protecting communities from preventable infectious diseases. The Korean government set up a dedicated workforce for national immunization in 2003, and since then has made strides in improving vaccination coverage across the nation. However, some groups remain relatively vulnerable and require intervention, and it is necessary to address unmet needs to prevent outbreaks of communicable diseases. This study was conducted to characterize persistent challenges to vaccination. Methods: The study adopted a qualitative method in accordance with the Consolidated Criteria for Reporting Qualitative Research checklist. Three focus group interviews were conducted with 15 professionals in charge of vaccination-related duties. The interviews were conducted according to a semi-structured guideline, and thematic analysis was carried out. Data saturation was confirmed when the researchers agreed that no more new codes could be found. Results: A total of 4 main topics and 11 subtopics were introduced regarding barriers to vaccination. The main topics were vaccine hesitancy, personal circumstances, lack of information, and misclassification. Among them, vaccine hesitancy was confirmed to be the most significant factor impeding vaccination. It was also found that the factors hindering vaccination had changed over time and disproportionately affected certain groups. Conclusions: The study identified ongoing unmet needs and barriers to vaccination despite the accomplishments of the National Immunization Program. The results have implications for establishing tailored interventions that target context- and group-specific barriers to improve timely and complete vaccination coverage.

  • In this study, the accuracy duplication and register rate of the vaccination registration data in National Immunization Registry Information System were evaluated and analyzed. Through which undocumented vaccination status data, duplicate data, missing data, errors data into the vaccination registration data were analyzed. In addition, the quality control for the vaccination registration database quality improvement, were proposed for standard error checking. In this paper, we propose an efficient validation of a quality management system of the database.

  • This paper examines the failure to promote adequate preventive health care in the U.S. It focuses specifically on the preventive health services of screening, counseling, and immunization. It explores evidence on their effectiveness, as well as coverage under current private and public health insurance plans. It concludes with a proposal to expand health insurance coverage for preventive services and to reimburse physicians directly for preventive health services provided to patients.
