  • Purpose: Identification of the infrapyloric artery (IPA) type is a key component of pylorus-preserving gastrectomy. As the indocyanine green (ICG) fluorescence technique is known to help visualize blood vessels and flow during reconstruction, we speculated that this emerging technique would be helpful in identifying the IPA type. Materials and Methods: From August 2015 to February 2016, 20 patients who underwent robotic or laparoscopic gastrectomy were prospectively enrolled. After intravenous injection of approximately 3 mL of ICG (2.5 mg/mL), a near-infrared fluorescence apparatus was applied. The identified shape of the IPA was confirmed by examining the actual anatomy following infrapyloric dissection. Results: The mean interval time between ICG injection and visualization of the artery was 22.2 seconds (range, 14-30 seconds), and the mean duration of the arterial phase was 16.1 seconds (range, 9-30 seconds). The overall positive predictive value (PPV) of ICG fluorescence in identifying the IPA type was 80% (16/20). The IPA type was incorrectly predicted in four patients, all of whom were obese with a body mass index (BMI) of more than $25kg/m^2$. Conclusions: Our preliminary results indicate that intraoperative vascular imaging using the ICG fluorescence technique may be helpful for robotic or laparoscopic pylorus-preserving gastrectomy.

  • Making a model that is an accurate replica of the oral structure requires precision and efficiency. Nowadays, rapid technological advances bring digitalization in dentistry. One of the most important works in digital dentistry is three-dimensional modeling of the oral cavity and digitizing the 3D data. Among the three components of CAD/CAM, (1) data capture component (digitizers), (2) design component (CAD software), (3) manufacturing component (CAM), the basic component that has a significant impact on the other processes is the data capture component, i.e. intra-oral scanners. This literature review discusses the principles and clinical use of intra-oral scanners in dentistry based on recent publications of the past 5 years using the PubMed and Google Scholar databases.

  • Background: Anatomic changes in the acromion have been considered a main cause of shoulder impingement syndrome (SIS). To evaluate the relationship between SIS and the acromion process, we devised a new morphological parameter called the acromion process cross-sectional area (APA). We hypothesized that the APA could be an important morphologic diagnostic parameter in SIS. Methods: We collected APA data from 95 patients with SIS and 126 control subjects who underwent shoulder magnetic resonance imaging (MRI). Then we measured the maximal cross-sectional area of the bone margin of the acromion process on MRI scans. Results: The mean of APAs were 136.50 ± 21.75 ㎟ in the male control group and 202.91 ± 31.78 ㎟ in the male SIS group; SIS patients had significantly greater APAs (P < 0.001). The average of APAs were 105.38 ± 19.07 ㎟ in the female control group and 147.62 ± 22.90 ㎟ in the female SIS group, and the SIS patients had significantly greater APAs (P < 0.001). The optimal APA cut-off in the male group was 165.14 ㎟ with 90.2% sensitivity, 91.4% specificity, and an area under the curve (AUC) of 0.968. In the female group, the optimal cut-off was 122.50 ㎟ with 85.2% sensitivity, 84.9% specificity, and an AUC of 0.928. Conclusions: The newly devised APA is a sensitive parameter for assessing SIS; greater APA is associated with a higher possibility of SIS. We think that this result will be helpful for the diagnosis of SIS.

  • The type of ultrasound transducer used influences the quality of a reconstructed ultrasound image. This study analyzed the effect of transducer type on ultrasound computed tomography (UCT) image quality. The UCT was modeled in an ultrasound simulator by using a 5 cm anatomy model and a ring-shape 5 MHz 128 transducer array, which considered attenuation, refraction, and reflection. Speed-of-sound images were reconstructed by the Radon transform as the UCT image modality. Acoustic impedance images were also reconstructed by the delay-and-sum (DAS) method, which considered the speed of sound information. To determine the optimal combination of transducers in observation, point-source, flat, and focused transducers were tested in combination as trasmitters and receivers; UCT images were constructed from each combination. The combination of point-source/flat transducer as transmitting and receiving devices presented the best reconstructed image quality. In UCT implementation, the combination of a flat transducer for transmitting and a point transducer for receiving permitted acceptable image quality.

  • Objectives : Acupoints education is important in that it can determine the clinical competency of Korean Medicine Doctors (KMDs). Accordingly, we aimed to develop a practical simulator for acupoints education, acupoints training, acupoints practice, and acupoints evaluation. Methods : Korean Medicine (KM) SMART Table can be divided into hardware, server and components, and is organically linked. We develop KM SMART Table that combines the hardware of a human-sized table with a UHD display capable of multi-touch in two cases and software that can teach acupoints. We make Augmented Reality (AR) contents linked with KM SMART Table contents and develop applications that can use contents using mobile devices. By developing an AR image tracking module to react with KM SMART Table, it enables acupoint learning according to the mobile device platform and human anatomy. Results : The current system is a prototype where some 3D technology has been implemented, but the AR function will be produced later. New learning using 3D and AR will be required during acupoints education and acupoints practice. It will be used a lot in OSCE (Objective Structured Clinical Examination) practices for strengthening the competency of KMDs, and it will be of great help not only in KM education as a unique simulator of KM, but also in the practice of acupuncture and chuna for musculoskeletal diseases. Conclusions : The KM SMART Table is a technology that combines 3D and AR to learn acupoints, and to conduct acupoints OSCE practice, and we suggest that it can be usefully used for educational evaluation.

  • The Cardiac Gated Blood Pool (GBP) scintigram, a nuclear medicine imaging, calculates the left ventricular Ejection Fraction (EF) by segmenting the left ventricle from the heart. However, in order to accurately segment the substructure of the heart, specialized knowledge of cardiac anatomy is required, and depending on the expert's processing, there may be a problem in which the left ventricular EF is calculated differently. In this study, using the DeepLabV3 architecture, GBP images were trained on 93 training data with a ResNet-50 backbone. Afterwards, the trained model was applied to 23 separate test sets of GBP to evaluate the reproducibility of the region of interest and left ventricular EF. Pixel accuracy, dice coefficient, and IoU for the region of interest were 99.32±0.20, 94.65±1.45, 89.89±2.62(%) at the diastolic phase, and 99.26±0.34, 90.16±4.19, and 82.33±6.69(%) at the systolic phase, respectively. Left ventricular EF was calculated to be an average of 60.37±7.32% in the ROI set by humans and 58.68±7.22% in the ROI set by the deep learning segmentation model. (p<0.05) The automated segmentation method using deep learning presented in this study similarly predicts the average human-set ROI and left ventricular EF when a random GBP image is an input. If the automatic segmentation method is developed and applied to the functional examination method that needs to set ROI in the field of cardiac scintigram in nuclear medicine in the future, it is expected to greatly contribute to improving the efficiency and accuracy of processing and analysis by nuclear medicine specialists.

  • Purpose: The aim of this study was to evaluate the root canal morphology of mesial roots of mandibular first molars. Materials and Methods: Forty extracted mandibular first molars were used in this study. The morphological examination of root canals was conducted in accordance with the Vertucci classification using micro-computed tomography (micro-CT). Any aberrant root canal configurations not included in the Vertucci classification were recorded, and their frequency was established using descriptive statistics. Intra-observer reliability was assessed using the Wilcoxon signed-rank test, while inter-observer reliability was assessed using the Cohen kappa test. Significance was evaluated at the P<0.05 level. Results: The mesial roots of mandibular first molars had canal configurations of type I (15%), type II (7.5%), type III (25%), type IV (10%), type V (2.5%), type VI (7.5%), and type VII (7.5%). The images showed 10 (25%) additional configuration types that were not included in the Vertucci classification. These types were 1-3-2-3, 1-2-3-2-3, 2-3-1, 2-3, 1-2-3-1, 2-1-2-3, 3-2-1, 1-2-3-1, 2-3-2-3, and 1-2-1-2-1. The intra-observer differences were not statistically significant(P>0.05) and the kappa value for inter-observer agreement was found to be 0.957. Conclusion: Frequent variations were detected in mesial roots of mandibular first molars. Clinicians should take into consideration the complex structure of the root canal morphology before commencing root canal treatment procedures to prevent iatrogenic complications. Micro-CT was a highly suitable method to provide accurate 3-dimensional visualizations of root canal morphology.

  • Purpose: Prior to cone-beam computed tomography (CBCT), orthodontic competence included radiological interpretation. Nevertheless, maxillary impacted canines (MICs), because of their position and adjacent complex anatomy, have been challenging to interpret, particularly with regard to root resorption. Although CBCT cross-sectional reconstructions of MICs yielded clearer insights into its diagnosis and treatment planning, the value of simultaneously using 2 different cross-sectional or multiplanar reconstructions of the CBCT datasets - orthogonal and curved/panoramic multiplanar reconstructions - has hitherto not been considered. Materials and Methods: Both orthogonal and curved/panoramic multiplanar reconstruction series of 5 screenshots were each reconstructed from the 5 cm×5 cm CBCT datasets of 15 separate MICs. Fifteen credentialled and experienced orthodontist volunteers reviewed 2 separate PowerPoints of 15 randomized series each, 1 week apart. Their review considered 6 factors that could affect treatment: the position and level of the MIC, the presence or absence of root resorption, ankylosis, cysts, and dilaceration. Results: All 15 orthodontists were statistically similar regarding overall years of experience and of CBCT use. Although either reconstruction alone allowed the orthodontists to determine whether ankylosis and, to a lesser extent, most of the other features were present or absent in the MIC, reviewing both reconstructions together was necessary to determine whether root resorption was present or absent in the adjacent tooth. Conclusion: Reviewing both orthogonal and curved/panoramic multiplanar reconstructions was necessary to evaluate the presence or absence of root resorption in the teeth adjacent to MICs and that of many other features.

  • This paper reports the clinical findings and surgical treatment of feline right patent ductus arteriosus (RPDA) with a left aortic arch. A two-month-old female Maine Coon was referred for an investigation of regurgitation after weaning. RPDA with a left aortic arch was diagnosed based on the echocardiographic and computed tomography (CT) findings. A right-fourth intercostal thoracotomy was found to be an appropriate approach to the duct. Preoperative diagnosis is crucial and diagnostic imaging, including radiography, echocardiography, and cardiac CT examination, is essential for determining if the aortic arch is right or left.

  • The root canal morphology undergoes aging-related changes, and relevant quantitative analyses have not yet been reported. We compared the cone beam computed tomography (CBCT) and micro-computed tomography (microCT) scans of extracted mandibular incisors to check the accuracy of morphological measurements. Thereafter, the root canal morphology and aging-related changes in the mandibular incisors of Japanese individuals were assessed using CBCT. Six extracted teeth were fixed in a phantom head and imaged using CBCT and micro-CT. The correlation between the findings of the two imaging modalities was examined. Further, CBCT reconstructed images of the mandibular incisors of 81 individuals were observed. Age-related changes of the root canals were compared between participants aged <30 years and those aged ≥30 years. The CBCT and micro-CT findings regarding the root canals of the extracted teeth coincided in 94.4% of the cases. Mandibular incisors exhibiting two root canals in either cross-section accounted for 9.9% of central incisors and 12.4% of lateral incisors. Mandibular central incisors with two root canals were observed in two (6.3%) individuals aged <30 years and six (12.2%) aged ≥30 years. Mandibular lateral incisors with two root canals were observed in one (3.1%) individual aged <30 years and nine (18.4%) aged ≥30 years. CBCT allows accurate evaluation of complex root canal morphologies and is useful for endodontic preoperative assessment. Mandibular incisors have more frequent occurrence of two root canals with aging.
