Suggestion for a New Grading Scale for Radiation Induced Pneumonitis Based on Radiological Findings of Computerized Tomography: Correlation with Clinical and Radiotherapeutic Parameters in Lung Cancer Patients

  • Background: The objective of this research is the computed axial tomography (CT) imaging grading of radiation induced pneumonitis (RP) and its correlation with clinical and radiotherapeutic parameters. Materials and Methods: The chest CT films of 20 patients with non-small cell lung cancer who have undergone threedimensional conformal radiation therapy were reviewed. The proposed CT grading of RP is supported on solely radiological diagnosis criteria and distinguishes five grades. The manifestation of RP was also correlated with any positive pre-existing chronic obstructive pulmonary disease (COPD) history, smoking history, the FEV1 value, and the dosimetric variable V20. Results: The CT grading of RP was as follows: 3 patients (15%) presented with ground glass opacity (grade 1), 9 patients (45%) were classified as grade 2, 7 patients (35%) presented with focal consolidation, with or without elements of fibrosis (grade 3), and only one patient (5%) presented with opacity with accompanying atelectasis and loss of pulmonary volume (grade 4). Both univariate and multivariate analysis revealed as prognostic factors for the radiological grading of RP the reduction of FEV1 and the V20 (P=0.026 and P=0.003, respectively). There was also a significant (P<0.001) correlation of radiological grading of RP with FEV1 and V20 (spearman rho 0.92 and 0.93, respectively). Conclusions: The high correlation of the proposed radiological grading with the FEV1 and the V20 is giving a satisfactory clinical validity. Although the proposed grading scale seems relevant to clinical practice, further studies are needed for the confirmation of its validity and reliability.

  • The application of neuroimaging techniques in psychiatry started in 1970s with the use of CT(computerized tomography). Neuroimaging methods can be categorized as anatomical and functional. Recently, attentions are focused on the functional neuroimaging methods those could give us various important informations. But results regarding to psychotropic medication effect on neuroimaging are not sufficient. Here, the study results of the medication effect with the functional imaging methods are mainly revieued.

In this paper, the authors investigated the effect of several clinician - friendly reconstrution filters on the resultant transverse slices of backprojected Profiles of radioisotope distribution from the Quantitative point of view, and reduced the filter parameters such as cutoff frequency and order of filter which are neces mary to minimize the quantification error using computer-generated phantoms.

카롤리병의 방사선학적 진단에 대한 고찰 (A Study on the Radiographic Diagnosis of Caroli's Disease)

  • 카롤리병은 섬유낭성 간질환이다. 상염색체 열성 장애로 담관의 선천적 다발성 확장이 특징이다. 카롤리병을 진단하는 방법으로 전산화단층촬영, 자기공명영상 검사, 담관 조영술, 초음파 검사가 있다. 전산화단층촬영검사는 섬유다낭성 간 질환을 감지하고 구별하는데 필수적이며, 간 내 담관의 확장을 판별하는 데 유용하다. 하지만 조영제를 사용함으로써 일어날 수 있는 부작용에 대한 인지가 필요하다. 자기공명영상검사는 대표적으로 자기공명담췌관조영술 방법을 이용한다. 비침습적 검사로 환자의 고통을 경감할 수 있고, 담췌관부의 해부학적 구조와 병변 유무를 쉽고 빠르게 관찰이 가능하다. 담관 조영술은 비대해진 담관 전체에 걸친 여러 낭성 확장을 직접 시각화할 수 있는 효과적인 진단 방법이다. 하지만 이 시술 또한 침습적인 시술이므로 진단이 아닌 치료 목적으로 권장된다. 초음파 검사는 전산화단층촬영과 유사한 소견을 확인할 수 있다. 간동맥 뿌리는 기존의 그레이 스케일 초음파에서 증명하기 쉽지 않다. 하지만 관 내 혈관 뿌리를 가진 확장된 담관을 묘사할 수 있을 뿐만 아니라 관 내 색상 도플러 신호를 쉽게 식별할 수 있다는 점에서 임상적 가치를 갖고 있다. 최근엔 영상진단의 발전으로 전산화단층촬영 검사, 자기공명영상 검사, 담관 조영술, 초음파 검사 등으로 조기 진단이 가능해졌다. 조기 진단을 통해 치료 후에 장기적인 예후가 개선될 수 있도록 영상 진단 발전에 더욱 기여하고자 각 검사별 카롤리병에 대하여 어떠한 양상이 나타나는지 고찰하였다

콘빔형 전산화단층영상을 이용한 자연치 치간거리의 평가 (Evaluation of interdental distance of natural teeth with cone-beam computerized tomography)

  • 목적: 본 연구 목적은 이상적인 임플란트 근원심적 식립 위치 결정에 필요한 정보를 제공하기 위해서 콘빔형 전산화단층영상(cone-beam CT)을 사용하여 건강한 자연치열에서 백악-법랑 경계부와 치조골 흡수를 가정한 그 하방 2 mm에서 전치, 소구치, 대구치의 치간거리를 평가하는 것이다. 연구 재료 및 방법: 원광대학교 치과대학병원에서 cone-beam CT를 촬영한 건강한 치열의 200명 환자를 선정하였다. Cone-beam CT 이미지를 DICOM (digital imaging and communication in medicine) 파일로 전환하여, 3차원 영상으로 재구성하였고, cone-beam CT 이미지를 표준화하기 위하여 head reorientation을 시행한 후, 전용 소프트웨어를 이용해 재구성된 파노라마 이미지를 얻었다. 모든 계측은 3명의 치과의사에 의해 최적화된 파노라마 이미지 상에서 시행되었다. 결과: 백악-법랑 경계부에서 상악 평균 치간거리는 전치 1.84 mm, 소구치 2.07 mm, 대구치 2.08 mm 그리고 하악은 전치 1.55 mm, 소구치 2.20 mm, 대구치 2.36 mm였다. 백악-법랑 경계부 하방 2 mm에서 상악 평균 치간거리는 전치 2.19 mm, 소구치 2.51 mm, 대구치 2.60 mm 그리고 하악은 전치 1.86 mm, 소구치 2.53 mm, 대구치 3.01 mm였다. 결론: 자연치열에서 치간거리는 전치부보다는 구치부에서 더 컸으며, 백악-법랑 경계부보다 그 하방 2 mm에서 더 크게 나타났다. 전 치열에서 가장 좁은 곳은 하악 전치, 가장 넓은 곳은 하악 대구치였다.

  • Particulate matter (PM) in dust causes serious pathological conditions, and it has been considered a critical health issue for many years. Respiratory disorders such as bronchitis, asthma, and chronic inflammation, are the most common illnesses due to PM that appears as dust. There is evidence that cardiovascular and neurological abnormalities are caused by PM. Although an extensive amount of work has been conducted on this topic, including studies on the nature of the particles, particle size measurements, particle distribution upon inhalation, the health effects of fine particles, disease prevention, diagnosis, and treatment, to this date, there is still a considerable lack of knowledge in these areas. Therefore, the identification of the key components that cause diseases owing to PM, and the specific diagnoses of the diseases, is important. This review will explore the current literature on the origin and nature of PM and their effects on human health. In addition, it will also highlight the approaches that have been adopted in order to diagnose the effects of PM using positron emission tomography (PET) or single-photon emission computerized tomography (SPECT).

Occipital Intradiploic Epidermoid Cyst with Intracranial Hypertension

  • Intradiploic epidermoid cysts of the cranium are rare, benign and slow growing lesion. However, these cysts may cause neurologic deficits due to mass effect. Intradiploic epidermoid cysts are thought to be derived from persistent ectodermal remnants present in the cranial bones during embryogenic development. Here, we report a case of an occipital intradiploic epidermoid cyst with posterior fossa extension. The patient developed intracranial hypertension associated with hydrocephalus due to aqueductal stenosis, and venous congestion secondary to compression of the torcular and the transverse sinus. The imaging studies included a computerized tomography[CT] venogram, which is essential for determining the pathophysiological mechanism of the clinical spectrum and for surgical planning. Near total removal was accomplished and the postoperative course was uneventful. Postoperative imaging studies demonstrated a reversal of the neural structures, but no patency of the sinus, which was presumably indicative of prolonged sinus stenosis. The patient gradually improved and was discharged free of symptoms.

  • The aim of the 3D medical imaging is to facilitate the creation of clinically usable image-based algorithm. Clinically usable imaging algorithm for image analysis requires a high degree of interaction to verify and correct results from registration algorithms, such as the Insight Toolkit (ITK) and the Visualization Toolkit (VTK) which are the class libraries. ITK provides segmentation algorithms and VTK has powerful 3D visualization. However, to apply those libraries to the medical images such as Computerized Tomography (CT), the algorithm based on the interactive construction and modification of data objects are necessary. In this paper we showed the 3D registration about mandibular premolar of human teeth acquired by micro-CT scanner. Also, we used the ITK to find the contour of pulp layer of premolar, furthermore, the 3D imaging was visualized with VTK designed to create one kind of view on the data of 3D visualization. Finally, we evaluated that the volume model of pulp layer would be useful for the tooth morphology in dental medicine.

영상 System의 처리의 근황-전산화 3차원 단층 영상처리 (Recent Developments in Imaging Systems and Processings-3 Dimensional Computerized Tomography)

  • 근래에 발전하고 있는 Artificial Intelligence 또는 Synthetic Image 등 넓은 의미에서의 영상처리에 관하여 해석학적인 설명을 시도하였다. 일반적으로 얻어지는 "영상" 또는 "사진"에 반하여 간접적으로 얻어진 Synthetic Image의 대표적인 예로서 3차원 영상 재 구성 (3-Dimensional Image Reconstruction)을 들 수 있으며, 이의 최근 의학 및 생명 과학 분야는 물론 공학 및 물리학 분야의 비파괴 검사(NDT)등 많은 분야에의 응용에 급격한 발전을 보고 있다. 본 논문은 3차원 CT (Computerized Topography)의 기본을 이루는 3차원 영상 재구성 처리에 관한 기본적인 문제를 two-dimensional signal processing의 관점에서 다루었다.

Point-based surface best fit 알고리즘을 이용한 디지털 치아 모형과 3차원 CT 영상의 중첩 정확도 (IMAGE FUSION ACCURACY FOR THE INTEGRATION OF DIGITAL DENTAL MODEL AND 3D CT IMAGES BY THE POINT-BASED SURFACE BEST FIT ALGORITHM)

  • Purpose: The goal of this study was to develop a technique for creating a computerized composite maxillofacial-dental model, based on point-based surface best fit algorithm and to test its accuracy. The computerized composite maxillofacial-dental model was made by the three dimensional combination of a 3-dimensional (3D) computed tomography (CT) bone model with digital dental model. Materials and Methods: This integration procedure mainly consists of following steps : 1) a reconstruction of a virtual skull and digital dental model from CT and laser scanned dental model ; 2) an incorporation of dental model into virtual maxillofacial-dental model by point-based surface best fit algorithm; 3) an assessment of the accuracy of incorporation. To test this system, CTs and dental models from 3 volunteers with cranio-maxillofacial deformities were obtained. And the registration accuracy was determined by the root mean squared distance between the corresponding reference points in a set of 2 images. Results and Conclusions: Fusion error for the maxillofacial 3D CT model with the digital dental model ranged between 0.1 and 0.3 mm with mean of 0.2 mm. The range of errors were similar to those reported elsewhere with the fiducial markers. So this study confirmed the feasibility and accuracy of combining digital dental model and 3D CT maxillofacial model. And this technique seemed to be easier for us that its clinical applicability can good in the field of digital cranio-maxillofacial surgery.
