Medical genomic approach to early-onset scoliosis

  • Early onset scoliosis (EOS) describes the onset of scoliosis before the age of 10 years and is associated with significant health risks. Compared to adolescent idiopathic scoliosis, studies on the etiology of EOS in children are more common. EOS includes inhomogeneous groups of patients, and the etiology of EOS may be congenital, neuromuscular, syndromic, or idiopathic. The identification of the molecular etiology underlying patients with EOS has been increasing. This could provide valuable information for optimizing the management and care of these children. In this review, EOS was reviewed from a medical genomic perspective.

근에너지기법(Muscle Energy Technique)과 Schroth Method를 이용한 특발성 척추 측만증 호전 1례 (A Clinical Case of Muscle Energy Technique and Schroth Method for Idiopathic Scoliosis)

  • The purpose of this study is to report the effect of muscle energy technique (MET) and Schroth method on idiopathic scoliosis. A patient with idiopathic scoliosis was treated by MET and Schroth method. To investigate the outcome of the patient's improvement, we observed the X-ray of cervical, thoracic, lumbar spine. And Cobb's angle, correctability, visual analog scale (VAS) were used to measure changes during treatment. After treatment, Cobb's angle and correctability, VAS were improved significantly. MET and Schroth method are proved to be helpful to improve idiopathic scoliosis. And, further studies will be needed.

  • Objectives : Hyun-Ga therapy, a creative method that involves meridian pathways for isometric exercise, has displayed the possibility of treating and alleviating idiopathic scoliosis in terms of theory. The researcher explored current research trends and introduced Hyun-Ga therapy in which meridian pathways theory is applied. Methods : We examined theses and books of oriental or western medicine that cover idiopathic scoliosis. By doing these, we looked into the role of Hyun-Ga therapy towards the prevention and treatment of idiopathic scoliosis, and gained the following results. Results and Conclusions : Hyun-Ga therapy, the manual technique that has introduced the concept of silence and movement into rectifying the body through isometric exercise on meridian pathways in the limbs, can be practiced more easily than other conventional manual techniques. Hyun-Ga therapy based on meridian pathways theory is highly evaluated for its clinical insight on the structural and functional roles of meridian pathways. The theory of Hyun-Ga therapy, however, needs to be supported by more objective and sufficient complementary data. For this, continuing research and analysis are required in the future.

요가운동프로그램이 특발성 척추 측만증에 미치는 효과 (The Effects of Yoga Program on Idiopathic Scoliosis)

  • 본 연구의 목적은 특발성 척추 측만증 환자를 대상으로 요가 운동 요법을 통하여 척추 측만각도 즉 요추 Cobb's angle의 정도를 분석하여 요가 운동 프로그램이 특발성 척추 측만연구에 어떠한 영향을 미치는지의 효과를 알아보는데 있다. 본 연구의 요가 운동 프로그램은 척추주변의 근육들의 지나친 긴장을 풀고 이완된 근육은 수축운동을 통하여 유연성과 탄력을 회복하여 운동 시 주동근과 길항근의 균형을 맞추고 협력근과의 조화로운 상태가 척추측만에 도움될 것으로 판단하여 골반운동과 척추 비틀기 운동을 중심으로 실시하였다. 실험대상은 광주광역시 소재 C병원에서 특발성 척추 측만증 진단을 받은 여자 중학생을 대상으로 하여, 요가 운동 프로그램으로 주5일, 1일 60분, 12주간 실시하여 요가 운동프로그램이 특발성 척추 측만증 변화에 미치는 영향을 알아보고자 실시하였다. 실험 전, 후 방사선을 통하여 요추의 Cobb's angle을 측정하여 효과를 검증하였다. 첫째, 요가 및 척추 전문가와 함께 연구하여 동작을 추축하였다. 둘째, 개발된 척추측만증에 대한 요가 운동 프로그램을 특발성 척추 측만증을 갖고 있는 여중생에게 12주간 실시하였다. 셋째, 특발성 척추 측만증에 대한 요가 운동 프로그램에 대한 타당성과 효과를 검증하였다. 그 결과 운동 전보다 5도가 통계적으로 유의하게 감소하여 (p=0.006)특발성 척추 측만증이 개선되었다.

  • PURPOSE: This study examined the effects of coordinative locomotor training on the spine appearance and quality of life of patients with idiopathic scoliosis. METHODS: This study included two patients with idiopathic scoliosis: one with a thoracic and lumbar type scoliosis and the other with thoracic type scoliosis. The study design was a single case study (A-B-A'), with a baseline-intervention/phase-post-intervention. The baseline (A) was designed and measured five times, intervention phase (B) ten times, and post-intervention (A') five times. The coordinative locomotor training program was divided into 10 minutes of warm-up exercise, 30 minutes of the main exercise, and 10 minutes of the finishing exercise, for 50 minutes each time. The primary outcome measurements were measured using the Cobb's angle, Adam's test, and Gait view pro 2.0 to determine the changes in the spine appearance. The secondary outcome measurements were compared before and after using the SRS-22 questionnaire to determine the quality of life of the scoliosis patients. A statistical test analyzed the mean and standard deviation, and the rate of change was presented by a visual analysis method using descriptive statistics and graphs. RESULTS: The findings showed that the spine appearance and quality of life of the two subjects were improved compared to the baseline measurements during the intervention phase, and the improved state was maintained during the post-intervention period. CONCLUSION: These findings indicate that coordinative locomotor training may help improve the spine appearance and quality of life of patients with idiopathic scoliosis.

슬링을 통한 코어운동프로그램이 청소년의 특발성 척추측만증의 Cobb각과 코어근력에 미치는 효과 (Effects of Sling-Based Core Exercise Program on Cobb Angle and Core Strength of Idiopathic Scoliosis in Adolescences)

  • Background: The purpose of this study was compare and analyze the Cobb's angle, core muscle strengthening in adolescents with idiopathic scoliosis and provide scientific basis for effectively treating idiopathic scoliosis in the future. Methods: The subjects of this study were 20 members of adolescent girls. Measurement tools were comprised Cobb's angle test, core muscle strengthening test. After 8 weeks of program, both groups had the same measurements as before program start. Results: The results of the study are as follows. First, the experimental group was decreased statistically significantly in Cobb's angle than the control group. Second, Both groups were increased statistically significantly in core muscle strengthening. The experimental group was increased statistically significantly direction in front, back l than the control group. Conclusion: Sling exercise with core exercise was effective in the decrease of Cobb's angle and core muscle strength than general scoliosis exercise in adolescents with idiopathic scoliosis.

특발성 척추 측만증(Idiopathic Scoliosis)애 대한 Schroth 운동요법에 대한 고찰 (A Study of Exercise treatment based on Schroth method of Idiopathic Scoliosis)

  • Current treatment for adolescent idiopathic scoliosis(AIS) consists of three phases: observation, bracing, and surgery. Although there are many nonsurgical treatment(bracing, electrical stimulation, exercise, manipulation, acupuncture, etc), their effect is still controversial. In many paper, Schroth method was reported good immediate response to conservative care, which could be considered a sign of good prognosis. Schroth method became effective thai specialists in physiotherapy for spinal deformities teach the patient how to perform a routine of 'curve pattern' specific exercises with the purpose to facilitate the correction of the asymmetric posture and to teach the patient to maintain the corrected posture in dally activities. This Principles of correction exercise treatment are based on those developed by the German physiotherepist K. Schroth.

청소년 척추 측만증 환자를 위한 근골격계 보정 웨어 개발 - 스포츠 테이핑 원리 응용 - (Development of Wearing of Musculo-Skeletal Functional Garment for Adolescents' Idiopathic Scoliosis -With the Principle of Sports Taping Applied-)

  • We aimed to analyze the muscle activity of adolescent patients with idiopathic scoliosis during gait and develop the wearing of musculo-skeletal functional garment by applying the principle of sports taping based on the result of the analysis. We selected 20 male students between the ages of 13 and 18 and divided them into 2 groups: one group consisted of 10 patients with idiopathic scoliosis <20 degrees of Cobb's Angle: the other group had 10 normal students. Using, we measured and analyzed the muscle activity of 8 different regions: left and right latissimus dorsi, left and right thoracolumbar fascia, left and right gluteus medius, and left and right biceps femoris during gait. Our results can be summarized as follows: Firstly, in patients with idiopathic scoliosis, the gait showed a significantly low activity of the right latissimus dorsi muscle when the left foot was supported on the ground(p<.05). Secondly, in the overall gait cycle, the patients showed a higher activity of the right thoracolumbar fascia and right gluteus medius than that seen in the normal students: however, this difference was not statistically significant. Thirdly, by applying sports taping on the bisis of the results, this study developed the wearing of musculo-skeletal functional garment that could maximize the stimuli of the right latissimus dorsi and alleviate muscle contraction of the right thoracolumbar fascia and right gluteus medius, while expanding the spinal line upward and downward, by focusing on the difference between left and right muscular strength of the muscle activity of the bright latissimus dorsi. Overall, we expect that by wearing of musculo-skeletal functional garment, the muscular functions in adolescents with idiopathic scoliosis.

특발성 척추 측만증이 있는 초등학생을 대상으로 한 조기 운동요법의 효과 (The Effect of the Early Therapeutic Exercise on Idiopathic Scoliosis in Elementary School Children in Seosan City)

  • The present study was performed to investigate the prevalence rate of idiopathic scoliosis and to determine the effect of exercise training on scoliotic angle in elementary school children. In this study, two out of five elementary schools in Seosan city were chosen by random sampling. Seven hundred sixty four students (from four grade to the sixth grade student) were selected in two schools. Screening tests were conducted to find idiopathic scoliosis. Among the 764 individuals, 139 subjects who showed positive sign in physical examination took whole spine radiography. Thirty six subjects who had a curve of 10 or greater and consented to participate in the exercise program were selected for the exercise program. The exercise program was performed four times a week for 5 months. The results of this study were as follows: 1) One hundred thirty nine subjects showed positive sign in the scoliosis screening test. 2) The overall prevalence of curve of $10^{\circ}or$ greater in X-ray finding was 8.15%. The prevalencies of curve of $10^{\circ}or$ greater in male and female were 7.1% and 9.2%, respectively. 3) Scoliosis curves were observed at thoracic area (48.4%), at thoracolumbar area (27.4%) and at lumbar area(24.4%). 4) Right side curve was 59.7%, and left side curve was 40.3%. 5) After the 5 month exercise program for scoliosis, the Cobb's angle was significantly decreased. 6) There was no significant difference of Cobb's angle change respect to sex, grades, and scoliosis curve site. Results shown here indicates that an early detection and early exercise for scoliosis can result in decreased the Cobb's angle in elementary school children.

추나요법을 시행한 특발성 척추측만증 환자 30례에 관한 임상적 고찰 (Clinical Study on Idiopathic Scoliosis with Massotherapy)

  • Objective : The purpose of this study was to inspect the effect of Massotherapy on idiopathic scoliosis patients, utilizing Cobb's angle to determine the degree of curvature progression in the thoracic and lumbar regions. Methods : To obtain this result, we examinated 30 case of parents who visit Dept. of Acupuncture and Moxibustion Oriental Medical Hospital. Taejon University. with idiopathic scoliosis. We observe their age, sex, symptoms and Cobb's angle. Results : As a result of this examination, we detected the fact that it's difficult to treat isiopathic scoliosis fundamentally. But, Their symptoms are improved. So it is necessary to treat scoliosis actively with massotherapy . Conclusion : We want to be put this knowledge to practical use on treating Scoliosis and think the study like this must be go on continuously.
