The Effect of Mental Practice with Task Activities on Upper Limbs Movement Speed and Accuracy of Ideomotor Apraxia Patients : Single Subject Experimental Research (상상연습과 과제활동 병행 치료가 관념운동 실행증 환자의 상지 움직임 속도와 정확성에 미치는 효과: 단일사례 실험연구)

  • Objective : The purpose of this study is to investigate the effect of mental practice with task activities through upper limbs movement speed and accuracy in ideomotor apraxia patient. Methods : As a single subject research design with multiple baseline across individuals, the patients were two ideomotor apraxia patients, employed in this study. The employed program included receiving mental practice and task activities, measuring change of the total performance times, time to put down the last cups and amount of water in cups for 4 weeks, including baseline and intervention periods. The subject were measured by Ideomotor apraxia test and MFT were analyzed. The analyses were performed using visually and two-standard deviation band method. Results : All subjects were a reduction in symptoms of apraxia, but noticeable change in upper limbs function was not there. Total time of performance from 376.67 to 355.24 seconds, 391.33 seconds to 434.60 seconds, reduced. Put down the cup of performance time from 15.67 to 13.30 seconds, 20.80 seconds to 10.73 seconds, reduced. The amount of water transferred from 52.38ml to 49.70ml, 50.89ml to 50.09ml, all results were closer to 50ml. Conclusion : As a result of this study, mental practice with task activities can be an effective method to ideomotor apraxia patients. This study would serve a fundamental data on ideomotor apraxia patients to the further studies.

  • Introduction : The Understanding of neurological lesion relating to apraxia will help to predict symptoms according to a lesion and to establish a proper treatment and goal. So, This study will explain causes and mechanism of a movement error of ideomotor apraxia through literature review and also will suggest the evidence using in treatment. Body : Ideomotor apraxia may occur a damage of the production part in praxis system, and is common in damage of a cortex rater than damage of a subcortex. According to study with gesture, movement of upper limbs is relate of left parietal lobe but finger movement is relate of frontal lobe. The visual and tactile stimulation through using real objects could guide into proper movement aside from memory of a skilled action. Conclusion : Praxis can occur through diverse neurological processing and various external stimulations can help praxis processing. Therefore, the treatment of ideomotor apraxia need to use this stimulations.

  • The purpose of this study was to review the limb apraxia. It includes the evaluation and neurophysiological aspects for limb apraxia. Limb apraxia comprised a wide spectrum of higher-order motor disorders that results from acquired brain disease affecting the performance of skilled and/or learned movements with the forelimbs and is a common sequela of left brain damage that consists of a deficit in performing gestures to verbal command or to imitation. There are two forms in limb apraxia; ideational apraxia and ideomotor apraxia. A assessment of limb apraxia typically includes pantomiming and imitation of transitive, intransitive, and meaningless gestures. Limb apraxia has been attributed to damage confined to the cerebral cortex, cortico-cortical connecting pathways, and basal ganglia.

Case Study of a Patient with Callosal Disconnection Syndrome Secondary to an ACA Infarction (전대뇌동맥 경색으로 인한 뇌량 분리 증후군 환자의 치험 1례)

  • A callosal disconnection syndrome results in the functional independence of each hemisphere caused by ACA infarction, head injury or callosal operation for epilepsy. Clinical features are characterized by left unilateral tactile anomia, left unilateral agraphia, left unilateral apraxia, difficulty in copying drawings, dyscalculia and alien hand syndrome. Taeumchowe-tang(太陰調胃湯) is used to treat Taeumin(太陰人) Wewansuhanpyohanbyung(胃脘受寒表寒病). A 47 year-old man, following callosal infarction, was detected on MRI with left agraphia, dyscalculia and alien hand syndrome, but no left ideomotor apraxia or left apraxia. He was treated with Taeumchowe-tang. Significant improvement of clinical symptoms was seen, so it is here reported.
