Borrmann 4형 위암의 임상병리학적 특성과 예후 (Clinicopathologic Characteristics of and Prognosis for Patients with a Borrmann Type IV Gastric Carcinoma)

  • 목적: Borrmann 4형 위암은 수술적 치료를 비롯한 다양한 치료방법의 발달에도 불구하고 매우 불량한 예후를 보인다. 본 연구에서는 4형 위암의 효과적인 치료전략을 세우기 위하여 임상병리학적 특성 및 예후인자들을 분석하였다. 대상 및 방법: 1990년부터 2001년까지 진행성 위암으로 진단 받고 절제술을 시행한 1098명을 대상으로 하여, 4형 위암 환자군(group I) 81명과 그 이외 육안형의 위암 환자군(group II) 1017명으로 나누어 임상병리학적 특성, 5년 생존율 및 재발양상을 비교하였다. 결과: 4형 위암은 여성의 발생빈도가 높았고(49.4% vs. 31.0%, P=0.001) 주로 미분화형의 조직형을 가지며(91.4%) 장막 침습(85.2%) 및 림프절 전이(87.7%)의 빈도가 유의하게 높았다. 81명의 환자 중 68명(84%)은 발견 당시 병기가 3기 또는 4기로 분류되었으며 다른 육안형에 비해 근치적 절제술의 빈도가 낮고(53.1%), 수술 당시 복막 파종의 빈도가 유의하게 높았다(25.9%). 다변량 분석에서 림프절 전이의 정도만이 4형 위암의 불량한 예후와 관련된 독립적인 인자였다. 근치적 절제술을 시행한 4형 위암 환자의 5년 생존율은 다른 육안형의 위암에 비하여 유의하게 낮았으며(26% vs. 63%), P<0.05), 동일한 병기에서도 유의미하게 낮은 생존율을 보였다(P<0.05). 4형 위암으로 근치적 절제술을 받고 재발이 확인된 29예 중 27예(93.1%)에서 복막 전이가 있었다. 결론: Borrmann 4형 위암은 수술적 치료에도 불구하고 예후가 매우 나쁘므로 선행화학요법 또는 복강 내 화학요법 등 다른 종류의 치료전략이 필요하며, 진단적 복강경 및 세포진 검사를 통해 미세 복막 전이 여부를 판단하여 병기 설정의 정확도를 높여야 한다.

  • In this paper, the study is researched for related safety standards having experiments concerning temperature changes in type IV cylinder of the Hydrogen fuel cell vehicle. Experiments were performed to acquire temperature data of type IV cylinder according to filling time. The experimental results are shown that internal temperatures of type Ⅳ vessel are over $85^{\circ}C$ at all measured points after 5 minutes at filling 35 MPa and the highest temperature is getting lower when the residual gases are more remained. Consequently, the safety standards need properly limited value through further study for filling flow rate and filling time.

  • Prenatally diagnosed neuroblastomas have been reported in increasing numbers over the past several years. The vast majority are in favorable stages of the disease (stage I, II, IV-S). The authors experienced one case of stage IV-S neuroblastoma of the adrenal gland with liver metastasis, which regressed spontaneously after removal by adrenalectomy. This patient was noticed to have an abdominal mass at prenatal ultrasonography performed at 36weeks of gestation. This tumor was a neuroblastoma of the left adrenal gland with multiple liver metastases. Left adrenalectomy and liver biopsy were performed at 3 months of age. Thirty-eight months after surgery, an MRI demonstrated that the hepatic metastatic lesions had completely regressed without chemotherapy or radiation.

  • Ti(IV)-Fe(III) oxide films were formed by MOCVD technique, and their photoelectrochemical properties were examined in 0.5M N $a_2$$SO_4$ solution by a photoelectrochemical polarization test. Ti(IV)-Fe(III) oxide films deposited at 40$0^{\circ}C$ by MOCVD have crystalline structure and are all n-type semiconductors. The photocurrent and the quantum efficiency of the films increase with increasing the iron cationic fraction ($X_{Fe}$ ) in the films. The energy band gap of the films increase linearly with increasing the iron cationic fraction in the films. Ti(IV)-Fe(III) oxide film of $X_{Fe}$ /=0.60 has high photocurrent response and corrosion resistance simultaneously.

  • IV Deep sedation is useful for Waardenburg syndrome Child undergoing extensive dental treatment. We experienced a case of dental treatment under W deep sedation in a 3 years old boy with Waardenburg syndrome. He was treated on an outpateint basis. He was diagnosed hypothyroidism and received thyroid hormone. Premedication was done using ketamine and atropine IM. Induction using $N_2O$-enflurane mask ventilation was uneventful and intravenous cathetering was carried out. IV deep sedation was maintained with midazolam-$N_2O$ combination. After monitoring the patient for 2 hours and confirming his recovery, he was discharged from the day care unit. In summary, we report this successful IV Deep sedation management of Waardenburg syndrome child during dental treatment in as out patient.

한국 Wechsler 유아지능검사 4판(K-WPPSI-IV)의 표준화연구: 신뢰도와 타당도분석 (A Study on Standardization of K-WPPSI-IV: Analyses of Reliability and Validity)

  • This study examined the reliability and validity evidence of the Korean version of Wechsler Primary & Preschool Scale for Intelligence(K-WPPSI-IV), which was standardized recently. Following the stratified sampling for Korean population in terms of region and maternal education level, 1,700 children aged 2.6~7.7 years were recruited for Korean norm development. The split-half reliability (N=1,700) was high(Fisher's z(.75-.89)) and test-retest reliability(N=100) was satisfactory ranging from .95 to .68. The inter-rater reliability(N=110) was very high(.96-1.0). Discriminant and convergent validities were revealed by correlational patterns among subtests and indices. Future studies are needed to extend both reliability and validity of K-WPPSI-IV by collecting data with a various sample group and a variety of instruments.

  • ETOT-3-ETOG-3 전산체계와 INDF/B-IV 평가핵자료를 이용하여 LEOPARD코드용 핵자료라이브러리를 생산하였다. 그리고 생산된 라이브러리의 신뢰성을 입증하기 위하여, 선정된 많은 실험자료에 대한 임계계산을 수행하였다. 이 결과를 토대로 경수형 $UO_2$핵연료계에 대한 수정, 평가계산을 수행하여 조정된 라이브러리가 유용함을 확인하였다.

  • Though the magnitude is decreasing, the compressive residual stress is always distributed during fatigue crack growth near the crack tip. This means that the residual stress is released during fatigue crack growth due to the alternative load. ${\Delta}K_{IV}$ is modified by using the initial residual stress and the redistributed residual stress in uniform and various thickness welded specimens. The former is denoted by ${\Delta}K_{IV.eff.i}$ and the latter is denoted by ${\Delta}K_{IV.eff.i}$. ${\Delta}K_{IV.eff.i}$ gives more accurate relations in da/dN vs. ${\Delta}K_{IV}$ curve, however the difference is very small.

  • An integrated R&D program is being conducted to study, qualify, and in some cases, develop materials with required properties for the reactor systems being developed as part the U.S. Department of Energy's Generation IV Reactor Program. The goal of the program is to ensure that the materials research and development (R&D) needed to support Gen IV applications will comprise a comprehensive and integrated effort to identify and provide the materials data and its interpretation needed for the design and construction of the selected advanced reactor concepts. The major materials issues for the five primary systems that have been considered within the U.S. Gen IV Reactor Program-very high temperature gas-cooled, supercritical water-cooled, gas-cooled fast spectrum, lead-cooled fast spectrum, and sodium-cooled fast spectrum reactors-are described along with the R&D that has been identified to address them.
