혈액 투석 환자에서 조혈 호르몬 치료 효과 향상에 대한 연구 (A Study for Improvement of Erythropoietin Responsiveness in Hemodialysis Patients)

  • 본 연구는 만성 신부전 환자의 빈혈 치료로 사용되는 EPO의 효과를 감소시키는 여러 원인을 조사하고 각각의 원인에 대한 적절한 치료를 함으로써 EPO의 치료 효과를 향상시키고자 2001년 4월 현재까지 영남대학교 의과대학 부속병원 인공 신장실에서 6개월 이상 혈액 투석중인 72명을 대상으로 전향적 연구를 시행하였다. 전체 대상군(n=72) 에서 IVAA 12주 치료와 철분 상태에 따른 IVAA 효과를 판정하여 가장 효과적인 기능성 철 결핍 군에게는 IVAA 치료를 8주 연장하였다. 혈중 알루미늄이 4 ${\mu}g/l$ 이상군과 저장철이 500 ${\mu}g/l$ 이상인 군에게는 DFO를 8주간 투여하였다. Tsat 20% 미만 군에서는 경구 철 투여를 증가시키면서 IVAA 12주간 투여하였고, i-PTH가 120 pg/ml 이상 군에게는 비타민 $D_3$를 투여하였다. 성적은 다음과 같다. 1. 전체 대상군(n=72) 에게 IVAA 12주 치료 후 혈색소는 투여 전에 비해 투여 후 2, 4, 6주에 유의한 증가를 보였으나, 이후 지속적인 효과를 보이지는 않았다. 2. 철분 상태에 따른 분류 군중 IVAA 12주 치료에 가장 효과적인 반응을 보인 군은 혈중 ferritin이 100-500 ${\mu}g/l$이고 Tsat이 30% 이하 군이었다. IVAA 20주간 투여 결과, 혈색소는 초기 $9.01{\pm}0.73$에서 2, 4주에 각각 $9.56{\pm}1.01$ g/dl, $9.45{\pm}0.93$ g/dl로 유의한 증가를 보였고, EPO는 초기 $116.4{\pm}50.8$ IU/kg/week에서 2, 4, 6, 10, 12, 16, 20주에 각각 $85.4{\pm}74.4$ IU/kg/week, $86.8{\pm}69.8$ IU/kg/week, $95.3{\pm}63.4$ IU/kg/week, $92.4{\pm}63.9$ IU/kg/week, $94.8{\pm}68.2$ IU/kg/week, $93.4{\pm}50.2$ IU/kg/week로 유의한 감소를 보였다. 또한, 초기 Tsat $20.3{\pm}5.4%$에서 2, 6, 16, 20주에 각각 $27.5{\pm}12.8%$, $28.2{\pm}15.2%$, $32.1{\pm}17.3%$, $29.2{\pm}12.9%$로 유의한 상승을 보였다. 3. 혈중 알루미늄이 4 ${\mu}g/l$ 이상 군(n=12)에서 IVAA 12주간 투여 후 DFO 8주간 치료 결과, IVAA 12주간 투여시 혈색소의 유의한 변화는 없었으나, DFO 투여사 투여 전 EPO $145.8{\pm}72.2$ IU/kg/week에서 투여후 2, 4, 6, 8 주에 각각 $104.5{\pm}95.7$ IU/kg/week, $88.2{\pm}99.9$ IU/kg/week, $90.9{\pm}67.1$ IU/kg/week. $99.1{\pm}58.9$ IU/kg/week로 EPO의 유의한 감소를 보였다. 그러나 혈중 알루미늄은 DFO 전후에 유의한 차이를 보이지 않았다. 4. 혈중 ferritin이 500 ${\mu}g/l$ 이상 군 (n=25)에서 IVAA 12주간 투여 후 DFO 8주간 치료 결과, 혈색소와 EPO 용량의 유의한 차이는 없었다. 5. Tsat 20% 미만 군(n=7) 에서 경구 철 투여를 증가시키면서 IVAA 12주간 투여 결과, 투여 10주에 Tsat의 유의한 상승을 동반한 EPO 용량의 감소를 보였으나, 혈색소의 유의한 변화는 없었다. 6. I-PTH 200 pg/rnl 이상 군(n=10)에서 비타민 $D_3$ 치료 결과, 비타민 $D_3$ 치료 후 2주부터 지속적으로 유의한 i-PTH의 억제에도 불구하고 혈색소와 EPO 용량의 유의한 차이는 없었다. 결론적으로 만성 신부전 환자의 빈혈 치료로 사용되는 EPO의 효과를 감소시키는 원인 중 기능성 철 결핍성 빈혈 환자에서의 IVAA 치료는 Tsat의 증가 및 혈색소의 증가와 EPO의 감량 효과가 있었고, 혈중 알루미늄이 증가된 경우, 저용량의 DFO 사용으로 EPO의 감량 효과를 나타내었다. 이러한 결론을 확인하려면 향후 대규모의 전향적 연구가 필요하리라 생각된다.

Ability of Biochemical Parameters to Distinguish between Bile Duct Cancer and Gall Bladder Stones - A Case Control Study in a Tertiary Care Hospital of Pokhara Valley

  • Background: The present study was designed to comparatively assess alteration of biochemical parameters in bile duct cancer and gall stone disease. Materials and Methods: A hospital based case-control study was carried out in the Department of Biochemistry of Manipal Teaching Hospital, Pokhara, Nepal between $1^{st}$ January 2010 and $31^{st}$ December 2012. The variables collected were age, gender, serum total cholesterol, total bilirubin, AST, ALT, serum alkaline phosphatase, albumin and hemoglobin. One way ANOVA was used to examine the statistical significance of differences between groups. A post-hoc LSD test was applied for the comparison of means of control versus case groups. A p-value of <0.05 (two-tailed) was considered significant. Results: The mean age of cases and controls was $53.2{\pm}21.2$ years. The levels of serum cholesterol were higher in cases of cancer $192.5{\pm}21.5$ mg/dl in comparison to stone cases $168.7{\pm}16.1$ mg/dl (p value: 0.0001). The total bilirubin showed the marked difference in cases of cancer $7.6{\pm}3.2$ mg/dl in comparison to stone cases $2.5{\pm}0.8$ mg/dl of bile duct. There was discernible divergence in values of alkaline phosphatase in cases of cancer $251.5{\pm}20.1$ IU/l when compared to stone cases $173.2{\pm}12.6$ IU/l of bile duct. In contrast, there was no apparent deviation in values of aspartate transaminases and alanine transaminases in cases of cancer $59.1{\pm}8.9$ IU/l and $105.5{\pm}26.5$ IU/l when compared to stone cases $56.9{\pm}7.9$ IU/l and $84.5{\pm}13.5$ IU/l respectively. Conclusions: LFT analysis for pre-operative assessment was a good predictive marker in setting apart bile duct cancer and gall bladder stone.

화상환자에서 발생한 췌장염의 임상적 고찰 (The Clinical Investigation Study of Pancreatitis Developed in Burn Patients)

  • Purpose: To find progression and prognosis of pancreatitis developed in massive burn patients through retrospective analysis. Methods: A retrospective study was conducted on 32 patients with abnormal increase of serum lipase level among 2523 acute burn patients admitted to our burn center from January 1, 2017 to June 30, 2018. Pancreatitis in this study was defined as a serum lipase concentration level that is higher than 180 IU/L which is three times more than the normal level (less than 60 IU/L). In this study, a retrospective analysis was performed on patients with serum lipase level higher than 300 IU/L to better understand causality of burns and pancreatitis. Results: 32 patients (1.27%) had serum lipase level higher than 180 IU/L among 2523 acute burn subjects. And 13 patients (0.52%) of these 32 patients had serum lipase level elevated more than 300 IU/L. The study indicated serum lipase level was increased around 7 days after the injury. It returned to normal level early as after 1 to 2 weeks and late as after 4 to 6 weeks of injury. The serum amylase level was increased as similar modality as to the serum lipase level increase. The serum bilirubin, AST, ALT, LD, and GGT were also observed to be elevated when serum lipase was more than 1000 IU/L. Conclusion: The pancreatitis developed in burn patients are mostly as mild symptom. It could due to the ischemic injury and can easily be treated by a temporary fasting, TPN, and Gabexate intravenous injection.

가미육군자탕 투여 후 발생한 급성 약인성 간손상 1례 (One Case of Drug-Induced Liver Injury after Taking Gamiyukgunja-tang)

  • Recently, acute toxic liver injury has been reported to be the most common cause of acute hepatitis. We witnessed one case of hepatic injury which suggested drug-induced hepatitis by herbal medication (Gamiyukgunja-tang). This patient, diagnosed cerebral infarction, was treated with herbal medication and physical therapy for improvement of right hemiparesis. In the course of treatment, this patient showed elevation of serum transaminase (ALT 129 IU/L, AST 150 IU/L), alkaline phosphatase (ALP 261 IU/L), and total bilirubin (2.0 IU/L), so we supposed toxic hepatitis by herbal medication. Saenggangeonbi-tang was administered for 8 days, ALT, AST, ALP, total bilirubin decreased within normal limits.

Evaluation of ${\beta}$-1,4-Endoglucanases Produced by Bacilli Isolated from Paper and Pulp Mill Effluents Irrigated Soil

  • A total of 10 cellulase-producing bacteria were isolated from soil samples irrigated with paper and pulp mill effluents. The sequencing of 16S rRNA gene revealed that all isolates belonged to different species of genus Bacillus. Among the different isolates, B. subtilis IARI-SP-1 exhibited a high degree of ${\beta}$-1,4-endoglucanase (2.5 IU/ml), ${\beta}$-1,4-exoglucanase (0.8 IU/ml), and ${\beta}$-glucosidase (0.084 IU/ml) activity, followed by B. amyloliquefaciens IARI-SP-2. CMC was found to be the best carbon source for production of endo/exoglucanase and ${\beta}$-glucosidase. The ${\beta}$-1,4-endoglucanase gene was amplified from all isolates and their deduced amino acid sequences belonged to glycosyl hydrolase family 5. Among the domains of different isolates, the catalytic domains exhibited the highest homology of 93.7%, whereas the regions of signal, leader, linker, and carbohydrate-binding domain indicated low homology (73-74%). These variations in sequence homology are significant and could contribute to the structure and function of the enzyme.

잡종견(雜種犬)의 혈액화학치(血液化學値)에 관(關)한 연구(硏究) (Blood chemical values of mixed breed dog in Korea)

  • The blood chemical values and blood enzyme activities were examined from 74 healthy mixed breed dogs in the area of Seoul. The results obtained are summarized as follows; 1. Mean${\pm}$SD values and ranges of glucose were $61.97{\pm}8.41mg/100ml$ and 47.28~81.67mg/100ml, of blood urea nitrogen(BUN) $15.99{\pm}2.31mg/100ml$ and 8.21~21.31mg/100ml, of total protein(TP) $8.17{\pm}0.93g/100ml$ and 6.06~9.91g/100ml, of albumin $4.16{\pm}0.47g/100ml$ and 2.81~5.15g/100ml, of globulin, $4.01{\pm}0.64g/100ml$ and 2.72~5.54g/100ml, of albumin/globulin(A/G) ratio $1.06{\pm}0.17$ and 0.71~1.42, of cholesterol(Chol) $187.33{\pm}19.78mg/100ml$ and 128.70~222.90mg/100ml, of total bilirubin(TB) $0.73{\pm}0.14mg/100ml$ and 0.43~1.16mg/100ml, of phosphorus(Pi) $5.25{\pm}1.00mg/100ml$ and 2.61~7.72mg/100ml, of calcium(Ca) $10.76{\pm}1.08mg/100ml$ and 8.24~12.60mg/100ml, of triglyceride(TG) $89.48{\pm}21.16mg/100ml$ and 47.80~133.00mg/100ml, respectively. 2. The glucose value in the age group of 7~12 months was higher (p<0.01) but in the age group of 3~4 years was lower (p<0.05) than the total glucose value. The TP value in the age group of 7~12 months was lower (p<0.01) but in the age group of 1~2 years was higher (p<0.05) than the total TP value. The globulin value in the age group of 7~12 months was lower (p<0.01) but in the group of 1~2 years was higher (p<0.01) than the total globulin value. The A/G ratio value in the age group of 7~12 months was higher (p<0.05) but in the age group of 1~2 years was lower (p<0.05) than the total A/G ratio value. The phosphorus values in the age group of 1~2 years and the age group of 3~4 years were lower (p<0.01, p<0.001) than the total phosphorus value. The calcium value in the age group of less than 6 months was higher (p<0.05) but in the age group of 7~12 months was lower (p<0.001) than the total calcium value. 3. Mean${\pm}$S.D. values and ranges of alkaline phosphatase(AP) were $72.47{\pm}19.73IU/l$, and 28.13~105.00IU/l, of lactic dehydrogense(LDH) $159.46{\pm}45.11IU/l$ and 60.63~265.30IU/l, of serum aspartate aminotransferase(AST) $38.64{\pm}8.62IU/l$ and 21.47~70.58IU/l, of serum alanine aminotransferase(ALT) $34.88{\pm}11.30IU/l$ and 14.51~73.17IU/l, respectively. 4. The AP value in the age group of 7~12 months was higher (p<0.05) but in the age group of 1~2 years was lower (p<0.01, p<0.001) than the total AP value. The LDH value in the age group of less than 6 months was higher (p<0.001) but in the age group of 1~2 years and the age group of 3~4 years were lower (p<0.05) than the total LDH value. The serum AST value in the age group of 3~4 years was lower (p<0.01) than the total SGOT value. The serum ALT value in the age group of 7~12 months was higher(p<0.05) than the total SGPT value.

인간 신장세포로부터 scu-PA의 경제적 생산을 위한 무혈청 배지의 개발 (The Development of Serum Free Medium for the Economic Production of scu-PA from HEK Cells)

  • Cytodex III 마립담체를 이용하여 무혈청배지에서 인간선장 세포를 배양하였다. 무혈청애지에서 미립담체 표변에 세포들은 정상적으로 부착하고 확 산하면셔 세포들은 담체에서 담체로 이동하여 생육 하였다. 미립담체에 접착 수율은 1%의 혈청 을 포 함된 배지에서는 약 93%인 반면에 무혈청배지에 서는 85% 정도로 나타났다. 접착된 세포의 90% 이상이 유가식 및 연속배양에서 탐체의 표변에 접 착한 후 6시간 후에 확산하며 생육하였다. 유가식 배양에셔 서1포농도와 최대 scu-P A 농도는 각각 $9.1{\times}10^5$ cells/m!와 1790 IU/ml이였고 연속배양에 셔는 각각 $2.5{\times}10^6$ cells/m]와 1820 IU/ml로 나타났다. 또한 저혈청배지에셔는 scu-P A가 tc-P A로의 전환율이 10% 정도로 높은데 비교하여 무혈 청배지에서는 전환율이 4.8%로 낮은 것으로 나타났 다. 이처럼 무혈청배지는 저혈청배지에셔보다 낮은 tc-P A로의 전환율을 유지할 수 있는 이점이 었다.

호르몬의 첨가가 돼지난포란의 체외성숙에 미치는 영향 (Effect of Hormone on In vitro Maturation of Porcine Follicular Oocytes)

  • This study was conducted to investigate the effect of hormones on in vitro maturation of porcine follicular oocytes. The results obtained were as follows : 1. The maturation rates of oocytes cultured in medium containing FSH 1, 10, 50 and 100$\mu\textrm{g}$/ml were 44.6, 58.5, 42.6 and 37.9%, respectively. And those were no difference to maturation rates among the medium containing FSH. However, the maturation rate(13.7%) of oocytes cultured without FSH was significantly(P<0.05) lower than those of oocytes cultured with FSH. 2. The maturation rates of oocytes cultured in medium containing hCG 0, 1, 10, 50 and 100IU/ml were 12.2, 11.9, 17.9, 21.9 and 45.6%, respectively. The maturation rate of oocytes cultured with 100IU/ml hCG was significantly(P<0.05) higher than those of oocytes cultured with 0~50IU/ml hCG concentrations. 3. The maturation rates of oocytes cultured in medium containing E2 0, 1, 10, 50 and 100$\mu\textrm{g}$/ml were 13.7, 10.5, 14.9, 11.4 and 5.9%, respectively. There were no differences to maturation rates among the E2 concentrations. 4. The FSH+hCG treatment was the highest maturation rate in medium containing different combination of FSH, hCG and E2.

대농갱이 인공종묘생산

  • 대농갱이는 체형이 동자개와 비슷하나 지금까지 인공부화는 물론 산란습성에 관해서도 잘 알려져 있지 않은 미개발 어종으로 양식품종의 다양화 및 경쟁력 있는 새로운 품종 개발을 위하여 인공종묘생산 시험을 시도하였다. 본 연구에 사용한 대농갱이 친어는 금강 중류역에서 서식하고 있는 자연산 친어로 암컷 210마리, 수컷 148마리를 사용하였다. 호르몬주사에 의한 인공 배란효과 조사를 위해 암컷 한 마리당 HCG 1,000 IU 주사군(75마리)과 2,000 IU 주사군(135마리)으로 나누어 배란율을 조사하였으며, 수컷은 주사하지 않고 정소를 적출하여 막자사발에 갈아서 등조액과 혼합하여 사용했다. 수정 방법은 난을 착출하면서 정자 등조액을 동시에 흘리는 방법, 난을 그릇에 받은 후 정자 등조액을 혼합 수정하는 방법, 채란용기에 정자 등조액을 미리 넣고 암컷의 난을 짜 넣어 수정시키는 방법 등 3가지 방법을 사용하였다. 부화된 자어는 초기먹이로 알테미아(3일 급이), 실지렁이(6일 급이), 배합사료 순으로 급이하였으며 수온 25~28$^{\circ}C$에 유지관리하면서 성장도 시험을 실시하였다. 호르몬 주사에 의한 배란효과는 HCG 2,000 IU 주사군(채란율 88.9%)이 1,000 IU 주사군(37.3%)에 비해 월등히 높았으며 난의 성숙상태도 좋았다. 수정방법에 있어서는 채란과 동시에 정자 등조액을 흘려 수정시키는 방법과 채란용기에 미리 정자 등조액을 넣고 암컷의 난을 한 마리씩 짜서 수정한 후 바로 부화지로 이동하여 고르게 뿌려 준 경우가 수정율 및 부화율이 좋았다. 부화수온은 23$\pm$1$^{\circ}C$에서 96시간, 26$\pm$1$^{\circ}C$에서는 72시간만에 부화되었으며 부화여건이 좋으면 단시간에 부화할 수 있도록 하는 것이 수생균 발생을 억제할 수 있어서 효과적이었다. 자치어의 성장도 실험에서는 사육수온 25~28$^{\circ}C$에서 직선식으로 나타낼 수 있어서 평균체장은 Y=0.9943X+0.7867, 평균체중은 Y=0.8800-0.6800로 각각 표시되었다.

시판 우유중의 잔류 Penicillin에 관한 실험연구 (A Study on Penicillin Residue In Market Milk in Seoul)

  • The study Was carried out to determine the residue concentrations of peniccillin in Merket milk samples collected from some of the retail shops located in Seoul City. During the period from June to September 1974, 160 milk samples were collected and examined, using the Filter Paper Disk Method, for Screening test and Quantitative test of penicillin residue. The results obtained in the study were as follows: 1. Among the 160 Market Milk samples. 28 samples (17.5%) proved to contain some antibiotics residue. 2. In the case of the antibiotics residue positive milk samples, the rate of penicillin residue alone was 46.4% and the rate of the other antibiotics was 53.6%. 3. In the distribution of milk plants examined in relation-to the antibiotics residue positive milk samples, it was shown that S.K.H. and M.Market Milk plants had 15.0, 17.5, 20.0 and 17.5 percent respectively. 4. According to the monthly distribution, the highest antibiotics residue positive rate (25.0% of the samples) was shown in the samples collected in August, and the next (20.0% of the samples) in July. While the samples collected in June and September 1974 Showed lower rates of 7.5% and 17.5% respectively. 5. The range of the residue concentraction of penicillin in the positive milk samples were as follows: a. 53.8% of the total samples were found in the lowest range of 0.02-0.05 IU/ml. b. 30.8% of the total samples were found in the lower range of 0.05-0.1 IU/ml. c. 7.7% of the total samlpes were found in the higher range of 0.1-0.2 IU/ml. d. 7.7% of the total samples were found in the highest range of over 0.2 IU/ml.
