Short Bowel Syndrome as the Leading Cause of Intestinal Failure in Early Life: Some Insights into the Management

  • Intestinal failure (IF) is the critical reduction of the gut mass or its function below the minimum needed to absorb nutrients and fluids required for adequate growth in children. Severe IF requires parenteral nutrition (PN). Pediatric IF is most commonly due to congenital or neonatal intestinal diseases or malformations divided into 3 groups: 1) reduced intestinal length and consequently reduced absorptive surface, such as in short bowel syndrome (SBS) or extensive aganglionosis; 2) abnormal development of the intestinal mucosa such as congenital diseases of enterocyte development; 3) extensive motility dysfunction such as chronic intestinal pseudo-obstruction syndromes. The leading cause of IF in childhood is the SBS. In clinical practice the degree of IF may be indirectly measured by the level of PN required for normal or catch up growth. Other indicators such as serum citrulline have not proven to be highly reliable prognostic factors in children. The last decades have allowed the development of highly sophisticated nutrient solutions consisting of optimal combinations of macronutrients and micronutrients as well as guidelines, promoting PN as a safe and efficient feeding technique. However, IF that requires long-term PN may be associated with various complications including infections, growth failure, metabolic disorders, and bone disease. IF Associated Liver Disease may be a limiting factor. However, changes in the global management of IF pediatric patients, especially since the setup of intestinal rehabilitation centres did change the prognosis thus limiting "nutritional failure" which is considered as a major indication for intestinal transplantation (ITx) or combined liver-ITx.

유연다물체동적해석을 이용한 무도상교량 침목패드의 최적 강성 산정 (Optimum Stiffness of the Sleeper Pad on an Open-Deck Steel Railway Bridge using Flexible Multibody Dynamic Analysis)

  • 레일장대화는 무도상교량의 소음, 진동, 충격 등의 문제점을 해결할 수 있는 경제적인 방안 중 하나이며, 최근 연동식 침목고정장치를 이용한 SSF 공법이 개발된 바 있다. 이 연구에서는 연동식 침목고정장치 적용 시 레일 높이 조정 및 열차 통과 시의 충격 흡수를 목적으로 교량침목 하부에 삽입되는 침목패드의 최적 연직강성을 결정하는 과정을 제시하였다. 침목패드의 최적 연직강성 결정을 위하여 관련 기존 기준을 검토하였으며, 유연다물체동적해석을 통하여 침목패드의 연직강성 변화에 따른 주행안전성, 승차감 및 궤도의 안전성에 대한 지표들과 교량 응답 변화를 검토하였다. 유연다물체동적해석은 상용프로그램인 ABAQUS와 VI-Rail을 이용하여 수행하였다. 수치해석은 30m 상로판형교에 대한 교량모델을 이용하여 수행하였으며, 침목패드의 연직강성이 7.5kN/mm ~ 240kN/mm로 변화할 때 ITX 새마을, KTX 및 화차 통과 시의 응답을 산정하였다. 수치해석에 적용된 궤도구성품 조건에서 침목패드의 최적 강성은 100kN/mm로 산정되었다.

Analysis of Business Performance of Local SMEs Based on Various Alternative Information and Corporate SCORE Index

  • Purpose: The purpose of this study is to compare and analyze the enterprise's score index calculated from atypical data and corrected data. Research design, data, and methodology: In this study, news articles which are non-financial information but qualitative data were collected from 2,432 SMEs that has been extracted "square proportional stratification" out of 18,910 enterprises with fixed data and compared/analyzed each enterprise's score index through text mining analysis methodology. Result: The analysis showed that qualitative data can be quantitatively evaluated by region, industry and period by collecting news from SMEs, and that there are concerns that it could be an element of alternative credit evaluation. Conclusion: News data cannot be collected even if one of the small businesses is self-employed or small businesses has little or no news coverage. Data normalization or standardization should be considered to overcome the difference in scores due to the amount of reference. Furthermore, since keyword sentiment analysis may have different results depending on the researcher's point of view, it is also necessary to consider deep learning sentiment analysis, which is conducted by sentence.

시계열모형을 활용한 춘천시 강촌역 단기수송수요 예측 (Forecasting short-term transportation demand at Gangchon Station in Chuncheon-si using time series model)

  • Purpose - This study attempted to predict short-term transportation demand using trains and getting off at Gangchon Station. Through this, we present numerical data necessary for future tourist inflow policies in the Gangchon area of Chuncheon and present related implications. Design/methodology/approach - This study collected and analyzed transportation demand data from Gangchon Station using the Gyeongchun Line and ITX-Cheongchun Train from January 2014 to August 2023. Winters exponential smoothing model and ARIMA model were used to reflect the trend and seasonality of the raw data. Findings - First, transportation demand using trains to get off at Gangchon Station in Chuncheon City is expected to show a continuous increase from 2020 until the forecast period is 2024. Second, the number of passengers getting off at Gangchon Station was found to be highest in May and October. Research implications or Originality - As transportation networks are improving nationwide and people's leisure culture is changing, the number of tourists visiting the Gangchon area in Chuncheon City is continuously decreasing. Therefore, in this study, a time series model was used to predict short-term transportation demand alighting at Gangchon Station. In order to calculate more accurate forecasts, we compared models to find an appropriate model and presented forecasts.

철도 승강장 소음의 청감실반응평가모형에 대한 적합도 검정 (Classification Accuracy Test of Hearing Laboratory Test Models for Railway Noise at Station Platform)

  • A statistical annoyance model to railway noise at platform was proposed by jury evaluation test performed in hearing laboratory. ITX-Saemaeul and Mugunghwa were chosen as the noise sources of the test, and announcement sound was included to simulate real situation. Logistic regression analysis produced %HALAB curve. Hosmer-Lemeshow test and classification accuracy test were used to verify the model's statistical significance. It was shown that the model which was generated from relatively small number of samples is statistically significant.

대중교통수단 객실 내 휘발성유기화합물류 거동 특성 (Study on the Movement of Volatile Organic Compounds in Public Transportation)

  • Objective: This study is aimed at investigating indoor air quality on public transportation (subway, train, and bus) according to changes in season and time. Methods: We evaluated TVOC and HCHO on public transportation based on the un-controlled parameters of the Ministry of Environment. We also measured temperature and humidity since they affect the concentration of TVOC and HCHO. For public transportation classification, subway lines were classified into Lines 1 to 4. Additionally, trains were classified as ITX and KTX. Results: When comparing summer and winter on public transportation, the concentrations of TVOC and HCHO did not show any particular tendency. However, the concentrations of TVOC and HCHO during traffic congestion was higher than levels during times of non-congestion on most public transportation. In summer and winter, the measurement results for temperature and humidity showed a normal range, so temperature and humidity did not affect the concentrations of TVOC and HCHO. In the case of TVOC, TVOC concentrations on new trains were found to be relatively higher than on older ones, but there was no statistically correlation. Conclusions: A survey was conducted on the indoor air quality on public transportation. This study also analyzed data based on TVOC and HCHO for designing policies and managing indoor air quality.

KTX 물금역 정차 확정이 양산시 철도 이용자에게 미치는 영향에 관한 연구 (A Study of the Effect of the KTX Mulgeum Station Stop on Railroad Users in Yangsan City)

  • 본 연구는 금번 일반철도가 정차하는 물금역이 KTX 정차역으로 확정됨에 따라 변화된 KTDB와 사회경제 지표를 반영하여 달라지는 교통 환경과 경제성 효과를 예측하는 데 그 목적이 있다. 본 연구의 자료를 분석하기 위해 전국 및 양산시를 중심으로 관련 통계자료를 참고한 사회경제 지표 및 교통시설 운영의 일반현황을 조사하였다. 또한 국토개발 및 교통망 구축에 관련된 국가 상위계획인 철도시설 건설계획 및 열차 운영계획에 대해서도 조사하고 참고하였다. 현재 양산시 물금역에 정차하는 일반철도는 ITX(4회/일)와 무궁화호(29회/1일) 뿐이어서 양산지역 열차승객은 고속철도를 이용할 수 없는 실정이다. 특히 양산지역 철도 이용객들은 KTX를 이용하기 위해서는 울산역까지 40분과 구포역까지 30분을 이동해야 하는 교통 불편을 가지고 있어 KTX 물금역 정차 필요성이 지속적으로 제기되어 왔다. 이번에 KTX 물금역 정차가 확정됨에 따라 장래 변화되는 철도교통 수요를 분석한 결과, 물금역 승하차는 2025년 기준 1,674인/일인 것으로 예측되었다. 또한, 물금역 정차로 인해 울산역과 구포역에서 전환되는 열차승객 수요는 2025년 기준 승차 승객은 594인/일, 하차 승객은 562인/일으로 예측되었다. 향후 양산 시민이 KTX 물금역을 이용할 경우, 물금역 접근 시간은 22분~65분까지 단축이 가능하다. 또한 철도 간(일반철도→고속철도) 환승의 불편함이 해소될 뿐만 아니라 환승 대기시간은 최대 40분까지 시간절감의 효과가 있는 것으로 예측되었다. 따라서 KTX 물금역 정차로 인한 경제성 효과는 일반철도 운영비 미고려 시 B/C는 1.823로 나타났고, 일반철도 운영비 고려 시 B/C는 2.127로 분석되었다. 결론적으로, KTX를 이용하여 수도권을 방문할 경우, 울산역이나 구포역을 이용하지 않고 물금역을 이용하면 2시간 43분이 소요되어 통행시간 단축 및 경제성 평가에서 효과가 매우 클 것으로 분석되었으며, 향후 양산시는 고속철도 이용의 오지란 불명예에서 벗어나 수도권까지의 접근성과 이동성을 보다 향상시킬 수 있을 것으로 기대되어진다.
