Effect of bitter melon (Momordica Charantia) on anti-diabetic activity in C57BLI/6J db/db mice (C57BL/6J db/db생쥐에서 여주 (Momordica Charantia)의 항당뇨 효과)

  • Many herbal extracts have been reported to have a preventive or therapeutic effect of on diabetes mellitus. Momordica Charantia commonly known as bitter melon or karela has been reported to be a medicinal plant for treating various diseases including cancers and diabetes. The objectives of this study were to investigate anti-diabetic effects of bitter melon (BM) as determined by blood glucose levels, glucose tolerance test (GTT), insulin tolerance test (ITT), insulin and HbA1C activities in serum, serum biochemical and lipid levels, histopathology, immunohistochemistry and AMPK-${\alpha}2$ expression of skeletal muscle in male C57BL/6J db/db mice. There were four experimental groups including vehicle control, BM 10 mg/kg, BM 50 mg/kg, and BM 250 mg/kg. BM at doses of 10, 50, and 250 mg/kg was orally administered to the diabetic mice everyday for 8 weeks. The treatments of BM 10, 50, and 250 mg/kg significantly decreased the blood glucose level in the diabetic mice compared with vehicle control (p < 0.05). The treatments of BM 10 and 50 mg/kg significantly decreased the GTT, ITT and HbA1c levels in the diabetic mice compared with vehicle control (p < 0.05). All BM groups significantly decreased GOT, GPT, BUN, LDL and glucose levels in the diabetic mice compared with the vehicle control mice (p < 0.05). The livers of mice treated with the BM 10, 50, and 250 mg/kg showed a remarkable decrease in the number of lipid droplets compared with the vehicle control. The pancreas of mice treated with the BM 10, 50, and 250 mg/kg showed a remarkable increase in insulin concentration of ${\beta}$-cells compared with the vehicle control. In addition, the treatments of BM 10, 50, and 250 mg/kg actually increased the expression of AMPK-${\alpha}2$ compared with vehicle control. These results suggest that BM has a respectable anti-diabetic effect resulting from inhibition of blood glucose level and lipid level in serum and that consumption of BM may give a benefit for controlling diabetes mellitus in humans.

Risk of Breast Cancer and Total Malignancies in Rheumatoid Arthritis Patients Undergoing TNF-α Antagonist Therapy: a Meta-analysis of Randomized Control Trials

  • Context: Interest exits in whether TNF-alpha antagonists increase the risk of breast cancer and total malignancies in patients with rheumatoid arthritis (RA). Objectives: To analyze the risk of malignancies, especially breast cancer, in patients with RA enrolled in randomized control trials (RCTs). Methods: A systematic literature search for RCTs from 1 January 1998 to 1 July 2013 from online databases, such as PubMed, WILEY, EMBASE, ISI web of knowledge and Cochrane Library was conducted. Studies included RCTs that compared the safety of at least one dose of the five TNF-${\alpha}$ antagonists with placebo or methotrexate (MTX) (or TNF-${\alpha}$ antagonists plus MTX vs placebo plus MTX) in RA patients for more than 24 weeks and imported all the references into document management software EndNote${\times}6$. Two independent reviewers selected studies and extracted the data about study design, patients' characteristics and the type, number of all malignancies. Results: 28 RCTs from 34 records with 11,741 patients were analyzed. Of the total, 97 developed at least one malignancy during the double-blind trials, and breast cancer was observed in 17 patients (17.5% of total malignancies). However, there was no statistically significant increased risk observed in either the per protocol (PP) model (OR 0.65, 95%CI [0.22, 1.93]) or the modified intention to treat (mITT) model (OR 0.75, 95%CI [0.25, 2.21]). There were also no significant trend for increased risk of total malignancies on anti-TNF-${\alpha}$ therapy administered at approved doses in either model (OR, 1.06, 95%CI [0.64, 1.75], and OR, 1.30, 95%CI [0.80, 2.14], respectively). As to the two models, modified intention to treat model analysis led to higher estimation than per protocol model analysis. Conclusions: This study did not find a significantly increased risk of breast cancer and total malignancies in adults RA patients treated with TNF-${\alpha}$ antagonists at approved doses. However, it cannot be ignored that more patients developed malignancies with TNF-${\alpha}$ antagonists therapy compared with patients with placebo or MTX, in spite of the lack of statistical significance, so that more strict clinical trials and long-term follow-up are needed, and both mITT and PP analyses should be used in such safety analyses.

  • How to handle missing data is a main issue in clinical trials. We impute missing data based on missing data that follows a mechanism according to the intention-to-treat rule. However, using the right imputation method for missing data is very important because this supposition is unclear. We suggest a new imputation method for missing data using agreement and maintenance introduced by Kang and Kim (1997). We give an example and adapt a Monte Carlo simulation to compare the performance between the established method and the suggested method.

Development of loop-mediated isothermal amplification method for the rapid and sensitive detection of bovine tuberculosis in Korea native cattle (한우 결핵의 신속 감별진단을 위한 등온증폭법 개발)

  • Loop-mediated isothermal amplification (LAMP) was developed to detect Mycobacterium tuberculosis complex (MTC) and non-tuberculous mycobacterium (NTM) genomic DNA in blood samples of Korea native cattle. A set of four primers, two outer and two inner, were designed from M. bovis and M. avium genomic DNA targeting the IS6110 and 16S rRNA gene, respectively. Based on 85 Intradermal Tuberculin Test (ITT) positive blood sample and using conventional PCR and LAMP, the agreement quotient (kappa), which measures agreement beyond chance were 0.93 (conventional PCR) and 0.97 (LAMP), respectively. The detection limit of the LAMP method was $2.0{\times}10^2$ copy/ml M. bovis and M. avium cells, compared to $2.0{\times}10^3$ copy/ml M. bovis and M. avium cells for conventional PCR. These results suggest that the LAMP is a powerful tool for rapid, sensitive, and practical detection of MTC and NTM in blood samples of Korea native cattle.

  • Busan Port is being operated inefficiently by a number of container terminal operators, which are dispersed in small groups. In particular, the small terminal at Busan Port is causing the dispersion of quantity, the increase of ITT cargo and the occurrence of a ship waiting at sea as the ship's Alliance became larger in 2016. As a result, studies such as integrated operations for the larger container terminals are being conducted. Therefore, in this study, we intend to set up a type of integrated operation for terminals at the new port of Busan and measure the practical effects through simulation to present an ideal direction for integrated operation.

The Therapeutic Effects of Nizatidine in Gastroesophageal Disease with Laryngopharyngeal Reflux Symptoms: Observational Study (위식도 역류성 질환 관련 인후두성 역류(Laryngopharyngeal Reflux : LPR)증상을 호소하는 환자에서의 니자티딘의 치료효과 연구)

  • Larygopharyngeal reflux(LPR) is one form of the Gastroesophageal Reflux Diseases(GERD). It is known to cause various kinds of otolaryngologic symptoms such as hoarseness, globus sensation in throat, chronic throat clearing, and chronic cough, Disease entities diagnosed by otolaryngologists as posterior laryngitis, globus pharyngeus and reflux laryngitis should be suspected as LPR-related diseases. The nizatidine(AXID), as a Histamine H2-receptor antagonist, reduces gastric acid secretion and improves gastric motility function. Objectives : The effect of nizatidine using 150mg b.i.d was evaluated for symptom relief and improvement of laryngoscopic findings in patients with LPR. Materials and Methods : In 30 multicenter, observational trial performed nationalwidely in Korea. 308 patients with LPR symptom were observed to evaluate their symptoms and larygnoscopic findings after 4weeks, 8weeks, 12weeks of treatment with nizatidine. Results : The symptoms of LPR including globus sensation, chronic throat clearing and hoarseness, are reduced significantly after 4 weeks, 8weeks, and 12weeks of treatment(p<0.05). The laryngoscopic findings including diffuse erythema, edema and granulation are improved after nizatidine treatment(p<0.05). and the efficacy of nizatidine on LPR-related sympoms after 4 weeks is 88.6%, and those of after 8 weeks and 12weeks were 92.6%, and 99.1% in ITT(Intent To Treatment) group(p<0.05). And PPA(Per Protocol Analysis)group showed 93.7%, 97.3%, and 99.1% of efficacy after 4, 8, and 12 weeks of nizatidine treatment(p<0.05). Conclusion : These results indicate that in patient with LPR, nizatidine 150mg b.i.d treatment very effectively reduces LPR symptoms and improves laryngoscopic findings as well as reduces gastric acid secretion and improves gastric motility function.

Comparative antidiabetic activity of different fractions of Enicostemma littorale Blume in streptozotocin induced NIDDM rats

  • Aqueous extract of Enicostemma littorale is reported to have antidiabetic activity. In the present investigation, we studied the effect of aqueous extract of E. littorale and its different fractions i.e., toluene, chloroform, ethyl acetate, n-butanol fractions and remaining residual fraction in streptozotocin (STZ)-induced neonatal type 2 diabetic rats. Fasting glucose and insulin levels in NIDDM were significantly (P<0.05) higher than control rats and they were significantly decreased by treatment with aqueous extract of E. littorale and its n-butanol and ethyl acetate fractions. Results of oral glucose tolerance test (OGTT) showed that aqueous extract and its n-butanol and ethyl acetate fractions significantly (P<0.05) decrease both $AUC_{glucose}$ and $AUC_{insulin}$ values in NIDDM treated groups. Insulin sensitivity $(K_{ITT})$ index of NIDDM control was significantly lower as compared to normal control and this was significantly (P<0.05) increased after treatment with aqueous extract, its n-butanol and ethyl acetate fractions. Treatment with aqueous extract of E. littorale and its n-butanol and ethyl acetate fractions lowered the elevated cholesterol and triglyceride levels observed in NIDDM rats. Treatment with aqueous extract of E. littorale and its n-butanol fraction showed significant decrease in creatinine, urea, SGPT and SGOT levels as compared to NIDDM control rats. However ethyl acetate fraction showed significant changes only in creatinine and SGOT levels, and not in the levels of urea, and SGPT as compared to NIDDM control rats. Treatment with toluene, chloroform and residual fractions of E. littorale did not produce any effect on glucose, insulin, triglyceride, cholesterol, creatinine, urea, SGPT or SGOT levels as compared to NIDDM control rats. Our data suggest that n-butanol and ethyl acetate fractions contain the active compounds which may be responsible for the above activity and associated complications in NIDDM diabetes mellitus.

High Efficacy of Levofloxacin-Dexlansoprazole-Based Quadruple Therapy as a First Line Treatment for Helicobacter pylori Eradication in Thailand

  • Background: Levofloxacin is an effective medication for second line Helicobacter pylori (H. pylori) eradication. However, limited studies have approved its use as an effective antibiotic in first line therapy. Dexlansoprazole is a new PPI and lacks of evidence in support of a role in H. pylori eradication. This study was designed to evaluate efficacy of levofloxacin-dexlansoprazole-based quadruple therapy for H. pylori eradication in Thailand. Materials and Methods: This prospective randomized control study was performed during June 2014 to December 2014. H. pylori infected gastritis patients were randomized to receive 7- or 14-day levofloxacin-dexlansoprazole based on quadruple therapy (levofloxacin 500 mg OD, dexlansoprazole 60 mg bid, clarithromycin MR 1000 mg OD, bismuth subsalicylate 1048 mg bid). CYP2C19 genotyping and antibiotic susceptibility tests were conducted for all patients. A 13C urea breath test was performed to confirm H. pylori eradication at least 4 weeks after treatment. Results: A total of 100 patients were enrolled, comprising 44 males and 56 females (mean age of 52.6 years). Eradication rate by PP analysis was 85.7% (42/49) with the 7-day regimen and 98% (48/49) with the 14-day regimen (85.7% vs 98%; p-value=0.059). ITT analysis was 84% and 96% with 7- and 14-day regimens, respectively (84% vs 96%; p-value=0.092). Antibiotic susceptibility testing demonstrated 35.1% resistance to metronidazole, 18.3% to clarithromycin, and 13.5% to levofloxacin. CYP2C19 genotyping revealed 54.1% RM, 34.7% IM and 11.2% PM. The 14-day regimen provided 100% eradication in patients with clarithromycin or dual clarithromycin and metronidazole H. pylori resistant strains. Moreover, the eradication rate was 96.6% in patients with CYP2C19 genotype RM. Conclusions: The 14-day levofloxacin-dexlansoprazole based quadruple therapy provides high H. pylori eradication regardless of CYP2C19 genotype, clarithromycin or dual clarithromycin and metronidazole resistant strains. This regimen could be use as an alternative first line therapy for H. pylori eradication in Thailand.

  • Antibiotic resistance of bacteria in the biofilm mode of growth contributes to the chronicity of infection and disease. The penetration of antibiotic, through biofilm developed in an itt vitro model system was investigated. Antibiotic resistant bacteria (E. coli) were obtained from Culture Collection of Antibiotic Resistant Microbes. Ca-alginate bead used as simulated biofilm and a cell entrapment test using compressed air were experiment for the improvement cell viability. Antibiotic susceptibilities though biofilms was measured by assaying the concentration of antibiotic that diffused through the biofilm to minimal inhibition concentration (MIC). Survival of immobilized cells were reduced as compared to free cells. In case of antibiotic susceptible E. coli reduced continuously, but antibiotic resistant E. coli kept up survival rate constantly. Survival was showed after exposed to the antibiotics that the more treated antibiotic resistant E. coli and low concentration of antibiotics) the more survived.

  • This paper describes the plan and procedure of a development test and evaluation that will be performed to verify the performance and technology of multi-mission unmanned surface vehicles (MMUSVs). In order to verify the design requirement of MMUSVs, we designed and manufactured the common platform of MMUSVs, which have an overall length of8.4 m, a displacement of 3,100 kg, and a speed of more than35 kts. The platform is equipped with several sub-systems, including radar and an EOTS/IRS. The EOTS/IRS, along with the search radar, is used for effective detection, identification, and targeting. The core technologies of MMUSV for DT&E will be investigated. The common platform design technologies, remote operating and control system technologies, autonomous navigation technologies, and unmanned operational technology of sensors and equipment will be studied for the development of the MMUSV's core technologies. The system will be able to make precise observations and track targets both manually and automatically during day and night conditions. Currently, the verification tests for each of the technologies and for the integrated system are in the pipeline for DT&E, which will be performed next year. Also, software reliability and life tests will be performed.
