10년간(1993~2002) Mycoplasma 폐렴의 역학적 양상과 최근 5년간 Mycoplasma 폐렴의 임상소견에 대한 고찰 (Epidemiological Pattern of Mycoplasma pneumoniae Pneumonia from 1993 Through 2002 and Clinical Characteristics during Recent Five Years)

  • 목 적 : 본 연구는 10년 동안 M. pneumoniae 폐렴의 역학적 양상과 최근 5년간의 임상증상 및 징후들을 조사하여 그 이전의 유행시기와 임상소견과 비교하여 어떠한 변화가 있는지를 알아보고자 하였다. 방 법 : 1993년 1월부터 2002년 12월까지 한일병원에 입원하여 M. pneumoniae 폐렴으로 진단받은 682례 환아의 역학적 양상과, 1998년 1월부터 2002년 12월까지 최근 5년간 M. pneumoniae 폐렴으로 입원한 328명의 환자의 임상증상 및 징후를 의무기록을 후향적으로 조사하였다. 결 과 : M. pneumoniae 폐렴이 발생한 연령은 4~6세의 환아에서 가장 많았고(286례; 41.9%) 남녀 비는 1 : 1.1로 나타났다. 월별분포를 보면 8월부터 12월 사이에 가장 많은 발생률을 보였고 특히 10월(106례; 15.5%)과 11월(119례; 17.4%) 그리고 12월(96례; 14.1%)에 가장 많은 분포를 보였다. 임상증상은 기침(660례; 96.8%), 발열(569례; 83.4%), 객담(522례; 76.5%)순으로 관찰되었고 그 외 구토, 콧물, 인후통, 복통, 호흡곤란, 두통, 권태 순으로 관찰되었다. 말초혈액 백혈구 수는 31.2%에서 기준치보다 증가되어 있었으며 적혈구 침강속도는 환아의 42.5%에서 20 mm/hr 이상 증가되어 있었으며 C-반응성 단백은 37.8%에서 양성이었다. 흉부 방사선 소견상 682례 중 557례(81.7%)에서 폐침윤이 관찰되었고 폐침윤의 양상은 기관지폐렴의 양상을 보인 경우는 78.0%이며, 대엽성 폐렴은 35.5%, 소엽성 폐렴은 19.2%, 그리고 간질성 폐렴은 28.7%에서 관찰되었다. 합병증으로는 부비동염이 41례에서 합병되었고 중이염이 23례에서 흉막삼출이 19례에서 경부 림프절염이 18례에서 그리고 신우신염이 1례에서 관찰되었다. 집중 발생을 보인 호발 연도를 알아보기 위하여 매년 소아과 호흡기 감염 입원환자 수에 대한 M. pneumoniae 폐렴환자 수의 연도별 비율 분포를 조사한 결과 1993년 19.1%, 1994년 13.0%, 1995년 5.6%, 1996년 12.8%, 1997년 18.6%, 1998년 22.6%, 1999년 1.1%, 2000년 13.3%, 2001년 9.1%, 2002년 6.0%, 그리고 2003년 19.9%로 1993년, 1997년, 1998년, 2000년, 그리고 2003년도에 집중 발생을 보이며 3~4년간의 유행주기를 보였다. 결 론 : 1996년 이전의 연구에서는 M. pneumoniae 폐렴의 유행시기가 3년의 주기를 보였고 주로 하절기에 유행을 보였던 반면, 1997년부터 2002년까지의 조사에서는 3~4년간의 집중발생시기와 주로 10월과 11월에 정점을 이루는 월별분포를 보였다.

Effects of Cordyceps Militaris Extract on Tumor Immunity

  • Background and Aims : Even though various strategies for cancer treatment have advanced with the remarkable development of genomic information and technology, it is far from giving relief to cancer patients. Recently there is accumulating evidence that the immune system is closely connected to anti-tumor defense mechanisms in a multistage process. This includes tumorigenesis, invasion, growth and metastasis. Cordyceps Militaris, a well-known oriental herbal medicine, is a parasitic fungus that has been used as an immune enhancing agent for a long period of time. However, little is known about the cancer-related immunomodulatory effects and anti-tumor activities. In the present study, we aimed to investigate the effects of Cordyceps Militaris extract (CME) on immune modulating and anti-tumor activity. Materials and Methods : To elucidate the effects of CME on macrophage and natural killer (NK) cell activity, we analyzed nitric oxide (NO) production, NK cytotoxicity and gene expression of cytokines related with macrophages and NK cell activity. Results and Conclusions : CME activated and promoted macrophage production of NO. It also enhanced gene expression of IL-1 and iNOS in RAW 264.7 cells. CME promoted cytotoxicity of NK cells against YAC-1 cells and enhanced NK cell related gene expression such as IL-1, IL-2, IL-12, iNOS, IFN-${\gamma}$ and TNF-${\alpha}$ in mice splenocytes. It also Promoted protein expression of IL-10, IL-12, IFN-${\gamma}$ and TNF-${\alpha}$ in mice splenocytes and inhibited lung tumor metastasis induced by CT-26 cell line compared with the control group. From these results, it could be concluded that CME is an effective herbal drug for modulating the immune system and anti-cancer treatment by promoting macrophage and NK cell activity.

Induction of Unique STAT Heterodimers by IL-21 Provokes IL-1RI Expression on CD8+ T Cells, Resulting in Enhanced IL-1β Dependent Effector Function

  • IL-1β plays critical roles in the priming and effector phases of immune responses such as the differentiation, commitment, and memory formation of T cells. In this context, several reports have suggested that the IL-1β signal is crucial for CTL-mediated immune responses to viral infections and tumors. However, little is known regarding whether IL-1β acts directly on CD8+ T cells and what the molecular mechanisms underlying expression of IL-1 receptors (IL-1Rs) on CD8+ T cells and features of IL-1R+ CD8+ T cells are. Here, we provide evidence that the expression of IL-1R type I (IL-1RI), the functional receptor of IL-1β, is preferentially induced by IL-21 on TCR-stimulated CD8+ T cells. Further, IL-1β enhances the effector function of CD8+ T cells expressing IL-21-induced IL-1RI by increasing cytokine production and release of cytotoxic granules containing granzyme B. The IL-21-IL-1RI-IL-1β axis is involved in an augmented effector function through regulation of transcription factors BATF, Blimp-1, and IRF4. Moreover, this axis confers a unique effector function to CD8+ T cells compared to conventional type 1 cytotoxic T cells differentiated with IL-12. Chemical inhibitor and immunoprecipitation assay demonstrated that IL-21 induces a unique pattern of STAT activation with the formation of both STAT1:STAT3 and STAT3:STAT5 heterodimers, which are critical for the induction of IL-1RI on TCR-stimulated CD8+ T cells. Taken together, we propose that induction of a novel subset of IL-1RI-expressing CD8+ T cells by IL-21 may be beneficial to the protective immune response against viral infections and is therefore important to consider for vaccine design.

Sinensetin Inhibits Interleukin-6 in Human Mast Cell - 1 Via Signal Transducers and Activators of the Transcription 3 (STAT3) and Nuclear Factor Kappa B (NF-κB) Pathways

  • Sinensetin, a pentamethoxyflavone, is known to exert various pharmacological activities including anti-angiogenesis, anti-diabetic and anti-inflammatory activities. However, its effects on the human mast cell - 1 (HMC-1) mediated inflammatory mechanism remain unknown. To explore the mediator and cellular inflammatory response of sinensetin, we examined its influence on phorbol 12-myristate 13-acetate (PMA) plus A23187 induced inflammatory mediator production in a human mast cell line. In this study, interleukin (IL)-6 production was measured using the enzyme-linked immunosorbent assay and reverse transcription polymerase chain reaction. Sinensetin inhibited PMA plus A23187 induced IL-6 production in a dose-dependent manner as well as IL-4, IL-5 and IL-8 mRNA expression. Furthermore, sinensetin inhibited signal transducer and activator of transcription 3 (STAT3) phosphorylation, suggesting that sinensetin inhibits the production of inflammatory mediators by blocking STAT3 phosphorylation. Moreover, sinensetin was found to inhibit nuclear factor kappa B activation. These findings suggest that sinensetin may be involved in the regulation of mast cell-mediated inflammatory responses.

Ginseng alleviates microbial infections of the respiratory tract: a review

  • The detrimental impact of air pollution as a result of frequent exposure to fine particles posed a global public health risk mainly to the pulmonary disorders in pediatric and geriatric population. Here, we reviewed the current literature regarding the role of ginseng and/or its components as antimicrobials, especially against pathogens that cause respiratory infections in animal and in vitro models. Some of the possible mechanisms for ginseng-mediated viral inhibition suggested are improvements in systemic and mucosa-specific antibody responses, serum hemagglutinin inhibition, lymphocyte proliferation, cell survival rate, and viral clearance in the lungs. In addition, ginseng reduces the expression levels of proinflammatory cytokines (IFN-γ, TNF-α, IL-2, IL-4, IL-5, IL-6, IL-8) and chemokines produced by airway epithelial cells and macrophages, thus preventing weight loss. In case of bacterial infections, ginseng acts by alleviating inflammatory cytokine production, increasing survival rates, and activating phagocytes and natural killer cells. In addition, ginseng inhibits biofilm formation and induces the dispersion and dissolution of mature biofilms. Most clinical trials revealed that ginseng, at various dosages, is a safe and effective method of seasonal prophylaxis, relieving the symptoms and reducing the risk and duration of colds and flu. Taken together, these findings support the efficacy of ginseng as a therapeutic and prophylactic agent for respiratory infections.

황금(黃芩) 발효 추출물이 LPS로 유발된 마우스대식세포에서 염증인자증가에 미치는 영향 (Effect of Fermented Scutellariae Radix Extract on Production of Inflammatory Mediator in LPS-stimulated Mouse Macrophages)

  • Objectives : This study aims at examining the immuno-modulating activity in the fermentative extract of the root of Scutellaria baicalensis Georgi (Scutellariae Radix) on the production of inflammatory mediator in LPS-stimulated RAW264.7 mouse macrophages. Method : Measurements were done for the influences on the cell viability, generation of hydrogen peroxide in cells and nitric oxide (NO) generation using the macrophage of mouse with the specimen SBS as the fermentative extract of Scutellariae Radix (SBS) with Saccharomyces cerevisiae STV89. Result : As a result of carrying out MTT assay to check the cellular toxicity of the fermentative extract of Scutellariae Radix, any excessive toxicity to the macrophage did not occur from treatments by concentration for SBS. SBS increased the generation of hydrogen peroxide in the macrophage. SBS suppressed the NO generated in macrophages and SBS concentration higher than $25{\mu}g/mL$ significantly suppressed the increased NO generated in LPS-stimulated macrophages. SBS concentration higher than $25{\mu}g/mL$ significantly suppressed the generation of IL-6, IL-10, IL-12p40 and MCP-1 in LPS-stimulated macrophages. Conclusion : Our findings indicate that SBS has an immuno-modulating activity in macrophage activation through suppressing the generation of inflammatory substances, NO, IL-6, IL-10, IL-12p40 and MCP-1.

The Alteration of Cytokine Expression and Goblet Cell Response by Cyclosporin A and Histamine Receptor Antagonists in C3H/HeN Mice Infected with Echinostoma hortense

  • Echinostoma hortense (E. hortense) is an intestinal trematode with the highest infection rate in South Korea. However, the immune response against E. hortense infection has not been explained well. In the present study, we investigated the effect of treatment with cyclosporin A (CsA) and histamine receptor antagonists on the cytokine expression and mucosal goblet cells in E. hortense-infected C3H/HeN mice. The alteration of cytokine mRNA expression ($TNF-{\alpha},\;IL-l{\beta},\;IL-4\;and\;IL-5$), intestinal worm recovery rate and goblet cell responses were measured weekly from 0 to 5 weeks post-infection (P.I.) in the control and the following three drug-treated groups: CsA, hydroxyzine and cimetidine. Compared with the control group, the expression of $TNF-{\alpha}$, IL-4 and IL-5 mRNAs decreased in the CsA- and hydroxyzine-treated groups, but only IL-4 mRNA expression did in the cimetidine-treated group. Worm recovery rate was significantly increased in the drug-treated groups. Mucosal goblet cells and their mucin response significantly decreased in the CsA-treated group (P<0.01), but significantly increased in the cimetidine- (P<0.05) and hydroxyzine- (P<0.01) treated groups. These data suggest that CsA treatment inhibits production of Th1- and Th2-type cytokines which are necessary for the worm expulsion. Histamine receptor increases goblet cells and their mucin activation, although it remains to be elucidated whether it directly affects the worm expulsion period of E. hortense in C3H/HeN mice.

  • Objectives : In this study, Samhwangsaejaegami extract was tested to prove its anti-atopic dermatitis effect on NC/Nga mice. Methods : In order to evaluate the external use effects of Samhwangsaejae extract on anti-atopic dermatitis, the expression of filaggrin, serine palmitoyltransferase(SPT), and COX-2 were analyzed. In vivo study, clinical skin severity score, IgE, IL-4, IL-5 and IL-6 level were analyzed through NC/Nga atopic mice model after 12 weeks external treatment. Results : In vitro study results showed the reduction in the expression of filaggrin, SPT, and COX-2. In vivo study results demonstrated the significant reduction in clinical skin severity score, IgE, IL-4, IL-5, IL-6 expression level. Conclusions : These results showed Samhwangsaejaegami extract can be a promising candidate for anti-atopic dermatitis treatment.

  • Purpose : Studies on the Regulatory Effect of Cytokine Production in Taumin Patients with Cerebral Infarction by Cheongsimyeonjatang Method : ELISA(enzyme-linked immuno-sorbent assay) Result : Chungsimyeunjatang(CYT) is a prescription for the cerebral infarction (CI) patients of Taeumin according to Sasang constitution philosophy. Taeumin patients with CI were treated with CYT during the acute stage. Clinical signs of CI disappeared markedly in about two to four weeks after oral administration of CYT in all patients. The mean interleukin (IL)-2 plasma levels were slightly lower in the patients with CI than in the normal groups, whereas the mean IL-4, IL-6 and IgE levels were significantly higher in the patients. There were no significant differences in $interferon-{\gamma}$ $(IFN-{\gamma})$ levels between the groups. Serum $IFN-{\gamma}$ and IL-2 levels derived from T helper (Th)1 cells were elevated significantly in the patients with CI by CYT administration. Significant reduced plasma levels of IL-4 and IL-6 derived from Th2 cells and IgE were observed in the patients treated with CYT. During the period of CYT administration, there were no other adverse effects. The data indicate that CYT has a good CI treatment effect, and that its action may be due to regulation of cytokine Production.
