Anti-Inflammatory Effects of Fermented Products with Avena sativa on RAW264.7 and HT-29 Cells via Inhibition of Inflammatory Mediators

  • This study investigated therapeutic candidates with anti-inflammatory potential among traditional dietary ingredients targeting inflammatory bowel disease (IBD). Both Avena sativa and traditional fermented products, such as Korean soy paste, are popular health foods. We investigated the anti-inflammatory effects of soy paste combined with A. sativa (KDA), compared with soy paste without A. sativa (KD) by evaluating the expression of pro-inflammatory cytokines in lipopolysaccharide-stimulated RAW264.7 mouse macrophages and HT-29 human colon epithelial cells. KDA significantly inhibited the production of nitric oxide (NO) and downregulated the pro-inflammatory cytokines, such as interleukin (IL)-1β, IL-6, and tumor necrosis factor-α in lipopolysaccharide (LPS)-induced RAW264.7 cells. In another in vitro experiment involving LPS-stimulated HT-29 cells, KDA suppressed the levels of IL-8, which is the chemokine elevated in IBD. In addition, KDA exhibited anti-oxidative properties, such as 2,2-diphenyl-1-picryl-hydrazyl-hydrate (DPPH) and 2,2'-azino-bis(3-ethylbenz-thiazoline-6-sulfonic acid) (ABTS) radical scavenging activity. Our findings revealed that A. sativa combined with soy paste exhibits a synergistic anti-inflammatory and anti-oxidant effect following fermentation. These results suggest that KDA may be used as a potential anti-inflammatory therapy against IBD.

  • Since the identification and characterization of toll-like receptors (TLR) in Drosophila, numerous scientific studies have examined the role of TLRs in host innate immunity. Recent studies have suggested a convergence of the nuclear factor kappa B (NF-${\kappa}B$) signaling and cytokine production regulated by the cytosolic elicitor known as NLRs (nucleotide-binding domain and leucine-rich repeat containing domain receptors) as a key modulator in inflammatory diseases. Among the NLRs, NOD1 and NOD2 have been intensively investigated for its role in inflammatory bowel disease (IBD). On the other hand, NLRs such as NLRP3, NLRP1, and NLRC4 (also known as IPAF) have been identified to form the inflammasome to activate downstream signaling molecules in response to pathogenic microbes. There is evidence to suggest that substantial crosstalk exists for the TLR and NLR signaling pathway in response to pathogen associated molecular pattern (PAMP). However, the substrate and the mechanistic role of NLRs are largely unknown in innate immune response. Understanding the signaling mechanisms by which NLRs recognize PAMP and other danger signals will shed light on elucidating the pathogenesis of various human inflammatory diseases such as IBD.

Lemierre Syndrome in Adolescent with Active Ulcerative Colitis

  • Inflammatory bowel disease (IBD) is a well-recognized risk factor for thrombotic events in adults but data on children are scarce. In the great majority of adult patients, thrombotic events are usually deep vein thrombosis and pulmonary embolism. Other sites such as jugular veins are extremely rare. We present a case of Lemierre syndrome in an adolescent girl with active ulcerative colitis and discuss possible risk factors. This is the first reported case of severe Lemierre syndrome with thrombus extension to cranial veins in a patient with ulcerative colitis. Early recognition of Lemierre syndrome in patients who present with rapidly worsening symptoms of neck pain, fever and signs of pharyngitis is imperative because it increases a chance of favorable prognosis. It is important for pediatricians treating IBD patients not to underestimate possible thrombotic events in children with IBD. Recognition of additional risk factors is crucial for prompt diagnosis and adequate treatment.

Immunofluorescence Microscopic Evaluation of Tight Junctional Proteins during Enterotoxigenic Bacteroides fragilis (ETBF) Infection in Mice

  • Inflammatory bowel disease (IBD) is increasing in prevalence in developed countries but the cause of this increase is unclear. In animal models of IBD and in human IBD patients, alterations in the tight junctional proteins have been observed, suggesting that the intestinal microflora may penetrate the underlying colonic tissue and promote inflammation. Enterotoxigenic Bacteroides fragilis (ETBF) causes inflammatory diarrhea in human and is implicated in inflammatory bowel diseases. However, it is unclear whether alterations in tight junctional proteins occur during ETBF infection in mice. In this brief communication, we report that ETBF infection induces up-regulation of claudin-2 and down-regulation of claudin-5 through B. fragilis toxin (BFT) activity in the large intestine of C57BL/6 mice. In contrast, BFT did not induce changes in tight junctional proteins in the HT29/C1 cell line, suggesting that analysis of biological activity of BFT in vivo is important for evaluating ETBF effects.

Osteoporosis and Osteoporotic Fractures in Gastrointestinal Disease

  • Patients with gastrointestinal disease (GI) are at risk for osteopenia or osteoporosis, which can lead to fractures. Although these patients may be at risk from a young age, gastroenterologists often overlook this fact in practice. There are well-known GI diseases associated with osteopenia and osteoporosis, such as the post-gastrectomy state, inflammatory bowel disease (IBD), and celiac disease. As there is an increase in the prevalence of IBD patients, newly diagnosed celiac disease in adulthood, and gastric cancer survivors following gastrectomy, bone disease in these patients becomes an important issue. Here, we have discussed osteoporosis and fractures in GI disease, especially in the postgastrectomy state, IBD, and celiac disease. Although the pathogenesis of bone loss in each disease has not been fully identified, we have confirmed that the prevalence of osteoporosis and fractures in each of these diseases is high. There are scarce studies comparing the prevalence of osteoporosis or osteoporotic fractures in GI disease patients with studies in postmenopausal women, and specific guidelines for their management in each disease have not been established. Intensive surveillance and management are needed to ensure that these patients attain peak bone mass for age and sex to prevent fractures.

Long lasting mesalazine-induced pancytopenia in a patient with ulcerative colitis

  • Inflammatory bowel disease (IBD) is an immune-mediated chronic inflammatory intestinal condition. With development of various treatment options for IBD, 5-aminosalicylic acid (5-ASA) agents became the drugs of choice due to high efficacy and low risk of complication, specifically effective at inducing and maintaining remission in ulcerative colitis(UC). Pancytopenia caused by 5-ASA agents, especially by mesalazine, has been rarely reported compared with azathioprine, which is commonly used for glucocorticoid-dependent IBD and known to have risk of bone marrow suppression. In the present report, we describe the case of a 57-year-old woman diagnosed with UC, who developed pancytopenia due to adverse effect of mesalazine after recovery from azathioprine-induced pancytopenia. After withdrawal of mesalazine, the laboratory values consistent with myelosuppression continued for 3 months while pancytopenia from azathioprine remained only for 2 weeks. Since pancytopenia can be fatal due to its risk of severe bleeding and infection, close monitoring of clinical presentation is important when starting mesalazine and laboratory data should be evaluated whenever the patients present related symptoms. Furthermore, we suggest that complete blood cell counts should be considered when resuming mesalazine following the development of pancytopenia from any cause, as routinely recommended for azathioprine use.

한약재 특성에 따른 궤양성 대장염 치료과 장내세균 상관관계 연구 (The Study about Relationship between Treatment of Ulcerative Colitis and Intestinal Bacteria According to Characteristic of Herbal Medicine)

  • Many modern people are exposed to chronic inflammatory diseases, such as inflammatory bowel disease (IBD), atopic dermatitis and immune disorder. Among those chronic diseases, the incidence ratio of IBD has been increased. IBD, including Crohn's disease and ulcerative colitis (UC), is known to cause abnormal inflammation in intestinal tissue. UC is accompanied by abdominal pain, bloody stool and diarrhea. Many therapeutic agents, such as sulfasalazine, corticosteroids, immunosuppressive agents, have been used for treating UC. However, those agents have side-effects and temporary effects on UC. The aim of this study was to investigate the effect of herbal medicine on UC and relationship between UC and intestinal bacteria according characteristic of herbal medicine. To determine the effect of herbal medicine on UC, various herbal medicine were chosen within oriental medicine category such as cheongyeol and onyeol medicine. In this study, we found carthami fructus, included in cheongyeol medicine, had stronger effect than onyeol medicine. Also, we determined influence of carthami fructus against lactic acid bacteria. Catthami fructus and lingon berry extracts affected the composition of mice intestinal bacteria in mice fecal. The symptoms of UC could be regulate by using herbal medicine, according to characteristic of herbal medicine. Also, herbal medicine might be change body condition to healthy by controlling intestinal bacteria composition. Herbal medicine characteristic could be a therapeutic agent by revealing relationship between intestinal bacteria and UC.

Stricturing Crohn's disease: what is the role of endoscopic stenting? A systematic review

  • Background/Aims: Endoscopic stenting for stricturing Crohn's disease (CD) is an emerging treatment that achieves more persistent dilatation of the stricture over time than endoscopic balloon dilatation (EBD). We aimed to explore the efficacy and safety of stenting for the treatment of CD strictures. Methods: A systematic electronic literature search was performed (PROSPERO; no. CRD42022308033). The primary outcomes were technical success, efficacy, complication rate, and the need for further interventions due to reobstruction. The outcomes of partially covered self-expanding metal stents (PCSEMS) with scheduled retrieval after seven days were also analyzed. Results: Eleven eligible studies were included in the review. Overall, 173 patients with CD were included in this study. Mean percentage of technical success was 95% (range, 80%-100%), short-term efficacy was 100% in all studies, and long-term efficacy was 56% (range, 25%-90%). In patients with a scheduled PCSEMS retrieval, the long-term efficacy was 76% (range, 59%-90%), the mean complication rate was 35% (range, 15%-57%), and the major complication rate was 11% (range, 0%-29%). Conclusions: Endoscopic stenting with scheduled PCSEMS retrieval may be considered a feasible second-line treatment for short CD strictures to postpone surgery. However, larger head-to-head prospective studies are needed to understand the role of stenting as an alternative or additional treatment to EBD in CD.

건강한 성인의 연령별 분변 칼프로텍틴의 농도 (Age-Related Fecal Calprotectin Concentrations in Healthy Adults)

  • 분변 칼프로텍틴(fecal calprotectin, FC)은 염증성 장질환의(inflammatory bowl disease, IBD)의 감별진단을 위한 표지자(marker)로 이용되고 있다. 또한 FC는 세균에 의한 분해가 어려워 실온에서도 1주일 정도 안정적으로 유지되며 대변 내에 균일하게 분포한다는 장점으로 치료효과의 판정과 재발을 에측하는 표지자로 활발히 연구되고 있다. 본 연구는 30~80대까지의 제주 거주 건강한 성인 남녀를 대상으로 하였다. 전체 건강대상군(N=45)의 FC 농도 범위는 0~545.9 ㎍/g로 아주 넓게 분포하였다. 건강한 성인들의 FC농도는 연령에 따라 유의한 차이를 보임을 알 수 있었으며 3개의 연령군 중에서 평균연령이 80.6세인 70세 이상의 연령군의 FC농도는 평균연령이 44세인 50세 미만의 연령군의 FC농도인 15.88 ㎍/g에 비해 약 10배 이상의 농도인 160.3 ㎍/g을 보였고 평균연령이 59.6세의 경우에도 50세 미만 연령층에 비해 2배 이상 농도인 35.46 ㎍/g 보여 연령과 FC 농도 간에는 유의한 상관관계를 보였다. 결론적으로 FC검사가 비침습적이며 비용 효과적이고 객관적인 IBD검사의 marker로 이용되기 위해서는 연령에 따른 정교한 cut-off 값이 필요하며 본 연구 결과가 IBD의 감별진단을 위한 기초자료를 제공할 수 있을 것으로 생각된다.

염증장병 환아의 혈액학적 소견 (Hematologic Features of Children with Inflammatory Bowel Disease)

  • 목 적: 최근 우리나라 소아에서 염증장병이 꾸준히 증가추세에 있으나 이들의 혈액학적 특징에 대해서는 알려진 바가 없다. 국내 염증장병 환아들의 장기간 관리에 도움이 되고자 혈액학적 소견을 분석하여 보았다. 방 법: 가천의대 길병원 소아청소년과에서 2002년 9월부터 2008년 9월 사이에 대장경과 생검으로 확진된 염증장병 환자 중 18세 이하인 25명의 염증장병 환아를 대상으로 하였다. 진단 당시의 혈색소, 적혈구용적률, 백혈구 수, 혈소판 수, 평균적혈구용적, 평균적혈구 혈색소량을 조사했다. 환아의 빈혈 빈도, 혈색소와 질병 이환기간과의 관계, 혈색소와 활동도지수와의 관계에 대하여 알아보았다. 결 과: 빈혈은 전체적으로 76% (19/25)에서 있었으며, 크론병 환아에서는 88% (14/16), 궤양결장염 환아에서는 56% (5/9)였다. 백혈구증가와 혈소판증가는 크론병 환아에서만 있었으며 빈도는 각각 56% (9/16), 25% (4/16)였다. 증상의 이환기간과 혈색소치는 크론병과 궤양결장염 모두에서 통계적인 유의성이 없었다. 활동도지수와 혈색소치도 크론병과 궤양결장염 모두에서 통계적인 유의성이 없었다. 결 론: 본 연구에서 염증장병 환아의 혈액학적 소견은 외국 문헌의 보고 내용과 큰 차이는 없었으나 빈혈의 빈도가 높았으며 질병의 이환기간 또는 활동도지수와 빈혈은 상관관계가 없었다. 이러한 사실은 염증장병환아를 장기간 관리하는 동안 빈혈에 대한 관리 역시 포함해야 하는 것을 시사하였다. 백혈구증가와 혈소판증가는 크론병에서만 관찰되어 궤양성장염과 감별진단하는데 도움이 될 수 있음을 제시하였다.
