Structural Equation Modeling of Self-Management in Patients with Hemodialysis (혈액투석환자의 자기관리 구조모형)

  • Purpose: The purpose of this study was to construct and test a hypothetical model of self-management in patients with hemodialysis based on the Self-Regulation Model and resource-coping perspective. Methods: Data were collected from 215 adults receiving hemodialysis in 17 local clinics and one tertiary hospital in 2016. The Hemodialysis Self-management Instrument, the Revised Illness Perception Questionnaire, Herth Hope Index and Multidimensional Scale of Perceived Social Support were used. The exogenous variable was social context; the endogenous variables were cognitive illness representation, hope, self-management behavior, and illness outcome. For data analysis, descriptive statistics, Pearson correlation analysis, factor analysis, and structural equation modeling were performed. Results: The hypothetical model with six paths showed a good fitness to the empirical data: GFI=.96, AGFI=.90, CFI=.95, RMSEA=.08, SRMR=.04. The factors that had an influence on self-management behavior were social context (${\beta}=.84$), hope and cognitive illness representation (${\beta}=.37$ and ${\beta}=.27$) explaining 92.4% of the variance. Self-management behavior mediated the relationship between psychosocial coping resources and illness outcome. Conclusion: This research specifies a more complete spectrum of the self-management process. It is important to recognize the array of clinical resources available to support patients' self-management. Healthcare providers can facilitate self-management through collaborative care and understanding the ideas and emotions that each patient has about the illness, and ultimately improve the health outcomes. This framework can be used to guide self-management intervention development and assure effective clinical assessment.

Structural Equation Modeling of Advance Directive Intent among a Korean Group in their Middle-Age (한국 중년층의 사전연명의료의향 의도 구조모형)

  • Objectives: The aim of this study was to construct and verify a structural equation model of advance directive intent among a Korean group in their middle-age. Methods: Data were collected between May 1 and 30, 2017, from 398 people. The endogenous and exogenous variables of the hypothetical model consisted of elderly parents' care burden, health status, attitude towards withdrawal of life sustaining treatment, advance directive efficacy, and advance directive intent. The collected data were analyzed using the SPSS/WIN 24.0 and Mplus 7.4. Results: The hypothetical model demonstrated a good fit: χ2=223.79(df=109, p<.001), CMIN/df=2.05 CFI=.96, TLI=.96, RMSEA=.05, SRMR=.06. Elderly parents' care burden and health status showed statistically significant direct effects with attitude toward withdrawal of life sustaining treatment(β=.17, p=.001; β=.21, p<.001) and advance directive efficacy(β=.11, p=.040; β=.19, p=.002), respectively. Attitude toward withdrawal of life sustaining treatment and advance directive efficacy showed statistically significant direct effects on advance directive intent(β=.15, p=.007; β=.48, p<.001). Elderly parents' care burden and health status had a significant indirect effect on advance directive intent through attitude toward withdrawal of life sustaining treatment(β=.01, p=.041; β=.05, p=.036) and advance directive efficacy(β=.06, p=.049; β=.16, p=.006), respectively. The variables accounted for 26.1% of advance directive intent of the Korean group in their middle-age. Conclusion: It is necessary to develop an advance directive education program based on variables affecting advance directive intent for individuals in their middle-age.

The Transport of Radionuclides Released From Nuclear Facilities and Nuclear Wastes in the Marine Environment at Oceanic Scales

  • The transport of radionuclides at oceanic scales can be assessed using a Lagrangian model. In this review an application of such a model to the Atlantic, Indian and Pacific oceans is described. The transport model, which is fed with water currents provided by global ocean circulation models, includes advection by three-dimensional currents, turbulent mixing, radioactive decay and adsorption/release of radionuclides between water and bed sediments. Adsorption/release processes are described by means of a dynamic model based upon kinetic transfer coefficients. A stochastic method is used to solve turbulent mixing, decay and water/sediment interactions. The main results of these oceanic radionuclide transport studies are summarized in this paper. Particularly, the potential leakage of 137Cs from dumped nuclear wastes in the north Atlantic region was studied. Furthermore, hypothetical accidents, similar in magnitude to the Fukushima accident, were simulated for nuclear power plants located around the Indian Ocean coastlines. Finally, the transport of radionuclides resulting from the release of stored water, which was used to cool reactors after the Fukushima accident, was analyzed in the Pacific Ocean.

  • Quantification of uncertainties in the source term estimations by a large computer code, such as MELCOR and MAAP, is an essential process of the current Probabilistic safety assessment. The main objective of the present study is to investigate the applicability of a combined procedure of the response surface method (RSM) based on input determined from a statistical design and the Latin hypercube sampling (LHS) technique for the uncertainty analysis of CsI release fractions under a Hypothetical severe accident sequence of a station blackout at Younggwang nuclear power plant using MAAP3. OB code as a benchmark problem. On the basis of the results obtained in the present work, the RSM is recommended to be used as a principal tool for an overall uncertainty analysis in source term quantifications, while using the LHS in the calculations of standardized regression coefficients (SRC) and standardized rank regression coefficient (SRRC) to determine the subset of the most important input parameters in the final screening step and to check the cumulative distribution functions obtained by RSM. Verification of the response surface model for its sufficient accuracy is a prerequisite for the reliability of the final results that can be obtained by the combined procedure proposed in the present work.

  • Purpose: This study was designed to construct a model which explains the career management, career plateau and career satisfaction of the factors influencing the career commitment of nurses. Method: The data collection was conducted from Aug. 24, 2009 to Sep. 22, 2009 through self-reported questionaries. Participants were 441 nurses who had worked in 2 national university hospitals, 1 university hospitals, and 1 hospitals founded by business enterprises. The data was analyzed by SPSS 12.0 and AMOS 15.0 for structural model. Results: Career commitment was directly impacted by career management, career plateau, career satisfaction. The predictable variables of the hypothetical model explained 28.4% of career commitment. Career commitment was significantly influenced by career management, career plateau, career satisfaction. Conclusion: The results of study suggest that some strategies are needed which focus on career management, career plateau and career satisfaction in order to promote the nurse's career commitment.

A Prediction Model for Health Promoting Behavior of The Korean Elderly (한국노인의 건강행위 예측모형구축)

  • This study was designed to construct a model that predicts the health promoting behavior of the Korean elderly. Data were collected by self-reported questionaires from 254 Korean elderly in seoul, from June 1 to July 15, 1998. Data were analyzed by descriptive statistics and correlational analysis using pc-SAS program. The Linear Structural Modeling(LISREL) 8.0 program was used to find the best fit model which predicts causal relationships of variables. The overall fit of the hypothetical model to the data was moderate[X$^2$=249.83(df=83, p=.00), RMR=.07, GFI=.90, NNFI=.92, NFI =.91]. The predictable variables of health promoting behavior of the Korean elderly were social activity. social support. self-integrity and helplessness except the perceived health status. These variables explained 17.1% of health promoting behavior of the Korean elderly.

  • The signal can be modeled as a linear combination of its past values and present and past values of a hypothetical input to system whose output is given signal. Using this model spectral estimation problem can be reduced to estimate the ARMA parameters. This paper presents recursive modified instrumental variable algorithm which can estimate AR and MA parameters. For more accurate estimation, overdetermined modified IV algorithm is also derived. Computer simulations are presented to illustrate the above methods.

  • A novel approach for modeling the thermic endfire interstitial applicator is presented. A hypothetical semi-infinite circular cylinder is added in the endfire direction in order to facilitate the theoretical modeling approach. The Fourier transform and mode-matching technique is utilized to obtain a solution in fast-convergent series Numerical computations for the input impedance are performed to check the Validity of the theoretical model.

  • Princeton Ocean Model (POM) is modified to construct a three-dimensional, semi-implicit hydro¬dynamic model with a wetting-and-drying scheme. The model employs semi-implicit treatment of the barotropic pressure gradient terms and the vertical mixing terms in the momentum equations, and the velocity divergence term in the vertically-integrated continuity equation. Such treatment removes the external mode and thus the mode splitting scheme in POM, allowing the semi-implicit model to use a larger time step. Applied to hypothetical systems, both the semi-implicit model and POM give nearly the same results. The semi-implicit model, however, runs approximately 4.4 times faster than POM showing its improved computational efficiency. Applied to a hypothetical system with intertidal flats, POM employing the mode splitting scheme produces noises at the intertidal flats, that propagate into the main channel resulting in unstable current velocities. Despite its larger time step, the semi-implicit model gives stable current velocities both at the intertidal flats and main channel. The semi-implicit model when applied to Kyeonggi Bay gives a good reproduction of the observed tides and tidal currents throughout the modeling domain, demonstrating its prototype applicability.

  • The modeling of interception and absorption of light was studied in plant canopies at Mt. Kwanak. Followering results were obtained. Light intensity passing through the stacked leaves is attenuated exponentially. This phenomenon seems to be more clearly applied to the plant canopies, if they have large cumulative leaf area and are matured densely. Light interception and absorption are influenced by leaf thickness, shape pigments, and leaf area, and they have great effect on the maturation of canopies. It was confirmed that the light penetrating through the stratified canopies is decreased exponentially in dual pattern. The cumulative leaf area of a definite space in a certain plant canopy is the same as the growth of leaf area of the canopy at that time. A hypothetical model for calculating the light absorption in plant canopies, was established on the bases of phenomena that incident light is captured at the maximum level and light inerception effect is minimized by leaves.
