Recurrent acinic cell carcinoma in the parotid gland with cardiac metastasis and hypertrophic osteoarthropathy (심장 전이와 비후성 골관절증을 동반한 재발성 이하선 선방세포암)

  • Acinic cell carcinoma (ACC) is an uncommon malignant tumor of the salivary glands that is difficult to diagnose. It grows slowly and shows distant metastasis rarely. We experienced a case of recurrent ACC in the parotid gland with cardiac metastasis and hypertrophic osteoarthropathy. The 29-year-old man had been suffering from severe multiple bones and joints pain for 2 months. Ten years earlier, he underwent superficial parotidectomy due to a right subauricular mass. The mass was diagnosed with ACC. After surgery, the tumor recurred twice. Then the patient was diagnosed with cardiac metastasis via positron emission tomography-computed tomography and trans-thoracic echocardiography. He also had hypertrophic osteoarthropathy with multiple bone metastasis. He was given palliative radiotherapy and conservative treatment. ACC in the parotid gland with cardiac metastasis and hypertrophic osteoarthropathy has not yet been reported in literature. From this case, it is recommended to evaluate multiple distant metastasis in the ACC of the parotid gland when joint and bone pain are present.

  • A 7-year old Holstein bull showing marked thickening of left forelimb was diagnosed as pulmonary hypertrophic osteoarthropathy on the histological basis of the exostotic bone of left metacarphus. Thickening of the bone was due to apposition of excessive cancerous bone beneath the periosteum on the shaft. The lung lesion of the bull, chronic suppurative bronchopneumonia was possibly related to pathogenesis of the bone thickening.

  • Hypertrophic osteoarthropathy(HOA) is a systemic disorder characterized by proliferative periostitis of long bone and polysynovitis. Clubbing of the fingers and toes usually accompanies HOA. This syndrome is most frequently with lung cancer. Although disappearance of HOA with removal of the tumor is well known, very few typical cases are reported in Korea We report case of lung cancer with HOA, which was dramatically relieved after the surgical resection of the tumor.

  • Hypertrophic osteoarthropathy (HOA) is a syndrome characterized by digital clubbing, periosteal bone formation, and synovial effusions. Secondary HOA is associated with intrathoracic malignancy in most cases; however, in rare cases, HOA can be caused by extrathoracic conditions. We report early ultrasound, computed tomography, magnetic resonance imaging, and bone scintigraphy findings of HOA in a patient with breast cancer. Its ambiguous clinical and imaging findings that mimicked malignant conditions are particularly interesting and informative.

  • Hypertrophic pulmonary osteoarthropathy classically consists of clubbing of the fingers and toes, periosteal new bone formation in the tubular bones, painful swelling of limbs, arthralgia and arthritis. Its pathogenesis is uncertain, but it is usually associated with malignant or inflammatory intrathoracic disease. We recently experienced a patient with the characteristic findings of this rare disease on bone scintigraphy using $^{99m}Tc-MDP$(methylene diphosphonate).

  • Hypertrophic osteoarthropathy(HOA) is a systemic disorder primary affecting the bones, joints, and soft tissues and characterized by several(or all) of the followings ; 1) Clubbing of digits, 2) Persistent new bone formation particulary involving long bones of the distal extremites, 3) Symmetric arthritis-like changes in the joints and periarticular tissue, most commonly the ankles, knees, wrist, and elbows, 4) Increased thickness of the subcutaneous soft tissues in the distal one-third of the arms and legs, and 5) Neurovascular changes of the hands and feet, including chronic erythema, paresthesis, and increase sweating. Most of cases of HOA are secondary to intrathoracic neoplasms, while the remaining few cases are secondary to other disease in the chest or elsewhere. We experienced a case of HOA in association with lung abscess in 26-yr-old male and reported with a review of literatures.
