A Study on the Byun Jeung Rok(辨證錄) Vol.XI (변증록(辨證錄) 권지십일(卷之十一)에 대(對)한 연구(硏究))

  • The topic of this study is the translation of "Byun Jeung Rok Vol.XI(辨證錄十一卷)". Byun Jeung Rok is a complete book of medicine which was written by Jin Sa tak(陣士鐸) in the early years of the Ching dynasty. Of this, the eleventh volume is composed of 6 disease-patterns(病證) and 41 subtopics. The organized characteristics of "Byun Jeung Rok(辨證錄) Vol.XI" is as follows. Chapter 1 refers to the symtom, agent and prescription of Hysterorrhea(帶下病). It explains five color Hysterorrhea; white, red, black, yellow and blue. Chapter 2 refers to the symtom, agent and prescription of Oligemic Menoschesis(血枯). It explains two cases of Menoschesis. Chapter 3 refers to the symtom, agent and prescription of Metrorrhagia(血崩). It explains eight cases of Metrorrhagia. Chapter 4 refers to the symtom, agent and prescription of Dysmenorrhea(月經不調). It explains fourteen cases of Dysmenorrhea. Chapter 5 refers to the symtom, agent and prescription of Infertilitas(不姙). It explains ten cases of Infertilitas. Chapter 6 refers to the symtom, agent and prescription of Hyperemesis(惡阻). It explains two cases of Hyperemesis. Obviously the Byun Jeung Rok(辨證錄) is one of the very important texts in the oriental medicine. But there is no exact Korean translation, so I study this six chapters and translate them into Korean. Finally, I hope this thesis can be an aid of development in the knowledge informatization of Korean medicine.

Subjectivity of Nausea and Vomiting of Pregnancy(NVP) in woman: Q Methodology (여성의 입덧 경험에 대한 주관성 연구)

  • Purpose: The purpose of this study was to explore the subjective opinions related to NVP experience in woman. Method: The research method used was Q-methodology. The statement of self-reference was derived from face to face interviews with 10 womans. Statement were categorized by researcher according to semantics. Twenty-seven of the self reference statement from categories of the Q-population were selected. Twenty-five of pregnant womans were selected by consideration of diversity in sociodemographic background. Based on a 1 to 9 point scale, the selected pregnant womans were made to participate in. Q-sorting. Analysis of Q-type was obtained by use of the QUANAL program. Result: There are three type of NVP experience of Korean womans. The first type focused on family support relate to NVP coping. The second type focused on physical self regulation related to NVP coping. The third type focused on actual strategy related to NVP coping. Conclusion: It is suggested that the results of this study may contribute to the development strategies for the purpose of decreasing the incidence of NVP.

Review on RCT Research Papers Related Nausea and Vomiting in Pregnancy - Focusing on Acupoint Treatment - (임신오조(姙娠惡阻) 관련 무작위대조군연구(RCT)에 관한 고찰 - 경혈을 이용한 치료를 중심으로 -)

  • Objectives: To observe randomized controlled trials(RCTs) related to investigating the effects of acupoint treatments on nausea and vomiting in pregnancy to establish the basis of korean medical treatments, and suggest the problems of future clinical research. Methods: After searching for national and international papers published after 2000 and restricting the parameters to 'clinical trial', 'controlled clinical trial', 'randomized controlled trial', then comparing the subjects, methods, research tools, and results to understand the effects and re-comparing the conflicting results to assume the cause. Results: Of the 9 research papers that applied acupressure, four of them used control and experimental groups, and three(75%) research papers indicated significant reduction in vomiting and nausea, and of the seven research papers that utilized the experimental and placebo groups, five(71.4%) reported significant reducing effects. Of the four research papers the used acupuncture treatments, four research papers using control and placebo groups, two showed improvements in symptoms but did not have significant results compared to sham acupuncture groups, and two showed significant reduction of symptoms compared to the control groups. The one research paper that used electrical stimulation showed significant results in the reduction of vomiting and nausea compared to the control group. Conclusions: Acupressure and electrical stimulation showed efficacy reducing the symptoms. Acupuncture treatments did not indicate efficacy some experiments. Therefore it is thought that future acupuncture treatments should use methods that resemble actual clinical medicine to establish efficacy to produce significant results.

The Latest Trends of Acupuncture Therapy on Obstetrics (Pub Med 검색을 통한 침구치료의 산과적 응용에 대한 최근 연구 고찰)

  • Purpose: We aimed at reviewing the studies of the acupuncture treatment for women related to pregnancy. Methods: We referred to PubMed by searching words of pregnancy and acupuncture and limited experimental method to randomized control trial. Results: We found 52 articles in 32 journals, which were divided into 3 parts: treatment for symptoms during pregnancy, treatment for symptom related to delivery and treatment for symptoms related to surgical operation for IVF-ET. In this study, articles on hypermesis gravidarum were the greatest in number and acupuncture for pain control related to pregnancy are most frequently appearing in terms of symptoms. Besides acupuncture treatment, acupressure band and acupressure were used for hyperemesis gravidarum, common acupuncture for low back pain in pregnancy and acupuncture or moxibustion to resolve breech presentation. Conclusion: In this study, acupuncture seems to be an efficacious means of reducing symptoms related to pregnancy and improving the quality of life of pregnant women.

A Study on the Survey for the Application Status and the Improvement of Korean Medical Health Insurance for Ob & GY Disease ( II ) (부인과 질환에 대한 한방건강보험 적용실태 및 개선방안에 대한 조사연구 2)

  • Purpose: This is a following study of preceding study about application and improvement of acupuncture benefits in OB & GY disease that was performed in 2007. And this study was performed to complement and compare with a preceding study. Methods: The questionnaire used in preceding survey was distributed to general korean medical doctors who have mainly worked in oriental medical clinic. And results of this survey was compared with results of preceding survey. Results: Results of two surveys were almost similar. 1. The common suitable the sick and wounded name of Ob & Gy disease thought be added in Intra-abdominal acupuncture(腹腔內; CV13 ${\cdot}$ CV16 ${\cdot}$ CV10) in two surveys were dysmenorrhea(K05). infertility(K15), JingHa(pelvic tumor) (K11). In Puncture each adjoining acu-points in one insertion(透刺; SP6-GB39)were dysmenorrhea(K05), climacteric syndrome(K04). amenorrhea (K03), In Puncture each adjoining acu-points in one insertion(透刺; PC6-TE5) were climacteric syndrome(K04), hyperemesis(K16.0), dysmenorrhea(K05). 2. The common acupuncture benefits items that postpartum pain syndrome(K29) should be added as suitable the sick and wounded name were Intra-articular acupuncture(關節內; LI15, SI10, GB30), Intervertebral acupuncture (脊椎間; GV14, 16, 6, 4, 12, 11, 9, 8, 3), Puncture each adjoining acu-points in one insertion (透刺; SP6-GB39). Conclusion: Based on this study, Survey to more KMDs should be performed. And the academic and clinical verifications that supports this results should be supplied.

Wernicke's encephalopathy in a child with high dose thiamine therapy

  • Wernicke's encephalopathy is an acute neurological disorder characterized by mental confusion, oculomotor dysfunction, and ataxia. It has been reported in individuals with alcohol dependence, hyperemesis gravidarum, and prolonged parenteral nutrition without vitamin supplementation. Here we present the case of a 13-year-old male patient with neuroblastoma and a history of poor oral intake and nausea for 3 months. After admission, he showed gait disturbances, nystagmus, and excessive dizziness; his mental state, however, indicated he was alert, which did not fit the classical triad of Wernicke's encephalopathy. A diagnosis of Wernicke's encephalopathy was made only after brain magnetic resonance imaging and serum thiamine level analyses were performed. The patient's symptoms remained after 5 days of treatment with 100-mg thiamine once daily; thus, we increased the dosage to 500 mg 3 times daily, 1,500 mg per day. His symptoms then improved after 20 days of replacement therapy. This case report describes a pediatric patient who was promptly diagnosed with Wernicke's encephalopathy, despite only 2 suspicious symptoms, and who completely recovered after high doses of thiamine were given intravenously.

  • Purpose : In the part of Ob & Gy disease, the health insurance application is very limited. This study has been performed for gaining the basic data of enlargement of insurance coverage and reform of the insurance system corresponded with real clinical conditions. Methods : The survey has been practiced twice, the subjective questionnaire was used at the first survey. Then the questionnaire written using the results of first survey was distributed to the Korean medical doctors(KMD) who participated in the autumn symposium of the society of Oriental Obstetrics & Gynecology. Results : 1. The main Ob & Gy disease that the acupuncture treatment has been used actually or thought be positively necessary on the clinic were Dysmenorrhea(including premenstrual syndrome), Climacteric syndrome, Menstrual disorder, Postpartum Pain syndrome. Amenorrhea, Low back pain with pregnancy, JingHa(pelvic tumor), Infertility etc. 2. The main additional complex the sick and wounded names given to visiting patients for Ob & Gy disease as the limits of acupuncture items of insurance coverage were Low back Pain(J10), Qi-stasis(B13.0), SimHwaHangYeom(C2l.1). 3. Suitable the sick and wounded name of Ob & Gy disease thought be added in BokGangNae(Intra-abdominal acupuncture: CV13 ${\cdot}$CV16${\cdot}$CV10). TuJa(Puncture each adjoining acu-points in one insertion : SP6-GB39), TuJa(PC6-TE5) among acupuncture items of insurance coverage were Dysmenorrhea(K05). Menstrual disorder(K02) and Dysmenorrhea(K05), Hyperemesis(K16.0). 4. Climacteric syndrome(K04) and Dysmenorrhea(K05) should be added as suitable the sick and wounded name of TuJa(SP6-GB39), and Postpartum pain syndrome(K29) as KwanJeolGangNea(Intra-articular acupuncture: S35, LE201). Conclusion : Standing on this study, additional survey to general KMD should be continued. And the academic verifications through the oriental medical literatures and RCT papers on acupuncture should be also required.

A Survey on Korean Medicine Doctors' Recognition and Treatment Method for Developing Clinical Practice Guideline of Nausea and Vomiting of Pregnancy (임신오조 한의표준임상진료지침 개발을 위한 한의사의 인식과 치료 실태에 관한 조사)

  • Objectives: This study was aimed to develop a Korean Medicine (KM) clinical practice guideline (CPG) of Nausea and Vomiting of Pregnancy (NVP). Methods: We conducted a questionnaire survey targeting KM doctors belonging to the Association of Korean Medicine by e-mail. We received 1,023 responds, and analyzed the answers. Results: 1. 83.0% of respondents knew the concepts and contents of CPG, and 98.1% had practical use plan. 2. 82.1% of respondents used pattern identification diagnosis for NVP patients, and the most commonly diagnosed pattern was spleen-stomach weakness (脾胃虛弱) with 41.3%. 3. The most frequently used treatment for NVP patients was KM combined treatment (46.7%). Herbal medicine+acupuncture (46.8%) was most used among KM combined treatments, and herbal medicine (37.1%) was most used among KM single treatments. 4. Among the contents of CPG for NVP, the fields of interest were selected in the order of KM treatment, KM-Western medicine cooperative treatment, KM diagnosis, prevention and regimen management. In the 'diagnosis part', the use of the symptom evaluation scale questionnaire was 41.8%, higher than the KM pattern diagnosis (34.4%). In the 'treatment part', herbal medicine accounted for 33.8%, higher than that of acupuncture (including electro-acupuncture) at 23.7%. 5. As for the expected development effects, opinions on evidence-based, safety, clinical use, and standardization were the most common. Conclusions: We figured out KM doctors' recognition of KM clinical practice guideline, clinical diagnosis, treatment on NVP to make the contents of the CPG reflecting the clinical situation.

A Survey on General Public's Experience and Perception of Korean Medicine Treatment for Developing Clinical Practice Guideline of Nausea and Vomiting of Pregnancy (임신오조 한의표준임상진료지침 개발을 위한 일반인 대상 의료 이용 경험 및 인식도 조사)

  • Objectives: This study was conducted to reflect the public's perspective when developing Korean Medicine (KM) Clinical Practice Guideline (CPG) for nausea and vomiting of pregnancy (NVP). Methods: 317 respondents who had experienced nausea and vomiting during current or past pregnancy completed the questionnaire online, and we analyzed their answers. Results: 1. 24% of all respondents had received KM treatment. The most common reason for choosing KM treatment was "it would be relatively safe for pregnant women and fetuses", and the reason for not choosing KM treatment was "they did not know much about it". 2. Respondents who had experienced KM treatment for NVP had felt the effects of KM treatment (79%), and chose herbal medicine as the most effective and preferred treatment. 3. 64.1% of respondents who had not experienced KM treatment for NVP preferred cooperative treatment with Western medicine. They answered that herbal medicine would be the most effective treatment, but preferred acupuncture. 4. The most important factor when treating NVP was "the safety of treatments". 59% of all respondents perceived KM treatment is safe for NVP. The treatments perceived as safest were herbal medicine and acupuncture, while the treatments with the least safety information were pharmacopuncture and electro-acupuncture. 5. Treatments that medical consumers were comprehensively interested in were acupuncture, KM treatment & Fluid, herbal medicine, moxibustion, and acupressure, in order. Conclusions: This study revealed the public's experience of using medical care, preference and perception of KM treatment for NVP. Therefore we would reflect the patients' clinical needs in the CPG.
