Geochemical Characteristics of Soils, Sediments and Waters in stream Of Hwasun area (화순지역 토양-퇴적물-하천수의 지구화학적 특성)

  • To consider environmental characteristics in the scope of geochemistry of streams, Hwasun area, soil, sediment and water samples near/in the streams were analyzed in texture and metal contents of soil and sediment and in quality in water. From those analyses, the soils are loamy sand, sandy loam, loam and silty loam in texture. And, the sediments are slightly gravelly sand, gravelly sand and gravelly muddy sand in facies. Metal contents in soils and sediments are of high near Hwasuneup and Hwasun coalfield. In peculiar, P, Co, Li, Ni, Zn and Pb exceed over crust mean contents. Physico-chemistry of above streams according to pH-Eh and Piper's diagrams indicates that the streams are, typically, assigned to natural river water. Water qualities of BOD, T-N and T-P in areas near Hwasun coalfield, Dongmyeon and Hwasuneup are polluted over V level. Enrichment factor(EF) representing metal condensation in P, Cu, Zn and Pb appear near Hwasun coalfield and Hwasuneup from the soil and sediment samples, in part. Additionally, river water in dry season is very high in BOD, T-N, $Na^+$ and ${SO_4}^{2-}$. It is suggested that the relatively high metal contents in the stream be connected with above coalfield and urban areas.

  • Sediments and river waters form the channel of Hwasun-cheon were studied in sedimentological size and geochemical analyses of metallic elements for the purposes of identification of depositional environments and geochemical characteristics. The sizes of sediments are assigned to pebble to coarse sand in mean size and polymodal in distribution. And the sediments are poorly to very poorly sorted and positively skewed. According to the grain size distributions of the sediments, the Hwasun-cheon belongs to gravel-bed river on the basis of the grain size distribution of the sediments. The behaviors of metallic elements in the sediments mainly depend on not grain size distribution but the geology connected with geomorphological reliefs near the stream. Contamination indices(CI) of Zn, Cu and Pb are 2.83 to 6.96 with average 4.31 in the sediments. Hwasun-cheon is assigned to general stream type in accordance with water quality of physical factors and chemical characteristics by Piper's diagram. Though meaningful values of BOD, T-N, T-P were locally depicted near Masan-ri, Hwasun-eup and Jiseok-cheon areas, artificial metal concentration do not represent in the most area of the stream. Sediments and river water are considered that the relatively more or less high metallic contents in the stream are originated from coal mine and urban area.

A Case of Dual Ectopic Thyroid (이중 이소성 갑상선 1예)

  • Ectopic thyroid is an uncommon embryologic aberration characterized by the presence of thyroid tissue in a site other than its usual pre-tracheal location. Dual ectopic thyroid is extremely rare, only few cases have been reported in the literature. Physical examination, laboratory test and radiologic examinations are usually performed for the diagnosis and therapeutic plan of dual ectopic thyroid. We report a 36-year old female with dual ectopic thyroid in lingual and hyoid regions but without thyroid tissue in its normal area. The diagnosis was made through pathologic examination after surgical resection.

Epithelioid Hemangioendothelioma Arising from Interanl Jugular Vein Mimicking Cervical Metastatic Lymphadenopathy

  • Epithelioid hemangioendothelioma (EHE) is a relatively rare and moderately aggressive type of vascular tumor. We describe a case of EHE that presented in a 24-year-old woman as a palpable nodule in the left cervical area. Radiological examinations and fine-needle aspiration cytology led to a preliminary diagnosis of metastatic lymphadenopathy with an unknown primary site. However, during surgery, we determined that the nodule was an intra-vascular tumor arising from the left internal jugular vein. The histopathological examination revealed cords of epithelioid endothelial cells distributed in a hyaline stroma, compatible with a diagnosis of EHE.

Neo-Adjuvant Chemotherapy Followed by Surgery for Extensive Calvarial Metastases of a Neuroblastoma

  • Neuroblastoma is a common tumor of children. We report a patient with extensive calvarial metastases of a neuroblastoma as an initial presentation. A 2-year-old girl presented with a history of gradually increasing head size and fever. A brain CT showed a multilobulated, large, extra-axial tumor involving both frontotemporoparietal areas with a sunray-spiculated hyperostosis of the skull and marked contrast enhancement. A brain MRI demonstrated extensive calvarial lesions with simultaneous involvement of the orbits. A biopsy was performed and a ganglioneuroblastoma was diagnosed. On systemic evaluation, an enlarged abdominal mass was detected. After neo-adjuvant chemotherapy, most of the tumors disappeared except for a tumor in the left parietal area; there was a corresponding decrease in the circumference of the head. We performed surgery for the remnant mass. Intensive chemotherapy was administered and a bone marrow transplantation was performed. Adequate neo-adjuvant chemotherapy followed by surgery to the neuroblatoma with extensive metastases to the skull and orbit may be helpful.

  • To suggest the field maintenance plan considering the geographical characteristics of the region, we selected representative regions(plain regione and mountain region) and compared spatial distribution of cultivated land in Muan-gun and Hwasun-gun. Firstly, we examined the distribution characteristics of cultivated land according to the scope of the maintenance object with Fragstats. As a result of that, it was found that the cultivated area except rice paddy had the highest aggregation effect. And then, we developed type classification of maintenance considering geographic characteristics and cultivated crops information. As a result of classification, plain land type Muan region was mostly cultivated land suitable for integrated maintenance. On the other hand, Hwasun, a mountainous terrain, needs small-scale maintenance and road maintenance. Based on these results, it was found that more detailed planning is needed for the upland field infrastructure improvement considering the topographic characteristics.

K-Ar Ages of Cretaceous Fossil Sites, Seoyuri, Hwasun, Southern Korea (화순 서유리의 백악기 화석산지에 대한 K-Ar 연대)

  • The Cretaceous fossil sites of Seoyuri in Hwasun was designated as the Korean Natural Monument No. 487 in November 2007. It provides important resources for paleoenvironmental studies, including theropod trackways, plant fossils, mudcracks, ripple marks, and horizontal bedding. The Cretaceous sedimentary strata contain a wide variety of volcanic pebbles, 5-40 cm in diameter in the lower portion and are overlain by the Late Cretaceous Hwasun andesite. Whole rock absolute K-Ar age determinations were performed on six volcanic pebbles from the Cretaceous sedimentary strata and on two samples from the overlaying Hwasun andesite. These ages indicate that the rocks belong to the period between the Turonian of the late Cretaceous (91-70 Ma) and the Pliocene age of the early Cenozoic ($63.4{\pm}1.2$ and $62.1{\pm}1.2$ Ma). Thus, the K-Ar ages indicate that the maximum geological age of the dinosaur track-bearing sedimentary deposits is about ca. 70 Ma. Therefore, it suggests that the age is comparable to the formation ages of the dinosaur footprints-bearing deposits in Sado area of Yeosu (71-66Ma).

Two Cases of Primary Osteolytic Intraosseous Meningioma of the Skull Metastasizing to Whole Skull and the Spine

  • We report here two cases of primary intraosseous meningioma with aggressive behavior. A 68-year-old man presented with a one year history of a soft, enlarging mass in the right parietal region. Magnetic resonance image (MRI) revealed a 6 cm sized, heterogeneously-enhancing, bony expansi1e mass in the right parietal bone, and computed tomograph (CT) showed a bony, destructive lesion. The tumor, including the surrounding normal bone, was totally resected. Dural invasion was not apparent Diagnosis was atypical meningioma, which extensively metastasized within the skull one year later. A 74-year-old woman presented with a 5-month history of a soft mass on the left frontal area. MRI revealed a 4 cm sized, multilobulated, strongly-enhancing lesion on the left frontal bone, and CT showed a destructive lesion. The mass was adhered tightly to the scalp and dura mater. The lesion was totally removed. Biopsy showed a papillary meningioma. The patient refused adjuvant radiation therapy and later underwent two reoperations for recurred lesions, at 19 and at 45 months postoperative. The patient experienced back pain 5 years later, and MRI showed an osteolytic lesion on the 11th thoracic vertebra. After her operation, a metastatic papillary meningioma was diagnosed. These osteolytic intraosseous meningiomas had atypical/malignant pathologies, which metastasized to whole skull and the spine.

Cerebral Aspergillosis with Multiple Enhancing Nodules in the Right Cerebral Hemisphere in the Immune-Competent Patient

  • Aspergillosis in the central nervous system (CNS) is a very rare disease in immune-competent patients. There was a case of a healthy man without a history of immune-compromised disease who had invasive aspergillosis with unusual radiologic findings. A 48-year-old healthy man with diabetes mellitus, presented with complaints of blurred vision that persisted for one month. Brain magnetic resonance imaging (MRI) showed multiple nodular enhancing lesions on the right cerebral hemisphere. The diffusion image appeared in a high-signal intensity in these areas. Cerebrospinal fluid examination did not show any infection signs. An open biopsy was done and intraoperative findings showed grayish inflammatory and necrotic tissue without a definitive mass lesion. The pathologic result was a brain abscess caused by fungal infection, morphologically aspergillus. Antifungal agents (Amphotericin B, Ambisome and Voriconazole) were used for treatment for 3 months. The visual symptoms improved. There was no recurrence or abscess pocket, but the remaining focal enhanced lesions were visible in the right temporal and occipital area at a one year follow-up MRI. This immune-competent patient showed multiple enhancing CNS aspergillosis in the cerebral hemisphere, which had a good outcome with antifungal agents.

Clinical Validation of a Protein Biomarker Panel for Non-Small Cell Lung Cancer

  • We validated the diagnostic performance of a previously developed blood-based 7-protein biomarker panel, $AptoDetect^{TM}$-Lung (Aptamer Sciences Inc., Pohang, Korea) using modified aptamer-based proteomic technology for lung cancer detection. Non-small cell lung cancer (NSCLC), 200 patients and benign nodule controls, 200 participants were enrolled. In a high-risk population corresponding to ${\geq}55years$ of age and ${\geq}30pack-years$, the diagnostic performance was improved, showing 73.3% sensitivity and 90.5% specificity with an area under the curve of 0.88. $AptoDetect^{TM}$-Lung (Aptamer Sciences Inc.) offers the best validated performance to discriminate NSCLC from benign nodule controls in a high-risk population and could play a complementary role in lung cancer screening.
