한국 여성의 탄수화물/지질 섭취가 대사증후군에 미치는 영향: 국민건강영양조사(2007-2016)를 중심으로 (Relationship of Carbohydrate and Fat Intake with Metabolic Syndrome in Korean Women: The Korea National Health and Nutrition Examination Survey (2007-2016))

  • 본 연구는 2007~2016 국민건강영양조사 자료를 이용하여 한국 여성의 탄수화물과 지방 섭취 수준에 따른 대사증후군 유병율에 대한 연관성을 파악하고자 실시하였다. 연구대상은 만 19~69세의 여성으로 임신 또는 수유중인 경우를 제외한 총 22,850명을 중심으로 분석하였다. 식이 섭취 조사는 24 시간 회상법을 이용하여 탄수화물과 지방의 섭취량에 따라 5가지 군으로 구분하였다. 교란 변수(연령, 가구소득, 흡연, 음주, 운동, 에너지 섭취량, 체질량 지수, 단백질 섭취량)을 통제한 후, 들을 통제한 후, 회귀분석과 일반 선형 모형으로 탄수화물 및 지방 섭취율에 따른 대사증후군 구성요소와의 관계를 분석하였다. 탄수화물을 가장 많이 섭취하는 군은 가장 적게 섭취하는 군에 비해 중성지방(p for trend=0.04), 허리둘레(p for trend<0.01), 그리고 수축기 혈압(p for trend<0.01) 이 유의하게 높았으며, HDL 콜레스테롤(p for trend<0.01)은 낮았다. 지방을 가장 많이 섭취하는 군은 적게 섭취하는 군에 비해 허리둘레(p for trend=0.02), 중성지방(p for trend<0.01), 그리고 수축기 혈압(p for trend<0.01)은 낮았던 반면, HDL 콜레스테롤(p for trend<0.01)은 더 높았다. 또한 탄수화물을 가장 많이 섭취하는 군에서 대사증후군 유병율이 나타났으며(5th quintile vs. 1st quintile, OR: 1.32; 95% CI: 1.11 to 1.57) 지방을 가장 많이 섭취한 군에서는 대사증후군 유병율이 더 적게(5th quintile vs. 1st quintile, OR: 0.73; 95% CI: 0.61 to 0.86) 나타났다. 연구 결과, 한국 여성에 있어서 과도한 탄수화물의 섭취와 적은 지방의 섭취는 대사증후군의 유병율과의 관계가 있음을 확인할 수 있었다.

임상시험심사위원회 위원과 연구자를 대상으로 임상연구에서 이해상충에 대한 설문조사연구 (Survey of Conflict of Interest in the Clinical Research for IRB Members and Researchers)

  • Purpose: To obtain opinions from Korean Institutional Review Board (IRB) members' self-evaluation on ability to conduct fairness review of clinical trial protocol with presence of conflict of interest and from investigators and IRB members on financial conflict of interest through surveying. Methods: IRB members and researchers in 9 different hospitals were asked to answer survey questions via email. Results: Responders were 115 personnel (IRB Chair/vice 18, medical member 30, non-medical member 28, and researcher 39) from 9 centers. Compared to IRB medical members, IRB chair/vice respondents scored higher with statistically significance on 10 point scale (8.44±1.381 vs. 7.30±1.685, p=0.005) when asked to self-evaluate fairness reviewing a protocol proposed by an investigator from the same department and a protocol from the company that supports the scientific committee of responders. When reviewing a protocol proposed by a hospital director, non-medical members scored statistically significantly higher than medical-members (7.47±1.76 vs. 8.07±2.70, p=0.034). When asked about the limitation of labor fee for principal investigator on phase 3 Human clinical trials of the Investigational new drug, while the responses range was wide, 60% answered that labor cost of principal investigator should be less than 30% of total budget for clinical trials with a budget of 100 million won. 51.3% answered that there is no need to disclose the labor cost of the principal investigator in the consent form. Since every investigator can be influenced unconsciously by conflict of interest, the answer that 'responder agrees that there is need for management' was the most chosen answer (IRB member 61.8%, investigator 64.1%, multiple answers allowed). Conclusion: Considering scores on questions of fairness by IRB members were between 7.23-8.56 on scale of 0 to 10 point when IRB members were asked about reviewing a clinical trial protocol, it cannot be said with absolute certainty that there is no issue regarding fairness in the review process. Therefore, there should be more ways to safeguard fairness for these issues. There is a need that the disclosure amount of honorarium from sponsor should be lower than 100 million Korean won. Considering the results of the survey in which respondents expressed their thoughts, it is likely that more education on the concept of conflict of interest is needed.

새로운 민주주의 국가건설의 과제 속에 직면한 AIDS와 이에 대한 교회의 반응과 과제: 남아프리카 공화국을 중심으로 (The Crisis of AIDS and responses of South African Churches in the task of new national building)

  • At the start of the new century, South Africa probably had the largest number of HIV-infected people of any country in the world. The only nation that comes close is India with a population of one billion people compared to South Africa's figure of 57 million. The tragedy is that this did not have to happen. South Africa was aware of the dangers posed by AIDS as early as 1985. In 1991, the national survey of women attending antenatal clinics found that only 0.8percent were infected. In 1994, when the new government took power, the figure was still comparatively low at 7.6 %. The 2004 figure which has been published is 26.5%. This article tracks the epidemic globally, in the region and in South Africa. I explain some of the basic concepts around the disease and look at what may happen with respect to numbers. The situation is bad, and the number of people falling ill, dying and leaving families will rise over next few years. This will impact on South Africa in a number of important ways. This article assesses the demographic, economic and social consequences of the epidemic. It disposes of a number of myths and present the real facts. The AIDS in South Africa is not related to individuals only. It warns that AIDS in Africa is becoming a community and systemic problem. The acuteness of the problem does not stem merely from the fact that communities are affected, or could even be wipe out by the end of this decade, but from the fact that AIDS will place incredible burdens and obligations upon medical services, health care and religious communities such as churches. The facts confront churches' mission with the important question: who is going to take care of all the patients and where? The reality is that people dying of AIDS will have to be cared for at home by relatives and friends. A further question that arises is whether our people are prepared for this. AIDS was considered to be a homo-plague and the hunt was on for a scapegoat in the light of the fatal implication of the disease. At present we are in the strategic phase where we all realize that it will be of no avail to scare people with the ominous threat of AIDS AIDS destroys the optimism of our achievement ethics. This exposure of the culture of optimism is also an exposure of the so-called 'human basic fear which accuses Christianity that their concept of sin is a damper on man's search for liberation and basic need to be freed from all Imitation. AIDS is also a test for our ecclesiastical genuineness and the sincerity of our mission sensibility. It poses the question: How unconditional is Christian love? Is there room for the AIDS sufferer in the community of believers, despite the fact he is an acknowledged homosexual? The question to put to the church is whether the community of believers is an exclusive to put to the koinonia which excludes homosexuals. They may be welcome on principle, but in actual fact are not acceptable to the church community. As South Africa enters the new century, it is clear that the epidemic is not having a measurable impact. However, the impact of AIDS is gradual, subtle and incremental. The author's proposal of what is currently most needed in South Africa is that the little things will make a difference. It's about doing lots of little things better at grassroots level, with the emphasis on doing. There are so many community, churches and NGOs initiatives worth building on and intensifying. One must not underestimate the therapeutic value of working together in small groups to overcome a problem

소아승낙 현황조사와 소아청소년과/소아신경과 전문의를 대상으로 면담조사를 통한 소아승낙서 공통기준 수립 연구 (Research to Establish a Common Standard for Assent by Assessing the Current State of the Assent Process and Conducting Interviews with Pediatrician/Pediatric Neurologist)

  • Purpose: The purpose of this study is to investigate the current status of pediatric assent in nationwide hospitals and to assess the children's comprehension for pediatric assent by interviewing pediatricians/pediatric neurologists to determine whether children of the age (elementary and middle school students) can understand the purpose, risks, benefits, and concepts of voluntary participation in clinical research described in the assent form, and to help improve the administrative efficiency of multicenter clinical trials. Methods: The status of pediatric assent was surveyed online using Google Forms at 141 university hospitals with administrative staff who are members of the Institutional Review Board (IRB) administrative staff subcommittee with in Korean Association of Institutional Review Boards (KAIRB). Additionally, face-to-face interviews were conducted with 7 pediatricians/pediatric neurologists. Survey and interview responses were summarized using descriptive statistics. Results: Out of the 141 institutions surveyed, 35 institutions (24.8%) responded. Among them, 30 institutions (85.7%) reported having age criteria for acquiring pediatric assent forms in the case of children. The age range for pediatric assent acquisition have been from 7 years old to 12 years old (15 institutions, 50%), and from 7 years old to 15 years old (7 institutions, 23.3%). Nine institutions (25.7%) have had criteria for obtaining both parents' consent in cases involving the participation of children. Nineteen institutions (54.3%) have had checklists or guidelines available for use by IRB members in study protocols involving vulnerable research subjects. Three pediatricians/pediatric neurologists have believed that upper-grade elementary school students (5th-6th grade) could comprehensively understand informed consent forms. Two have believed that middle school students would be able to understand them if they included personal information. Two pediatricians/pediatric neurologists have believed that even lower-grade elementary school students (1st-4th grade) could understand the explanations if they were made simpler. Conclusion: It is suggested that not only elementary school students (7-12 years old) but also middle school students (13-15 years old) should receive pediatric assent forms, as it would facilitate a comprehensive understanding of the forms. To enhance the comprehension of assent form content, it is necessary to use age-appropriate words, language, and expressions in the forms hospital. It is also recommended to create comics or videos to make the content of the assent forms more accessible for children.

영산강 하구의 방류와 강우의 규모 및 빈도 상관성 분석 (Relationships on Magnitude and Frequency of Freshwater Discharge and Rainfall in the Altered Yeongsan Estuary)

  • 하구둑으로 인해 담수의 공급이 간헐적으로 발생하는 영산강 하구에서는 담수의 방류가 하구의 유동패턴, 염분농도의 변이, 영양염 공급 등 하구환경과 생태계 반응을 주도하는 요인이다. 담수의 방류는 유역의 강우조건에 영향을 받으므로 담수의 방류시기 및 규모를 파악하기 위해서는 담수방류를 유발하는 강우조건과 강우-방류간 상관관계에 대한 이해가 필요하다. 또한 담수방류가 하구에 미치는 영향을 평가하기 위해서는 방류의 규모와 더불어 빈도에 대한 고려가 필수적이다. 이 연구는 영산강 하구역의 담수방류를 예측하고 평가하는 도구로서 영산강 하구둑의 담수 방류자료를 대상으로 강우에 보다 직접적으로 반응하는 극치방류의 확률분포함수를 파악하고 극치방류를 유발하는 강우조건을 판별하여 극치방류를 유발하는 강우와 극치방류 간의 빈도-규모 관계식을 도출하는 데 목적을 두었다. 13.7년(1997.1.1-2010.8.31)간의 일방류량 자료에 대하여 일연속방류를 분석의 기본단위인 방류이벤트로 정의하되 4일 이상의 일연속방류의 경우는 방류패턴에 토대를 둔 이벤트 분리 알고리듬을 적용하여 개별 이벤트를 식별하였다. 총 529건의 방류 이벤트에 대하여 14년간의 연 최고치 중 최솟값에 해당하는 133,656,000 $m^3$을 기준으로 극치방류이벤트를 선별하고 부분시계열 빈도분석법을 적용하여 극치방류의 확률분포함수가 Weibull(k=1.4)함수를 따름을 확인하였다. 극치방류를 기준으로 대비되는 강우 l일전 3일합 강우량이며 최솟값은 50.98 mm인 것으로 나타났다. 이 기준에 따라 추출된 방류유발기능 강우군은 총 102건으로 극치방류이벤트의 수보다 많았다. 정준판별분석을 통해 3일합 강우량 이외에 관리수위대비(-1.35 m EL.) 하구호 수위가 방류유발 강우를 규정하는 중요한 요소라는 점과 방류유발가능 강우군을 선별하는 임계값을 104mm로 재조정할 경우 3일합 강우량만으로 방류유발 강우를 규정할 수 있음을 확인하였다. 극치방류 유발 강우만을 대상으로 강우-방류 관계식을 수립한 결과 3일합-강우량($\overline{r_{3day}}$), 3일합-강우량 재현주기($T_{r3}$), 방류량(Discharge, D), 방류량 재현주기($T_d$)의 관계는 $D=1.111{\times}10^8+1.677{\times}10^6{\overline{r_{3day}}$, (${\overline{r_{3day}}{\geqq}104$), $T_d=1.326T^{0.683}_{r3}$, $T_d=0.117{\exp}[0.0155{\overline{r_{3day}}]$로 나타났다. 100년 주기 3일합 강우(357mm)에 의해 유발되는 방류량은 영산강 하구둑 방류량의 재현주기는 30.8년 정도이며 그 양은 $7.0979{\times}10^8m^3$이다. 담수방류의 재현주기 및 재현주기에 기초한 강우-방류 관계식은 영산강 하구역을 비롯하여 인위적으로 담수의 방류가 조절되는 하구역에서 담수의 영향을 평가하고 예측하는 데 기여할 수 있을 것이다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. To use as the structural matrix of the composite, collagen was purified from human umbilical cord. We propose a simple adaptive algorithm to increase capability of noise cancelation in QRS complex detection with two stage adaptive filter. At the first stage, background noise is removed and at the next stage, only spectrum of QRS complex components is passed. Two adaptive filters can afford to keep track of the changes of both noise and QRS complex.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. We use combination of MLP and fuzzy filter for pattern recognition to estimate the direction of a muscle and Hogan's method for the force estimation. EMG signals are acquired by using a amputation simulator and 2 dimensional joystick motion. 민담 '주인집을 망하게 한 하인'의 분석심리학적 이해: 트릭스터 원형을 중심으로 (An Interpretation of the Folktale 'the Servant Who Ruined the Master's House' from the Perspective of Analytical Psychology: Centering on the Trickster Archetype)

  • 본 논문을 통해 한국 민담 '주인집을 망하게 한 하인'의 심리학적 의미를 고찰하였다. 민담 속 주인과 하인의 대립은 보편적인 인간 정신의 문제로, 경화된 기존의 집단적 의식과 이를 보상하고 갱신하려는 새로운 의식의 대립으로 볼 수 있다. 다른 각도에서 설명해보자면 인간의 정신적인 측면과 본능적인 측면 사이의 혹은 의식과 무의식 사이의 대립이며, 자아와 그림자 사이의 대립이라고도 할 수 있다. 민담 속 주인은 애먹이는 막내 하인을 없애버리려 여러 차례 시도하지만, 하인은 꾀와 속임수를 써서 주인으로부터 음식과 말(馬), 막내 누이, 전 재산, 마침내 목숨까지 빼앗아 버리고, 이야기는 막내 하인과 막내 누이의 혼인 생활로 끝을 맺는다. 주인이 죽고 하인이 새로운 주인이 되는 대극반전(enantiodromia)은 낡은 집단적 의식이 파괴되고 집단적 무의식으로부터 올라온 새로운 의식이 지배적 위치에 서게 되는 것으로, 개인의 심리적 상황에서는 기존의 자아의 태도가 해소되고 새로운 태도로 변환되는 것으로 이해해볼 수 있다. 이야기의 중간 과정에서 하인은 그를 죽이려고 주인이 써준 등편지를 순박한 사람들을 이용해 새롭게 바꿔 써서 막내 누이와 혼인한다. 이 모습은 집단적 의식의 도덕관념에서는 부정적으로 이해될 수 있으나, 아낙네, 꿀장수, 배고픈 중으로 상징되는 조선 시대 집단적 의식에서 무시되어오던 정신요소를 통합하는 과정으로 볼 수 있다. 하인으로 대변되는 새로운 의식성은 기존의 틀에 구속받지 않는 트릭스터적 특성을 갖기에 집단적 의식에서 무시되어온 정신적 내용을 통합하여 조선 후기 집단적 의식에 대한 보상과 대안으로서 제시하는 요소라고 할 수 있다. 주인은 다시 하인을 죽이려고 하인을 가죽 부대에 넣어 나무에 매다는데, 가죽 부대 속에 들어가 매달렸더니 눈을 떴다는 하인의 속임수에 넘어간 소경이 대신 매달려 죽고 하인은 달아나게 된다. 주인과 하인의 대극 문제가 마침내 나무로 상징되는 전체 정신(Self)에 맡겨지자 소경이 제거되는 것은 트릭스터에 포함되어있는 맹목성, 어리석음, 탐욕적 요소를 구분하고 정화하려는 자기(Self)의 의도로 이해해볼 수 있다. 이런 과정들을 거쳐 집단적 의식의 새로운 변화 혹은 새로운 자아의 태도를 상징하는 하인은 기존의 문제들을 해결하고 주인의 자리에 서게 된다. 그러나 꾀 많은 하인의 활약상을 들으며 청중들은 유쾌함과 해방감을 느끼는 한편, 소경이 대신 죽고 주인집 식구가 몰살되고 하인이 주인이 되는 부분에서는 하인의 위험성과 통제 불가능에 대한 우려와 두려움의 감정을 경험하기도 한다. 해외 유화들에 등장하는 트릭스터들 역시 철저히 이기적이고 오직 욕구를 채우고 위험에서 빠져나가기 위해 무고한 존재들을 속이거나 죽게 만드는데, 이들 트릭스터를 처단하거나 교화하려는 노력은 허사로 돌아가고 그들은 달아나버린다. 그러므로 본 민담 역시 이런 원형적 그림자가 매우 위험하다는 것, 그리고 의식이 통제하거나 의식에 동화될 수 없다는 것을 알게 해주고, 그것을 외경하고 관조하도록 하는 목적 의미도 있다고 볼 수 있다. 트릭스터는 기존의 구조와 질서가 경화되었을 때 보상작용으로서 무의식으로부터 올라오는 재생시키는(revivifying) 자연 에너지의 비합리적 발현 양상이다. 그 현상은 기존의 집단적 정신의 입장에서는 파괴적이고 비도덕적일 수 있으나, 도덕적으로 규정할 수 없는 보다 근원적 정신인 집단적 무의식의 기능이라고 보아야 한다. 트릭스터 원형상으로 볼 수 있는 하인은 변환을 가져오는 존재로 파괴성과 창조성이라는 양면성과 모순성을 지니고 있다. 본 민담의 유화들의 결말은 여러 갈래인데 이는 트릭스터의 양면성으로 인해 청중의 마음 반응이 그만큼 다양하다는 것을 반영하며, 트릭스터 문제에 대한 무의식의 다양한 반응을 제시하는 것이기도 하다. 또한 트릭스터란 그만큼 결론이 안 나고 논란을 일으키는 모순덩어리 존재로 의식적 합리적 태도로 통제할 수 없다는 것, 우리 안의 트릭스터 원형에 대해 진지하게 관조할 수 있을 뿐이라는 것을 보여준다.
