Vitamin B-6 Nutritional Status of Breast-fed and Formula-fed Preterm Infants (모체의 비타민 B-6 섭취상태가 조산아의 비타민 B-6 영양상태에 미치는 영향)

  • Concentrations of total vitamin B-6 in human milk as well as individual, B-6 vitamers have important implications for the nutritional management of breast-fed(BF) infants. Vitamin B-6 status was assessed in 3 groups of infants : two groups preterm (PT) BF infants whose mothers were supplemented with 2 or 27mg pyridoxine(PN)-HCI ; a sub group of formula-fed (FF) PT infants. Mothers and infants were assessed weekly during the 28-day post feeding. Throughout the neonatal period, levels of total vitamin B-6 and percentages of pyridoxal(PL) in breast milk were lower in PT than T mothers, even in mothers supplemented with 27mg PN-HCI. Total vitamin B-6 levels in PT milk paralleled maternal supplementation but percentage distributions of B-6 vitamers did not change. Vitamin B-6 intakes of BF preterm infants paralleled their mothers' level of infants in the 2mg group was suggested by vitamin status parameters. Vitamin B-6 inadequacy of infants correlated with their plasma pyridoxal-5-phosphate(PLP) levels and erythrocyte alanine aminotransferase(E-ALAT) activity; all parameters such as plasma PLP, PL/PLP ratio and stimulation % of E-ALAT were highest for FF PT infants. The positive correlation of vitamin B-6 levels in breast milk gestational age may contraindicate its adequacy for some PT infants.

A Survey on the Awareness of Human Milk Bank in Korean Female Health Care Providers

  • Objective: Human milk bank is known as best alternative option for sick infants. However, only two human milk banks (HMBs) are working in Korea. In the present study, we evaluated the awareness on the HMB in female health care provider to find out the issues that would help facilitating HMB establishment in Korea. Methods: The survey questionnaire was developed by a team composed with neonatologists, obstetricians, nurses, nutritionists, and health care specialists. The survey was performed for female health care workers in Soon Chun Hyang Cheonan Hospital between March and June in 2016. Eighty-eight of 110 questionnaires were collected and the results were analyzed. Results: Most of subjects were found to have correct knowledge on the breast milk feeding. However, only 49% of subjects had awareness of the presence of HMB. Unfortunately, less than half of them could get information about HMB by official way including mass media. In addition, 76% of subjects were found not to want to use HMB. Most important reason was found as a concern on the safety of donor milk including life style of donors, infection controls, and possible nutritional loss of banked milk. Conclusion: The purpose and function of HMB was not widely educated and it seems to be one of most important reasons why women have negative concept about HMB in Korea. Thus, further study is warranted with more numbers of subjects after active public education about the HMB.

A Longitudinal Study on Human Milk Volume and Lactational Performance of Korean Lacto-ovo-vegetarians (채식을 하는 수유부의 수유기간별 모유분비량과 수유양식에 관한 연구)

  • The longitudinal changes on human milk volume per day and lactational performance of 23 Korean lacto-ovo-vegetarians(primiparae=11, multiparae=12) from 0.5 to 5 months after parturition have been studied by test weighing method. The human milk volume per day tended to increase during lactation. The mean volume (SD, ml/day) at 0.5, 1, 2, 3, 4, 5 months were 502(102), 692(127), 697(100), 684( 125), 757(52), 703(70), respectively. The overall mean volume was 661$\pm$132ml/day. The peak milk volume during the lactation was observed at most lactating women at the 1st month. The distributions of the individual mean volume during the first 5 months of lactation were found over 750m1(8.7% ), 650-750ml(47.9% ), 550-650m1(2l.7% ), and 450-550ml(2l.7% ) The number of feeding per day was 8.1 ($\pm$1.0) at 0.5 months postpartum, which was consistently decreased. However, the mean volume per feeding was increased from 62($\pm$9.5)ml at 0.5 month to 112($\pm$13.3)ml at 5 months postpartum. The milk volume was correlated with the peak volume. and maternal age. but not with weight before delivery. maternal height and birth weight. No differences of milk volume and lactational performance between facto-ovo-vegetarian and nonvegetarian women were observed. This study suggests that the human milk volume cited in the recommended dietary allowances for Koreitns is over estimated.

  • Infanthood is a crucial period when mental and emotional developments take place along with physical growth. Adequate supply of nutrition is essential during this period and there is an urgent need for proper education and guidance on balanced nutrition. Therefore, this study was designed to assess to current status of lactation and to investigate mothers' perspectives on human milk and formular milk. As random samples for this study, a total of 352 mothers having infant under the age of 4 were surveyed. 27.3% of mother surveyed were breast-feeding, 38.0% were formula-feeding and 34.7% were mixed-feeding. According to the result of this study, the percentage of breast feeding was on the decrease, while the dependence on formular milk was on the rise. The main reason of formula-feeding was insufficient secretion of human milk on mothers part. The major factors that affect to select feeding method were employment status, degree of education, family income and age. Also, the interviewees desired the product fortified with special ingredients that are hot contained in formular milk currently available in the market. The most concerned problem of formula milk was indigestion and the interviewees wanted the formular milk to be improved digestibility.

The Yields of Volume, Thiamin and Riboflavin from the Milk of Cesarian-Section Women, and Their Ingestion in Infants (제왕절개 분만 수유부의 모유, 티아민, 리보플라빈의 분비량과 영아의 섭취량)

  • The present study was conducted to investigate the yields of volume, thiamin and riboflavin from the milk of twenty-five Cesarean-section women, and their ingestion in infants. We measured the consumed volume of human milk of infants by the test-weighing method, and thiamin riboflavin contents were determined by fluorescence spectroscopy during the first 7 days postpartum. Thiamin contents of the milk averaged 24.3, 24.0, 27.4 and 30.2ug/100g. Thiamin and riboflavin contents of the milk both increased during the lactation ps mod(p<0.05). The first secretion of breast milk occurred from the 4th to 5th day postpartum in Cesarean-section women. The consumed volume of human milk was 63, 81, 104 and 133g/day at 4, 5, 6 and 7 days postpartum and the consumed volume of formula milk was 58, 176, 292, 323, 308, 321 and 318g/day at 1, 2, 3, 4, 5, 6 and 7 days postpartum and the consumed volume of formula from the human and formula milk were 26.6, 91.0, 151.0, 172.5, 169.2, 175.9 and 172.9ug/day. Total daily riboflavin ingestion from the human and formula milk were 26.6, 91.0, 151.0, 172.5, 169.2, 175.9 and 172.9ug/day. Total daily riboflavin ingestion from the human and formula milk was 48.7, 166.8, 277.6, 318.2, 312.0, 327.0 and 335.7ug/day at 1, 2, 3, 4, 5, 6 and 7 postpartum. These results suggest that the milk secretion of Cesarean-section women is delayed a few days and the yield volume of milk is less than that of normal delivery women, but thiamin and riboflavin contents of the milk are not different from those of milk of normal-delivery women.

A Comparative Sudy of the Lactose, Total Nitrogen, Total Lipid Content of Preform and Term Human Breast Milk (조기분만 및 만기분만 산모 모유의 젖당, 단백질, 총지질 함량에 관한 비교 연구)

  • The lactose, total nitrogen and total lipid contents of human breast milk were determined longitudinally after protein and term deliveries. Milk samples were obtained from day 2 to 5, on weeks 1, 2, 4, and 6 after preform (PM : n = 24) and term (TM : n = 22) deliveries. The lactose contents of PM and TM increased from 5.28 g/dl and 5.93 g/dl at days 2 to 5, to 7.95 g/dl,7.87 g/dl at 6 weeks, respectively. The total nitrogen contents of PM and TM decreased significantly during the first 6 weeks after birth (from 374 to 220 mg/dl versus 382 to 220 mg/dl respectively). However the total lipid contents of the PM and TM increased during the same periods. Significant differences in the total lipid contents between the PM and TM were 2.15 versus 3.27 g/dl at 2 weeks, and 3.26 versus 2.52 g/dl at 6 weeks, respectively. The energy intakes of preform infants was 134.8 ㎉/kg/d during the 6 weeks of lactation, which satisfied with energy requirements (85-130 ㎉/kg/d) of the preform infants. In Vitro Apoptosis Triggering in the BT-474 Human Breast Cancer Cell Line by Lyophilised Camel's Milk

  • Breast cancer is a global health concern and is a major cause of death among women. In Oman, it is the most common cancer in women, with an incidence rate of 15.6 per 100,000 Omani females. Various anticancer remedies have been discovered from natural products in the past and the search is continuing for additional examples. Cytotoxic natural compounds may have a major role in cancer therapy either in potentiating the effect of chemotherapy or reducing its harmful effects. Recently, a few studies have reported advantages of using crude camel milk in treating some forms of cancer. However, no adequate data are available on the lyophilised camel's milk responsibility for triggering apoptosis and oxidative stress associated with human breast cancer. The present study aimed to address the role of the lyophilised camel's milk in inducing proliferation repression of BT-474 and HEp-2 cells compared with the non-cancer HCC1937 BL cell line. Lyophilized camel's milk fundamentally repressed BT-474 cells growth and proliferation through the initiation of either the intrinsic and extrinsic apoptotic pathways as indicated by both caspase-3 mRNA and its action level, and induction of death receptors in BT-474 but not the HEp-2 cell line. In addition, lyophilised camel's milk enhanced the expression of oxidative stress markers, heme-oxygenase-1 and reactive oxygen species production in BT-474 cells. Increase in caspase-3 mRNA levels by the lyophilised camel's milk was completely prevented by the actinomycin D, a transcriptional inhibitor. This suggests that lyophilized camel's milk increased newly synthesized RNA. Interestingly,it significantly (p<0.003) repressed the growth of HEp-2 cells and BT-474 cells after treatment for 72 hours while 24 hours treatment repressed BT-474 cells alone. This finding suggests that the lyophilised camel's milk might instigate apoptosis through initiation of an alternative apoptotic pathway.

  • In this study, the nutritional composition of colostrum milk obtained from 22 mothers delivering perterm infants and 17 mothers delivering term infants was analyzed. The energy content of colostrum was yielded based on nitrogen, lactose, and lipid data. Preterm milk is more close the nutritional needs of the preterm infant than dose other breast milk. The results are as follows : 1) Protein concentration was significantly higher, but carbohydrate and lipid concentration were lower in preterm milk than fullterm milk. Energy content yielded from total nitrogen, lactose and lipid concentration was higher in the fullterm milk than preterm milk. 2) Calcium was higher in the preterm milk than fullterm colostrum. Phosphate was lower in fullterm milk than pretem milk. Calcium/Phosphate ratio were 2.61/1 in the preterm and 2.06/1 in the fullterm milk. Magnesium was higher in the preterm milk than fullterm milk. Zinc, Copper and Manganese concentrations were tended to lower in the preterm milk than fullterm milk.

  • 모유의 뮤신 복합체는 rotavirus에 특이적으로 결합하여 항 바이러스활동을 보여주는 것으로 나타났다. 자연 상태에서의 뮤신은 몇몇 작은 분자들과 복합적으로 연합되어 있는데 70kda의 glycoprotein, butyrophilin, 그리고 glycosylated component, lactadherin을 포함하고 있다. 그중 rotavirus에 가장 높은 결합력과 항바이러스 활동을 나타내는 Lactadherin은 모유의 유단백질의 하나인 뮤신과 결합되어 분비되는 당단백질의 하나로 분자량이 46kda이고, 지방구막 속에 연합되어있다. 이러한 배경에서 본 연구에서는 한국 여성의 breast tissue로부터 lactadherin 유전자의 cloning 및 in vitro에서의 발현을 유도하여 lactadherin band만을 purify하였고 여기서 얻은 lactadherin을 항체 생산을 위한 항원으로 사용하여 anti-lactadherin antibody를 확보하였다. 이 항체는 human milk에서의 lactadherin을 동정하는데 사용하였는데 western blot결과 lactadherin을 포함하여 몇 몇 단백질들이 확인되었다. Human milk내 mucin은 몇몇 작은 분자들과 복합체를 형성하는 것으로 확인되어졌는데 70kda의 glycoprotein, butyrophilin 그리고 46kda의 glyxosylated component, lactadherin을 포함하고 있는 milk mucin은 associated molecular임을 확인할 수 있었다.

A Web-based Health Information Survey for Korean Human Milk Bank and Current Issues

  • Objective: We evaluated the Korean mothers' level of awareness of human milk bank (HMB) in order to find out the issues in establishing HMB in Korea. These mothers had infants younger than 18 months old to find out the issues that would help establishment of HMB in Korea. Methods: The survey questionnaire was developed by a team composed by nurses, neonatologists, obstetricians, nutritionists, health care specialists and faculty members. The internet survey was performed by sending emails to mothers who are registered to M family members and have children younger than 18 months. In total, 864 questionnaire were gathered from 114,469 answers and the results were analyzed. Results: Fifty two point eight percent of the respondents had awareness of HMB. 79.4% of subjects agreed with the necessity of HMB. On the basis of multivariate logistic regression analysis, we found that the numbers of children, awareness of HMB, the concern on the HMB in Korea, and the expected problems in establishing HMB in Korea were significant factors when evaluating the necessity of HMB in Korea. The common factors associated with positive thinking of potential users, donors and recipients were the awareness of HMB, and the experience of feeding own breast milk to other's babies. Conclusion: More than half of Korean women were found to agree with the necessity of HMB in Korea, even though they had concern on infection controls and management of HMB and the purpose and function of HMB was not widely educated. The establishment of HMB in Korea would be effective with public education and considering their demands.
