통합보건지소 설치 전후 주민들의 보건지소 이용율 변화 및 관련요인 (Utilization Rate and Related Factors of Unified Health Sub-center Among Rural Residents)

  • 이 연구일개 통합보건지소 관할 지역 주민들의 통합보건지소 설치 전후의 보건지소 이용 양상 및 관련 요인, 그리고 보건지소 이용 만족도에 대한 설문조사 결과와 통합 전후 보건지소의 진료 및 보건사업 실적을 비교, 분석함으로써 통합보건지소 운영이 해당 지역 주민들의 보건지소 이용에 미친 영향을 평가하여 향후 효율적인 통합보건지소 운영을 위한 기능 강화와 역할 방향을 제시하는데 기초자료를 제공하고자 수행되었다. 연구 대상지역은 통합 후 보건지소가 폐쇄되는 지역인 경주시 감포읍과 통합보건지소가 설치되는 지역인 양북면이었으며, 통합지역에 인접한 대조지역으로 선정하였다. 연구 대상지역 주민들 중 표본 추출된 일부 주민들을 대상으로 통합 전에 1차 조사(1996년 9월)를 실시하고, 보건지소가 통합된(1997년 11월)후 2000년 2월에 2차 조사를 실시하여 연구 분석 자료를 수집하였고, 통합 전후 각 보건지소의 진료 및 보건 사업 실적 등의 기존 자료를 참고하였다. 최종 연구 분석 대상자는 감포읍, 양북면, 양남면이 각각 156명, 147명, 146명이었으며, 보건지소 이용 경험률은 감포읍 지역에서는 통합 전 26.9%에서 통합 후 21.2%로 감소하였으나, 양북면 지역은 24.5%에서 29.3%로, 양남면 지역은 23.3%에서 35.6%로 증가하였다. 평균 이용횟수는 감포읍 지역에서는 2.6회에서 1.0회로 통계적으로 유의하게 감소하였으나, 양북면 지역과 대조 지역에서는 다소 증가하였다. 통합지역과 양남면에서 보건지소 이용 경험 여부와 이용 횟수의 변화에 모두 유의하게 관련된 요인은 만성질환 유무이었고, 통합지역 내 감포읍과 양북면으로 구분한 지역 변수는 통합보건지소 이용 경험에 유의한 관련 요인이었다. 만성질환 이환자들의 치료 의료기관으로는 병 의원이 가장 많았으며, 통합 지역 중 양북면에서는 통합 후 보건지소 이용이 증가하였다. 보건지소 이용 만족도는 전체적으로 통합 후 증가하였으나, 의료서비스의 유용성 점수는 다른 만족도 항목에 비하여 점수가 낮았고, 지리적 접근성 항목의 점수는 감포읍이 통합 후 유의하게 감소하였다. 통합보건지소의 일반 진료 실적은 통합지역 내 감포읍과 양북면 보건지소의 통합 전 일반 진료 실적의 총계보다 증가하였고, 예방접종 건수, 가정방문 건수, 방사선 촬영 건수, 보건교육 인원 및 상담 건수 등도 증가하였으나, 일부 만성병 관리 및 영유아 관리 인원은 감소하였다. 보건증 발급, 자동차 운전면허 적성검사, 그리고 임상병리 검사는 통합보건지소에서 새로 추가된 의료서비스였다. 양남면 보건지소에서는 일반진료 조제 건수와 보건교육 실시 인원 및 상담 건수는 증가 하였으나, 예방접종 건수, 학교보건사업실시 인원 등은 감소하였다. 이상의 결과를 정리하면 감포읍 주민들은 통합 후 보건지소 이용에 어느 정도 장애를 받고 있고, 통합보건지소 운영 성과는 기존 보건지소의 운영 성과에 비하여 크게 개선되었다고 할 수는 없으나, 통합보건지소에 대한 전반적인 만족도는 증가하였으며 새로 추가된 의료서비스는 주민들의 의료이용에 편의성을 제공하였다고 볼 수 있다. 향후 보건지소의 농어촌지역에서의 역할과 기능을 유지하고 통합보건지소 고유의 설립 취지를 살리며, 지역주민들에게 형평성 있는 일차보건의료가 제공될 수 있도록 보다 효율적인 운영 방안의 모색이 이루어져야 할 것이다.

Improving the brittle behaviour of high-strength concrete using keratin and glass fibres

  • Keratin fibres are waste products of the poultry industry. Natural materials made from chicken feather fibres (CFFs) are used in concrete-reinforced composites in this study. Brittleness is a major problem of high-strength concrete (HSC) that leads to sudden failure at the ultimate capacity of concrete. Hence, this work aims to investigate effects of using CFFs on improving the brittle behaviour of HSC. Two scenarios are performed to analyse the effectiveness of using CFFs. HSC containing different ratios of CFF (0% as the control, 0.5%, 1%, 1.5%, 2%, and 3%) by volume are tested in the first scenario. Glass fibres (GF) are used to replace CFFs in the other scenario. Tests of fresh, hardened and morphological properties for concrete are performed. Results showed the enhanced brittle behaviour of HSC when using both types of fibres. The preferable ratio of both types of fibres is 1% by volume. Flexural and splitting tensile strengths increased by about 44.9 % and 42.65 % for mixes containing 0.1% GF, respectively. While they were increased by about 21.6 % and 21.16 % for mixes containing 0.1% CFF, respectively.

횡구속재의 종류에 따른 고강도콘크리트 기둥의 내력 및 연성에 관한 연구 (A Study on the Behavior of High-Strength Concrete Columns with Variable Lateral Confinement)

  • High-strength concrete(HSC) is a new construction material with enormous potential. Structures using high-strength concrete are to be coming more and more popular. But high-strength reinforced concrete columns show brittle behavior. It, therefore, is necessary to improve the ductility of HSC members. The purpose of this study is to investigate the ductility and strength of high-strength columns with variable lateral confinement under concentric axial load. Five HSC columns with compressive strength 68㎫ are designed with variable lateral confinements such as carbon fiber sheet(CFS), glass fiber sheet(GFS), and metal lath. Test results indicate that specimen confined by CFS show 11% higher maximum strength, 2.74 times ductility than A specimen using hoop. On comparing with the specimen A and B confined metal lath instead of hoop, strength decrease of 3%, and ductility increase of 1.21 times were observed.

  • Hepatic stellate cells (HSC) and the derived myofibroblasts are known to play a central role in liver fibrogenesis. Rhus verniciflua Strokes (RVS) has traditionally been used in Korea herbal medicine for a stomachic tonic. In this study, we observed the effect of RVS acetone extract (Ra) and its five major components on the proliferation, the collagen synthesis, and hepatic fibrosis related proteins mRNA levels in HSC-T6 cells which is a fully activated rat hepatic stellate cell line. Ra inhibited the proliferation and decreased the content of collagen in the HSC-T6 cells. (omitted)

사람구강편평상피암세포에서 유지놀에 의한 세포자멸사 유도 효과 (The effect of eugenol on the induction of apoptosis in HSC-2 human oral squamous cell carcinoma)

  • Objectives: Oropharynx tumors(oral cancer), are caused by tobacco, alcohol consumption, and high-risk human papillomavirus(HPV) infection. Oral squamous cell carcinoma(OSCC) is the most common type of oral cancer and frequently arises from the mucosa of the oropharynx and oral cavity. Despite advances in the diagnosis and treatment(chemotherapy, radiotherapy, and surgery) of oral cancer, over the past two decades, the overall survival rates remains at about 60%. Methods: We pretreated HSC-2 cells with various doses of exposed the cells to eugenol and then we measured cell viability by MTT assay. Results: Cell proliferation was markedly inhibited after eugenol treatment compared to the control. The majority of HSC-2 cells in the control groups showed normal morphology with round regular nuclei. In contrast, apoptotic bodies were seen in the 0.5 mM, 1 mM, 2 mM group. However, the pretreatment with eugenol increased HSC-2 cells apoptosis according to dose-dependency. PI staining quantitatively confirmed the anti-apoptotic effects of propofol. The expression levels of cleaved caspase 3, and Bak significantly increased in HSC-2 cells. Conclusions: These findings indicate that eugenol could be a potential anti-cancer agent for human OSCC and provide valuable data for the development of a novel anticancer strategy.

헤디드 바, 고장력 철근 및 CFRP 바로 전단보강된 세장 고강도콘크리트 보의 전단 거동 평가 (Shear Behavior of Slender HSC Beams Reinforced with Stirrups using Headed Bars, High Strength Steels, and CFRP Bars)

  • 일반 강도의 철근을 고강도콘크리트 구조물에 사용하는 경우, 고강도콘크리트의 취성을 보완하고 그 성능을 십분 활용하기 위해서는 많은 양의 철근이 요구된다. 이는 곧 철근의 과밀 배근이나 자중의 증가와 같은 구조, 시공상의 문제를 야기할 수 있다. 따라서 고강도콘크리트 부재에 적합한 새로운 보강 재료 혹은 공법이 필요한 것이다. 이에 본 연구에서는 100 MPa의 고강도콘크리트를 사용하여 4개의 실물크기 보 시험체를 제작하고, 전단보강재를 달리하여 전단 거동에 대한 효과를 관찰하기 위해 실험을 수행하였다. 4개의 시험체는 각각 일반 철근, 고장력 철근, 헤디드 바, CFRP 바를 전단 스터럽으로 사용하였으며, 이 중 2개의 시험체에는 강섬유를 콘크리트에 혼입하여 그 효과를 관찰하였다. 고장력 철근의 사용은 전단철근에 의한 전단력과 부착력의 증가로 인해 전반적인 전단 성능의 향상을 가져왔다. 헤디드 바로 전단 보강된 부재의 경우에도 헤디드 바의 상당히 높은 정착강도로 인해서 우수한 전단 거동을 보였다. 하지만 본 실험에서 사용된 CFRP 바의 경우, 부착력이 매우 떨어져 전단보강재로서 적합하지 못한 것으로 나타났다. 강섬유 보강 콘크리트를 통해 부재의 연성 및 균열 제어 능력이 향상되었다.

Subaru Strategy for 2020's

  • Strategic plan of Subaru science and operation will be introduced. Currently, Subaru has wide variety of instruments, conducts only classical observations, with less than 5 nights allocation for each proposal. Near future, Subaru will emphasize on surveys, introduce queue mode observations, reduce the number of instruments, and concentrate on large size programs. Large surveys are called Subaru Strategic Programs (SSPs). HSC-SSP is on-going (300 nights for 5 years), PFS-SSP will start at around 2020 (360 nights for 5 years), and IRD-SSP from 2016 (TBD). HSC science includes 1) cosmology with gravitational lensing, 2) lensing studies of galaxies and clusters, 3) photometric redshifts, 4) the Solar system, 5) the Milky Way and the Local Group, 6) AGN/quasars, 7) transients, 8) galaxies at low/high redshifts, and 9) clusters of galaxies. PFS science includes 1) cosmology, 2) galaxy & AGN, and 3) galactic archaeology. Subaru is planning the third pillar instrument, so called ULTIMATE-Subaru, which is the GLAO optical-NIR wide field camera & multi-IFU spectrograph for finding galaxies at ultra high redshift (z>10). Finally the strategy from Subaru to TMT will be presented. Subaru will conduct four major SSPs (HSC, PFS, IRD, ULTIMATE-Subaru) in coming decade to provide targets to TMT. HSC performs wide field surveys to reveal the distribution of dark matter in the Universe. IRD surveys Earth-like young planets to discover ~20 Earth-like habitable planets. PFS studies the expanding Universe to provide a few million emission line galaxies to TMT.

Redistribution of moments in reinforced high-strength concrete beams with and without confinement

  • Confinement is known to have important influence on ductility of high-strength concrete (HSC) members and it may therefore be anticipated that this parameter would also affect notably the moment redistribution in these members. The correctness of this "common-sense knowledge" is examined in the present study. A numerical test is performed on two-span continuous reinforced HSC beams with and without confinement using an experimentally validated nonlinear model. The results show that the effect of confinement on moment redistribution is totally different from that on flexural ductility. The moment redistribution at ultimate limit state is found to be almost independent of the confinement, provided that both the negative and positive plastic hinges have formed at failure. The numerical findings are consistent with tests performed on prototype HSC beams. Several design codes are evaluated. It is demonstrated that the code equations by Eurocode 2 (EC2), British Standards Institution (BSI) and Canadian Standards Association (CSA) can well reflect the effect of confinement on moment redistribution in reinforced HSC beams but the American Concrete Institute (ACI) code cannot.

고강도 콘크리트와 강섬유 보강 콘크리트의 장기거동 특성에 관한 상관관계 연구 (An experimental study on the relationship between SFRC and HSC at long-term response.)

  • In recent years, according to the development of construction technique, the constructions of longer span bridges, taller buildings, deeper offshore structures, and other megastructures are calling for construction materials with increasingly improve properties. So, the demand for high-strength concrete(HSC) have been increased and many new structures have been built using HSC with the compressive strength about 100MPa. However, it is well-known that as the strength of concrete increases, concrete becomes more brittle. Recent studies, however, shown that the brittleness of HSC can be improved by adding some fibers to the concrete. Especially steel fiber reinforced concrete(SFRC) can be used in this case. Many research works have shown that SFRC results in better crack and deflection control, higher shear strength, improved fatigue performance, increased impact strength, reformed flexural strength, advanced fracture toughness and enhanced postcracking resistance. So, this is a study on the long-term response of SFRC applied to HPC about 40MPa. Therefore, in this study, the test results of twenty-six high-strength concrete specimens and steel fiber-reinforced concrete specimens, with steel fiber content of 1 $\%$ by volume were presented. And the results are analyzed by using of the factors of time, mix properties, humidity/temperature, and loading conditions.

Flexural ductility of reinforced HSC beams strengthened with CFRP sheets

  • Externally bonding fiber reinforced polymer (FRP) sheets with an epoxy resin is an effective technique for strengthening and repairing reinforced concrete (RC) beams under flexural loads. Their resistance to electro-chemical corrosion, high strength-to-weight ratio, larger creep strain, fatigue resistance, and nonmagnetic and nonmetallic properties make carbon fiber reinforced polymer (CFRP) composites a viable alternative to bonding of steel plates in repair and rehabilitation of RC structures. The objective of this investigation is to study the effectiveness of CFRP sheets on ductility and flexural strength of reinforced high strength concrete (HSC) beams. This objective is achieved by conducting the following tasks: (1) flexural four-point testing of reinforced HSC beams strengthened with different amounts of cross-ply of CFRP sheets with different amount of tensile reinforcement up to failure; (2) calculating the effect of different layouts of CFRP sheets on the flexural strength; (3) Evaluating the failure modes; (4) developing an analytical procedure based on compatibility of deformations and equilibrium of forces to calculate the flexural strength of reinforced HSC beams strengthened with CFRP composites; and (5) comparing the analytical calculations with experimental results.
