A Facial Nerve Palsy Cases Managed by Yinyang Balancing Appliance (YBA) of FCST, a TMJ Therapy for the Balance of Meridian and Neurological System (턱관절자세음양교정술(FCST)의 음양균형장치를 활용한 구안와사 증례보고)

  • Therapeutic effect of temporomandibular joint postural training by functional cerebrospinal therapy (FCST) for the balance of the meridian and neurologic system was observed in a facial palsy case. An idiopathic facial palsy case of House-Brackmann grade V was managed with FCST, combined with acupuncture and postural training of the cervical and pelvic structures. Clinical outcome measurement was performed by House-Brackmann grade and Yanagihara's scale. The patient showed positive changes in the degree of palsy and subjective symptoms within one month. Further research on the effect of FCST is expected.

Study on the Correlation between Bell's Palsy Severity and Facial Thermography (Bell's palsy의 중증도와 얼굴 체열진단의 상관성에 대한 연구)

  • The aim of this study was to investigate that thermal imaging is objective and effective to evaluate the severity of Bell's palsy. We investigated 19 cases of patients in college of Oriental Medicine, Iksan Oriental medical hospital at Wonkwang university who were diagnosed with Bell's palsy. The degree of palsy was evaluated with House-Brackmann grading system(HBGS) before treatment, then the thermal difference of the face was calculated by thermal imaging, and finally the correlation between HBGS and thermal difference was analyzed. The significance correlation was found between HBGS and thermal imaging's thermal difference. According to the images taken from 17 patients' DITI's temperature value and HBGS's palsy grade, they significantly were correlated. As thermal imaging's thermal differences got larger in number, the palsy grade had increased. These results suggest that thermal imaging identified to be a relatively objective examination method for evaluating the severity of Bell's palsy.

Effect of Facial Chuna Manual Therapy Combined with Korean Medicine Treatmeant for Bell's Palsy

  • The effect of combined Korean medicine treatment including facial chuna manual therapy was retrospectively investigated in patients with Bell's palsy. There were 16 patients (14 males) who received 2 months outpatient treatment following > 14 days of hospitalization. The Yanagihara score and House-Brackmann grade were used to measure the severity of symptoms and recovery. The patients (mean age 58.5 ± 24.74 years) were categorized based on their medical history of hypertension (n = 10), diabetes millitus (n = 10), or oral intake of steroids early in the diagnosis of Bell's palsy (n = 10). There was no difference between patients who had or did not have a previous history of diabetes mellitus or oral steroid. Both the Yanagihara-score and House-Brackmann grade improved significantly in the total patient sample after treatment (p < 0.001). Combined Korean medicine including facial chuna manual therapy may have a beneficial effect on patient symptoms and recovery.

  • Objectives : The aim of this study was to report that effect of Korean medicine treatment for patient with peripheral facial nerve palsy accompanied by hemifacial spasm through hospitalization. Methods : We treated a patient with facial nerve palsy by acupuncture, electroacupuncture, herbal medicine, pharmacopuncture and cupping. The effect of treatment was evaluated using the House-Brackmann scale, the Yanagihara grading system, and the visual analogue scale(VAS). Results : During the 18 days hospitalization period, the symptoms of hemifacial spasm disappeared and the facial palsy improved. The House-Brackmann scale changed from 4 grade to 2 grade, the Yanagihara grading system changed from 19 to 47, and the VAS changed from 3 to 0. Conclusions : This case report suggests that Korean medicine could be effective for facial palsy accompanied by hemifacial spasms.

  • Background and Objective : The evaluation of facial nerve function is a complex procedure. The House-Brackmann facial nerve grading system(H-B grade)and the Yanagihara grading system(Y-system) have been recommended as universal standards for assessing the degree of facial nerve palsy. The purpose of this study is to compare the effectiveness of Y-system and H-B system and to know the advantages of each system for each stage of incomplete recovery or sequelae of facial paralysis. Materials and Methods : A comparison between H-B grade and Y-system was studied with 137 evaluations of 61 cases of incomplete recovery or sequelae of peripheral facial paralysis. Each case was graded by using H-B system as a gross system and Y-system as a regional system before treatment, after 12 weeks and 6 months Results and Conclusions : The range of score in the Y-system 0-6, 8-16, 14-22, 24-34, 32-38, 38-40 were matched with grade VI, V, IV, III, II and I in the H-B system. The percentage of H-B grade III was the greatest among 137 evaluations and y-system showed the greatest score range. H-B system is easy to use but Y-system is more objective, quantitative and convenient to use in the incomplete recovery or sequelae state.

  • Objective: This study is designed to evaluate the effects of Korean medicine treatment for facial nerve palsy. Method: The patient was administered Korean medicine treatment, including acupuncture, pharmacopuncture, and herbal medicine. We used the House-Brackmann Grading System and Yanagihara's Unweighted Grading System to assess the symptoms of facial nerve palsy. Results: The patient's House-Brackmann grade was reduced from grade IV to grade II, and Yanagihara's grade was increased from 19 to 32. The patient's symptoms, which were facial pain, facial asymmetry, reduced wrinkling forehead, incomplete closing eye, epiphora, and tinnitus, improved during the hospitalization. Conclusion: This study shows that Korean medicine treatment may be an effective treatment for facial nerve palsy.

  • Objectives : This study was performed to find the relation between prognosis of peripheral facial palsy and blink reflex. Methods : Data was collected from patients who were admitted and treated by East-West Collaborative Treatment of Facial Palsy Center in Kyung Hee University Hospital at Gangdong from January 2012 to June 2013. Patients were screened by inclusion/exclusion criteria and 163 patients' medical records were reviewed including blink reflex performed $14{\pm}2$ days after onset and House-Brackmann grade. Patients were divided into three groups, normal, delayed and absent group by blink reflex test. Trends of each recovery rate to House-Brackmann grade II and I depending on three group were analyzed at 1 month, 3 months and 6 months after onset using Linear by linear association. Results : At 1 month, 3 months and 6 months after onset, both recovery rate to House-Brackmann grade II and I had a tendency to be decreased statistically significantly in order of normal, delayed and absent group. Conclusions : We could find feasibility of blink reflex as a prognostic factor of peripheral facial palsy. Further study will be necessary.

  • Objective : The purpose of this study is research on three cases of recurrence facial palsy with sequelae. Method : The authors observed patients by the House-Brackmann grade system and by the Scale of Peitersen for operated acupuncture treatment, herbal treatment and herbal acupuncture. Result : After the treatment, the House-Brackmann grades were decreased in two cases of three and the Scale of Peitersen grades were decreased in two cases of three. Conclusion : These result suggest that early admission with Korean medicine treatment could be more effective in recovery of recurrent peripheral facial nerve palsy with sequelae.

Changes in Facial palsy Patient's Quality of life based upon Oriental-Western Medicine Treatment (한양방 협진치료가 안면마비환자의 삶의 질 변화에 미친 영향)

  • Objective : In period of convalescence and aftereffect, facial palsy patients suffer from social and psychological problems, besides experiencing physical inconvenience. So Quality of life is important Evaluation in treatment or facial palsy. Nevertheless the aims of recent study were only trying to explain about objective symptoms. Therefore, Oriental-Western Medicine was performed, effectiveness of treatment were measured in Quality of life. Methods : Acute facial palsy patients who visiting whin 5days completed questionnaire about Quality of life, if he(or she) participated voluntarily. Questionnaire are comprised of general characteristics, Facial Disability Index(FDI), WHOQOL-BREF, VAS and House-Brackmann grade. Questionnaire used two times, the first medical examination and 4weeks later after starting Oriental-Western Medicine. The statistical analysis was performed by GraphPad Prism 4.0. T-test was used to verify effectiveness between the two groups. Results : 1. When we compared the first medical examination with 4weeks later, score of FDI-Physical function and FDI-Social/Well-bieng function increased but they were not valid statistically. 2. When we compared the first medical examination with 4weeks later, in WHOQOL-Brefoverall domain and physical domain, score increased. In WHOQOL-Bref-psychological, Social, Environment domain, score decreased. but, they were not valid statistically. 3. VAS, House-Brackmann grade decreased, but, they were not valid statistically. Conclusion : The number of subjects with facial palsy in our study(N=5) was too small, and the period of study(4 weeks) was short, too. For this reason, our data were not valid statistically. But Facial palsy Patient's Quality of life has risen.

Correlation of Internal & External Factors with the Beginning Period of Improvement in Idiopathic Facial Paralysis (특발성 안면마비에서 내외적 요인과 호전시기와의 상관관계)

  • Objectives : The purpose of this study was to investigate the correlation between patients' characteristics and the beginning period of improvement, as well as contribute to the efficient management of Bell's palsy patients. Methods : The subjects were 94 patients with Bell's palsy. This study was carried out through the use of an administrative database that included patients' characteristics and clinical information. The analysis of the beginning period of improvement by gender, hypertension, diabetes, drinking history, smoking history and facial palsy history was conducted by independent sample t-test. The analysis of the beginning period of improvement by age, House-Brackmann grade, Yanagihara scale and period receiving Korean medical treatment was conducted by Pearson's correlation analysis. Further analysis of the beginning period of improvement by associated symptoms and seasons was conducted by one-way analysis of variance. Results : 1. Significant correlations were not found between the beginning period of improvement and gender, age, season, smoking history, drinking history, facial palsy history, House-Brackmann grade, Yanagihara scale, hypertension, diabetes or associated symptoms. 2. There was significant correlation between the period of receiving Korean medical treatment and the beginning period of improvement. Conclusion : In this study, the earlier that patients received korean medicine treatment after onset, the earlier that the beginning period of improvement could be seen. Therefore, for the efficient management of facial paralysis patients, it is expected to help secure a baseline.
