Atypical triggers in trigeminal neuralgia: the role of A-delta sensory afferents in food and weather triggers

  • Background: Trigeminal neuralgia is a debilitating craniofacial pain syndrome that is characterized by paroxysms of intense, short-lived electric shock-like pains in the trigeminal nerve distribution. Recently, the presence of triggers has become one of the key diagnostic criteria in the 3rd edition of the International Classification of Headache Disorders. Light touch is the most common trigger, however other non-mechanical triggers, such as cold weather and certain foods, have been thought to provoke trigeminal neuralgia anecdotally. We aimed to characterize the prevalence and characteristics of these atypical triggers. Methods: We conducted a retrospective, cross-sectional study of atypical triggers in trigeminal neuralgia patients seen in a tertiary pain clinic in Singapore. Patients were recruited via clinic records, and study data were identified from physician documentation. Results: A total of 60 patients met the inclusion criteria. Weather triggers were observed in 12 patients (20%), of which five patients (8%) reported strong winds, 4 patients (7%) reported cold temperatures, and 3 patients (5%) reported cold winds as triggers. Fifteen patients (25%) had a specific food trigger, of which 10 patients (17%) reported hard or tough food, 5 patients (8%) reported hot/cold food, 4 patients (7%) reported spicy food, and 2 patients (3%) reported sweet food as triggers. Conclusions: Although trigeminal neuralgia is most commonly triggered by mechanical stimuli, atypical triggers such as cold temperatures and certain foods are seen in a significant proportion of patients. These atypical triggers may share a common pathway of sensory afferent Aδ fiber activation.

  • The following conclusions are obtained from the studies on the chapters concerning phisiology, pathology, daily conduct, materia medica, pulse, and urinalysis from , of the "Four Tantras". 1. The theoretical basis forming the Tibetan Medicine is the substance of phlegm, bile, and wind each of which is divided into five kinds. These phisiological substances can be transformed into pathological factors when certain environment is formed. 2. In embryology, the semen of the father and menstral blood of mother is considered the most important condition in conception, and the Five factors are regarded as important. There is a detailed explanation of development of the fetus while it stays in the womb of mother during 38weeks, such as the formation of the viscera, channels, sense organs, etc. 3. There is metaphor which compares the human body with the king's palace. With the development of human anatomy, there is the detailed anatomical picture of anterior and posterior aspects of human body. And also there is the measurement of physiological constitution and the three fluids. 4. In division of the channels, they concerned the now of the blood and distribution of the nerve fibers, and each channel is connected with one another. The division of the cannel is namely embryonic channel, channel of existence, channel of connection, and the course of life principle. 5. The seven bodily constituents and three factors of phlegm, bile, and wind are important in sustaining the life of human body as well as growth and maturization, and when their equilibrium is broken, the human body is degnerated, and finally death comes. 6. The signs of death is divided into distant sign, remote sign, certain sign. and uncertain sign, and is used as a clue in diagnosis of the disease. Especially there is a mention about the mechanism of the dreams, and different dreams according to the condition of the patient. 7. In pathology, there is the cause of the disease, the environmental factors which can induce disease, the path by which disease come into the human body, the characteristics of the disease, and the kinds of disease. 8. There is a mention about the conduct, and it is divided into the daily conduct, the mental attitude, and the side-effects which can occur when one puts up with the physiological actions of the body. 9. The daily diet is divided into food and beverage. The food is divided again into grain, meat, fat, boiled food, and spiced food, and the beverage into milk, water, and wine. 10. The pulsation should be taken in lift hand to diagnose heart, small intestine, stomach, spleen, kidney, reproductive organ, and in right hand jungs, large intestine, liver, gall bladder, kidney, and bladder. In the healthy person, the pulse moves 5times in one perspiration, and the type of pulse is constant while the pulse moves 100times. But unhealthy person's pulse is different from this. The urinalysis is the unique part of Tibetan Medicine, and is important in examination of the disease. One should decide which of the three factors are dominant by the obervation of the color, amount of the steam and how long it lasts, odour, foam, and the shape of the swirl in three different states when the urine is hot, warm, and cold. One can determine the life of the patient and which viscera is the cause of the disease by the pulse, and whether the nature of the disease is hot or cold by the urinalysis. 11. The materia medica contains gemstones, minerals, plateau medication, and meat products.

황제내경(黃帝內經)에 보이는 한(汗)관련 서술(敍述)의 특징(特徵)에 대한 고찰(考察) (A Study on the Characteristics of Descriptions of the Perspiration in "Hwangjenaegyeong(黃帝內經)")

  • In Korean Traditional Medicine(abbreviated to K.T.M.), hyperhidrosis and anhidrosis are the targets of the medical treatment. Furthermore sweating appearance is also one of the important symptoms which explain a particular situation of the patient in K.T.M. And at "Sanghanron(傷寒論)" which is a traditional chief clinical bible written by Jang Gi(張機) later Han dynasty(漢代) in China made full use of the various kinds of diaphoresis[汗法] as a main medical treatment with purgation therapy[下法] and emetic therapy[吐法]. So the sweat in itself not only is the disease, but also is one of the symptoms explain a disease pattern. This thesis inquires into "Hwangjenaegyeong(黃帝內經)" referring to sweat which is the origin of recognition to the sweat in K.T.M. Some theses similar to this research had been made progresses and already reported, but most of them have classified the contents into biology, pathology, diagnosis, treatment after the model of western medical theory. In the aspect of comparative studying with other literature and clinic practical using, we found characteristics of referring to sweat in "Hwangjenaegyeong(黃帝內經)". And we classify the characteristics into some categories as follows. 1. There are some terms which make a title including sweat and symbolize the characteristics, for example sweat of soul[魄汗], sweat of death[絶汗], sweat of streaming[灌汗], sweat of weakness[白汗], sweat of sleep[寢汗], sweat of bright and heat[炅汗], sweat of kidney[腎汗], sweat of escaping[漉汗], cold sweat[寒汗], sweat on the head[頭汗], hyperhidrosis[多汗], heavy sweat[大汗]. But there aren't spontaneous sweat[自汗] or sweat like a thief[盜汗] which are the normal terms referring to sweat in history of K.T.M. And there are several descriptions about sweat appearance such as sweating in half of body[汗出偏沮], sweating in the rear end and thigh and knee[汗出尻陰股膝], hyperhidrosis in the neck and aversion to wind[頸多汗惡風], hyperhidrosis in the head and face and aversion to wind[頭面多汗惡風], cannot stopping the sweating under head[頭以下汗出不可止], make a person sweat to one's feet[令汗出至足], sweating like escaping[漯漯然汗出], sweating like soaking[汗出如浴], sweating become moist[汗出溱溱], hardly escaping sweat[汗大泄], escaping sweating[漉漉之汗], sweat moisten the pores [汗濡玄府], ceaseless sweating like pouring[汗注不休] sweating like pouring and vexation[汗注煩心], damp with sweat[汗汗然], sweating spontaneously[汗且自出], removal of fever with sweat drying[熱去汗稀]. That can be divided into sweat region and sweat form. 2. There are detailed explanations of the principle of perspirations caused by hot weather, hot food, hard working and meeting damp pathogen. 3. There are some explanations of the principle of removing fever due to the excessive heat from internal and external body through sweating by replenishing the body fluid. And many descriptions about overcoming the febrile disease by dropping temperature through sweating and many diaphoresis for curing. 4. There are some descriptions about five Jang organs perspirations and attachment of five mucous body fluid to five Jang organs. 5. There are pathogenic progresses after sweating affected by the Six Atmospheric Influences and water. And detailed explanations of disease mechanism a sweat leading to another disease. 6. There are descriptions about various sweat absent situations.

方藥合編 皮膚外科 處方에 대한 分析 (Analysis on the Dermatosrugical Prescriptions in BangYakHapPyun(方藥合編))

  • Subjects : We tried to analysis dermatosurgical prescriptions including 477 WonBang(元方) prescriptions for SangJungHaTong(上中下統) introduced by HwangDoYeon(黃道淵). Methods : Prescriptions in BangYakHapPyun(方藥合編) is generally categorized into SangTong(上統), JungTong(中統), HaTong(下統) which are called PoJe(補劑), HwaJe(和劑), KongJe(功劑) respectively. This study classified and analyzed major diseases and symptoms appeared in dermatosurgical prescription and composition of medicine, as well as in BangYakHapPyun(方藥合編). Results and conclusions : The results of examining dermatosurgical prescriptions in WonBang(元方) of SangJungHaTong(上中下統) in BangYakHapPyun(方藥合編) are as follows; 1. The proportion of dematosurgical prescriptions was SangTong(上統) $\frac{10}{126}$(7.9$\%$). JungTong(中統) $\frac{22}{181}$(12.1$\%$), and HaTong(下統) $\frac{16}{163}$(9.8$\%$), which means that JungTong(中統)(HwaJe 和劑) takes up relatively the largest portion. 2. As for SangTong(上統), upper level herbs used in medicine are Glycyrrhiza uralensis(甘草), Paeonia japonica(白芍藥), Angelica gigas(當歸). Astragalus membranaceus(황기). Ginseng(人蔘), Poria cocos(복령), Atractylodis macrocephalae rhizoma(白朮). Cinnamon(肉桂), Rehmaniniae radix preparat(熱地黃). And these herbs are the components of Sipjundaebo-tang(十全大補湯), one of the most well-known medicine for weak energy and blood(補氣血). 3. As for JungTong(中統), in addition to medicine for weak energy and blood. Ledebouriella seseloides(防風) that removes ill elements on skin surface and Pung(風) called "wind". Limonium tetragonum(桔梗) that eliminates discharges and sputum, Angelica dahurica(白芷) that removes discharge and suppress tumor are applied. Other herbs are Ostericum koreanum(羌活). Skullcap(황령),Schizonepeta tenuifolia(荊芥), Aurantii fructus(地殼), Cimicifuga heracleifolia(升麻), Bupleurum falcatum(柴胡), Lonicerae flos(金銀花). These herbs are more effective for wind-calming treatment. cooling down fever, clearing skin irritation, detoxication. removal of tumor and discharge than replenishing energy and blood. 4. As for HaTong(下統), Angelica gigas(當歸) and Ledebouriella seseloides(防風), the two major herbs for SangTong(上統) and JungTong(中統), are mostly used. In addition, Skullcap(黃芩), Gardenia jasminoides(梔子), Eisenia bicyclis(大黃) are other major components and their key efficacy is to lower fever and KongHa(功下). 5. Herbs applied for SangTong(上統), JungTong(中統), and HaTong(下統) in large quantity are Glycyrrhiza uralensis(甘草) that harmoniously combine different herbal elements and Poria cocos(복령) that discharges humidity and watery elements out of body, removes humid and hot elements, and strengthen gastrointestinal system. Based on this, it is inferred that prescriptions for this study focus largely on treatment of humid and hot elements. In the composition of this prescription, Angelica gigas(當歸), Paeonia japonica(白芍藥), and Cnidium officinale(川芎) are taking up relatively large proportion, which are basic herbs for Samul-tang(四物湯). Therefore, it is incurred here that the concept of "replenishing blood" bears importance in dermatosurgical treatment. 6. As for herb medicines used for more than two types of prescriptions of SangTong(上統), JungTong(中統), and HaTong(下統), most of them are simultaneously used for SangTong(上統) and JungTong(中統), or for JungTong(中統), and HaTong(下統) except for Atractylodis macrocephalae rhizoma(白朮) and Gleditsia sinensis(조각자). This finding implies that prescription or treatment that are simultaneously applied are replenishing and harmonizing, or harmonizing and attacking while replenishing and attacking never go together.

Local Climate Zone과 토지피복에 따른 여름철 야간의 인간 열환경 분석 - 경기도 수원시 호매실 택지개발지구 (Human Thermal Environment Analysis with Local Climate Zones and Surface Types in the Summer Nighttime - Homesil Residential Development District, Suwon-si, Gyeonggi-do)

  • 도시지역의 여름철 밤 시간대 열환경을 알아보기 위해, 토지피복 대분류에 따라 10개의 도시기후지역 지점에서 미기후 자료를 측정하여 인간 열환경지수를 분석하여 보았다. 미기후 요소 비교 분석 결과, 그린인프라 지역이 그레이인프라 지역보다 기온에서는 평균 1.6℃ 최대 2.4℃ 낮은 결과를 보였으며, 상대습도에서는 반대로 그린인프라 지역이 평균 9.0% 최대 15.0% 높은 결과를 보였다. 풍속에서는 그린인프라 지역과 그레이인프라 지역에서 차이가 거의 없는 것으로 나타났으며, 수목의 영향으로 가장 낮은 풍속을 보인 산림지점을 제외하고는 모든 지점에서 유의성이 없는 것으로 나타나, 건물이나 수목의 높이에 의해 결정되는 표면거칠기 뿐만 아니라 지역풍의 영향이 풍속을 결정하는 주 요인인 것으로 보인다. 평균복사온도에서는 수목의 영향으로 산림지역이 가장 높은 값을 보였으며, 산림지역을 제외하면 그린인프라 지역이 그레이인프라 지역에 비해 평균 5.5℃의 저감 효과를 보였다. 평균복사온도를 결정하는 주 요인으로는 하늘시계지수라고 할 수 있다. 인간 열환경지수 분석에서는 그린인프라 지역은 거의 덥지도 춥지도 않는 '중립'의 열지각 단계를 나타내었으며, 그레이인프라 지역은 그것보다 한 단계 높은 '약간 따뜻함'의 단계를 나타내었다. 가장 높게 나온 산림지역을 제외하면, 그린인프라 지역이 그레이인프라 지역에 비해 3.2℃ 저감 효과를 보여 1/2 단계의 인간 열환경 개선 효과가 있는 것으로 나타났다.

  • In the orient, ancestor's perception to natural environment was related on the change of climate. Man had been a essence of the warm blood animal, which had suited in mild climate for living and acting. The weather of hot or cold was not only comfort in human life but influences negatively on human behavior. The most people of oriental was particularly interrelated to seasonal change of climate in earlier time. Because, there are elements such as temperature, humidity and wind in the category of climate, these elements differentiated seasonal change. The main methods of perception of natural environment were observed and classified. Although these methods were in the primitive stage, these coincided the tendency of modern science. For example, confucian was recognized the law of vertical air current that warm air rised and cold air sinked. And they could classify all elements nature based on the principles of 'Yin and Yang(陰陽論)' such as male and female, the sun and moon. shade and light etc. Thus results of the observation and the classification concerned with physical environment can be utilize a wisdom for progressive life of inhabitants. It was a origin of the education in natural environment. Commonness in the viewpoint of environment in the orient if recognized the law of circulation. Buddhism, originated in india, realized that all of phenomena in the world was changed slowly through the principles of cycle(輪廻說) such as male and female, the sun and moon, shade and light etc. Thus results of the observation and the classification concerned with physical environment can be utilized a wisdom for progressive life of inhabitants. 전류센서가 없는 열전모듈의 최대전력점 추적방식 (Maximum Power Point Tracking operation of Thermoelectric Module without Current Sensor)

  • 최근, 지구온난화 등의 문제로 인해 새로운 에너지 기술의 개발이 화제가 되고 있다. 중규모 이상의 출력을 얻도록 최적화된 태양광 및 태양열, 풍력 발전과 같은 신재생에너지 기술과 다르게 에너지 하베스팅기술은 출력전력이 매우 작아 크게 주목받지 못하고 있다. 하지만 최근 모바일 산업이 활성화 되면서 에너지 하베스팅기술의 활용가치가 재평가 받고 있다. 또한, 최대전력점 추적방식기술 역시 활발히 연구가 이루어지고 있다. 본 논문에서는 일정한 저항부하를 위한 열전모듈의 새로운 최대전력점추적 제어방식을 제안한다. 열전 모듈(이하 TEM: Thermoelectric Module)의 V-I곡선특성과 내부저항을 분석하고. 기존의 MPPT제어방식을 비교하였다. P&O(Perturbation and Observation)제어방식은 전압, 전류를 측정하기위한 센서 2개를 사용해야하기 때문에 CV제어방식보다 경제성이 떨어지지만 보다 정확히 MPP를 찾는다는 장점을 가진다. CV(Constant Voltage)제어방식은 전압센서 1개만 사용하기 때문에 경제적인 측면에서는 P&O제어방식보다 뛰어나지만, MPP가 정확히 못하다는 단점을 가지고 있다. 본 논문에서는 두 제어방식의 장점만을 가지고 TEM의 최대전력점(MPP)을 추적하도록 설계하였다. 제안된 MPPT 제어 방식은 PSIM 프로그램을 이용한 모의실험으로 확인하였으며, 하드웨어제작을 통해 제안된 MPPT제어 방식을 검증하였다.

층류박리 후향계단 유동의 이중주파수 가진 (Double Frequency Forcing of the Laminar Separated Flow over a Backward-Facing Step)

  • The effect of local forcing on the separated flow over a backward-facing step is investigated through hot-wire measurements and flow visualization with multi-smoke wires. The boundary layer upstream of the separation point is laminar and the Reynolds number based on the free stream velocity and the step height is 13800. The local forcing is given from a slit located at the step edge and the forcing signal is always defined when the wind tunnel is in operation. In case of single frequency forcing, the streamwise velocity and the reattachment length are measured under forcing with various forcing frequencies. For the range of 0.010〈S $t_{\theta}$〈0.013, the forcing frequency component of the streamwise velocity fluctuation grows exponentially and is saturated at x/h = 0.75 , while its subharmonic component grows following the fundamental and is saturated at x/h = 2.0. However, the saturated value of the subharmonic is much lower than that of the fundamental. It is observed that the vortex formation is inhibited by the forcing at S $t_{\theta}$ = 0.019 . For double frequency forcing, natural instability frequency is adopted as a fundamental frequency and its subharmonic is superposed on it. The fundamental frequency component of the streamwise velocity grows exponentially and is saturated at 0.5 < x/h < 0.75, while its subharmonic component grows following the fundamental and is saturated at x/h= 1.5 . Furthermore, the saturated value of the subharmonic component is much higher than that for the single frequency forcing and is nearly the same or higher than that of the fundamental. It is observed that the subharmonic component does not grow for the narrow range of the initial phase difference. This means that there is a range of the initial phase difference where the vortex parring cannot be enhanced or amplified by double frequency forcing. In addition, this effect of the initial phase difference on the development of the shear layer and the distribution of the reattachment length shows a similar trend. From these observations, it can be inferred that the development of the shear layer and the reattachment length are closely related to the vortex paring.

학질(瘧疾)의 증상(症狀)과 기전(機轉)에 대한 문헌적(文獻的) 고찰(考察) -청대(淸代)까지 중국의서(中國醫書)를 중심(中心)으로- (A study of symptoms and pathogenesis of hakgil(瘧疾) in the chinese traditional medical literature until chung(淸) dinasty)

  • The hakgil is the important disease in the oriental medicine historically. In the preseant time also this disease continually appear all over the world. So purpose of this study is that consider the symptoms and pathogenesis of hakgil(瘧疾) with the point of view of oriental medicine. And in this study, the results are summarized as the followings. 1. The symptos of hakgil(瘧疾). 1) Rigor and heat spasm : The main symptoms of hakgil is the severe and periodical rigor and heat spasm. Generally the rigor first appear and later the heat spasm appear. According to the first and last, severe and weak, the hakgil is classified to hanhak(寒瘧), onhak(溫瘧), danhak(癉瘧), binhak(牝瘧). 2) The regulation of the time of spasm : The spasm occour in the same time daily or one time in two days, three days or several days. And the spasm time is regulary in day or night. 3) The term between the spasm and next one become later or faster. It can be decided that the becoming worse and better in the disease with the signs. 4) The seasonal property Generally the hakgil appear in summer and early autumn. 5) The other kind of hakgil there are five-organ hakgil(五臟瘧), six-kyung hakgil(六經瘧), janghak(瘴瘧), kuihak(鬼瘧), six-gi hakgil(六氣瘧), damhak(痰瘧), sikhak(食瘧), and so on. 6) The pulse condition of the hakgil is chiefly hyun(弦). 2. The pathogenesis of the hakgil 1) The cause of the hakgil The causes of the hakgil first are the seo(暑) or heat(熱) that make the problem in the cycle of five phases(五行). In the consequence, il open the hole of skin so that the pathogenic factors easily invade the humanbody and at the same time the pathogenic factor in the inside easily come out, that make the spasm. In the second time the pathogenic factor of yin(陰) - wind(風), cold(寒), water(水) invade through the opened skin to combine with the factor in the inside. Such condition make the hakgil and the accessory spasm. 2)The pathogenesis of hakgil(瘧疾) (1) The rigor and heat spasm of hakgil(瘧疾) appear because in summer the human body don't accomplish a task of summer because of hot weather or heat, so in autumn the ki(氣) of human body separate into yin(陰) and yang(陽), and the skin of human body is weaken so the saki(邪氣: pathogenic factors) is easily come into the human body. At this time the circulation of ki(氣) is obstructed, so the jungki(精氣: vital substance) apply to straighten the circulation of ki(氣), if the jungki(精氣: vital substance) help the yin(陰) the rigor spasm appear in the opposit direction the jungki(精氣: vital substance) help the yang(陽) the heat spasm appear. (2) The period of circulation of ki(氣) and jungki(精氣: vital substance) is one day, so the general period of spasm of hakgil(瘧疾) is one day, But if the saki(邪氣: pathogenic factors) come into the human body deeply, the jungki(精氣: vital substance) cannot apply 10 straighten the circulation of ki(氣) every day so the period of spasm become longer.

  • 접착제는 구조물에 두 가지 복합재료를 연결하는 데 사용된다. 과거의 연결방식인 볼트, 리벳, 그리고 핫 멜트 방식과는 다르게 접착제의 경우 위와 같은 재료를 필요로 하지 않는다. 이는 복합재료의 경량화 및 연결부위로 인한 응력 집중점이 없다는 장점이 있다. 본 연구에서는 아크릴계 및 비스페놀-A형 에폭시계 접착제, 더 나아가 탄소나노튜브 강화재를 투입했을 때에 기계적 및 계면특성을 파악하였다. 접착제 자체의 기계적 강도 변화를 보았고, 전단 및 전단 피로실험을 통하여 물리적 특성을 파악하였다. 전단 실험 후에 파단면을 반사현미경을 통하여 관찰하였다. 실험결과 비스페놀-A계 에폭시 접착제를 사용했을 때의 인장강도 및 기계적 피로 저항성이 아크릴 접착제 보다 좋은 것을 확인했다. 또한 탄소나노튜브 입자를 에폭시 접착제에 첨가했을 때 기계적 향상을 확인했다.
