The burden of cancer is growing globally and is one of the top leading causes of death. Information on cancer patterns are essential for effective planning of cancer control interventions. There is limited published information available on pattern of cancer for the state of Odisha, India. The present study was an attempt to explore the pattern and trend of cancer in Odisha. To fulfill the objectives retrospective data available from 2001-2011 at Acharya Harihar Regional Cancer Center (AHRCC), Cuttack, Odisha, were analyzed. Medical records of cancer patients were reviewed and relevant information on diagnosis, primary site and demographic data were retrieved. Data were entered and analyzed using SPSS 16.0 (SPSS Inc.). A total of 74,861 cancer inpatients were registered at AHRCC for the years 2001-2011. The proportion of females outnumbered males with female:male ratio 1.1:1. The number of female cases increased four folds and that of males three fold over the period studied. Malignancies such as oral cancer (16.93%), acute lymphocytic leukemia/non Hodgkins lymphoma (14.09%) and cancer of gastrointestinal tract (21.07%) are leading cancers among males and carcinomas of breast (28.94%), cervix (23.66%) and ovary (16.11%) were leading among females. Findings from this study indicate an overall increase in cancer reporting which could be regarded as proxy measure for overall cancer situation in Odisha. There is scope and need for integrating other government hospitals, existing private health service providers and research institutions across the state for better planning of cancer control program.
The National Health Insurance Expenditure has been increased rapidly since the introduction of the separation of prescription and dispensing in 2000, and this trend of rapid growth in overall spendings rate has been observed predominantly among medical practitioners. This study was conducted to investigate the growth rate and distributional changes in private medical practitioners' expenses from 1999 to 2002 and its determinants using the National Health Insurance claims data. The total increasing rate of all medical practitioners' expenditure paid by the National Health Insurance between 1999 and 2002 was $41.71\%$, which exceeding that of general hospitals by $20\%$p. But the income distribution among each practitioner was improved as the changes in Gini coefficient(from 0.40 to 0.38) and decile distribution ratio(from 0.25 to 0.29) during the same period showed. However, this improvement in distributional patterns is not enough since even in 2002 it turned out that the highest $10\%$ income group earned 33times more than the lowest $10\%$ income group did. Also, higher Gini coefficient was observed in larger cities and some department like plastic surgery, obstetrics and gynecology. The major causes of this differentials in medical practitioners' expenses were factors related to medical demand like proportion of old population, residential economic status in a given area. In addition, providers' economic incentives also played an important role in determining their income distribution. The large income differentials among physicians may imply a skewed distribution of patients and thus long waiting time, inefficient utilization of resources and potential inadequate quality of care. In this sense, unreasonable distributional gaps should be reduced, so effective measures as well as ongoing monitoring would be necessary to correct current distributional problems.
The objective of this study is to examine relevant factors of the service level of aged care facilities. The sample used in this study consisted of 357 aged care facilities in Korea. Data were collected with self-administered questionnaire and 140 returned questionnaire were analyzed by SPSS Version 12.0. The major findings of the study are as follows: First, there was no significant mean difference in the service level by the facility characteristics, except the length of operation. Second, it was found that both administrative characteristics and employer characteristics were positively associated with the level of nursing and supportive services. Third, the study results revealed that the following three variables of employee education and training, community networks, and employer's philosophy and management principles had significant positive effects on the level of nursing services. Meanwhile, the following two variables of employee education and training, and community networks had significant positive effects on the level of supportive services. In conclusion, in order to improve their service level, the managers of aged care facilities in Korea should make efforts to provide more employee education and training, establish networks with the community stakeholders, for example, local clinics and hospitals. It is also recommended for the government to make a policy inducing more qualified private investors to enter the aged care market, as well as to strengthen the qualification of the managers of the public aged care facilities.
Purpose: The purpose was to investigate the relations among job stress, depression, social support, and coping strategies of nurses. Method: The data were collected from 362 nurses. A self-administered questionnaire was used to assess general characteristics, job stress, depression, social support and coping strategies. Results: The prevalence of depression was 41.7%. Scores of job demand and insecurity, and organizational climate were very high. Logistic regressions showed that nurses, who were single, their 20s, had less than a career year, or working in private hospitals, associated with an increased risk of depression. The sub-scales of job stress except interpersonal conflict and lack of autonomy contributed to an increased risk of depression (lower group; OR=0.248, 95% CI:0.14-0.43). Also individual and organizational support and control coping strategies were associated with depression(lower group: OR=2.993, 95% CI: 2.11-6.30; OR=2.993, 95% CI: 1.51-5.65; OR=2.372, 95% CI=1.43-3.93). Conclusion: These findings indicated that the job stress, especially organizational climate, insecurity of job, lack of reward, individual and organizational support, and control coping strategies contributed to a risk of depression. In order to prevent the depression, the organizational support and strategies will be needed. The depression in specific context and organizational climate should be considered in future studies.
Mourning culture has tended to be reduced to mere empty formalities with more simplified regulations. Changes in modern life style make it difficult to perform extended mourning ceremonies and the venues for mourning ceremony have shifted from private homes to chapels of rest in hospitals or Funeral Homes. Mourning clothing, the symbol of filial duty, has gradually been changed in shape. The study purposes were to research in the shapes of modern mourning clothing through field study on mourning clothing manufacturers and to compare traditional mourning clothing with the modern varieties through the actual making of traditional male mourning clothing based on old regulations. The study of mourning clothing through actual making prevents transformation and provides practical research data. The study methods were inquiry into old documents, field study, and actual clothing making. The study results are as follows. First, in terms of shape, traditional and modern mourning clothing are different in Garyeong, Lim and Daehacheok of Choiui. In case of Choisang, traditional clothing has one central plait in its front and rear sides while modern clothing one has 3 single plaits in each side. Second, in terms of sewing, traditional mourning clothing leaves an exterior margin to sew up in Choiui and an internal one in Choisang. However, modern mourning clothing has various types of sewing and plaits depending on the manufacturers and all sewing is done by machine. Third, in terms of material, traditional mourning clothing is made of Korean hemp and features narrow width, while modern clothing is made of Chinese hemp and features broad width.
Background: It is important to understand the perceptions of oncologists to understand the comprehensive picture of clinical presentation of breast cancer. In the absence of clear evidence, clinical practice involving patients of breast cancer in India should provide insights into stages of breast cancer with which women present to their clinics and mode of screening of breast cancer prevalent in Andhra Pradesh. Materials and Methods: A qualitative study was conducted to understand the perceptions of oncologists regarding clinical presentation of breast cancer, stages at which women present to clinics, and mode of screening of breast cancer prevalent in Andhra Pradesh. In-depth interviews (IDI) were conducted with ten practising oncologists from various public and private cancer hospitals in Hyderabad city to understand their perspectives on breast cancer and screening. The data were triangulated to draw inferences suitable for the current public Health scenario. Results: Late presentation was indicated as the most important cause of decreased survival among women. Most women present at Stage 3 and 4 when there is no opportunity for surgical intervention. The results indicate that there is a huge gap in awareness about breast cancer, especially in rural areas and among poor socioeconomic groups. Even despite knowledge, most women delay in reporting due to reasons like fear, embarrassment, cost, ignorance, negligence, and easy going attitude. Conclusions: It is important to improve awareness about breast cancer and screening methods for promoting early screening. The study inferred that it would be beneficial to establish cancer registries in rural areas. Also, the policymakers need to make key decisions which among three methods (breast self examination (BSE), clinical breast examination and mammography) can best be used as a screening tool and how to successfully implement population wide screening program to prevent mortality and morbidity from breast cancer in India.
Background: Financial distress due to the cost of cancer treatments is prevalent among cancer patients. Identifying the level of financial distress and its affecting factors has an important role in providing supportive services. Accordingly, the aims of this study were to determine these parameters among Iranian cancer patients. Materials and Methods: This descriptive-correlational study was undertaken among 262 cancer patients admitted to both private and public hospitals in East Azerbaijan province, Iran. The financial distress/financial well being scale was used to determine financial distress. The data were analyzed using SPSS software using descriptive and inferential statistics (multiple linear regression). Results: Among the 262 cancer patients, 57.3% were male and their mean age was 47.0 years. The mean score for financial distress was 4.12 (2.01). The final regression model demonstrated that the independent variables (predictors) of income less than living expenses, income equal to living expenses, having an employed spouse in governmental job and living with parents, with regression coefficients of -1.029, -0.515, 0.198, and 0.096, respectively, were predictors of financial distress among cancer patients. These variables accounted for 50% of changes in variance of financial distress. Conclusions: Iranian cancer patients have moderate to high levels of financial distress. Considering policies for managing direct and indirect costs of cancer treatments must be followed.
The main purpose of this study was to investigate attitudes toward mental illness and the mentally ill of psychiatric nurses in Korea and to identify the relationships between the nurses' attitudes and demographic variables. Subjects in this study were 122 psychiatricnurses and 111non-psychiatric nurses in active service at 12 hospitals in Seoul. The insrtument used for measuring attitudes was a questionaire developed by Cohen and Struening (1962) referred to as the“Opinions about Mental illness(OMI) Scale”made up to 51 Likert-type items. The findings of this study indicated hatnurses showed negative attitudetoward mental illness and the mentally ill: Very high on social restrictiveness (factor D), low on Mental Health Ideology (factor C), and Interpersonal Etiology (factor E). Since the high score on Factor A, B and the low score on Factor B, C, E reflect a negative altitudes toward mental illness, this study population related extremely negative attitudes compared to any other surveys. And of the demographic variables that related to their attitudes the education degree and the kind of the hospital in active service showed very significant differences. On the basis of the investigation the findings indicated the followings; 1) Althohg attitudes toward mental illness and the mentally ill of psychiatric nurses were not significant differences from non-psychatric nurses, there was a trend that attitudes of psychaitric nurses did show more negative responses rather than those of non-psychiatric nurses. 2) Demographic variables that relate to their attitudes on the OMI scale were the education degree of the respondent and the kind of their hospital in active service. 3) About attitudes toward mental illness and the mentally ill baccalaureate nursesshowed more positive attitudes in Factor A (Authoritarianism), Factor C (Mental Health Ideology) and Factor D (Social Restrictiveness) than diploma program murses. 4) Nurses in active Service in the private hospital revealed more positive attitudes(A,B,C,D) except Factor (E) than those in the national or public hospital. 5) The ages, duration of work, wanted or unwantedG roup of psychiatric ward and satisfactory level of psychiatric nursing service were non-significant.

APP based on the smart phone is being utilized to various scopes such as medical services in hospitals, financing services at banks and credit card companies, and ubiquitous technologies in companies and homes etc. In this service environment, exposures of smart phones cause loss of assets including leaks of official/private information by outsiders. Though secret keys, pattern recognition technologies, and single image authentication techniques are being applied as protective methods, but they have problems in that accesses are possible by utilizing static key values or images like pictures. Therefore, this study proposes a face authentication technology for protecting smart phones from these dangerous factors and problems. The proposed technology authenticates users by extracting key frames of user's facial images by real time, and also controls accesses to the smart phone. Authentication information is composed of multiple key frames, and the user' access is controlled by distinction algorism of similarity utilizing DC values of image's pixel and luminance.
Pan-Korean multi-center trial of doxycycline in the treatment of upper and lower respiratory infection was carried out in the summer(from May to October) of 1975. A total of 204 patients between the ages of 3 and 75 was treated by 37 physicians including the internal medicine, pediatrics and ENT specialist in 17 general hospitals and 2 private clinics all-over the country. The dosage recommended was 200 mg on the first day of therapy followed by 100 mg daily thereafter. The pediatric dosage was 4 mg/kg on the first day and then 2mg/kg on each successive day. The subjective and objective signs of respiratory infection such as temperature, cough, sputum, pain and associated inflammatory symptoms were assessed using the carefully designed protocols and it showed rapid improvement by the fifth day (80%) of therapy. Only minor side-effects were reported and these were mild and mainly limited to gastrointestinal tract. The overall evaluation of the trial showed that 170 patients(83.3%) had a very good or good result, 13 patients (6.4%) a moderate and another 13 patients (6.4%) a negative result.
