The Effects of Aroma Hand Massage on Anxiety and Sleep in Cancer Patients during Hospitalization (아로마 손 마사지가 입원한 암 환자의 불안과 수면에 미치는 효과)

  • Purpose: This study was conducted to investigate the effects of aroma hand massage on anxiety and sleep in cancer patients during hospitalization. Methods: This study used a pre-post non-synchronized quasi-experimental design. Data were collected from 66 participating patients from July to December 2008. The participants were divided into an aroma massage group (n=25), a hand massage group (n=15), and a control group (n=26). The experimental group received aroma hand massage using 1% diluted lavender aroma oil once per day for 3 days. The massage was given by the researcher following the Korea Aromatherapy Health Professional Association massage protocol In contrast control group A received only a hand massage. The state anxiety, sleep condition, sleep satisfaction levels, and sleep duration were measured before and after the intervention trial. Results: The aroma hand massage group experienced in a significant decrease in their state anxiety (F=7.658 p=.001). Moreover, there was a significant increase in their sleep condition (F=18.047, p=.001), sleep satisfaction (F=30.016, p=.001) and sleep duration (F=23.019, p=.001) compared to the other groups according to one-way ANOVA and Scheffe's multiple comparison test. Also, noted was a significant increase in the sleep condition of the hand massage group as in the aroma hand massage group according to the Scheffe test. Conclusion: The study findings indicates that aroma hand massage decreases anxiety and increases the sleep condition, sleep satisfaction, and sleep duration of cancer patients.

Development of Long-Term Hospitalization Prediction Model for Minor Automobile Accident Patients (자동차 사고 경상환자의 장기입원 예측 모델 개발)

  • The cost of medical treatment for motor vehicle accidents is increasing every year. In this study, we created a model to predict long-term hospitalization(more than 18 days) among minor patients, which is the main item of increasing traffic accident medical expenses, using five algorithms such as decision tree, and analyzed the factors affecting long-term hospitalization. As a result, the accuracy of the prediction models ranged from 91.377 to 91.451, and there was no significant difference between each model, but the random forest and XGBoost models had the highest accuracy of 91.451. There were significant differences between models in the importance of explanatory variables, such as hospital location, name of disease, and type of hospital, between the long-stay and non-long-stay groups. Model validation was tested by comparing the average accuracy of each model cross-validated(10 times) on the training data with the accuracy of the validation data. To test of the explanatory variables, the chi-square test was used for categorical variables.

A Case Study on Short Term Hospitalization Program of Korean Medicine Treatment for Postmenopausal Hot Flush and Sweating (단기 입원 프로그램 후 호전된 갱년기 환자의 상열감 및 발한과다에 대한 치험 1례)

  • Objectives: Postmenopausal hot flush and sweating is troublesome symptom to most of climacteric women. It is well known that Korean medicine is safe and effective on climacteric symptoms. So this study aims to report the clinical effect of Korean medicine on postmenopausal hot flush and sweating. Methods: The patient received herbal medication, acupuncture, moxibustion, cupping therapy, Gong-jin therapy, and Korean medicine music therapy during hospitalization (about 8 days). Results: Hot flush and sweating was effectively reduced after the treatment. Visual Analogue Scale (VAS) for hot flush and sweating decreased from 9 to 2. Hot flush score decreased from 20 to 8. Area of hot flush and sweating also markedly decreased. Conclusions: This case shows that Korean medicine is effective on climacteric symptoms.

Weight Gain of the Psychiatric Female Hospitalized Patients (정신과 여자 입원 환자의 체중 증가)

  • Objectives : The purposes of present study were to identify weight increment in female psychiatric patients during hospitalization and to evaluate the relationship between weight gain and daily calorie intake, daily activity and other variables of disease itself. Methods : 20 patients were studied. Body weight were measured once a week, and daily activity(total amounts of walking/day) and total amount of daily calorie intake were measured twice a week. We examined psychiatric and medical illness history, eating disorders' history and family history of eating disorders and obesity. Results: Mean body weight and Body Mass Index(BMI) at admission are 49.40kg and 19.59kg/$m^2$. Mean weight increment during hospital stays of mean 65 days is 4.90kg. Mean amounts of daily calorie intake and daily activity(daily walks) were increased during hospital stays, but not statistically significant. The degree of weight increment is higher in longer hospital stay group, but not statistically significant. Mean body weight at admission of mood disorder group is higher than that of schizophrenia group, but not statistically significant. Conclusion : This results suggested that weight increment in female psychiatric hospitalized patients is present. However, it is not resulted by amount of calorie intake and daily activity level.

Change in Healthcare Utilization by Disease Severity after Case Management for Medicaid (의료급여 사례관리 후 질병 중증도에 따른 의료이용 변화)

  • Purpose: This study examined change in healthcare utilization by disease severity after case management (CM) for Medicaid. Methods: Data were extracted from survey data on "Healthcare utilization and health status of Medicaid beneficiaries" conducted in 2007 and 2008 by the Ministry for Health, Welfare and Family Affairs. This study was designed to compare change in healthcare utilization between the CM group and the non-CM group. The subjects were 528 Type I Medicaid beneficiaries who utilized healthcare more than 365 days during 2006. Results: In beneficiaries having fewer than 3 among the 11 notified diseases, the CM group showed a significantly larger decrease in outpatient day, outpatient expense, medication day, and medication expense than the non-CM group. In beneficiaries having 3 or more among the 11 notified diseases, however, there was no significant difference in healthcare utilization between the CM group and the non-CM group. Conclusion: CM worked effectively on Medicaid beneficiaries outpatient healthcare utilization for mild diseases. However, its effects on hospitalization, which is a major cause increasing the total expense, were not observed. Therefore, a future study is needed to develope strategies to reduce hospitalization and care for Medicaid beneficiaries with severe diseases.

A Case Report of Liver and Renal Function Changes in Stroke Patients Taking Long-term Herbal Medicine (장기간 한약 복용한 뇌졸중 환자의 간 및 신장 기능 수치 변화에 대한 증례 보고)

  • Objectives: This case report describes a patient who had suffered a stroke and who was treated for a long period with herbal medicine in combination with a Western medicine. Methods: Changes in liver function and renal function laboratory tests were measured while the patient took herbal medicine during the 1190 days of hospitalization. Results: During the hospitalization period, the liver function test (LFT) levels were elevated two fold, but after taking the herbal medicine, the LFT levels were stabilized and have remained stable since. Conclusions: The high risk of prescribing herbal medicine by an unlicensed person and the possibility of long-term herbal medicine usage for stabilizing liver function test were shown.

  • We perform an econometric assessment of the role that pharmaceutical innovation-the introduction and use of new drugs-has played in improving the health of Koreans, by investigating whether diseases for which more new drugs were launched had larger subsequent increases in longevity and smaller subsequent increases in hospitalization. Drugs launched during 1993-2012 are estimated to have increased mean age at death from all diseases by 1.71 years between 1995 and 2015 and 1.09 years between 2005 and 2015. We also estimate that new drugs increased the five-year relative survival rate from all cancers combined by 23.2 percentage points-78.5% of the total increase-between 1993-1995 and 2011-2015, and that new drugs launched during 2008-2010 reduced the number of hospital days in 2017 by 13.0 million. If the drugs launched during 2003-2012 had had no effect on other medical expenditure in 2015, the cost per life-year gained would not have exceeded 6332 USD. Therefore, even if we ignore the effect of new drugs on hospital utilization, the drugs launched during 2003-2012 were very cost-effective, overall. When reduced hospital utilization is accounted for, the evidence indicates that, in the long run, pharmaceutical innovation was cost-saving as well as life-year saving.

Factors Influencing Stress in Spouses of Hospitalized Women Diagnosed with Preterm Labor (조기진통으로 입원한 임부 배우자의 스트레스 영향요인)

  • Purpose: The purpose of the study was to identify to identify the nursing needs and stress levels among spouses of women hospitalized with preterm labor, and to determine factors influencing spousal stress. Methods: Data were collected from 95 spouses of hospitalized pregnant women due to preterm labor at a hospital in Gyeonggi province from June to December of 2016. The data were analyzed by descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient, and multiple linear regression. Results: The mean score of spouses' nursing needs was 3.06±0.42 and stress was 1.85±0.44 out of 4.00. The highest score of nursing needs was 3.37±0.51 in assurance and the highest score for stress was 2.26±0.72 for patient's illness and prognosis. There was a significant positive correlation between stress in spouse and nursing needs (p=.004). Stress was explained by nursing needs (β=.28) and hospitalization days (β=.21). Conclusions: The results of this study suggest that appropriate nursing interventions are required to address the nursing needs at the beginning of hospitalization and to reduce the stress among spouses of hospitalized pregnant women diagnosed with preterm labor.

A Case Report on the Effect of Peripheral Body Temperature Increase and Pain Relief in a Patient with Complex Regional Pain Syndrome Type 1 Improved by Buja-tang and Aconitum ciliare Decaisne Pharmacopuncture (부자탕과 초오약침을 병행한 한방복합치료로 호전된 복합부위통증 증후군 제1형 환자의 말초체온 상승 및 통증 경감 효과에 관한 증례 보고)

  • The aim of this study was to investigate the effect of complex Korean medicine treatment especially Buja-tang and Aconitum ciliare decaisne pharmacopuncture on patients with type 1 complex regional pain syndrome caused after a traffic accident. The patients was treated with Buja-tang, Aconitum ciliare decaisne pharmacupuncture, acupuncture and physical therapy during 20 days of hospitalization. Numerical rating scale (NRS), short form McGill pain questionnaire (SF-MPQ), body temperature measurement of the foot, manual muscle test (MMT) were used for assessment. After treatments, NRS of burning pain in the left ankle decreased from 8 to 4-5 and NRS of burning pain in the right ankle decreased from 7 to 4-5. Also, SF-MPQ results, foot temperature measurement and MMT showed improvement during hospitalization. This study shows that complex Korean medicine treatment especially Buja-tang and Aconitum ciliare decaisne pharmacopuncture are effective in alleviating symptoms in patients with type 1 complex regional pain syndrome.

A Case of Acute Facial Nerve Palsy with Hemifacial Spasm Treated with Korean Medicine (편측 안면경련을 동반한 급성기 안면신경마비 한방 치험 1례)

  • Objectives : The aim of this study was to report that effect of Korean medicine treatment for patient with peripheral facial nerve palsy accompanied by hemifacial spasm through hospitalization. Methods : We treated a patient with facial nerve palsy by acupuncture, electroacupuncture, herbal medicine, pharmacopuncture and cupping. The effect of treatment was evaluated using the House-Brackmann scale, the Yanagihara grading system, and the visual analogue scale(VAS). Results : During the 18 days hospitalization period, the symptoms of hemifacial spasm disappeared and the facial palsy improved. The House-Brackmann scale changed from 4 grade to 2 grade, the Yanagihara grading system changed from 19 to 47, and the VAS changed from 3 to 0. Conclusions : This case report suggests that Korean medicine could be effective for facial palsy accompanied by hemifacial spasms.
