MicroRNA super-resolution imaging in blood for Alzheimer's disease

  • We propose a novel blood biomarker detection method that uses miRNA super-resolution imaging to enable the early diagnosis of Alzheimer's disease (AD). Here, we report a single-molecule detection method for visualizing disease-specific miRNA in tissue from an AD mice model, and peripheral blood mononuclear cells (PBMCs) from AD patients. Using optimized Magnified Analysis of Proteome (MAPs), we confirmed that five miRNAs contribute to neurodegenerative disease in the brain hippocampi of 5XFAD and wild-type mice. We also assessed PBMCs isolated from the whole blood of AD patients and a healthy control group, and subsequently analyzed those samples using miRNA super-resolution imaging. We detected more miR-200a-3p expression in the cornu ammonis 1 and dentate gyrus regions of 3 month-old 5XFAD mice than in wild-type mice. Additionally, miRNA super-resolution imaging of blood provides AD diagnosis platform for studying miRNA regulation inside cells at the single molecule level. Our results present a potential liquid biopsy method that could improve the diagnosis of early stage AD and other diseases.

  • The concept of the Hospital run for medical treatment changing to the forms of theme complex hospital with design by the impact of a boutique design. Especially, this design's element emboss with strongly to form a ring around the special hospital as the dentistry, dermatology and plastic surgery, the woman hospital of a lying-in hospital etc. that is the higher value-added industrial among groups of medical, even general hospital that is not depend on the design relatively, the application of the interior design is extending farther. Also, this special hospital shape up become bigger, form a group and competition for admission to medical industry. For dentistry, the more departmentalize and definite service as implant, children dental clinic holding an absolute dominant position about management, and effective environmental exchange can be essential element for the most profits. Accordingly, In this study I would like to see about the space composition of the dentistry that changing with the times, suggest direction from now on the bigger, specialization and a characteristic of a space focused on recently dentistry project.

  • After 1980's, more and more people expect to the mysterious treatment of herb medicine about chronic and degenerated disease, according to growth of income level and old population. So the herb medicine hospital is begin to increase rapidly. But, they apply western hospital plan to the herb medicine hospital project which cause a lot of problem. To solve the problem, we suggest the new design standards for the purpose of this thesis which considered treatment method and system of herb medicine hospital.

  • The development of a general hospital has a close connection with the trend of the society in general and it flows to the ways that satisfies the demanded value of the time. Therefore, various internal changes have occurred since a hospital has built. The changes is appearing through the time not only in the quantitative changes, but also in functional changes and correlations between each department. This study focused on these changes, and observed the St. Mary's Hospital - one of the major hospitals - which is constructed in 1980's. And then, it was divided into the D/T, O.P.D and ward as function that focused on the time of hospital's establishment (1980), the present (2006), and new hospital (2009) to observe the changing aspect of the function and department. As a result, according as age passes, this study shows function of each department is mixed. Among the D/T, O.P.D and ward, mixing functions of O.P.D is most notable. The mixing function brings changes in hospital's arrangement type. In the past, verticality style hospital was evaded, but recently verticality style hospital is appearing. Through this study, it is expected to forecast the new aspects of hospital and apply it to the new project.

  • In overseas-hospital construction, the digital hospital is a trend that is based on the developments of the information and communication technologies, state-of-the-art medical equipment, smart health, and telemedicine. Along with the increasing proportion of IT, this has resulted in the spreading of the concept throughout city-like hospitals and their transformation into digital hospitals. In the hospital-construction business, IT is a key element that will link the modernization of the mechanical, electrical, and equipment systems, construction, and medical equipment for efficiency maximization through integration. The purpose of this study is the analysis of the market-expansion success factors through the construction of a success-story-based, IT-driven overseas-hospital business. The digital-hospital concept and the development process are analyzed through a literature review, and the success factors are analyzed in terms of the cost, time, and quality that are proposed in the project-management body of knowledge. The main contributions of this study regarding the success factors are as follows: First, a cost-side need exists regarding the establishment of strategic-value engineering in terms of increasing the value from the perspectives of the IT and operational infrastructures; second, in terms of the construction time, all of the hospital systems must comply with the established deadlines for the integrated test and commissioning; and lastly, in terms of quality, it is important to ensure that the System Integration digital-hospital services are delivered according to the user requirements.

Development of OSCE by Nurse Managers of One University-Affiliated Hospital for Skill Test of Nurse Recruitment Process (간호사 채용을 위한 간호관리자의 OSCE 개발)

  • To recruit nurses with skills, the hospital must evaluate the skills of the applicants. But objective evaluation of the skills is a difficult. Many hospitals are using written examination or oral test only and are not testing the nursing skills directly when they recruit new nurses. OSCE(Objective Structured Clinical Examination) is a method developed for the evaluation of clinical skills objectively. In this study, 16/20 nurse managers of one university-affiliate hospital were assembled for the project of developing 7 OSCE modules by themselves, and then they ran OSCE using those modules for 40 nursing students. They scored nursing skills of the students using checklists they designed. The whole process was proceeded phase by phase using step-wise approach model. The project was successful; inter-personal consistency of the scores were high; nurse managers became confident in objective skill evaluation; subjective responses for this project were favorable. The hospital adopted OSCE in recruitment process thereafter. This experience might be applied to other hospitals who want to recruit nurses with skills.

Is the Legal Basis for Physical Therapist's Home-Based Rehabilitation Appropriately Prepared?

  • Purpose: This study examined the legal basis related to a physical therapist's home-based rehabilitation. Methods: The policy data were referenced to the comprehensive plan for national health promotion and the guidebook Community Health Promotion Project issued by the Korea Health Promotion Institute and other institutes. The legal data were referenced to the Bill information system on the National Assembly website. Results: The physical therapist's home-based rehabilitation did not have a legal basis in the community-based rehabilitation project (CBR) of the HP 2020 project. On the other hand, according to the Home health care part of the community health promotion project that began in 2013, physical therapists were allowed to play a role from 2020 under Article 16-2 (Public Officials in Exclusive Charge of Visiting Health Management) of the REGIONAL PUBLIC HEALTH ACT. Conclusion: Policies and laws are being developed in the field of healthcare, but the necessary policies and laws in the field of rehabilitation are still insufficient. A bill to rehabilitate in a variety of fields and spaces by modifying the scope of work of physical therapists will be needed.

  • In Japan, a new nursing insurance system was enforced in April 2000, where premiums were paid according to the level of necessary care. Our project, Nutrition Care and Management(NCM) for the elderly was started in 1995, funded by the Ministry of Health and Welfare of Japan. The NCM project was to provide appropriate nutrition care for the elderly and to see that it effectively functions as part of the health care services. There were 4 stages to the project : the first stage was to find out the PEM status among the elderly patients in hospital and home-care settings in Japan. The 2nd stage was to develop and evaluate nutritional assessment methods, anthropometry, resting energy expenditure measuring methods using of portable indirect calorimeter, and the convenient protein energy intake assessment methods, etc. for the elderly patients with PEM risk. The 3rd stage was to examine the effectiveness of the nutrition care plan induced of protein energy supplement and team care in improving nutrition among the elderly patients. The last stage was to develop the NCM set for the elderly patient based on the past three years of scientific evidence. it is expected that the NCM system for the elderly will provide adequate nutritional care management, improve the elderly care environment and create effective resource management.

  • From the perspective of the client, the value of information for use in facility management is very low at the point of use. This is because the facility owner, who is a non-professional, cannot accurately request necessary information from the perspective of long-term facility life cycle management. The purpose of this study is to define the information necessary for facility management and to analyze the project operation method to procur this information to the client using BIM. To this end, this study analyzed the role of client and contractor, project delivery process, standards, and submissions in the BIM-based procurement area through USACE Air Force Hospital project case analysis. As a result of the analysis, if the client, who is the subject of facility operation, defines in advance the type and input method of necessary information in the operation stage, the client can receive the BIM optimized for operation from the contractor. The results of this study can be used to establish standards for receiving information necessary for establishment of procurement process and facility management of public institutions considering BIM introduction.

  • This project is to establish an annex clinic center under 5 floors with gross floor area of 3,000 Pyeong behind existing Chosun university Hospital main building with 600 beds. The goal of the design is connecting the new clinic center with the existing main building and completing a combined hospital that has synergy effects. Given the old condition of the hospital and characteristics of the ground, I tried to embody the image of Medical Pulse, which is very alive with strong pulse, and suggest vision of 21st century hospital. First, one main entrance as a combined hospital. For optimum access and functions, I used the entrance of existing building as the only main entrance of new combined hospital, rearrange the road in front of the main building and improve traffic system, and upgrade the image and function of combined main entrance by planning new atrium united robby that makes link to the main building easier. Second, section planning considering the optimum functions and convenience. I established 4 floors linking functionally with the existing building, minimized vertical traffic line, and enhanced convenience through barrier free environment, which is a horizontal traffic line without barriers, crucial to hospital. Third, combined zoning generating synergy effects. I linked sterilizing room with logistics center by operating room and service bridge, and arranged central treatment department horizontally, Also, by horizontally arranging cancer center and department of nuclear medicine close with PET and cyclotron center, I established identity of specialized departments.
