Case Report of 6 Outpatients Complaining of Uterine myoma (자궁근종(子宮筋腫) 6례(例)에 대한 임상경과 보고)

  • Uterine myoma is benign tumor formed from muscular and connective tissues, which is one of the most common tumors found in women's reproductive organs. Uterine myoma is the most common pelvic tumor and they are diagnosed in up to15 to 20% of women in pubertal period. Although myoma is generally considered to be a slowly growing tumor, in 20-40% of women at the age of 35 and more have uterine myoma of significant sizes with severe clinical symptoms. The most common symptoms include abdominal pain or discomfort, low abdominal pressure sensation, hypermenorrhea and urinary symptoms. The treatment of uterine myoma is just observation, operation and hormone replacement therapy. We treated 6 outpatients who has uterine myoma and visited Kyungwon InCheon Oriental Hospital with Herb-medication and acupuncture. So we have gotten good result from them. After 6 outpatients had taken Guichulpazing-tang(歸朮破?湯), the symptoms were improved and the size of the uterine myoma was reduced significantly, It proves that oriental medical treatment is effective on the patients with uterine myoma, but continuous studies are needed.

Factors Associated with the Use of Medical Care at Hospitals among Outpatients with Hypertension: A Study of the Korea Health Panel Study Dataset (2010-2016) (우리나라 고혈압 환자의 병원급 의료기관 외래이용 관련 요인: 한국의료패널자료(2010-2016)를 이용하여)

  • Background: As the prevalence of hypertension is increasing in Korea, the government is seeking policy actions to manage patients with hypertension more efficiently. In this paper, we aimed to identify factors associated with the use of medical care at hospitals among outpatients with hypertension. Methods: We analyzed a total of 15,040 cases of 3,877 outpatients with hypertension obtained from the Korea Medical Panel database from 2010 to 2016. The dependent variable was whether a patient with hypertension visited a hospital or not; and independent variables were the patient's various socio-demographic, health-related, and heath-status characteristics. We conducted a generalized linear mixed model analysis with logit link for all the cases and then conducted it stratified by gender. Results: As a result of a multivariable analysis, women were less likely than to visit at a hospital (odds ratio [OR], 0.44; 95% confidence interval [CI], 0.32-0.61) and people aged 65 years and older than those aged less than 65 years (OR, 0.71; 95% CI, 0.57-0.89). Residents in Busan, Ulsan, and Gyeongnam were more likely than those in than Seoul, Gyeonggi, Incheon, and Jeju to visit a hospital (OR, 1.40; 95% CI, 1.05-1.86). The likelihood of visiting a hospital was high in people belonging to a group of: the highest level of annual household income (OR, 1.73; 95% CI, 1.30-2.29); Medical care aid recipients (OR, 1.94; 95% CI, 1.34-2.83); people having three or more complex chronic diseases (OR, 1.59; 95% CI, 1.19-2.11); people having diabetes (OR, 1.81; 95% CI, 1.41-2.32); or people having ischemic heart disease or cerebrovascular disease (OR, 6.80; 95% CI, 5.28-8.76). Also, we found that factors associated with the use of medical care at hospitals among outpatients with hypertension differed between genders. Conclusion: A variety of factors seem to be associated with the use of medical care at hospitals among outpatients with hypertension. Future research needs to find a way to help patients with hypertension visit an appropriate medical institution between clinics and hospitals.

  • This study was carried out during 3 years from March 1992 to March 1995, 140 healthy people and 140 C.V.A patients were investigated, at an oriental clinic in Taejon. The data collected was analysed using a SAS. The results are as follows. 1. The Relationships between patients who have undergone one or more abortion procedures and C.V.A occurrence. Prescription Patterns of Benzodiazepine for Outpatients in a Psychiatric Department in Korea (일 종합병원 정신건강의학과 외래환자에서 벤조디아제핀 사용)

  • Objective : This study examined benzodiazepine prescription patterns of outpatients visiting the psychiatry department in a single general hospital in Korea. Methods : A retrospective descriptive analysis of benzodiazepine prescriptions was performed on a database from 2014 in a general hospital in Korea. We analyzed the following factors of adult outpatients: demographic factors such as sex and age, amount of benzodiazepine prescribed, treatment duration, and diagnosis based on the ICD-10. Results : In 2014, benzodiazepines were prescribed to 46.4% of the outpatients. Percentage of benzodiazepine prescription increased with age and was highest in the age group 40-59 years. Prescription was more prevalent in women and the prescription percentage increased by treatment duration. Patients with the F4 diagnosis (neurotic, stress-related and somatoform disorders) were the most highly prescribed group. For all diagnosis groups, prescription was more prevalent in females or similar for both sexes except for patients with F5 diagnosis (behavioral syndromes associated with physiological disturbances and physical factors), with males being more predominant. Conclusion : Despite the concern regarding the rate of benzodiazepine prescription and administration to geriatric patients, long-term prescription and usage among older patients is still prevalent.

Prevalence and Its Correlates of Night Eating Syndrome in Schizophrenic Outpatients (조현병 외래 환자에서 야간식이증후군의 유병률과 관련요인)

  • Objectives : The aim of study was to examine the prevalence of night eating syndrome(NES) and its correlates in schizophrenic outpatients. Methods : The 14 items of self-reported night eating questionnaire(NEQ) was administered to 201 schizophrenic patients in psychiatric outpatient clinic. We examined demographic and clinical characteristics, body mass index(BMI), subjective measures of mood, sleep, binge eating, and weight-related quality of life using Beck's Depression Inventory(BDI), Pittsburgh Sleep Quality Index(PSQI), Binge Eating Scale(BES) and Korean version of Obesity-Related Quality of Life Scale(KOQoL), respectively. Results : The prevalence of night eaters in schizophrenic outpatients was 10.4%(21 of 201). Comparisons between NES group and non-NES group revealed no significant differences in sociodemographic characteristics, clinical status and BMI. Compared to non-NES, patients with NES reported significantly greater depressed mood and sleep disturbance, more binge eating pattern, and decreased weight-related quality of life. While 'morning anorexia' and 'delayed morning meal'(2 of 5 NES core components in NEQ) were not differed between groups, 'nocturnal ingestions', 'evening hyperphagia', and 'mood/sleep' were more impaired in NES group. Conclusions : These findings are the first to describe the prevalence and its correlates of night eaters in schizophrenic outpatients. These results suggest that NES has negative mental health implications, although it was not associated with obesity. Further study to generalize these results is required.

Analysis of the Treatment and Adverse Events of Whiplash Associated Disorder Patients During Pregnancy in a Korean Medicine Hospital (임신 중 교통사고로 일개한방병원에 내원한 환자의 치료 분석 및 이상반응 보고)

  • Objectives: The purpose of this study is to analyze both treatment and adverse events of Korean medicine treatment for whiplash associated disorder patients during pregnancy. Methods: We investigated the medical records of 44 pregnant patients who had got into car accidents from April 1, 2018 to May 31, 2019. Results: 100 percent of the inpatients and 88 precent of the outpatients were treated by acupuncture. 79 percent of the inpatients and 72 precent of the outpatients were treated by chuna, and 86 percent of the inpatients and 64 precent of the outpatients were treated by pharmacopuncture. Out of 32 patients with low back pain, 94 percent were treated by acupuncture, 47 percent by phramacopuncture and 38 precent by chuna. 90 percent of 31 patients with cervical pain were treated by acupuncture, 71 percent were treated by pharmacopuncture and 77 percent were treated by chuna. There were adverse events like cough, rhinorrhea, dilatation of cervix and worsen low back pain during or after the treatment. Conclusions: Korean medicine treatment including chuna and pharmacopuncture can be applied to pregnant women injured by traffic accident.

An Empirical Study on Effectiveness of Hospital Information System and Its Influencing Factors (With Emphasis on the Order Communication System) (병원정보시스템의 유효성 평가와 영향요인에 관한 실증적 연구 - 처방전달시스템(OCS : Order Communication System)을 중심으로 -)

  • The purpose of this study is to find out the successful way of the hospital management focusing on the OCS. More than 40 educational hospitals have OCS. However, only five of them were chosen for the research. Questionnaires are collected from more than 750 persons working in five educational hospitals and some of them were interviewed. The major conclusions of the study can be summarized as follows: o The OCS has simplified the treatment procedure for outpatients, and the outpatients' waiting time has been shortened. o Higher rate of satisfaction was found among the personnel in general. o The number of patients who visited outpatients clinics has increased, so as the hospital profits: the personnel are rescheduled or sent to other departments the number of employees in hospitals have decreased. o The system has the positive effect on large hospitals with over 1000 beds, highly invested hospitals, well computerized hospitals and where the personnel are much interested in the system. o The managers' and the doctors' intention to utilize the OCS is the most important factor influencing the patients' convenience, the simplification of the work and the rate of the employees' satisfaction about their duties. The suggestions for more efficient hospital management through the OCS are as follows ; o The managers and the doctors are need to decide to make active use of the OCS. o The hospital can be run more effective under the clear management purpose. o The work in the hospital must be standardized based on patients' needs. o The OCS must be built and developed mainly for the users' utility. o The education of the personnel and the inspection of the program are necessary before the OCS is introduced. o The hardware should be thoroughly benchmarked before the purchase.

Clinical Analysis of the 991 Outpatients with Gastrointe stinal Symptoms and Extra-gastrointestinal Symptoms (소화기 증상을 주소로 내원한 외래환자 991명에 대한 위장관 증상 및 동반 증상 분석)

  • Objectives: Clinically, patients with dyspepsia often complain of several gastrointestinal (GI) and extra-GI symptoms. However, there have been few studies investigating the relationship between GI symptoms and extra-GI symptoms. The aim of this study was to characterize the clinical features of the 991 outpatients with GI and extra-GI symptom and the relationships between GI and extra-GI symptoms. Methods: 991 patients newly visiting Weedahm Oriental Hospital for GI symptoms were included. Those who had disorders caused by the hepatobiliary-pancreatic system were excluded. The 991 outpatients were interviewed with a standardized questionnaire inquiring about demography, past medical history, past institutional care, GI symptoms and extra-GI symptoms. Results: Among 991 patients, 780 (78.7%) had more than three GI symptoms and 451 (45.5%) had more than five. Among 991 patients, 545 (55.0%) had more than three extra-GI symptoms and 285 (28.8%) had more than five. There was a statistically significant correlation between GI symptoms and extra-GI symptoms such as headache, fatigue, forgetfulness, eyeball pain, unrest, dizziness, muscle pain, chest pain, and dyspnea. Conclusions: We found that there was statistically significant correlation between GI symptoms and extra-GI symptoms, which suggests the possibility of main common pathophysiology between GI symptoms and extra-GI symptoms. Further studies will be required to elucidate the main common pathophysiology between GI symptoms and extra-GI symptoms.

Self-perceived Stigma, Self-efficacy and Quality of Life in Psychiatric Outpatients (정신과 외래 환자가 지각하는 낙인과 자기효능감 및 삶의 만족도에 관한 연구)

  • Purpose: The purpose of this study was to explore self-reported perceived stigma, self-efficacy, and quality of life among psychiatric outpatients Methods: In the present study, researchers analyzed the survey responses of 195 patients recruited in the S hospital psychiatry outpatient clinic. The measurement tools used in this study were the "Devaluation & Discrimination" scale to determine perceptions of stigmatization, general and social efficacy scales to measure self-efficacy, and the "General Well-Being Index" to measure quality of life in psychiatric outpatient. Statistical analysis included means with standard deviation, t-test, ANOVA and Pearson correlation coefficients to identify relationships between the variables. Results: There was a significant difference in self-perceived stigma and quality of life in psychiatric outpatients (r=-.233, p=.001) and a negative correlation between self-efficacy and quality of life (r=-.424, p=.000). Correlation analysis results support the hypothesis that higher self-perceived stigma scores were related to reduced quality of life among patients with schizophrenia (r=-.231, p=.021), while there was a positive correlation between self-perceived stigma scores and self-efficacy scores among patients with bipolar disorder (r=.362, p=.013). Conclusion: The findings suggest that nursing imtervention strategies should include education programs to reduce stigmatization and enhance self-efficacy and quality of life among patients with chronic psychiatric illnesses.

Hospital Financial Performances and Separation of Dispensary from Medical Practice (의약분업 전후 병원 재무성과의 변화)

  • This study analyzes the effect of separation of dispensary from medical practice on hospital management performance. The results are as follow. The earning ratios of large size hospitals become deteriorated significantly after the separation of dispensary. The growth ratios of revenues from inpatient were unchanged, but the growth ratios of revenues from outpatient of large and middle size hospitals were decreased significantly. Hospitals can't sell the medicines to the outpatient after the separation of dispensary, but they can do to the outpatient. The labor cost ratios of small and large size hospitals are increased significantly after the separation of dispensary. There are two reasons for increase of labor cost ratios. One is the decrease of material cost ratio. The other is the increase of doctor's salary. The material cost ratios of every size hospital are decreased significantly after the separation of dispensary. Because medicines costs of outpatients are decreased. The labor cost and doctors' salary per patient of middle and large size hospitals are increased significantly after the separation of dispensary. And average treatment fees per day of inpatients of middle and large size hospitals are increased significantly after the separation of dispensary. But those of outpatients are decreased significantly. Average numbers of outpatients per bed of small and large hospitals are decreased significantly after the separation of dispensary. And average numbers of inpatients per bed of large hospitals are decreased significantly. In summary, as a consequence of separation of dispensary from medical practice, management performances of large size hospitals become deteriorated significantly.
