간호·간병통합서비스 병동 간호사의 조직몰입과 환자안전문화인식이 환자안전간호활동에 미치는 영향 (The Effects of Organizational Commitment and Perceived Patient Safety Culture on Patient Safety Nursing Activities among Nurses in Comprehensive Nursing Care Units)

  • Purpose: The purpose of this study is to identify the effects of organizational commitment (OC) and perceived patient safety culture (PPSC) on patient safety nursing activities (PSNA) among nurses in comprehensive nursing care units. Methods: Participants were 173 nurses working at five general hospitals in Chungcheong area. Data were analyzed using descriptive statistics, 𝑥2 test, t-test, ANOVA, Pearson's correlation coefficient, and multiple regression analysis with SPSS/WIN 23.0 programs. Results: The mean scores of the OC and PPSC were 3.28±0.50 and 3.85±0.35, respectively. The mean score of PSNA was 4.55±0.41, and PSNA was significantly different by the experience of participating in hospital's safety culture campaigns (t=2.70, p=.008). The results of the multiple regression analysis showed that 'patient safety knowledge and attitudes' (β=.27, p=.006) and 'unpunished environment to error' (β=.22, p=.004) as the sub-categories of PPSC were affecting factors on PSNA with an explanatory power of 26.0% (F=6.40, p<.001). Conclusion: The results of this study suggest that in order to promote PSNA among nurses in comprehensive nursing care units, it is necessary to develop a program to enhance patient safety-related knowledge and attitudes. In addition, the hospital's organizational efforts such as operating safety campaigns and creating an unpunished environment to error should be needed.

신생아 집중치료실 간호사의 이직의도에 미치는 영향요인 (Factors Affecting Turnover Intention of Nurses Working in Neonatal Intensive Care Units)

  • Purpose: This study was conducted to assess the factors affecting turnover intention of the nurses working in neonatal intensive care units, including the extent of fatigue, work stresses, and work overload related to infection control. Methods: This was a descriptive correlational study to test factors affecting the turnover intention of the nurses working in neonatal intensive care units. Data was analyzed utilizing descriptive statistics, t-test, One-way ANOVA, Pearsons' Correlation Coefficient, and multiple regressions by using the IBM SPSS Statistics 23.0 Program(IBM Corp., Armonk, NY). Results: The turnover intention of nurses had a positive correlation with the extent of fatigue (r=.54, p<.001), work stress (r=.40, p<.001), and with the work overload (r=.43, p<.001) related to infection control. In addition, factors affecting the turnover intention of nurses included the extent of fatigue (β=.52, p<.001) related to infection control and the number of pediatric patients assigned to each nurse (β=.26, p=.001) and the variances explained by the regression model was 37.0%. Conclusion: It is necessary to establish and implement strategies to lower the extent of fatigue related to infection control by reducing the nurses' workload. Securing additional nurses to ensure an appropriate number of pediatric patients assigned to each nurse, and providing efficient management and available resources to reduce the turnover intention of nurses working in neonatal intensive care units are suggested.

  • Purpose: This study investigated the influence of nurses' organizational communication and self-leadership on patient safety competence in comprehensive nursing care units of small and medium-sized hospitals. Methods: A descriptive design was used and self-reported questionnaires were used to collect data from 165 nurses in Seoul from February to March, 2020. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficient, and multiple linear regression with SPSS/WIN 22.0 program. Results: A mean score of organizational communication was 3.20±0.49, self-leadership 3.58±0.50, and patient safety competency 4.01±0.49 out of 5. A significant positive correlation was found between patient safety competence, organizational communication and self-leadership. Self-leadership and combined ward explained 32% of the variance of patient safety competence (Adjusted R2=.32, p<.001). Conclusion: Patient safety competence in comprehensive nursing care units are associated with self-leadership and organizational communication. Systemic education to improve patient safety competence in small and medium sized hospitals should be implemented to promote organizational communication as well as self-leadership.

호스피스 병동과 일반병동의 말기암환자의 간호중재 비교 (A Comparison of Nursing Interventions with Terminal Cancer Patients in a Hospice Unit and General Units)

  • Purpose: The purpose of the study was to compare symptoms, medical therapies, and nursing interventions with terminal cancer patients during the last four weeks of their lives in a hospice unit and general units. Method: For the descriptive survey study, data were collected by reviewing the medical records of 243 patients who died of terminal cancer at K hospital in Seoul. The data was analyzed by using Chi-square test and t-test. Result: The study findings are summarized as follows: There were higher frequencies in physical symptoms of constipation, itching sensation, pain, sleeping disturbance, soreness and dysuria for those patients in the hospice unit than those patient in general units. All emotional symptoms were recorded significantly higher for those patients in the hospice unit than those in general units. Regarding the major medical interventions, pain management was used more significantly for those patients in the hospice unit, but antibiotic therapy and resuscitation were used more significantly for those patients in general units. Conclusion: The hospice unit provided more comprehensive nursing interventions including psychological, spiritual, and family cares as well as physiological care for terminal cancer patients. The facts showed that those patients who would need hospice care in general units should be referred to the hospice unit at an appropriate time.

전실업무와 연계간호에 대한 신경계 중환자실 간호사의 인식 (Perception of Transfer Job and Liaison Nursing among Nurses in Neurological Intensive Care Units)

  • Purpose: This study aimed to describe how nurses in neurological intensive care units (ICU) perceive their roles about patient transfer and liaison. Methods: A cross-sectional survey was conducted using a questionnaire developed for the study. Data were collected from a convenience sample of 115 nurses working in the neurological intensive care units ofsix university hospitals. Data were analyzed using SPSS software (version 15.0). Results: Staff nurses were mainly in charge of patient transfer, and the awareness about its importance was significantly related with the burden about the practice of patient transfer (p<.001). Patient liaison was mainly conducted by staff nurses as well. Liaison practice wassuggested to be started from the ICU at the time of transfer; the suggested time for completion of liaison service and evaluation was when patients were adjusted to the transferred ward. Preparing job descriptions and increasing nursing staffing for patient liasion service were strongly recommended. Conclusion: The results suggest that the ICU nurses' burden in relation to patient transfer is commensurate with their awareness about itsimportance. To improve the continuity of care from ICU to transferred ward, it isrequired to reduce the burden about patient transfer and improve perceptions about patient liaison.

  • Purpose: Methicillin Resistant Staphylococcus aureus (MRSA) has become a major clinical problem and one of the major nosocomial pathogen worldwide. The aim of the study was to investigate the epidemiological characteristics of genotypes of MRSA isolated in the A-hospital ICU. Methods: In the period between December 2007 and May 2008, MRSA was isolated from ICU patients and its surrounding environment. Polymerase Chain Reaction (PCR) was conducted for the detection of MRSA gene. The incidence of MRSA in the clinical isolates of Staphylococcus aureus was examined by using a multiplex PCR. The spa gene of Staphylococcus aureus encodes protein A and is used for typing of MRSA. We used sequence typing of the spa gene repeat region to study the epidemiology of MRSA at a hospital. Results: Two different genotypes of MRSA were identified with 90 isolated from the patients and its surrounding environments in the ICU. Conclusion: This study may contribute to the development of effective strategies for preventing nosocomial infections. Genotyping may have more general application for the study of MRSA epidemic outbreak in hospital and community infection.

중환자실 간호사가 경험하는 윤리적 딜레마와 대처행위 (Nurses' Experiences of Ethical Dilemmas and their Coping Behaviors in Intensive Care Units)

  • Purpose: This study was aimed to describe ethical dilemmas and types of coping behaviors among nurses who worked in intensive care units (ICUs). Methods: Data were collected by 2 focus group interviews with 12 ICU nurses in an academic affiliated hospital in Seoul, Korea. All interviews were tape-recorded and transcribed, and data were analyzed by modified qualitative content analysis. Results: Three themes emerged from the focus group interviews: "Respect for Persons (2 contents)", "Beneficence (13 contents)", "Justice (1 content)". Coping behaviors against the dilemmas were consultations with the doctors or colleagues, acceptance, guilt, reflection, forgetting, endurance, and frustration. Conclusion: The results of this study help us to understand ethical dilemmas that nurses experienced in ICUs and their coping behaviors. It would be useful to develop education programs for nurses in ICUs to support coping strategies for ethical dilemmas.

  • Purpose: This study aimed to identify stress levels due to end-of-life care, coping strategies, and psychological well-being among nurses in neonatal intensive care unit, and to investigate the effect of stress levels and coping strategies on their well-being. Methods: A total of 128 nurses in the neonatal intensive care units of general hospitals in B city participated. The data were collected using a self-report questionnaire. The collected data were analyzed using descriptive statistics, the t-test, ANOVA, the Pearson correlation coefficient, and hierarchical regression with SPSS version 22.0. Results: The coping strategy that nurses most often used was seeking social support. The factors affecting the well-being of the participants were wishful thinking, problem-focused coping and seeking social support, in order. Those 3 variables explained 21 % of the total variance in psychological well-being. Problem-focused coping and seeking social support were positively associated with psychological well-being, while wishful thinking showed a negative association. Conclusion: In order to improve the psychological well-being of nurses in neonatal intensive care units, it is necessary to provide nurses with a program to build a social support system and to improve their problem-based coping skills.

중환자실 간호사의 의사소통 능력, 소진이 간호업무성과에 미치는 영향 (Influence of Communication Competence and Burnout on Nursing Performance of Intensive Care Units Nurses)

  • Purpose: The purpose of this study was to investigate the influence of communication competence and burnout on nursing performance in intensive care units (ICU). Method: The participants were 209 nurses from four university hospitals. Measurements included a socio-demographic and job related survey, communication competence inventory, the Maslach burnout inventory and a nursing performance scale. Data were collected from February 6 to 24, 2012, with a self-report questionnaire. The statistical analyses were performed with SPSS 18.0 software. Results: The mean scores for communication competence, burnout and nursing performance were 50.49, 84.72, and 62.18, respectively. Communication competence (r=.44, p<.001) and burn out (r=-.32, p<.001) were significantly correlated with nursing performance. In the multiple linear regression, factors influencing nursing performance were communication competence, age and burnout. These variables explained about 46% of the total variance of nursing performance. Communication competence (${\beta}$=.34, p<.001) was the most influential factor. Conclusion: Nurses' poor communication skills and burnout can influence the occurrences of medical errors in ICU nursing performance. The results suggest that good communication and burnout resolution skills programs should be implemented to improve nursing performance efficiency.

중환자실 퇴원환자의 집중치료 후 증후군과 삶의 질 (Post-intensive Care Syndrome and Quality of Life in Survivors of Critical Illness)

  • Purpose: To investigate the post-intensive care syndrome (PICS) and to analyze the factors affecting the quality of life (QoL) of survivors of critical illness. Methods: Subjects were 114 outpatients who had been discharged from intensive care units of a university hospital in B city, Korea. From July 30 through September 30, 2015, PICS was assessed using the Korean Montreal Cognitive Assessment, Hospital Anxiety-Depression Scale, Korean Instrumental/Activities of Daily Living (K-I/ADL) index, and handwriting transformation, while physical and mental health-related QoL was measured using the SF-12. Results: Of the subjects, 39.5% were screened for mild cognitive disorder and 23.7% experienced handwriting transformation after discharge. Multiple regression analysis revealed that restraint application, current job, time of ${\geq}36$ months after discharge, depression, anxiety, and handwriting transformation accounted for 40.9% of the physical health-related QoL, and depression, anxiety and experience of delirium accounted for 62.4% of the mental health-related QoL. Conclusions: It is necessary to make efforts to reduce restraint application in intensive care units and prevent the occurrence of delirium, with the objective of reducing PICS and improving the QoL of critical illness survivors.
