Purpose: The purpose of this study was to describe nurses' experience of health communication with doctors in the clinical fields. Methods: Eight experienced clinical nurses working in various units of general hospitals participated. Data were collected through in-depth individual interviews and analyzed using the Colaizzi' method. Results: Five themes of nurses' experience of communication were identified: exploring the speaker seriously, becoming aware of emotions and controlling them, developing relationships after lining up for battle, having superficial and withered relationships with doctors, having few resources to promote communications with doctors. Conclusion: The results of the study indicate that humanistic and warm interpersonal relationships and genuine sharing of information are necessary to ensure effective communication. Thus, practice guidelines should be re-evaluated and developed to meet the proposed standards; safety-net programs and education for health communication should also be established. Nurse managers should create a high-touch work environment.
Purpose: This study was conducted to assess the knowledge and performance of developmentally supportive positioning for premature infants (DSPP) among neonatal intensive care unit (NICU) nurses. Methods: The participants in this study were 131 nurses working in the NICU at five university hospitals in Daejeon and the city of Cheong-ju. The collected data were analyzed with descriptive statistics, the t-test, analysis of variance, and Pearson correlation coefficients using SPSS for Windows version 22.0. Results: NICU nurses' average DSPP knowledge score was 24.7 out of 33.0, and their average DSPP performance score was 3.2 out of 4.0. A statistically significant positive correlation was observed between knowledge and performance of DSPP (r=.32, p<.001). Conclusion: To improve the level of NICU nurses' knowledge and performance of DSPP, educational programs should be developed and their effectiveness should be verified.

Purpose: At the moment, There is a facility standard for intensive care units on the Korea medical services Act. However, the contents of regulation are so limited and difficult to apply. Therefore, it is necessary to draw up the guideline to complement the regulation for efficient operation of intensive care units. This study aims to suggest basic data for guideline of intensive care units. Methods: 2 Methods have been used in this paper. 1) Comparative analysis between the national regulation of Korea with the design standard of the United States America, the United Kingdom and Australia. 2) A literature survey about operation method for intensive care unit, focused on materials published in intensive care medicine society of each country. Results: In comparison with guideline of abroad cases, additional supplement are required, with regard to item such as operation environment, size, nursing manpower ratio, bed space area, space program of intensive care units for healing environment. Implications: This study is a basic research for guideline of intensive care unit, and need to be followed by further study using various perspectives and methods.
Purpose: This study aimed at the effectiveness to investigate the performance of evidence-based pain assessment and management guidelines. Methods: Participants were 140 nurses at the med-surgical units. Data were collected in early July, 2014 using Registered Nurses Association of Ontario (RNAO) guideline (2007) revised and validated by Hong and Lee (2012) and analyzed by descriptive statistics, t-test, ANOVA using SPSS/WIN18.0. Results: The score of performance of pain assessment guideline was higher than the score of pain management. Categories with high score were pain screening, parameter of pain assessment, documentation, assessment of opioids side-effects, and record of pain caused intervention. Categories with low score were comprehensive pain assessment, multidisciplinary communication, establishing a plan for pain management, consultation and education for patients and their families, and education for nurse. Non-pharmacological management was the lowest one. Conclusion: Assessing and managing pain is a complex phenomenon. It might be useful if institutions host training programs to ensure that nurse are better able to understand and implement pain assessment and management. Since non-pharmacological management is less likely to be used by nurses it may be helpful to include these methods in a training program.
Purpose: This study was aimed at providing scientific evidence for minimizing ventilator-associated pneumonia(VAP) by identifying appropriate timing of exchange of circuit for mechanical ventilator that is well suitable for the medical environment of intensive care units of hospitals in Korea. Method: This was a quasi-experimental study with a convenience sample of 19 adult subjects aged over 18 years who were admitted to the NS ICU of C university hospital, and placed on mechanical ventilator. The subjects were placed in two groups, compared on the incidence rate of VAP after they received exchange of circuit either at 1-week interval (N =10) or 2-week interval (N = 9). Result: 1) When considering 1000 days as the standard unit of analysis for incidence, the incidence rate of VAP was 7.19 cases at the 1-week cycle exchange group and 15.23 at the 2-week cycle exchange group, showing no statistically significant difference between the two groups. 2) There were a total of 3 types of bacteria isolated from the patients with VAP, including 2 cases with P. aeruginosa, 1 case with Streptococcus group F and A. baumannii. Conclusion: With thorough hand washing and strict management of tracheal tube of mechanical ventilator as well as use of tracheal intubation techniques, exchange cycle of circuit of mechanical ventilator by nurses may be changed from 1-week to 2-week interval.
Purpose: This study was conducted to identify changes in limb circumferences among patients admitted to the intensive care unit (ICU) and related factors. Methods: We conducted a prospective observational study with 27 patients from 3 ICUs at a university hospital located in B city of Korea, from September 1 to October 30, 2015. The circumferences of the left and right upper arms, thighs, and lower legs were measured on the first, third, fifth, and seventh days of ICU admission. Information on the related factors was collected from the medical records. The data were analyzed using a linear mixed model method. Results: The limbs circumferences significantly reduced from day 3, and the changes continued till day 7. These changes were related to the gender of the subjects, restraints application, use of steroids, and continuous renal replacement therapy. Conclusions: Based on the above results, it can be concluded that ICU-acquired weakness begins before the third day of admission. Thus, early mobilization protocols for ICU patients need to be developed and implemented in order to improve long-term outcomes.
Purpose: This study aims to develop Tablet PC-based multimedia educational contents on the procedures from surgery preparation to recovery for hospitalized patients who are going to undergo thoracic surgery and their families. Methods: The educational contents were created based on literature review and a survey of needs in patients and their families who were admitted to a hospital in Seoul from Dec. 2013 to May 2014. The contents were developed in conjunction with a film production company. The educational materials including texts, images, sounds, and videos were developed appling the ADDIE (analysis, design, development, implementation, and evaluation) model. Results: Two multimedia educational contents were developed for the patients with coronary artery bypass graft or lung surgery. Each content consisted of six subdivisions of self-learning for 20 minutes. The developed educational contents were completed after testing content validity by 30 medical experts. Conclusion: This study recommends that the Tablet PC-based educational contents for thoracic and cardiovascular patients and their families be used in general wards and intensive care units to relieve their anxiety before surgery and reduce postoperative complications.

Purpose: Methicillin-resistant Staphylococcus aureus (MRSA) is a major nosocomial pathogen in the intensive care units (ICUS). The purpose of this case-control study is to identify risk factors for acquisition of MRSA during ICU stays in patients with and without MRSA. Method: The study was conducted in a 16 beds-neurosurgical intensive care unit of a 2200-bed tertiary care university hospital in Seoul, Korea. Medical record and Critical Classification Scoring System were reviewed retrospectively in patients who were admitted more than 3 days from August 1, 2003 to May 30, 2004. Cases and controls were matched for age and gender. The obtained specimens were nasal swab and sputum. Result: There were 950 patients' admissions during the period. Among them, MRSA was isolated from twenty-three patients who were considered as hospital acquired. Artificial airway (p=.045), frequency of suction (p=.002), nasogastric tube (p=.004), wound drain (p=.045), and vancomycin (p=.019) were risk factors for MRSA acquisition in univariate analysis. Frequency of suction (p=.012, OR 3.5) was revealed as the only risk factor in multivariate conditional logistic regression. Conclusion: Our findings give support to recent studies that suggest that frequent physical contact maγ increase the nosocomial acquisition of MRSA in a neurosurgical ICU.

Purpose: The purpose of this study was to identify factors which influence the management of MultiDrug -Resistant Organisms (MDROs) by nurses in Intensive Care Units (ICUs). Methods: Data were collected from December 8 to 20, 2013 and participants were 163 ICU nurses working in one general hospital. The Health Belief Model tool and knowledge and management of MDROs infection tools were used in the study. Descriptive statistics, t-test, analysis of variances, Pearson correlation coefficients and multiple regression were used to analyze the data. Results: Knowledge, perceived susceptibility, and perceived benefits had a significant influence on MRSA (Methicillin Resistant Staphylococcus Aureus,) and MDRAB (Multidrug Resistant Acinetobacter Baumannii) infection management when all the other variables were considered. Significant variable which had influence on VRE (Vancomycin Resistant Eenterococci) infection management were perceived susceptibility and perceived benefits. Conclusion: Perceived susceptibility and perceived benefits had significant influence on MDROs infection management. Emphasis needs to be on the perceived susceptibility and perceived benefits of MDROs infection management when providing an educational program for ICU nurses.
Purpose: This study aimed to identify the relationships among attitudes towards death, perceptions of hospice care, and hospice care needs as perceived by family members of patients in the intensive care unit (ICU). Methods: This study used a descriptive correlational method. A structured questionnaire was used to collect data from 114 participating families in the ICU at Dong-A University Hospital, from October 10 to November 1, 2019. The data were analyzed in terms of frequency, percentage, and mean and standard deviation. The t-test, one-way analysis of variance, and Pearson correlation coefficients were also conducted. Results: Perceptions of hospice care showed significant differences according to age (F=3.06, P=0.031) and marital status (t=3.55, P=0.001). However, no significant differences in attitudes towards death or hospice care needs were found. A significant positive correlation was found between perceptions of hospice care and hospice care needs (r=0.49, P<0.001). Conclusion: In order for families to recognize the need for hospice care and to receive high-quality palliative care at the appropriate time, it is necessary to increase public awareness of hospice care through various educational and awareness-raising efforts, thereby providing opportunities for families of terminally ill patients to request hospice care.
