Recent Research Trends in Korean Medicine Treatment of Diabetes Mellitus - Focusing on Domestic Articles from 2008 to 2013 - (당뇨병의 한의학적 치료에 대한 최신 연구 동향 - 2008년부터 2013년까지 국내 학술논문을 중심으로 -)

  • Objectives : This study was performed to research trends in Korean medicine treatment of diabetes mellitus (DM). Methods : We reviewed 73 studies about diabetes mellitus, which had been published from 2008 to 2013. The article search was performed on the Korean institute of oriental medicine (KIOM) database. Results : 1. Patterns of study design were as follows : 14 review studies, 44 experimental studies, 7 clinical studies, 8 case reports. 2. From the 14 review studies, we noted that literature studies about definition, pathologic mechanism, treatment, pathologic pattern identification of DM and previous study analyses were done. 3. From the 44 experimental studies that are the most common type of DM studies, various Korean medicine treatments such as single medicinal herbs, herbal complexes, diets and pharmacopuncture were used. Anti-diabetic effects were demonstrated in most of the studies. 4. 7 clinical studies about acupuncture, electroacupuncture, herbal medicine, etc. were reported. 5. 8 cases about DM complications and comorbidities were reported. Clinical symptoms improved in all of studies, blood sugar level decreased in four studies. Conclusions : Further clinical and case study based on experimental researches should be performed.

The Effects of Korean Medicine Treatment for Meniscus Tears: A Retrospective Chart Review

  • Changes in symptoms and dysfunction related to meniscus tears following the use of Korean medicine for ≥ 4 days were studied. The medical charts of 53 cases of diagnosed meniscus tears (magnetic resonance imaging) with an admission Numeric Rating Scale (NRS) score ≥ 4, between 2017 and 2022 were retrospectively reviewed. Treatments included acupuncture, pharmacopuncture, herbal treatment, Chuna therapy, and physiotherapy. The NRS, Western Ontario and McMaster Universities Osteoarthritis Index, and European Quality of Life 5 Dimensions were performed at admission and discharge. There were 42 females and 11 males in this study. Patients were more likely to be in their 60s (38.18%), have an unknown etiology (81.13%), and have complex tears (50.94%). After receiving a combination of alternative Korean medicine during hospitalization, the mean NRS score improved from 6.82 ± 1.19 to 3.66 ± 1.83 (p < 0.001), the Western Ontario and McMaster Universities Osteoarthritis Index score improved from 46.47 ± 20.99 to 37.98 ± 19.23 (p < 0.001), and the mean European Quality of Life Five Dimensions score improved from 0.61 ± 0.18 to 0.68 ± 0.14 (p < 0.001) after treatment. These results suggest that Korean medicine treatment of meniscus tears alleviated pain and improved physical function.

A Case Report of a Patient with Multiple Cerebral Infarction Caused by Posterior Cerebral Artery Occlusion Treated with Yikgeebohyul-tang (후대뇌동맥 폐색으로 발생한 다발성 뇌경색 환자에 대한 익기보혈탕 치험 1례)

  • Objectives: The aim of this study is to describe the effects of treatment with traditional Korean medicine on a patient with multiple cerebral infarction caused by posterior cerebral artery occlusion. Methods: The patient was treated with herbal medicine (Yikgeebohyul-tang) and acupuncture. The effects of these treatments were evaluated using the Manual Muscle Test, the Korean version of the Modified Barthel Index, the Modified Rankin scale, and a subjective percentage of sensation in the left lower extremity and face. Results: After treatment, the Modified Rankin Scale score, subjective percentage of sensation in the left lower extremity, and gait disturbance improved. Conclusion: The results suggest that traditional Korean medicine may be effective for treating the symptoms of multiple cerebral infarction caused by posterior cerebral artery occlusion.

  • The Korean Society of Heart Failure guidelines aim to provide physicians with evidence-based recommendations for diagnosing and managing patients with heart failure (HF). In Korea, the prevalence of HF has been rapidly increasing in the last 10 years. HF has recently been classified into HF with reduced ejection fraction (EF), HF with mildly reduced EF, and HF with preserved EF (HFpEF). Moreover, the availability of newer therapeutic agents has led to an increased emphasis on the appropriate diagnosis of HFpEF. Accordingly, this part of the guidelines will mainly cover the definition, epidemiology, and diagnosis of HF.

The Clinical Study on 194 Patients of Temporomandibular Disorder (측두하악관절장애 환자 194례에 대한 임상적 고찰)

  • Objectives : These studies are designed to make a survey of the effectiveness of oriental medicine treatment on temporomandibular disorder. Methods : The clinical study was done on 194 cases of patients with temporomandibular disorder who was treated in Ja-Seng Oriental Medicine Hospital from January 2008 to June 2008. After treatment we checked VNRS(verbal numerical rating scale) score and five-point linkert scale. Also we observed the change of symptom of click sound and trismus. Results : After the last treatment, The mean VNRS(verbal numerical rating scale) score changed 4.63 to 2.25. The click sound was disappeared of 53% patients, and the trismus was disappeared of 57% patients. And The five-point linkert scale were checked 3.78. Conclusions : It is concluded that oriental medicine treatment has on useful effect on temporomandibular disorder.

  • Objectives : The Purpose of this study is to investigate the clinical application of conservative treatment including quadratus lumborum MST for lumbar HIVD(Herniated Intervertebral Disc) with sacralization. Methods : We used acupunture, herbal acupuncture, herbal medicine, Quadratus Lumborum MST for this patient. This patient received hospital treatment for 25 days. Results : After conservative treatment, the patient's pain was controlled and NRS score was decreased. ODI and ROM also were improved. Conclusions : Oriental conservative treatment including quadratus lumborum MST has a positive effect to control pain of lumbar HIVD(Herniated Intervertebral Disc) with sacralization.

  • We report a case of gastric adenoma which was found incidentally on $^{18}F$-FDG PET/CT study for cancer screening in asymptomatic patient. It showed focal and intensely increased. FDG uptake in the antrum of stomach. On the gastroduodenoscopy, it showed flat elevated lesion with irregular margin. Histologically, the lesion was confirmed gastric adenoma with high grade dysplasia and removed by endoscopic mucosal resection.

  • Although dysphagia and globus is a common comlication in Functional gastrointestinal disorder, there are few studies or reports about its treatment in oriental medicine. This study was to report the effect of Bokryungyeum-tang in Functional gastrointestinal disorder patients with dysphagia. Functional gastrointestinal disorder patients with dysphagia hospitalized in our hospital were treated by Bokryungyeum-tang every day. three times a day for two weeks. The patient showed constant improvement during the two weeks. Not only their objective measure, but also their subjective symptoms such as chest discomfort, insomnia, and depression improved too. This suggests that Bokryungyeum-tang is quite effective when treating functional gastrointestinal disorder patients with dysphagia.

Repeated Postoperative Adhesive Small Bowel Obstruction Treated with Daeseunggi-tang and Acupuncture Treatment in Elderly: A Case Report

  • Postoperative adhesive small bowel obstruction (ASBO) is an intractable disorder which sometimes leads to adhesiolysis or small bowel resection. These therapeutic reoperations, however, also have many limitations including complications. An 80-year-old female, who had undergone 4-abdominal surgeries, visited the hospital with continuous vomiting. Based on her clinical symptoms and history, multiple air-fluid levels and distention of the small bowel in an abdominal X-ray, we diagnosed her with postoperative incomplete ASBO. We conducted acupuncture and an herbal medicine enema to stimulate bowel movement and relieve pain. The patient came in complaining of abdominal pain and vomited more than 10 times on hospital day 0 stopping on hospital day 4. Comparing hospital day 0 with hospital day 4, the abdominal pain decreased from a numerical rating scale (NRS) 10 to 4. There were no side effects such as redness or burns during the treatment process. This study presented an acupuncture-based treatment will be helpful for clinicians managing cases of ASBO with poor performance in elderly individuals.

A Case of Organizing Pneumonia Associated with FOLFIRI Chemotherapy

  • The combination chemotherapy of irinotecan with 5-fluorouracil and leucovorin (FOLFIRI regimen) was recently proven to be beneficial in patients with advanced colorectal cancer. Pulmonary toxicity is very rare in adverse effects of irinotecan. No case of organizing pneumonia (also known as bronchiolitis obliterans organizing pneumonia) associated with FOLFIRI chemotherapy has been reported. We experienced a case of a 62-year-old man who presented persistent dry cough and progressive dyspnea after receiving chemotherapy with FOLFIRI regimen. After surgical lung biopsy, the patient was diagnosed with FOLFIRI chemotherapy-induced organizing pneumonia which was successfully treated with steroid therapy.
