병원 간호사의 갈등과 갈등 해소 경험 (Hospital Nurses' Experiences of Conflict and Conflict Resolution)

  • Purpose: The purpose of this study was to investigate conflicts and the experiences of conflict resolution of hospital nurses. Methods: This qualitative study used phenomenological methodology. The research participants were 8 nurses of C hospital. Data were collected through in-depth individual interviews from November 2016 to February 2017. The data analysis method was based on Colaizzi's approach. Results: The study results showed that the experiences of nurses consisted of 15 themes and 6 theme clusters: 1) Ambiguous role, 2) Recognizing lack of competence, 3) Difficulty maintaining relationships, 4) Unsatisfactory work environment, 5) Efforts to resolve conflicts, and 6) Conflict resolution process through labor union. Conclusion: The findings of this study recommend that hospital organizations and managers should pay attention to improving the work environment and organizational culture of members. The results of the present study can improve the understanding of various aspects of the daily life of hospital nurses.

  • The purpose of this study is to analyze a case of unit management system introduced and operated in a university hospital. The system was designed and applied to six clinical departments and centers to help to achieve the medical revenue and profit targets. The case hospital is now in the second year of the system operation. Major findings of the study are as follows; Firstly, the leadership style of the unit manager is the most important factor in management of the unit. The transformational leadership style was more effective than the transactional one. Secondly, unit manager's managerial ability is another key factor to the success or failure in achieving the responsibility targets. Thirdly, the degree of divisionalization and responsibility should be strengthened to activate both unit managers and medical specialists.

간호서비스 질, 만족 및 병원 재이용 의도에 관한 소비자와 제공자간의 지각차이 비교 (A Comparative Study of Consumers and Providers on Nursing Service Quality, Satisfaction, and Hospital Revisiting Intent)

  • Purpose: The purposes of this study were to compare the nursing service quality, gap, overall nursing service satisfaction, overall medical service satisfaction and intent to revisit the hospital perceived by consumers and providers. Methods: Data were collected with self-administered questionnaire and analyzed using frequency, %, mean, standard deviation, t-test, Pearson correlation coefficient, and multiple regression analysis. Result: Nursing service gap perceived by consumers was smaller than that of providers. Consumer's overall satisfaction with nursing and medical service was higher than that of nurses. In consumers, nursing service satisfaction alone accounted for 62.9% of the variance in their intent to revisit the hospital, while explained 3% of the variance in providers. Conclusion: There are definitely perception gap between consumers and providers. Therefore nursing and hospital managers must recognize it, and carry out the internal marketing strategies for nurses.

  • We have observed discitis developed after Intradiscal radiofrequency thermocoagulation in a patient with chronic discogenic low back pain. Recently, it is becoming more common that pain-managers perform disc-manipulation or, a nerve block after penetrating a disc, so postprocedural infection of disc can be a problem. To prevent discitis, very carefull attention must be given to ensure aseptic conditions during this procedure. However, if it occurs, it needs to be treated properly. In this case, a good result was obtained by treatment with absolute bed rest, pain management, and antibiotics.

모바일 병원정보시스템 사용성에 대한 실증연구 (A Case Study of a Mobile Hospital Information System Usability)

  • This study is a proposal of quality scale and measurement for the Mobile Hospital Information System Usability used to the Chungnam National University Hospital in Daejeon Korea. This study is to provide decision- making guideline for hospital managers and to provide feedback for the users' information needs in Mobile Hospital Information System. The target people of the study were medical doctors in Chungnam National University Hospital. The service contexts of usability were request of medical Care, Vital sign check of Patient, Care Agreement of Patient, Blood management, Check of care state, Sign of choice doctors, Query of doctor order and patient measurement result. The Usability factors were the efficiency, effectiveness and satisfaction of quality model ISO9241-11. This study shows that the Mobile HIS is used for the communication and education between doctor and patients. Especially, The Care Agreement of Patient is a part of the most utilized in the Mobile HIS.

국내 질 향상부서 중심의 질 지표 측정 현황, 장애요인과 평가 (The Current State of and Barriers to Quality Measurement, and Quality Managers' Reported Evaluation on Quality Indicators in Korea)

  • The purposes of this study were to Identify the .level of measurement on quality Indicators and evaluate the existing indicators in order to determine the priority of quality indicators' application in Korean general hospitals. A survey was conducted using a questionnaire. The subjects were quality managers working at general hospital having over 300 beds. The criteria were relevance, reliability, precision, impact, application, and preference to evaluate quality indicators. According to these six criteria, each indicator was evaluated on a five point scale(5: excellent, 1: poor). The response rate was $40.4\%$. The hospitals have monitored the average of 3.8 indicators(median 4). The indicators such as return to operating room, unplanned readmission, cancellation of booked operations, death, hospital infection, cesarean section rate, volume per disease or procedure, readmission, re-operation, blood transfusion, and post-procedural complications were frequently measured. The top ten quality indicators in the evaluation by its relevance, validity, reliability, impact, preference and application were decubitus ulcer, clean wound infection, fall, unplanned return to operation room, transfusion reactions, foreign body left In during procedure, unplanned readmission, wound infection after contaminated surgery, postoperative hemorrhage/hematoma, and cesarean section rate in order. The high priority quality indicators frequently measured could be used as primary national indicators. Standardized guidelines about monitoring indicators and the utilization will preliminarily be needed to compare and reuse the data for various purposes and improve the quality of care continuously.

환자안전 문화에 대한 의료 종사자의 인식과 경험 (Experience and Perception on Patient Safety Culture of Employees in Hospitals)

  • Purpose: The objectives of this study were to understand and compare perception and experience between clinical staffs(nurses and pharmacists) and Quality Improvement managers. Method: A qualitative study was conducted with 14 clinical staffs and QI managers who are working at tertiary hospitals in Korea. Interviews were recorded and transcribed for systematic analyses of qualitative data. Results: Most critically, while QI managers acknowledged that establishment of the patient safety culture and reduction of medical errors are urgent tasks for QI effort, clinical staffs don't seem to share such perceptions. All participants agree that staff shortage and no compliance to safety procedures were major reasons for medical error occurrences. Many suggested that an organizational culture where errors were perceived as a systematic problems rather than individual failures or carelessness should be formed to promote voluntary reporting of medical errors. Conclusion: A more systematic effort and attention at the hospital leadership and public policy level should be promoted to constitute societal consensus on the urgence of promoting patient safety culture and more specific approaches to tackle the patient safety problems.

자기 개발 훈련이 일선 간호 관리자의 인간 관계와 내재적 모티베이션에 미치는 영향 (The Effects of Self Development Training on the Human Relationship and Intrinsic Motivation of the First-Line Nurse Managers)

  • Purpose: The purpose of this study was to examine the effects of Self-Development training on the Human Relationship and Intrinsic Motivation for first-line nurse managers. Method: This was an empirical study on the Human Relationship and Intrinsic Motivation of Self Development Training. The researcher developed a new Self-Development Training Program, and the two-hour long training session was performed twice a week for each group. The program was performed for 4 session in two weeks. The subjects consisted with 24 nurse managers from C University Hospital in Seoul, Korea. The subjects were divided into two groups for the training. Two weeks before and 4 weeks after the training, subjects completed questionnaires that measured Human Relationship and Intrinsic Motivation. Analysis was completed by using SPSS PC 10.0 for percentile, mean, standard deviation and paired t-test. Result: The results of this study showed that Self-Development Training Program resulted in a significant effects on the Human Relationship. But the Intrinsic Motivation was not significantly affected by the program. Conclusion: This Self-Development training program had a positive effect on the Human Relationship and Intrinsic Motivation.

SNS 정보 특성이 병원선택과 만족에 미치는 영향 (The Effects of SNS Information Characteristics on Hospital Selection and Satisfaction Degree)

  • This study is to confirm that SNS information characteristics have an effect on choosing hospital care and satisfaction on hospital. This study is conducted with a total of 301 respondents (150 of document investigation and 151 of mobile SNS research) from 16th January 2014 to 29th January. 168 respondents who have experience to collect SNS health information within 6 months recently was selected subject of analysis. In the results, we could confirm the followings. The reliability among SNS information characteristics has effect on convenience that is source of choosing hospital care. The reliability and agreement have effect on the professionalism among source of choosing hospital care. In the influence on having hospital satisfaction, the more reliable and agreeable, the more increasing of hospital satisfaction. In the influence between hospital choice element and hospital satisfaction, the more convenient and service available, the more increasing of hospital satisfaction. In conclusion, it is expecting to contribute SNS managers or marketer of medical institutions to taking out commitment on medical consumer's SNS.

터지털 시대의 병원경영전략 수립에 관한 연구 - 병원경영자의 경영개선활동에 관한 인식을 중심으로 - (Hospital Management Strategy in Digital Era)

  • This study purports to examine the current management and information technology related strategy of Korean hospitals and suggest the effective management strategy in the 21st century when is digital era. Specifically the study tries to analyze the changing trends of strategic orientation and investigate the general management and information technology strategy of Korean hospitals. Self-administered Questionnaires were distributed to 721 hospitals nationwide and finally 98 Questionnaires were analyzed for the study. The results of the study are as follows : 1) Half of the respondent hospitals reported that they have an analyzer orientation in 2000, whereas 19.4% were prospectors, reactors 16.4%, and defenders 14.3%. However, the respondent hospitals intended to have a prospector orientation in the future (2002), while 29.6% planned on being analyzers, 17.3% reactors, and 3.1% defenders. 2) Hospital services for improving patient satisfaction were the most common. strategy for the respondent hospitals, followed by cost containment, organizational restructuring, employee education, purchasing system change, specialization of clinical services, quality improvement of medical care, strengthening the networking with the stakeholders, public relations and marketing strategy, diversification, and installing the information system. However, the strategies of annual salary system, retrenchment of unprofitable services, merit payment based on performance were still not popular for the respondent hospitals. 3) As for the strategies related with information technology, most hospitals have not implemented actively, except for the establishment of home-pages, order communication systems, and insurance claims through electronic data interchange system. 4) There were significant differences in the level of strategy implementation in terms of the ownership, bed size, financial performance, and the top managers I knowledge of information technology. The larger bed size, the higher financial performance, the better knowledge of information technology the top managers have, the more strategies the respondent hospitals implemented. The managerial and political implications for Korean hospitals in digital era were also discussed.
