노인 일자리 정책과 노인 일자리 인식에 관한 융합 연구 (A Convergence Study on the Senior Employment Policy and Senior Job Awareness)

  • 본 연구는 노인 일자리에 대한 인식 개선을 위한 실마리를 제시할 목적으로 시행되었으며, 최종 연구대상은 110명 이다. 연구방법은 student t-test, ⲭ2-test, one-way ANOVA(Scheffe)를 실시하였다. 연구결과는 다음과 같다. 연소노인(65-74세)이 노인 일자리 사업에 대한 인식, 경제 상태가 유의하게 높았으며, 공공형 일자리 인식은 기혼, 종교가 있을 때, 월소득이 400만원 초과일 때, 사회서비스형은 기혼, 종교가 있을 때, 민간형은 월소득이 400만원 초과로 나타났다(p<0.05). 공공형 직종 에서는 '주정차 질서 계도 봉사'가, 사회서비스형 직종 중에서는 '노인 관련 시설지원'이, 민간분야 직종에서는 '패스트푸드원, 주방, 식당 보조원, 음식배달서비스 종사자'에 대한 선호도가 가장 높게 나타났다. 결론적으로 연령이 증가함에 따라 낮아지는 노인 일자리 정책에 대한 인식을 향상 시키기 위해 노인 일자리 인식 개선 교육 프로그램이 필요하다. 추후 노인연령에 따라 하고 싶은 직종과 할 수 있는 일자리 창출을 위한 기초자료로 활용될 것으로 기대된다.

A형 간염의 자연항체와 예방접종을 통한 항체 생성률의 역가 비교분석 (The Comparative Analysis of the Titer of Seroconversion Rate Through the Natural Antibody and Antibody after Vaccination of Hepatitis A)

  • 2008년부터 A형 간염 환자들이 급속히 증가하고 본원에 내원하여 건진을 받는 대부분의 수검자들이 A형 간염(IgG) 항체 생성 유무에 관심이 많아지며 검사 건수가 증가하였다. 그에 따라 항체 검사결과가 cut-off값에 걸리는 검체가 많아져 원인을 분석하였더니 대부분 A형 간염 예방접종을 한 수검자들이었다. 이에 저자들은 건강증진센터에서 설문조사를 통하여 자연면역을 획득한 수검자들 그룹과 본원에서 A형 간염 예방접종(1차, 2차)을 실시한 직원들 그룹으로 나누어 검사를 시행하였고 cut-off값을 기준으로 항체 생성률과 그에 대한 역가를 비교하고 진단검사의학과와 핵의학과에서 사용하는 진단 시약간에 항체 생성률과 그에 대한 역가를 비교해 보고자 했고, 2012년 8월 한 달 동안 건진 수검자 185명을 설문조사하여 자연면역을 획득한 119명과 본원에서 예방 접종을 실시한 직원들을 대상으로 1차 접종자 53명, 2차 접종자 59명으로 대상을 분류했다. 항체 생성률은 cut-off값 1을 기준했을 때 0.90-1.10 (${\pm}$), 0.60-0.89 (1+), 0.30-0.59 (2+), 0.01-0.29 (3+)로 나누어 역가를 비교하고, 같은 기준으로 제조사별 백신 접종 후 항체 생성률에 대한 역가를 비교평가 해 보았다. 그 결과, 건진 수검자 중 자연 면역을 획득한 수검자는 cut-off값 1을 기준했을 때, 0.90-1.10 (${\pm}$)가 0%, 0.60-0.89 (1+)가 0%, 0.30-0.59 (2+)가 4.2%, 0.01-0.29 (3+)가 96%로 역가가 <0.60 ($${\geq_-}2+$$)가 100%였다. 그리고 예방접종을 실시한 직원들의 항체 생성률은 1차 접종자 중 ${\pm}$가 59.1%, 1+가 18.1%, 2+가 18.1%, 3+가 4.6%로 총 45.3%였고, 역가는 $${\geq_-}$$ 0.60 ($${\leq_-}1+$$)가 77.3%였다. 2차 접종자의 항체 생성률은 ${\pm}$가 1.9%, 1+가 15.4%, 2+가 36.54%, 3+가 46.2%로 총 88.1%였고 역가는 <0.60 ($${\geq_-}2+$$) 82.7%가 였다. 또한 제조사별로 비교 하였을 때 1차 접종자의 항체 생성률은 BNIBT 20.8% (${\pm}24.5%$), GB 15.7% (${\pm}7.8%$), RIAKEY 94.3% (${\pm}3.8%$), ROCHE 83% (${\pm}0%$), Abbot 73.1% (${\pm}5.8%$)였고, 2차 접종자의 항체 생성률은 BNIBT 86.4% (${\pm}1.7%$), GB 88.5% (${\pm}1.9%$), RIAKEY 100% (${\pm}0%$), ROCHE 98.3% (${\pm}0%$), Abbot 98.2% (${\pm}0%$)였다. 즉 자연면역 항체가 예방접종에 의한 항체보다 역가가 높다는 것을 알 수 있었고, 1차 접종 후 보다는 2차 접종 후 검사를 시행했을 때 항체 생성률과 역가가 대부분 높아짐을 알 수 있었다. 따라서 결과 보고시 negative, index (${\pm}$), weak positive (1+), positive (2+), strong positive (3+)로 역가를 나누어 보고를 하거나 결과값에 index값을 같이 적어서 결과를 상세히 보고한다면 과거결과와 비교도 가능할 것이다. 또 제조사별 비교 시 1차 예방접종 후의 항체 생성률과 역가에서 시약간에 많은 차이를 보이고 있었고, 매년 예방 접종률이 높아지고 있는 시점에서 이러한 차이를 줄이기 위해서 각 제조사들은 민감도나 재현성에 더 주의를 기울여야 하겠고, 자연면역항체와 예방접종을 통한 항체간에 생길 수 있는 미지의 차이를 감안하여 검사자들이 사용하는 시약을 신뢰할 수 있도록 더 연구하고 개발해야 할 것이다.

물리치료사의 근무실태와 직무만족 및 직장애착 (Working Conditions, Job Satisfaction and Organizational Commitment of Physical Therapists)

  • The main objective of this paper is to examine the working conditions of physical therapists and to investigate the level of job satisfaction and organizational commitment under different working conditions. A survey was conducted through structured questionnaire for the physical therapists working in Busan area, and data from 175 therapists were utilized in the final analysis. Survey items included general characteristics of the therapists(sex, age, education, marital status, religion, income, career years, etc.), their working conditions(kinds of organizations they work in, location of PT room, weekly working hours, average number of patients, number of co-workers, equipments/facilities, etc.), and organizational effectiveness measured in terms of job satisfaction and organizational commitment. Major results are as follows: 1) As for the general characteristics of the surveyed physical therapists, male(51.4$\%$) slightly exceeded female(48.6$\%$) and majority(more than 90$\%$) was 20-39 in age. Also, there were more single(57.7$\%$) than married(42.3$\%$), and 54.9$\%$ of the respondents had religion while 45.1$\%$ did not. In terms of the ranks in their organizations, only a few of them(6.9$\%$) were managers, and monthly salary mostly ranged from $\₩$1,000,000 to $\₩$2,500,000. More than half of them worked in various kinds of hospitals, with the remaining in health centers or social welfare institutions. 2) In regard to the working conditions of the respondents, 19.4$\%$ of PT rooms were located in basement areas while the remaining 80.6$\%$ in first to third floors. 34.3$\%$ of them treated 15 patients or less per day, whereas 25.1$\%$ treated 31 patients or more. Also, 52$\%$ recorded physical therapy charts periodically. As for the equipments and facilities, 25.1$\%$ felt 'sufficient' and 40$\%$'insufficient.' 3) The respondents provided various kinds of suggestions for the improvement of their working conditions, where the most important were 'salary raise' and 'reduction of working hours.' In addition, their requests to the PT Association included 'permission of opening of independent practice,' 'permission of legal specialty,' and 'vitalization of the Association.' 4) The comparative analysis of job satisfaction and organizational commitment among different characteristics of the respondents revealed that there was no significant difference between male and female, but in terms of age, the group of 40 or older was highest in both aspects. Besides, therapists who had 15 or more years of hospital career reported higher job satisfaction and organizational commitment than the others. It was also found that the level of organizational commitment increased as the rank of the respondents got higher. There were no significant differences in terms of marital status and religion. 5) Comparison with regard to working conditions showed that therapists employed in university hospitals, health centers and social welfare institutions felt higher job satisfaction and organizational commitment than those in community hospitals and clinics. Also, respondents who worked in basement areas had significantly lower job satisfaction than otherwise. The length of weekly working hours did not really affect the level of job satisfaction, but it was important to maintain 'regular working hours' for the hospitals to improve the employees' organizational commitment. As for the number of patients per day, 'medium level' (21-25 patients) had highest scores in both aspects. And, finally, sufficiency of equipments and facilities was found to be important for the improvement of organizational commitment.

병원 간호행정 개선을 위한 연구 (A Study for Improvement of Nursing Service Administration)

  • Much has teed changed in the field of hospital administration in the It wake of the rapid development of sciences, techniques ana systematic hospital management. However, we still have a long way to go in organization, in the quality of hospital employees and hospital equipment and facilities, and in financial support in order to achieve proper hospital management. The above factors greatly effect the ability of hospitals to fulfill their obligation in patient care and nursing services. The purpose of this study is to determine the optimal methods of standardization and quality nursing so as to improve present nursing services through investigations and analyses of various problems concerning nursing administration. This study has been undertaken during the six month period from October 1971 to March 1972. The 41 comprehensive hospitals have been selected iron amongst the 139 in the whole country. These have been categorized according-to the specific purposes of their establishment, such as 7 university hospitals, 18 national or public hospitals, 12 religious hospitals and 4 enterprise ones. The following conclusions have been acquired thus far from information obtained through interviews with nursing directors who are in charge of the nursing administration in each hospital, and further investigations concerning the purposes of establishment, the organization, personnel arrangements, working conditions, practices of service, and budgets of the nursing service department. 1. The nursing administration along with its activities in this country has been uncritical1y adopted from that of the developed countries. It is necessary for us to re-establish a new medical and nursing system which is adequate for our social environments through continuous study and research. 2. The survey shows that the 7 university hospitals were chiefly concerned with education, medical care and research; the 18 national or public hospitals with medical care, public health and charity work; the 2 religious hospitals with medical care, charity and missionary works; and the 4 enterprise hospitals with public health, medical care and charity works. In general, the main purposes of the hospitals were those of charity organizations in the pursuit of medical care, education and public benefits. 3. The survey shows that in general hospital facilities rate 64 per cent and medical care 60 per-cent against a 100 per cent optimum basis in accordance with the medical treatment law and approved criteria for training hospitals. In these respects, university hospitals have achieved the highest standards, followed by religious ones, enterprise ones, and national or public ones in that order. 4. The ages of nursing directors range from 30 to 50. The level of education achieved by most of the directors is that of graduation from a nursing technical high school and a three year nursing junior college; a very few have graduated from college or have taken graduate courses. 5. As for the career tenure of nurses in the hospitals: one-third of the nurses, or 38 per cent, have worked less than one year; those in the category of one year to two represent 24 pet cent. This means that a total of 62 per cent of the career nurses have been practicing their profession for less than two years. Career nurses with over 5 years experience number only 16 per cent: therefore the efficiency of nursing services has been rated very low. 6. As for the standard of education of the nurses: 62 per cent of them have taken a three year course of nursing in junior colleges, and 22 per cent in nursing technical high schools. College graduate nurses come up to only 15 per cent; and those with graduate course only 0.4 per cent. This indicates that most of the nurses are front nursing technical high schools and three year nursing junior colleges. Accordingly, it is advisable that nursing services be divided according to their functions, such as professional, technical nurses and nurse's aides. 7. The survey also shows that the purpose of nursing service administration in the hospitals has been regulated in writing in 74 per cent of the hospitals and not regulated in writing in 26 per cent of the hospitals. The general purposes of nursing are as follows: patient care, assistance in medical care and education. The main purpose of these nursing services is to establish proper operational and personnel management which focus on in-service education. 8. The nursing service departments belong to the medical departments in almost 60 per cent of the hospitals. Even though the nursing service department is formally separated, about 24 per cent of the hospitals regard it as a functional unit in the medical department. Only 5 per cent of the hospitals keep the department as a separate one. To the contrary, approximately 12 per cent of the hospitals have not established a nursing service department at all but surbodinate it to the other department. In this respect, it is required that a new hospital organization be made to acknowledge the independent function of the nursing department. In 76 per cent of the hospitals they have advisory committees under the nursing department, such as a dormitory self·regulating committee, an in-service education committee and a nursing procedure and policy committee. 9. Personnel arrangement and working conditions of nurses 1) The ratio of nurses to patients is as follows: In university hospitals, 1 to 2.9 for hospitalized patients and 1 to 4.0 for out-patients; in religious hospitals, 1 to 2.3 for hospitalized patients and 1 to 5.4 for out-patients. Grouped together this indicates that one nurse covers 2.2 hospitalized patients and 4.3 out-patients on a daily basis. The current medical treatment law stipulates that one nurse should care for 2.5 hospitalized patients or 30.0 out-patients. Therefore the statistics indicate that nursing services are being peformed with an insufficient number of nurses to cover out-patients. The current law concerns the minimum number of nurses and disregards the required number of nurses for operation rooms, recovery rooms, delivery rooms, new-born baby rooms, central supply rooms and emergency rooms. Accordingly, tile medical treatment law has been requested to be amended. 2) The ratio of doctors to nurses: In university hospitals, the ratio is 1 to 1.1; in national of public hospitals, 1 to 0.8; in religious hospitals 1 to 0.5; and in private hospitals 1 to 0.7. The average ratio is 1 to 0.8; generally the ideal ratio is 3 to 1. Since the number of doctors working in hospitals has been recently increasing, the nursing services have consequently teen overloaded, sacrificing the services to the patients. 3) The ratio of nurses to clerical staff is 1 to 0.4. However, the ideal ratio is 5 to 1, that is, 1 to 0.2. This means that clerical personnel far outnumber the nursing staff. 4) The ratio of nurses to nurse's-aides; The average 2.5 to 1 indicates that most of the nursing service are delegated to nurse's-aides owing to the shortage of registered nurses. This is the main cause of the deterioration in the quality of nursing services. It is a real problem in the guest for better nursing services that certain hospitals employ a disproportionate number of nurse's-aides in order to meet financial requirements. 5) As for the working conditions, most of hospitals employ a three-shift day with 8 hours of duty each. However, certain hospitals still use two shifts a day. 6) As for the working environment, most of the hospitals lack welfare and hygienic facilities. 7) The salary basis is the highest in the private university hospitals, with enterprise hospitals next and religious hospitals and national or public ones lowest. 8) Method of employment is made through paper screening, and further that the appointment of nurses is conditional upon the favorable opinion of the nursing directors. 9) The unemployment ratio for one year in 1971 averaged 29 per cent. The reasons for unemployment indicate that the highest is because of marriage up to 40 per cent, and next is because of overseas employment. This high unemployment ratio further causes the deterioration of efficiency in nursing services and supplementary activities. The hospital authorities concerned should take this matter into a jeep consideration in order to reduce unemployment. 10) The importance of in-service education is well recognized and established. 1% has been noted that on the-job nurses. training has been most active, with nursing directors taking charge of the orientation programs of newly employed nurses. However, it is most necessary that a comprehensive study be made of instructors, contents and methods of education with a separate section for in-service education. 10. Nursing services'activities 1) Division of services and job descriptions are urgently required. 81 per rent of the hospitals keep written regulations of services in accordance with nursing service manuals. 19 per cent of the hospitals do not keep written regulations. Most of hospitals delegate to the nursing directors or certain supervisors the power of stipulating service regulations. In 21 per cent of the total hospitals they have policy committees, standardization committees and advisory committees to proceed with the stipulation of regulations. 2) Approximately 81 per cent of the hospitals have service channels in which directors, supervisors, head nurses and staff nurses perform their appropriate services according to the service plans and make up the service reports. In approximately 19 per cent of the hospitals the staff perform their nursing services without utilizing the above channels. 3) In the performance of nursing services, a ward manual is considered the most important one to be utilized in about 32 percent of hospitals. 25 per cent of hospitals indicate they use a kardex; 17 per cent use ward-rounding, and others take advantage of work sheets or coordination with other departments through conferences. 4) In about 78 per cent of hospitals they have records which indicate the status of personnel, and in 22 per cent they have not. 5) It has been advised that morale among nurses may be increased, ensuring more efficient services, by their being able to exchange opinions and views with each other. 6) The satisfactory performance of nursing services rely on the following factors to the degree indicated: approximately 32 per cent to the systematic nursing activities and services; 27 per cent to the head nurses ability for nursing diagnosis; 22 per cent to an effective supervisory system; 16 per cent to the hospital facilities and proper supply, and 3 per cent to effective in·service education. This means that nurses, supervisors, head nurses and directors play the most important roles in the performance of nursing services. 11. About 87 per cent of the hospitals do not have separate budgets for their nursing departments, and only 13 per cent of the hospitals have separate budgets. It is recommended that the planning and execution of the nursing administration be delegated to the pertinent administrators in order to bring about improved proved performances and activities in nursing services.

  • A purpose of this study was to compare the breast massage and manual expression of the breast before engagement after delivery with the time of engagement, the throbbing pain in breast, the first amount of breast milk and involution of the uterus. The subjects selected for this study were 138 women (experimental group;69, control group :69) who were admitted to the Dept. of Obtest. and Gyneco. of Kyung Hee University Hospital from Jan. 5 to June 5, 1975. The results of study were as follows; 1 The average age of the women 26.9 years old in the experimental group and 27.6 years old in the control group. As to religion, the number of those who had no religion was 58.0 percent and 62.4 percent respectively. Classified according to occupation, there were 87.0 percent in house wives of the booths group. Educational background; 87.0 percent of high school graduates or above, 78.3 percent respectively. The occupation of husband 53.7 percent of company employees stood highest and 42.0 percent respectively. In according to the grade of wards, 55.1 percent and 52.2 percent of four-men room stood highest. 2. Physical condition: Body weight before this Pregnancy(T=0.4962, N.S.), the size of breast(X²df2 = 0.1728, N.S.), the shape of nipple(X²df3 =1.3804, N.S.), hemoglobin's level of the first day after delivery(T=1.2572, N.S.), the above were showed non significant between the experimental group and control group The investigator found any difference between the two groups of the health condition during the pregnancy, 3. The rate of no experience of breast massage during pregnancy was 85 percent and 75.4 percent (X²df1=2.2562, N.S.). 4. As to the meal during hospitalization after delivery: The booth of the groups in ordinary food took usually of meyer soup and milk(X²df8=2.5957, N.S.). 5. The relation between the first step of breast massage, second step of the manual expression of breast before engagement after delivery and time of engagement : average time of engagement in the experimental group (2.1 days±0.8) was shortened than the control group (3.3 days±1.2). (T=-6.9045, P< 0.005). It toot less time in the experimental group of primipara(2.2days±0.7) than in the control group (3.1day±1.2) and it also took less time in the experimental group of multipara (2.0 days±0.9) than in the control group (3.5days±1.4). (Primipara T=-3.9266, 0< 0.005. Multipara T= 5.2356, P<0.005). 6. The relationship between the first step of the massage and second step of manual expression and the throbbing pain at the time of engagement: The experimental group showed less effect than control group (X²df4= 27.3342 P<0.005). The separate study of primipara and multipara showed remarkable difference in the group of primipara)X²df4=20.7285, p<0.005) and little difference in the multipara group (X²df4=8.8351, p< 0. 10). 7. The relationship between the first step of the breast massage, second stop of the manual expression and first amount of breast milk: The average amount of breast milk increased more conspicuously in the experimental group (33.8㎖±23.4) than in the control(29.8㎖±25.3) (T=0.8262, N.S.). No remarkable difference was found in the respective groups that investigated in the groups of primipara and of multipara. (Primipara T=1.1467, N.S., Multipara T=-0.0354, N.S.). 8. The relationship between the first step of breast massage and second step of manual expression of breast and involution of uterus : Average time needed for uttering involution was sooner in the experimental group of primipasa(-3.3 F.B.±1.1), than the control group of primipara (-2.5F. B.±1.2), and it was sooner in the experimental group of muitipara (-3.0 F. B.±l..3), than the control group of multipara(-2.3 F.B±0.9). Primipara T=-2.9272, p< 0.005, Multipara T=2.5557, p< 0.01).

가습기살균제 참사의 진행과 교훈(Q&A) (Questions and Answers about the Humidifier Disinfectant Disaster as of February 2017)

  • 'The worstest environment disaster', 'World's first biocide massacre', 'Home-based Sewol ferry disaster' are all phrases attached to the recent humidifier disinfectant disaster. In the spring of 2011, four of 8 pregnant women including 1 adult man passed away at a university hospital in Seoul due to breathing failure. Epidemiologic investigation conducted by the Korean CDC soon revealed the inhalation of humidifier disinfectant, which had been widely used in Korea during the winter, to be responsible for the disease. As well as lung fibrosis hardening of the lungs, other diseases including asthma, rhinitis, skin disease, liver disease, fetal disease or cancers have been researched for their relation with exposure to the products. By February 9, 2017, 5,342 cases had registered for health problems and 1,131 of them were already dead (20.8% mortality rate). Based on studies by government agencies and a telephone survey of the general population by Seoul National University and civic groups, around 20% of the general public of Korea has used these products. Since the market release of the first product by SK Chemical in 1994, over 7.1 million items from around 20 brands were sold up to 2011. Most of the products were manufactured by well-known large conglomerates such as SK, Lotte, Samsung, Shinsegye, LG, and GS, as well as some European companies including UK-based Reckitt Benckiser and TESCO, the German firm Henkel, the Danish firm KeTox, and an Irish company. Even though this disaster was unveiled in 2011 by the Korean government, the issue of the victims was neglected for over five years. In 2016, an unexpected but intensive investigation by prosecutors found that Reckitt Benckiser manipulated and concealed animal tests for its own brand and brought several university experts and company employees to court. The matter was an intense social issue in Korea from May to June with a surge in media coverage. The prosecutor's investigation and a nationwide boycott campaign organized by victims and environmental groups against Reckitt Benckiser, whose product had been used by more than 70% of victims, led to the producer's official apology and a compensation scheme. A legislative investigation organized after the April 2016 national election revealed the producers' faults and the government's responsibility, but failed to meet expectations. A special law for the victims passed the National Assembly in January 2017 and a punitive system together with a massive environmental epidemiology investigation are expected to be the only solutions for this tragedy. Sciences of medicine, toxicology and environmental health have provided decisive evidence so far, but for the remaining problems the perspectives of social sciences such as sociology and jurisprudence are highly necessary, similar to with the Minamata disease and Wonjin Rayon events. It may not be easy to follow this issue using unfamiliar terminology from medical and chemical science and the long, complicated history of the event. For these reasons the author has attempted to write this article in a question and answer format to render it easier to follow. The 17 questions are: Q1 What is humidifier disinfectant? Q2 What kind of health problems are caused by humidifier disinfectant? Q3 How many victims are there? Q4 What is the analysis of the 1,112 cases of death? Q5 What is the problem with the government's diagnostic criteria and the solution? Q6 Who made what brands? Q7 Has there been a recall? What is still on sale? Q8 Was safety not checked by any producers? Q9 What are the government's responsibilities? Q10 Is it true that these products were sold only in Korea? Q11 Why and how was it unveiled only in 2011 after 17 years of sales? Q12 What delayed the resolution of the victim issue? Q13 What is the background of the prosecutor's investigation in early 2016? Kano Test for Social Nicotine Dependence(KTSND-K) 설문지를 통한 한국인의 사회적 니코틴의존성의 평가 (Evaluation of Social Nicotine Dependence Using the Kano Test for Social Nicotine Dependence (KTSND-K) Questionnaire in Korea)

  • 연구배경: 흡연은 많은 질병과 사망의 가장 중요한 원인이나 니코틴의존성으로 인해 금연이 어렵고 이로 인한 사회적, 의료적 손실이 증가함에도 불구하고 개선되지 못하고 있다. 이러한 니코틴의존성 중 물리적의존성과 독립적으로 관여할 것으로 생각되는 사회, 문화적 니코틴의존성을 알아보고자 연구하였다. 방 법: 2006년 5월부터 8월까지 일개 대학병원 직원과 일개 의과대학생, 일반 대학생 및 건강 검진자들을 대상으로 KTSND-K 설문지를 이용하여 흡연에 관한 의식 조사를 시행하였다. 결 과: 총 741명 중 평균 연령은 $31.8{\pm}11.6$세이었고, 남자는 428명(57.8%)이었다. 흡연력은 현재흡연자 102명(13.8%), 과거흡연자 95명(12.8%), 비흡연자 544명(73.4%)이었다. 흡연력에 따른 KTSND-K 총점은 현재흡연자, 과거흡연자, 비흡연자에서 각각 $17.1{\pm}5.4$, $14.3{\pm}5.5$, $12.3{\pm}5.5$로 흡연자일수록 의미 있게 높게 나타났다(p<0.001). 성별에 따른 총점의 차이는 남녀 각각 $14.3{\pm}5.7$, $11.7{\pm}5.4$로 남성에서 유의하게 높았다(p<0.001). 그러나 현재흡연자 중 흡연 행태 및 금연의지에 따라 각 군을 나누어 비교하였을 때 각 군간의 KTSND 총점의 차이는 보이지 않았다. 비흡연자 중 62.5%에서 간접 흡연의 피해를 느꼈으며, 간접 흡연을 경험하는 주요 장소는 주점 56.8%, 식당 32.3%, 학교 30.2%, 노래방 22.8%, 길거리 18.6% 등 공공 장소가 많았다. 결 론: KTSND-K 설문지는 한국 흡연자의 니코틴의존성 중 사회적 니코틴의존성을 평가할 수 있는 유용한 방법이 될 수 있다고 생각된다.

공공기관 종사자들의 자동제세동기에 대한 인식과 시행의도 및 교육경험에 관한 연구 (A Study on Awareness of Automated External Defibrillator, Usage Intention and Related Educational Experience in Workers at Public Institutions)

  • 본 연구는 공공기관 종사자들의 자동제세동기에 대한 인식과 시행의도 및 교육경험을 조사하고 그를 통해 효율적인 교육과 응급상황 시 자동제세동기의 실제 시행률을 높이는데 그 목적이 있다. 이러한 목적을 달성하기 위하여 2016년 6월 13일부터 8월 21일까지 강원도 공공기관에 근무하는 성인 남녀 243명을 연구대상으로 조사하였다. 수집된 자료는 빈도분석과 ${\chi}^2$-test로 분석하였다. 연구결과는 다음과 같다. 첫째, 공공기관 종사자들의 일반적 특성에 따른 자동제세동기 사용방법에 대한 인식의 차이에 대한 결과, 성별($X^2=6.32$, p<.05), 최종학력($X^2=5.91$, p<.05)에서 유의미한 인식의 차이가 있었다. 둘째, 공공기관 종사자들의 일반적 특성에 따른 자동제세동기 시행의도 차이에 대한 결과, 최종학력($X^2=5.70$, p<.05)에서 시행의도에 유의미한 차이가 있는 것으로 나타났다. 셋째, 공공기관 종사자들의 자동제세동기 시행의도가 없는 경우의 원인은 '자동제세동기 사용법을 몰라서'가 31.5%로 가장 높게 나타났다. 넷째, 공공기관 종자사의 일반적 특성에 따른 자동제세동기 교육경험은 연령($X^2=6.15$, p<.05), 최종학력($X^2=11.56$, p<.01), 근무경력($X^2=11.92$, p<.01)에서 유의하게 나타났다. 다섯째, 공공기관 내 종사자들 자동제세동기 교육의 형태는 이론과 실습의 병행교육이 55.9%로 가장 높게 나타났다. 이러한 결과들로 보아 많이 사람들이 이용하는 공공기관의 종사자들은 표준화된 자동제세동기 교육을 필수적으로 이수하고 실제 응급상황시 심정지환자의 소생률을 높일 수 있도록 노력해야 할 것으로 사료된다.

진폐증환자에서의 임파구 분획 및 싸이토카인의 역할 (The Role of Lymphocyte Compartment and Cytokine in Coal Workers Pneumoconiosis)

  • 연구배경 : 진폐증은 주로 분진흡입에 의해 폐실질에 섬유화를 일으키는 질환으로 만성염증에 의해 폐포내에 대식세포를 축적시키는 특정이 있으며, 크게 단순진폐증(SP)과 진행성 종괴성 폐섬유증(PMF)으로 구분된다. 본 연구의 목적은 진폐증 환자에서 질환의 심한 정도를 ILO 분류에 따라 SP, PMF로 구분한 후, 감염이나 기타 기저질환이 없는 상태에서 혈중 임파구 분획 및 싸이토카인의 농도를 비교한 것이다. 방 법 : 태백 중앙 병원과 영동 병원의 진폐증 환자 중 정밀 검사를 시행하기 위해 병원을 방문한 단순진폐증 환자와 입원 중이거나 정밀검사를 시행한 진행성 종괴성 폐섬유증 환자, 대조군을 대상으로 하였고, 대조군(C;10), 단순진폐증 (S;19), 진행성 종괴성 폐섬유증 (P;30명)에서 혈중 임파구 분획을, C(10), S(14), P(14명)에서 싸이토카인 (IFN-${\gamma}$, TNF-${\alpha}$, IL-6, IL-4))의 농도를 비교하고, 특히 흡연력이나 연령, 폐기능에 의해 변할 수 있는 싸이토카인의 변화를 동시에 고려하여 진폐증의 심한 정도와의 연관성을 관찰하였다. 결 과 : C, S, P 3 군간의 B-임파구, T, $T_H$, Ts, 및 NK-세포를 측정하였을 때 대조군과 단순진폐증환자 사이에서 T, $T_H$는 통계학적으로 의미있게 증가하였으며(P<.01) $T_H$는 두 군간에 의미있게 감소하였으나(P<.01), S, P 두 군 사이에는 특별한 변화가 없었다. 세 군 사이의 싸이토카인 분비 양상은 통계학적인 의의를 발견할 수 없었다. 결 론 : 혈청 임파구 분획은 C군과 비교하여 S군에서 T, $T_H$는 통계학적으로 의미 있게 증가하였으며 Ts는 감소하는 경향을 나타내어 진폐증의 진행 초기에 이들이 연관성이 있다고 추측되어진다.

소규모 사업장 보건관리대행기관의 간호업무 운영관리 지원체계 (Management and Supporting System on the Occupational Health Nursing Services Provided in Group Occupational Health Agencies of Korea)

  • This study was carried out to investigate the management and support system affecting to the occupational health nursing services(OHNS) provided in group occupational health agencies(GOHA). Questionnaire was developed and distributed to 82 nurses who were working in GOHA and who agreed to participate in the survey. The results were as follow: 1. OH nurses responded were mostly in the age of twenty to thirties(89%), married(73.7%), technical college graduates(88.9%), worked in hospital(85.4%) and participated more than 1 year in group occupational health services (96.3%). 2. Fifty eight point four percent of the OH nurses worked in number of workplace more than 30 to less than 60 in the OHNS form. The figure of workplaces undertaken by nurses was ranged greatly from 9 to more than 100. Number of employees who cared by nurses were mostly under 5,000 peoples in 93.3%. The types of industry was mostly manufacturing and located in the order of factory complex area, suburban, urban and others. 3. Most OH nurses(87.8%) were fully involved in the OHNS for the SSE. Their working days to visit SSE was 5 days per week(77.8%) and one day in the GOHA at 41.3%. 4. The OH documents using by nurses were found in more than 23 different types. However, they were largely summarized in the types of 'Workplace Health Management Card', 'Personal Health Counselling Card', 'Daily Health Management Report', 'Visiting List of Workplace' and 'Sick Employee List'. 5. The items of laboratory test provided by GOHA were mostly achieved in the purpose of basic health examination. They were used to be the blood pressure check(98.8%), blood sugar test (98.8%), urine sugar and protein(91.4%), SGOT and SGPT(85.3% each), cholesterol (82.9%), hepa vaccine immunization(82.9%), r-GPT(81.7%), hemoglobin(79.3%) and triglyceride(75.5%). 6. The OH nurses(92.7%) followed the work pattern to visit the GOHA before and after small-scale enterprises(SSE) visit by car driven by nurses in 74.3%. They were payed by GOHA for transportation fees in certain amounts. However, nurse is the main person(75.0%) who covers up in case of traffic accident. If the GOHA has no transportation regulation for the formal workplace visit, data showed that nurses had been responsible to take charge(31.7%). 7. The personnel manager who takes in charge for nursing services was 'nurse' in 61.7% and 41.2% worked as the final decision maker related to nursing work. The OH nurses' opinions about factors affecting to the management were classified in the four areas such as 'Nature(Quality) of health professional'. 'Content of OHNS', 'Delivery system of the GOHS', and 'Others'. The factors were indicated highly in 'Authority as health professional', 'Level of perception of director on the OH' and 'Physical work condition for OHNS'. The things that this study suggests in the recommendation would be summarized in such as the management and supporting system working for SSE in the OHNS is necessary to reform thoroughly. The reconsidered aspects might be in the matters of number of workplaces undertaken by nurses, development of effectively practical health documents, preparation for guideline of the laboratory test in the workpleces, establishment of convenient and encouraging support system and cooperation between other health professionals with respect and skill.
