Femoral Periprosthetic Fractures after Total Knee Arthroplasty: New Surgically Oriented Classification with a Review of Current Treatments

  • Purpose: As the number of total knee arthroplasties (TKAs) increases, the incidence of femoral periprosthetic fractures after TKA is also increasing. This review aimed to suggest a new surgically oriented classification system for femoral periprosthetic fractures. Methods: We investigated the classifications, and current treatment trends for femoral periprosthetic fractures after TKA by means of a thorough review of the relevant literature. Results: Numerous studies reported good results of surgical treatment with modern fixatives including locking compression plates and retrograde intramedullary nails. However, few classifications of femoral periprosthetic fractures reflect the recent developments in surgical treatment. Conclusions: We recommend that surgical management be considered the first-line treatment for femoral periprosthetic fractures after TKA. Our new classification will help in deciding the surgical treatment option for femoral periprosthetic fractures after TKA.

Korean Medical Treatment for Improving Symptoms of Four Patients Diagnosed with Grade II Lumbar Spondylolisthesis

  • Objectives : To investigate the clinical results of patients who were diagnosed with grade II spondylolithesis treated with Korean medicine of relaxative Chuna and Gangchuk herbal medicine. Methods : We reviewed medical records of four patients diagnosed with grade II spondylolisthesis. Each patient has taken intensive Korean medical admission treatment at Mokhuri Hospital. All patients received treatments of Gangchuk herbal medicine, relaxative Chuna and acupuncture during admission period. Before treatment and every after 7-day treatment the patient's pain and walking distance without pain were assessed. The zero minimum to ten maximum pain numeric rating scale(NRS) was used to determine the degree of pain. Results : Average admission duration was 23 days. All the patients' NRS decreased from median figure of 7.5[the minimum 7 to the highest 8] to median figure of 1[the minimum 0 to the highest 2]. Walking distance without pain increased from a median of 55 m[the minimum 20 m to the highest 100 m] to median of 165 m[the minimum 100 m to the highest 250 m]. Conclusions : Intensive conservative korean medical treatment was effective in reducing pain and functional improvement for grade II lumbar spondylosisthesis patients.

대퇴신경지각이상증 환자에 대한 한의학적 치료 효과: 증례보고 (Effects of Korean Medicine Treatment for a Meralgia Paresthetica Patient: A Case Report)

  • Meralgia paresthetica is a rare femoral disease and various symptoms appear such as pain, numbness, and paresthesia in the anterolateral thigh due to entrapment of the lateral femoral cutaneous nerve. We treated the meralgia paresthetica patients with Korean medicine treatment including herbal medicine, acupuncture, Chuna manual therapy and pharmacopuncture during 12 days. Numerical rating scale (NRS), Euroqol five dimension (EQ-5D) index, and the changes of symptoms were measured for assessment. After 12 days inpatient treatment, NRS decreased from 7 to 4, EQ-5D index and the symptoms of the patient also were improved. In conclusion, this case shows Korean medicine treatment might be an effective treatment for Meralgia paresthetica.

당뇨병성 족부 골수염 치료에서 보존적 치료에 대한 임상적 고찰 (Clinical Analysis of the Conservative Treatment for Diabetic Foot Osteomyelitis)

  • Purpose: The question of surgical versus non-surgical treatment for diabetic foot osteomyelitis remains subject to debate. The aims of this study were to analyse the outcome of conservative treatment (antibiotic treatment and conservative surgery) for diabetic foot osteomyelitis and identify the predictive factors of remission in conservative treatment of diabetic foot osteomyelitis. Materials and Methods: Seventy-seven patients with diabetic foot osteomyelitis who initially received conservative treatment from January 2004 to July 2013 were identified, and their medical records were reviewed. Diabetic foot osteomyelitis was defined by imaging studies or histological evidence. Remission was defined as the absence of any sign of infection at the initial or contiguous site assessed at least 12 months after the end of treatment. The demographic, clinical, and therapeutic factors were analysed. Results: The mean age of the patients was $62.7{\pm}12.2$ years, and 47 patients (61.0%) were male. The median diabetes duration was $15.7{\pm}11.2$ years and mean HbA1c was $8.7%{\pm}2.4%$. Forty-eight patients (62.3%) healed with conservative treatment (antibiotic treatment and conservative surgery). Twenty-five patients (32.5%) underwent amputation. In the multivariate analysis, concomitant peripheral artery disease and inadequate antibiotic therapy were associated with failure of conservative treatment. Conclusion: Antibiotics alone, or with conservative surgery, were successful in treatment of diabetic foot osteomyelitis in 62.3% of the patients. Concomitant peripheral artery disease and inadequate antimicrobial therapy were risk factors for remission in conservatively treated diabetic foot osteomyelitis.

Evaluation of Dosimetric Effect and Treatment Time by Plan Parameters for Endobronchial Brachytherapy

  • This study aims to analyze dose distribution and treatment time of endobronchial brachytherapy (EBBT) by changing the position step size of the dwell position. A solid water phantom and an intraluminal catheter were used in the treatment plan. The treatment plans were generated for 3, 5, 7, and 10 cm treatment lengths, respectively. For each treatment length, the source position step sizes were set as 2.5, 5, and 10 mm. Three reference points were set 1 cm away from the central axis of the catheter, along the axis, for uniform dose distribution. Volumetric dose distribution was calculated to evaluate the dosimetric effect. The total radiation delivery time and total dwell time were estimated for treatment efficiency, which were increased with position step sizes. At half-life time, the differences between the position step sizes in the total radiation delivery time were 18.1, 15.4, 18.0, and 24.0 s for 3, 5, 7, and 10 cm treatment lengths, respectively. The dose distributions were more homogenous by increasing the position step sizes. The dose difference of the reference point was less than 10%. In brachytherapy, this difference can be negligible. For EBBT, the treatment time is the key factor while considering the patient status. To reduce the total treatment time, EBBT can be performed with 2.5 mm position step size.

Computed Tomography Findings Associated with Treatment Failure after Antibiotic Therapy for Acute Appendicitis

  • Objective: To identify the CT findings associated with treatment failure after antibiotic therapy for acute appendicitis. Materials and Methods: Altogether, 198 patients who received antibiotic therapy for appendicitis were identified by searching the hospital's surgery database. Selection criteria for antibiotic therapy were uncomplicated appendicitis with an appendiceal diameter equal to or less than 11 mm. The 86 patients included in the study were divided into a treatment success group and a treatment failure group. Treatment failure was defined as a resistance to antibiotic therapy or recurrent appendicitis during a 1-year follow-up period. Two radiologists independently evaluated the following CT findings: appendix-location, involved extent, maximal diameter, thickness, wall enhancement, focal wall defect, periappendiceal fat infiltration, and so on. For the quantitative analysis, two readers independently measured the CT values at the least attenuated wall of the appendix by drawing a round region of interest on the enhanced CT (HUpost) and non-enhanced CT (HUpre). The degree of appendiceal wall enhancement (HUsub) was calculated as the subtracted value between HUpost and HUpre. A logistic regression analysis was used to identify the CT findings associated with treatment failure. Results: Sixty-four of 86 (74.4%) patients were successfully treated with antibiotic therapy, with treatment failure occurring in the remaining 22 (25.5%). The treatment failure group showed a higher frequency of hypoenhancement of the appendiceal wall than the success group (31.8% vs. 7.8%; p = 0.005). Upon quantitative analysis, both HUpost (46.7 ± 21.3 HU vs. 58.9 ± 22.0 HU; p = 0.027) and HUsub (26.9 ± 17.3 HU vs. 35.4 ± 16.6 HU; p = 0.042) values were significantly lower in the treatment failure group than in the success group. Conclusion: Hypoenhancement of the appendiceal wall was significantly associated with treatment failure after antibiotic therapy for acute appendicitis.

감압요법을 이용한 요추 추간판 탈출증 치료 3례 (Case Report of L-spine HIVD treated with Spine Decompression)

  • Objectives : This study reported 3 cases of L-spine HIVD which was treated with spine decompression, as a new medical treatment alleviates lumbago by non-surgical treatment, and oriental medicine treatment. Methods : Each patient has been treated with spine decompression and oriental medicine treatment, and the degree of improvement has been evaluated by VAS and Rating scale for LBP. Results : Through the result, the medical treatment proved to have valid effect for L-spine HIVD, but patients did not become healthy enough restored to their job before. Conclusions : At the result of this, every case can be certified to give appropriate effect, and there needs more research into synergy between spine decompression and oriental medicine treatment.

경추 후만에 대한 추나치료 치험례 (The Case Report of Chuna Treatment on Neck pain Patients with Kyphotic Cervical Curvature)

  • Objectives : This study is performed to evaluate the clinical effect of Chuna therapy on the neck pain associated with kypotic cervical curvature. Methods : This study carried out on two patients with neck pain & kyphotic cervical curvature who have received treatment in Department of Oriental Rehabilitation Medicine, Dong-seo Oriental Medical hospital from 4th April 2011 to 26th May 2012. Pre and post treatment, We evaluated the cervical angle, Jackson's angle, Jochumsen method, VAS and effective score of treatment. Results & Conclusions : Two patients who received Chuna treatment recovered cervical curvature and improved neck pain.

  • Objectives : The purpose of this study is to report the efficacy of using oriental medical treatment(herbal medicine, acupuncture) with melonis Calyx Vomiting Therapy(MCVT) for trigeminal neuralgia. Methods : We experienced one cases of trigeminal neuralgia treated with oriental medical treatment and melonis Calyx Vomiting Therapy. MCVT is a traditional treatment, which induces vomiting by eating melonis Calyx powder with water. To evaluate the results of this treatment, we used the Visual Analogue Scale(VAS) and sleeping time. Results : After using MCVT with oriental medical treatment, VAS, sleeping time and clinical symptoms of the patient were improved remarkably. Conclusions : This study suggests that using MCVT with oriental medical treatment is effective on trigeminal neuralgia.

A Review of Randomized Controlled Trials of Pes Anserinus Tendinitis/Bursitis Syndrome in the China National Knowledge Infrastructure Database

  • The purpose of this study was to review randomized controlled trials (RCTs) of pes anserinus tendinitis or bursitis (PATB) syndrome in the China National Knowledge Infrastructure database to investigate the efficacy of traditional Chinese medicine treatment for PATB syndrome. There were 20 RCTs published from 2001 to 2021 which were selected for analysis by publication year, number of samples, evaluation criteria, treatment duration, and treatment method. Out of the 142 retrieved RCTs, 20 were relevant to this review, and had performed Chinese medicine treatments including acupuncture (the most common treatment typically using acupoints SP10, ST35, SP9, and LR8), manipulation (typically using acupoints ST35, SP10, and SP9), and external application therapy (typically herbal medicine) in the treatment of PATB syndrome. Chinese medicine treatments were used widely in the treatment of PATB syndrome. We hope in the future, this review may initiate the development of treatments for PATB syndrome using Korean medicine.
