Continuous Irrigation of Brain Abscess Using a Double Lumen Catheter - Technical Note - (이중 도관을 이용한 뇌농양의 지속적 세척술 - 수술 수기 -)

  • We present a modified method for the treatment of brain abscess. The double lumen extraventricular drainage (EVD) catheter which was developed for the intracerebral hematoma management, was applied for the treatment of brain abscess drainage. We placed the double lumen EVD catheter into the abscess cavity by free-hand technique and irrigated the abscess cavity continuously with antibiotics solution for 7 days. Simultaneous intravenous antibiotics was administered for 4 weeks. The continuous irrigation with double lumen catheter was found to be safe and effective treatment modality in the brain abscess.

  • Meralgia paresthetica is a rarely encountered sensory mononeuropathy characterized by paresthesia, pain or sensory impairment along the distribution of the lateral femoral cutaneous nerve (LFCN) caused by entrapment or compression of the nerve as it crossed the anterior superior iliac spine and runs beneath the inguinal ligament. There is great variability regarding the area where the nerve pierces the inguinal ligament, which makes it difficult to perform blind anesthetic blocks. Ultrasound has developed into a powerful tool for the visualization of peripheral nerves including very small nerves such as accessory and sural nerves. The LFCN can be located successfully, and local anesthetic solution distribution around the nerve can be observed with ultrasound guidance. Our successfully performed ultrasound-guided blockade of the LFCN in meralgia paresthetica suggests that this technique is a safe way to increase the success rate.

Complex open elbow fracture-dislocation with severe proximal ulna bone loss: a case report of massive osteochondral allograft surgical treatment

  • We report a case of a 69-year-old right-dominant man who had an open Monteggia-like lesion of the right elbow (Gustilo-Andersen IIIA) with severe proximal ulna bone loss associated with an ipsilateral ulnar shaft fracture due to a motorcycle accident. The patient underwent two-stage surgery. Wound debridement and bridging external fixation were performed at first. Three months later, a frozen massive osteochondral ulnar allograft was implanted and fixed with a locking compression plate. A superficial wound infection appeared 5 weeks after the second surgery. Superficial wound debridement, negative pressure therapy, and antibiotics were administered for 3 months, achieving infection healing. At 3 years post-surgery, the elbow range of motion was satisfactory with a Disabilities of the Arm, Shoulder and Hand (DASH) score of 16.7. Radiographs and computed tomography scans showed good allograft-bone integration without allograft reabsorption or hardware loosening. Although not complication-free, massive ulna osteochondral allograft implantation can be considered a valid option in cases of open Monteggia-like lesions associated with ulnar shaft fracture and severe bone loss in active patients, whenever osteosynthesis or joint replacement is not a proper solution. This type of bone stock restoration allows for future surgery, if needed.

The Clinical Trial of Quicure Vaginal Irrigation on Cenvico-Vaginitis (Quicure Vaginal Irrigation에 의한 자궁경부염 및 질염치료에 관한 임상적 연구(예보))

  • This study was intended to investigate the effectiveness of Quicure vaginal irrigaticn for treatment of cervico-vaginitis. 53 patients who visited to severance hospital during July to December 1982 were included in this study. Antiseptics used for irrigation were 1% KMnO4, 10% Betadine and Albright solution, each solution was prepared for warm ($37^{\circ}C$) and cold ($15^{\circ}C$) form respectively, Acridine Orange immunofluorescent stainiry of cervico-vaginal smear was used for assessment of effectivenese. The results were as followed. 1. On unitial examination 62%s of patients had trichomonas infection, 19% had moniliasis infection, 19% had non specific infection. 2. Smear findings of patients~who had trichomonas infection or moniliasis infection, treated with cold 1% KMnO4 or Albright solution, became>negative except case of on each group. But the findings of all patients treated with warm solution became negative. Changes of findings of patients who had non specific infection showed similar result. This results indicated that warm solution would be better than cold solution. 3. The findings of all patients treated with Betadine solution became negative except, patient who had non specific infection, showed that Betadine solution would be better than 1% KMnO4 or Albright salution. 4. Patients' feeling of using warm saline was more satisfactory than that of cold solution

Optical Sensor of Coplanar Structure Study and Design for Intravenous Solution Exhaustion Alarm System (수액 소진 알람시스템을 위한 동일평면형 광센서의 연구 및 제작)

  • Intravenous (IV) infusion set is one of the most common treatment methods applied to hospitalized patients. However, it is necessary to check the injection of IV solution in order to prevent patients from any possible medical injuries. In this paper, using the optical sensors to detect exhaustion of IV solution was proposed. The optical sensor is coplanar structure composed of LED and photodiode which is installed according to focal distance of the lens. These two elements detect exhaustion of IV solution at the desired point conveniently. Through the results of experiments using various wavelength of LED (R.G.B), the blue LED was selected to the optimum light source. The suggested optical sensor can detect exhaustion of IV solution by the differences in the amount of light which is caused by properties such as total reflection, refractive index and scattering. From the implementation, the detector is applicable to both containers of IV solution, glass bottle and plastic pack. And also the result shows apparent differences according to existence of IV solution even if the IV solution color and illumination were changed.

Effects of Verapamil in Cardioplegic Perfusates on the Ischemic Myocardium in Isolated Rat Heart (흰쥐의 적출된 심장에서 Verapamil이 허혈성 심근에 미치는 효과)

  • Using isolated rat heart preparations, we observed the protective effe ts of verapamil cardioplegia on ischemic myocardial injury. Isolated rat hearts were subjected to global ischemia at $25^{\circ}C$ Twenty four isolated Sprague Dawley rat hearts underwent 30 minutes of the retrograde nonworking perfusion with Krebs-Henseleit buffer solution followed by $25^{\circ}C$ cardioplegic solution (St. Thomas'Hospital Cardioplegic Solution) for 60 minutes. Before ischemic arrest, rat hearts were treated with cold cardioplegic solution in control group (n=12) and cold cardioplegic solution with verapamil (1 mg/L) in experimental group (n=12). After 60 minutes of ischemia, hemodynamic and biochemical parameters such as heart rate, left ventricular pressure (LVP), + dp/dt max, coronary flow and creatine phosphokinase (CPK) were measured before giving cardioplegia and 30 minutes after reperfusion. Verapamil group exhibited greater recovery of heart rate, LVP, +dpldt max, coronary flow and CPK than control group (p < 0.05).

Clinical Evaluation of Human Papillomavirus Detection by careHPVTM Test on Physician-Samples and Self-Samples using The Indicating FTA Elute® Card

  • Objective: To make the clinical evaluation of a solid-state human papillomavirus (HPV) sampling medium in combination with an economical HPV testing method ($careHPV^{TM}$) for cervical cancer screening. Methods: 396 women aged 25-65 years were enrolled for cervical cancer screening, and four samples were collected. Two samples were collected by woman themselves, among which one was stored in DCM preservative solution (called "liquid sample") and the other was applied on the Whatman Indicating FTA $Elute^{(R)}$ card (FTA card). Another two samples were collected by physician and stored in DCM preservative solution and FTA card, respectively. All the samples were detected by $careHPV^{TM}$ test. All the women were administered a colposcopy examination, and biopsies were taken for pathological confirmation if necessary. Results: FTA card demonstrated a comparable sensitivity of detecting high grade Cervical Intraepithelial Neoplasia (CIN) with the liquid sample carrier for self and physician-sampling, but showed a higher specificity than that of liquid sample carrier for self-sampling (FTA vs Liquid: 79.0% vs 71.6%, p=0.02). Generally, the FTA card had a comparable accuracy with that of Liquid-based medium by different sampling operators, with an area under the curve of 0.807 for physician &FTA, 0.781 for physician &Liquid, 0.728 for self & FTA, and 0.733 for self &Liquid (p>0.05). Conclusions: FTA card is a promising sample carrier for cervical cancer screening. With appropriate education programmes and further optimization of the experimental workflow, FTA card based self-collection in combination with centralized $careHPV^{TM}$ testing can help expand the coverage of cervical cancer screening in low-resource areas.

Protective Effect of Chlorpromazine for the Isolated Rat Heart from Reperfusion Injury (Chlorpromazine 이 과분극 정지심장의 재관류 손상에 미치는 보호효과)

  • This study was designed to investigate the protective effect of chlorpromazine against the reperfusion injury of myocardium after high potassium cardioplegic arrest. Langendorff`s preparations of rat heart were infused with high potassium cardioplegic solution[St. Thomas Hospital Solution] at 25oC. Chlorpromazine [10-7M] increased the recovery of myocardial contractility[dp/dt], left ventricular pressure[LVP], and coronary flow rate of the reperfused heart. Both in control and experimental groups, the restoration of myocardial activity could not reach to the level of preplegic control. These results suggest that the etiologic factors of the reperfusion injury include the influence of high potassium cardioplegic solution and/or reperfusion itself, and that chlorpromazine protects myocardium from the reperfusion injury.

  • Donor availability is a major limiting factor in heart transplantation. Prolonging donor heart preservation would facilitate distant heart procurement. The setup used was the isolated retrograde perfused nonworking rabbit heart model and 4 hours of preservation at 2oC. And the electron microscopic findings of the myocardium were evaluated after reperfusion for 5 minutes. The following three groups [each group, n=4] were prepared: Group I: the heart was arrested with the St. Thomas Hospital solution [STH] and stored in Ringers lactate solution [RLS]; Group II: the heart was arrested with STH and stored in Modified Collins-Sachs solution [MCS]; Group g: the heart was arrested with and stored in MCS. The result was the most severe myocardial injury in the Group III on electron microscopic study.

A Clinical Evaluation of Spontaneous Pneumothorax; A Review of 612 Cases (자연기흉의 임상적 고찰)

  • Possible enhancement of myocardial protection with oxygenated crystalloid cardioplegia and blood cardioplegia were evaluated in a cardiac enzyme study. The bicarbonate-containing solution equilibrated with 100% oxygen becomes highly alkaline as carbon dioxide is released. 95% oxygen and 5% carbon dioxide was added to the crystalloid cardioplegic solution [St. Thomas` Hospital No. 2 Solution] for prevention of severe alkalinity of oxygenated crystalloid cardioplegia. Heart was arrested and reinfused every 20 minutes throughtout the ischemic period with crystalloid cardioplegia or oxygenated crystalloid cardioplegia or blood cardioplegia. Group I was a patient with crystalloid cardioplegia in 11 patients. Group II was a patient with oxygenated crystalloid cardioplegia in 9 patients. Group III was a patient with blood cardioplegia in 15 patients. The value of CK-MB was evaluated from the patient`s serum at 6 hours, 24 hours, and 48 hours postoperatively. In Group I and II, there was no significant change of CK-MB. In Group I and III, the value of CK-MB at postoperative 6 hours was 114$\pm$83 ng/ml and 56$\pm$22 ng/ml [P < 0.05]. In conclusion, blood cardioplegia was superior to crystalloid cardioplegia.
