노인전문병원의 병동부문 공간구성에 관한 연구 (A study on the Spacial Composition for Ward in Geriatric Hospital)

  • The purpose of this study is to provide a basic data for architectural planning on the wards and special quality of physical remedy spaces of geriatrics hospital. This research, to analyse dependency degree of inpatients in geriatric hospital, shows relativeness between remedy space and wards, and to show behavior of each patients reaction to there space. Result of interviewed inpatients who were sorted by age, sex, dependency degree, and days in hospital, suitable space programming is to need for physical and mental state. On the bases of survey and analysis, the basic data for planning and design the hospital for the elderly with dementia has been proposed.

  • Mental hospital has been expanding its function to psychiatric treatment in the meaning of therapy. Therefore systematic research on thr mental hospital is urgent. This study aims to prepare the basic data which are needed on planning and designing the mental hospital with emphasis on the wards, which take the largest area ratio and play a principal role to psychiatric patients. The process of study is to 1) to survey facility of Mental hospital in Korea, 2) to analize charactor and behavior of psychiatric patients 3) to suggest element of ideal ward in general hospital.

체계중심병원설계를 위한 공조실 면적 계획에 관한 연구 (A Study on Area Planning of Air Conditioning Room for Hospital Design Focused on System)

  • Purpose: As the improvement in quality of medical environment and the spatial consideration for the prevention of infection in the hospital have become increasingly important in the recent years, specific and practical planning for air conditioning room's area has become a major issue accordingly. However, the air conditioning room tends to be discussed focusing on technical factors such as air conditioning systems that are irrelevant to building plans, while discussions of most departments related to the medical functions have actively taken placed. Therefore, this study aims to investigate the factors influencing the planning for the air conditioning room area, and through subsequent analysis of the area of the air conditioning room, to propose a way to improve effective planning for the air conditioning room area. Methods: This study examines the case hospitals that fall into two types: each floor supply system and concentrated supply system, and compares and analyzes the air conditioning room area-related factors and the characteristics of changes in the interior of air conditioning room before and after remodeling through air conditioning floor plan of those case hospitals. Results: The air conditioning room can be classified into the net area of the functional space such as duct passages, and the public area such as aisle space, and of those the public area is an important factor in calculating the area of the air conditioning room. The public area of the air-conditioning room should not be planned considering only the spaces for mobile passage or maintenance activities, but should be planned taking into account internal changes such as expansion and replacement of equipment in the future from the beginning. Implications: When planning a medical facility, it is used as basic data for the planning of the air conditioning room area, which is a significant fixed factor in the initial setting.

QFD 기법을 이용한 병원 위탁급식 운영전략 수립 (Strategic Planning for the Contract-Managed Hospital Foodservice Through QFD Methodology)

  • At present, health care industries throughout the world are struggling with the challenges to set up financial structures as cost-effective ways and means of satisfying customer needs for health care services. Many hospitals consign foodservice management to foodservice companies for the purpose of efficiency. The companies taking charge of hospital foodservice are also striving to gain an advantage over keen competitions. This study applied Quality Function Deployment(QFD) to one hospital (which will be shown as $\ulcorner$A hospital$\lrcorner$ below) managed by a contract foodservice company for the purpose of strategy planning to provide sustainable competitive advantage. First of all, this study scanned internal and external environment of $\ulcorner$A hospital$\lrcorner$ by means of a Quality Measurement Tool and a fieldwork study. With the result of environment scanning, this study elicited 20 strategies through SWOT analysis, which were categorized by 4 perspectives such as financial, customer, internal process, learning and growth perspectives. Finally, the priorities of 20 strategies were extracted from QFD methodology. According to the results obtained by applying QFD to $\ulcorner$A hospital$\lrcorner$'s foodservice, the strategies which $\ulcorner$A hospital$\lrcorner$ foodservice was obliged to introduce and implement were : the specialization of Children's hospital foodservice, scientific foodservice management through the standardization of foodservice operations, the maintenance of sanitary quality through sanitary system, the remodeling of facilities, the introduction of new equipment, the prompt and accurate response to customer needs, the development of appropriate patient menus, the provision of competitively priced meals for patient selection, the development of a demand forecast model by considering the characteristics of a children's hospital, improvement of productivity and the reduction of labor costs through the employment of experienced employees based on their seniority.

Attitude and treatment options in implant-supported prosthetics: A survey among a cohort of German dentists

  • PURPOSE. The aim of the current study was to analyze treatment concepts of a cohort of German dentists for planning, fabrication, and maintenance of implant-supported fixed and removable restorations. MATERIALS AND METHODS. A questionnaire including queries about experiences with implant-supported restorations as well as prosthetic and maintenance treatment concepts for supplying patients with fixed and removable implant-supported prosthetic restorations was developed and sent to 350 dental offices registered in the municipal area of Leipzig, Germany. RESULTS. An overall total of 62 returned questionnaires were included in the analyses, which relates to a response rate of 17.7%. Participating dentists were more involved in the prosthetic aspects of implant dentistry rather than surgery, while prosthetic concepts such as backward planning, digital processing, and application of all-ceramic materials were not commonly performed. Simple attachments were preferred over complex retention systems in removable implant-supported restorations. Tooth/implant-supported fixed denture prostheses as well as removable denture prostheses with supporting posterior implants were not regarded as a favorable treatment option. CONCLUSION. Within the limitations of the study, the data indicate that dentists favor simple and conventional treatment approaches in implant prosthetics. Prosthetic aspects in the planning of implant-supported restorations are often neglected. Prosthetic treatment guidelines and aspects should commonly be considered in the planning phase of implant-supported prosthetic restorations, and awareness should be increased in postgraduate education.

Comparative Evaluation of Two-dimensional Radiography and Three Dimensional Computed Tomography Based Dose-volume Parameters for High-dose-rate Intracavitary Brachytherapy of Cervical Cancer: A Prospective Study

  • Background: Dosimetric comparison of two dimensional (2D) radiography and three-dimensional computed tomography (3D-CT) based dose distributions with high-dose-rate (HDR) intracavitry radiotherapy (ICRT) for carcinoma cervix, in terms of target coverage and doses to bladder and rectum. Materials and Methods: Sixty four sessions of HDR ICRT were performed in 22 patients. External beam radiotherapy to pelvis at a dose of 50 Gray in 27 fractions followed by HDR ICRT, 21 Grays to point A in 3 sessions, one week apart was planned. All patients underwent 2D-orthogonal and 3D-CT simulation for each session. Treatment plans were generated using 2D-orthogonal images and dose prescription was made at point A. 3D plans were generated using 3D-CT images after delineating target volume and organs at risk. Comparative evaluation of 2D and 3D treatment planning was made for each session in terms of target coverage (dose received by 90%, 95% and 100% of the target volume: D90, D95 and D100 respectively) and doses to bladder and rectum: ICRU-38 bladder and rectum point dose in 2D planning and dose to 0.1cc, 1cc, 2cc, 5cc, and 10cc of bladder and rectum in 3D planning. Results: Mean doses received by 100% and 90% of the target volume were $4.24{\pm}0.63$ and $4.9{\pm}0.56$ Gy respectively. Doses received by 0.1cc, 1cc and 2cc volume of bladder were $2.88{\pm}0.72$, $2.5{\pm}0.65$ and $2.2{\pm}0.57$ times more than the ICRU bladder reference point. Similarly, doses received by 0.1cc, 1cc and 2cc of rectum were $1.80{\pm}0.5$, $1.48{\pm}0.41$ and $1.35{\pm}0.37$ times higher than ICRU rectal reference point. Conclusions: Dosimetric comparative evaluation of 2D and 3D CT based treatment planning for the same brachytherapy session demonstrates underestimation of OAR doses and overestimation of target coverage in 2D treatment planning.

한방의료환경의 변화에 대응한 한방병원의 건축계획적 연구 (A Study on the Architectual Planning of the Oriental Medical Hospital Corresponding with the Change of the Oriental Medical Environment)

  • Corresponding to the change of the environment of oriental medicine, oriental medicine hospital in today is adjusting between the specific element of oriental medicine and the systematic application of occidental medicine hospital. According to this situation, this study aims to represent the material of the architectural planning of the the oriental medicine hospital. As its method, including the concept of the cooperative examination and treatment between oriental and occidental medicine hospital, the circulation of the patient and staff, and the type of the plane figure of the outpatient department are estimated.

  • From the latter half of 20's century, the numbers of dental service workers and dental facilities are noticeably increased and dental patients want the higher-classed and specialized dental service. For the increased facilities and user's needs at dental college hospital, the present condition of dentistry, special features and the field study at dental college hospital are surveyed for this study. The purpose of this study is to provide a architectural planning data on the dental college hospital for keeping up with the development of dentistry.

  • Considering the rapid growth of elderly population and the increase of medical demand for aged, many of geriatric hospitals are needed in Korea. On the other hand, supplies of hospitals for acute care are considered to be excessive. So many of small and medium hospitals facing management problems have been converged to geriatric hospitals. These attempts are considered resonable for efficient utilization of health care resources in Korea. This paper aims to identify the concept of geriatric hospital and to analyze the problems and then to seek the alternatives for architectural planning on the basis of surveying the geriatric hospitals converged from hospitals for acute care.
