  • In Korea's distribution and production field establishment of logistics information system and automation of logistics were greatly developed. But there is no characteristic and professionalism about hospital logistics, and distribution center is located in outside warehouse, logistics system is very important. Since hospital has human power to receive product, system is not as much required, but outside warehouse requires system for arrival from medicine and medical supplies wholesale dealer and vender, warehousing, storage, and automation facilities, and system for various works such as peaking instruction, classification, packing, cargo-working, delivery, order of medicine from hospital and non-medicine in hospital is essential. Therefore, This study is about development of automatic identification of slide which cannot be overlooked in efficiency of hospital logistics, establishment of database, and information interlocking between automatic storage system and outside warehouse.

TIAM2 Enhances Non-small Cell Lung Cancer Cell Invasion and Motility

  • Background: TIAM2, a Rac guanine nucleotide exchange factor, is closely associated with cell adherence and migration. Here, we aimed to investigate the role of TIAM2 in non-small cell lung cancer (NSCLC) cells. Materials and Methods: A small interference RNA (siRNA) was introduced to silence the expression of TIAM2. Invasion and motility assays were then performed to assess the invasion and motility potential of NSCLC cells. GST-pull down assays were used to detect activation of Rac1. Results: TIAM2 was highly expressed in NSCLC cells. Knockdown of TIAM2 inhibited the invasion and motility, and suppressed activation of Rac1. Further experiments demonstrated that knockdown of TIAM2 could up-regulate the expression of E-cadherin, and down-regulate the expression of MMP-3, Twist and Snail. Conclusions: Our data suggest that TIAM2 can promote invasion and motility of NSCLC cells. Activation of Rac1 and regulation of some EMT/invasion-related genes may be involved in the underlying processes.

The Effect of Hospital SCM on Logistics Performance (병원 통합물류시스템(SCM)이 물류 업무성과에 미치는 영향)

  • Purpose: The purpose of this study was to examine the causal relationships among quality of hospital SCM (system quality, information quality, service quality), perceived usefulness, user satisfaction and logistics performance by hospital staff. Methods: A survey using a structured questionnaire was conducted with 276 hospital staff. Analysis of the data was with both SPSS Win 18.0 for descriptive statistics and AMOS 8.0 for the structural equation model. Results: The hypothetical model yielded Chi-square=8.121 (p=.087), df=4, $x^2$/df=2.030. GFI=.989, AGFI=.942, RMSEA=.066, NFI=.995, CFI=.997 TLI=.990 and showed good fit indices. Three dimensions of quality of SCM had significant direct effects on perceived usefulness. System and service of quality of SCM had significant direct effects on user satisfaction. Perceived usefulness had significant direct effects on user satisfaction. User satisfaction had significant direct effects on logistics performance. Conclusion: These results suggest that logistics performance can be enhanced by user satisfaction, user satisfaction can be enhanced by perceived usefulness, and perceived usefulness can be enhanced by quality of hospital SCM. Further study with various hospitals and various variables is necessary.

Economic Analysis of Introduction of Third Party Logistics System in a Tertiary Hospital (상급종합병원 3자 물류외주시스템 도입의 경제성 분석)

  • Purpose: This study purposed to analyze the economic feasibility of introducing a third party logistics system as a strategic alternative to hospital management. Methods: Economic analysis was performed by measuring changes in costs and benefits before and after implement the third party logistics system and estimating the size of net benefits for the next five years for the target hospital. A questionnaire survey and in-depth interview with stake-holders were conducted to find out the satisfaction and effectiveness of the system. Findings: According to results, the cost-benefit ratio for the implementation of the system was 1.18. For the next five years, the cost-benefit ratio was 1.48, the net present value was about 1.7 billion won, and the internal rate of return was 64%. The satisfaction of internal stake-holders was relatively high, in terms of improving the concentration of unique tasks and increasing the efficiency of inventory management. Practical Implications: It was found that the increase in benefits had a greater effect on the change in the cost-benefit ratio than the increase in costs resulting from the expansion of logistics, and the increase in the present value of net benefits gradually decreased as the cost increased. In addition, overall job satisfaction and satisfaction with outsourcing companies were relatively low, which means that stabilization of the new system is important. Further study is needed for more accurate economic analysis.

Whole body hypoxic preconditioning-mediated multiorgan protection in db/db mice via nitric oxide-BDNF-GSK-3β-Nrf2 signaling pathway

  • The beneficial effects of hypoxic preconditioning are abolished in the diabetes. The present study was designed to investigate the protective effects and mechanisms of repeated episodes of whole body hypoxic preconditioning (WBHP) in db/db mice. The protective effects of preconditioning were explored on diabetes-induced vascular dysfunction, cognitive impairment and ischemia-reperfusion (IR)-induced increase in myocardial injury. Sixteen-week old db/db (diabetic) and C57BL/6 (non-diabetic) mice were employed. There was a significant impairment in cognitive function (Morris Water Maze test), endothelial function (acetylcholine-induced relaxation in aortic rings) and a significant increase in IR-induced heart injury (Langendorff apparatus) in db/db mice. WBHP stimulus was given by exposing mice to four alternate cycles of low (8%) and normal air O2 for 10 min each. A single episode of WBHP failed to produce protection; however, two and three episodes of WBHP significantly produced beneficial effects on the heart, brain and blood vessels. There was a significant increase in the levels of brain-derived neurotrophic factor (BDNF) and nitric oxide (NO) in response to 3 episodes of WBHP. Moreover, pretreatment with the BDNF receptor, TrkB antagonist (ANA-12) and NO synthase inhibitor (L-NAME) attenuated the protective effects imparted by three episodes of WBHP. These pharmacological agents abolished WBHP-induced restoration of p-GSK-3β/GSK-3β ratio and Nrf2 levels in IR-subjected hearts. It is concluded that repeated episodes of WHBP attenuate cognitive impairment, vascular dysfunction and enhancement in IR-induced myocardial injury in diabetic mice be due to increase in NO and BDNF levels that may eventually activate GSK-3β and Nrf2 signaling pathway to confer protection.

  • The traditional barcode based medicine logistics system have the problems such as loss, management error, online track, public information due to the hand-operated logistic process. Otherwise, quick response allows that the company observe the requests consistently and design medicine and establish manufacture schedule rapidly so that they could prohibit the product stocked. In this paper, we analyzed how to process medicine circulation logistics and information steps practically and proposed the RFID based medicine logistics system for the quick response in hospital. The proposed method provided the medical history tracing service, the traditional system connection, the efficiency logistic service, and the narcotic specified management service using the medicine logistics information from RFID tag. Accordingly, the satisfaction and the productivity will be improved the efficient medicine logistics by reducing the loss as well as the management error.

A Study on Relationship between Outsourcing and Organizational Effectiveness in Hospital (병원의 아웃소싱과 조직유효성의 관계에 관한 연구)

  • The purpose of this study is to make managerial information regarding outsourcing more concrete by identifying and evaluating how outsourcing as an useful strategic tool for hospitals influences organizational effectiveness. The survey was performed to 311 general hospitals and tertiary hospitals, and 63 questionnaires were recovered and analyzed. As the result of measuring organizational effectiveness after introduction of outsourcing, non-financial performance(3.34) was higher than financial performance(3.25) and satisfaction(3.08). According to the characteristics of organizational structure, financial performance showed statistically significant difference when categorizing the hospitals. It was higher in the general hospitals than in the tertiary hospitals. In addition, the hospitals that outsource the logistic and patient affairs parts have higher financial performances than non-financial ones. Especially, there was statistically significant difference depending on the sub-parts of the logistics, which means the hospitals outsourcing the logistic part have higher financial performance than the hospitals without outsourcing the logistics.

  • New infectious diseases such as MERS have been in need of many measures such as initial discovery, isolation, and crisis response. In addition, the culture of hospitals is changing, such as the general public 's visiting and Nursing Care Integration Services. However, as the qualifications and regulations of medical personnel in hospitals become rigid, overseas such as linens, wastes movements are replacing possible works with robots. we have developed a hospital logistics robot that can carry out various goods delivery within a hospital, and can move various kinds of objects safely to a desired location. In this thesis, we have studied a hospital logistics robot that can carry out various kinds of goods delivery within the hospital, and can move various kinds of objects such as waste, and linen safely to a desired location. The movement of a robot in a hospital may cause a collision between a person and an object, so that the collision must be prevented. In order to prevent collision, it is necessary to recognize whether or not an object exists in the movement path of the robot. And if there is an object, it should recognize whether it moves or not. In order to recognize human beings and objects, we recognize the person with face/body recognition technology and generate the context awareness of the object using 3D Vision image segmentation technology. We use the generated information to create a map that considers objects and person in the robot moving range. Thus, the robot can be operated safely and efficiently.

Job Analysis of Hospital Administrator Based on the DACUM Method (DACUM 법에 의한 병원행정사 직무분석)

  • The purpose of this study was to analyze of hospital administrator based on the DACUM(Developing A Curriculum) method. The contents of this study were to extract the duties, tasks consisting of job of hospital administrator and to investigate levels of importance, difficulty, frequency and entry level on each task, and to make out a job model of hospital administrator. A DACUM committee was composed to analyze job of hospital administrator and the committee members were total 9, a facilitator, 7 hospital administrator and a recorder. The major findings of this study were as the followings; first, duties in job of hospital administrator were total 13, which were organization of hospital administration affairs, health insurance review & assessment, general affairs, personnel management, hospital planning & management, medical quality improvement, hospital financial affairs, logistics management, facilities management, computerized system management, education & study supports, medical staffs' perceptions of service quality both directly and indirectly through their perceptions of patient orientation. These findings implied that HPWS would be a way of survival in drastically changing hospital environments.

A Study on Expiration Date on Ethylene Oxide Gas Sterilization Products - Related to Storage Environment - (Ethylene Oxide 가스 멸균물품의 유효기간에 관한 연구 - 보관환경을 중심으로 -)

  • Purpose: This was a study on the expiration date of Ethylene Oxide (EO) gas sterilization and effects of the environmental factors of temperature, humidity and type of cabinet in sterile goods storage area on the expiration date. Methods: Sterile goods storage areas from 13 departments in one hospital were selected and 455 EO gas sterilization samples were prepared and kept in those areas over the 14 months of the study. Each sample was tested with a microbiological culture in the laboratory every week. If the result was positive, the sample was regarded as contaminated. The researcher visited once a month to check the temperature, humidity and type of cabinet. Results: With the exception of 1 sample which was positive at 56th week. 454 samples were confirmed as negative. The environment of the samples storage area was measured monthly. The annual average temperature was $24.2{\pm}1.6^{\circ}C$, and the mean relative humidity $34.7{\pm}15.2%$. The types of cabinet were 7 open and 6 closed. Conclusion: The results of the microbiological culture at 13 months showed that none of the samples were contaminated. Therefore the hospital's existing Expiration Date can be extended from 6 months to 13~14 months.
