ln general, hospital information system should provide interoperability hat usually and operate independence of other HIS. This study proposes a new HIS paradigm that can be implemented within standard HL7 Interface engine and clinical data repository (CDR). We have developed an alternative architecture relying on agent solutions with distributed queries to heterogeneous databases. This architecture creates a very fine and flexible repository that can handle queries with the bases of standard HL7 messaging structure. Deploying Agent solutions to manipulate autonomy of storage management and sociality for communication with open world is another issue that keeps this system from reinventing existing wheels in medical informatics. This study the first attempt to construct CDR based private clinic. We used the information stored in the clinical patient record system of the internal medicine private hospital which is used rational database. We were searched increasing the 1,000 data entry from 1,000 to 10,000. By the result, experimental CDR showed highly efficient performance more than 6,000. In the future, the CDR can be further extended for clinical information among private hospitals estranged from EHR (Electronic Health Records).

It is difficult to settle the well-designed local terminology for imaging report in the hospital information system (HIS). One of the major reasons is the local terminology with poor contents have been used in the hospital. Thus, we mapped the locally used terms in nuclear medicine imaging report to the SNOMED-CT, which had been widely used in the electronic medical record system, for implementation of hospital information system. Preliminary construction of terminology dictionary was done by mapping of local terms to SNOMED-CT and LexCare Suite. Further study may be warranted.

본 논문은 현재 간호학과에 재학 중인 학생들의 컴퓨터 활용 능력과 실제 임상에서의 컴퓨터 활용 실태를 조사하여 현장에서 컴퓨터 활용에 있어서 부족한 부분을 개선하기 위하여 필요로 하는 사항을 제시하였다. 정보과학의 발달로 인한 병원업무는 병원 정보 시스템을 이용한 시스템적 관리 체제가 이루어지고 있다. 그러므로 정보 분야에 대응하는 간호학과에서의 컴퓨터 활용교육은 교양 과목으로 진행되는 일방적이고 획일적인 공급자 중심의 교육이 아닌 현장 환경에 맞는 학습자 중심의 교육이 이루어져야 한다. 따라서 본 논문에서는 현재의 단순한 간호 정보 지식 교육에서 정보 시스템 지식 습득 및 실습으로 변경될 것을 제안한다.

유비쿼터스 환경에 대한 기술적 발전에 따라 다양한 u-Health 서비스 시스템이 개발되어 시행되고 있다. u-Health 시스템은 서비스 인프라인 병원, 의료기관의 의료정보시스템 연동을 통한 의료정보활용이 불가피하다. 그러나 HL7 의료정보 표준안은 헬스케어 시스템에서 활용하는 의료정보에 대한 명세를 제시하지 못하고 있다. 즉 의료정보시스템으로부터 정보를 요청/제공 받기 위한 HL7 표준 메시지가 명시되어 있지 않다. 따라서 본 논문에서는 헬스케어 시스템에서 필요로 하는 병원 내 의료정보를 분석하고 적합한 HL7 표준 메시지 구조를 설계하였다.

A controlled medical vocabulary is a vital component of medical information management because it enables computers to use information meaningfully and different institutions to share the medical data. There are currently many standard medical vocabularies - SNOMED-CT, ICD-10, UMLS, GALEN, MED, etc, but none is universally accepted as an optimal controlled medical vocabulary for application to medical information system. Moreover, it is difficult to settle the well-designed local data dictionary consisting of controlled medical vocabularies for the individual hospital information system (HIS). One of the major reasons is the local terminology with poor contents have been used in the hospital. Thus, as a trial, the local controlled vocabulary referencing system has being constructed in a limited medical field - nuclear medicine. We selected practical nuclear medicine terms from interpretation reports and electronic medical records, and removed ambiguity and redundancy, mapping the selected terms to standard medical vocabularies. Relationship and hierarchy structure between terms have being made, referring to standard medical vocabularies. Further studies may be warranted.

Purpose: The purpose of this study was to analyze changes in the work patterns of nurses working in tertiary hospitals in Seoul, Korea Method: Structured self-administered questionnaires were sent to the nursing department of each hospital, and distributed to nurses. The questionnaires were designed to identity changes in work patterns, frequency of contact inside & between departments, and interpersonal relationship inside & between departments. And also variables for analysis included the followings ; acknowledgement of convenience, information applicability, willingness to participate in, and job satisfaction after implementation of a Hospital Information System. A total of 249 nurses from 13 hospitals replied. Results: Changes in direct nursing time showed the most improvement and the frequency of contact was decreased while interpersonal relationships among coworkers was improved. As most nurses expressed a positive opinion of the HIS including its convenience, and information applicability, their willingness to participate in it and job satisfaction, it can be concluded that the implementation of the HIS has resulted in positive influences in the work patterns of clinical nurses.

HL7은 병원정보 시스템에서 사용되는 정보와 관련된 표준화된 프로토콜이다. 생체 정보 웹 뷰어 시스템 또한 의료 영상 및 전송에 대해 표준화된 프로토콜이다. 원격 진료의 새로운 기술을 받아들이기 위해 병원에서 서로 다른 시스템들 사이의 표준화 프로토콜을 사용하는 것이 매우 중요하며, 또한 외부 병원과의 연동을 위한 통신방식도 중요하다. 본 논문에서 우리는 병원정보 시스템과 생체 정보 웹 뷰어 시스템사이의 통합 방법을 제안한다. 적절한 생체정보 데이터의 교환과 정보 관리의 수정을 통해 병원정보 시스템은 모든 병원 관계자의 보다 나은 업무처리를 향상시킬 수 있다.

현재 종합병원에서는 21세기 최첨단 의료 IT시대에 맞지 않는 단순한 검체관리의 부주의 및 비효율적인 관리로 인해서 병리조직의 인식을 수기로 기록하여 정보의 불일치와 업무적인 미스매칭으로 인하여 의료사고가 증가하고 있다. 본 논문에서는 병리조직 검사 시스템 정보화를 위한 카세트, 슬라이드 출력기 및 정보관리 소프트웨어를 설계하였고, 카세트 프린터에 RFID를 장착하여 정보를 슬라이드 프린터와 연동하여 관리할 수 있는 시스템을 설계하여 의료사고를 방지 및 u-Hospital 분야에서 효율적으로 활용될 수 있다.

통신기술과 의료장비의 발달은 환자에 대한 의료혜택의 기회를 확대하고, 최상의 의료서비스를 제공하려는 병원들에 있어서 전산 시스템은 빼놓을 수 없는 병원내 주요 기반시설로서 등장하게 되었다. 멀티미디어 기술과 정보통신의 발달은 의료분야의 정보화 및 자동화 기술 발전에 커다란 영향을 주어 의료 영상 및 각종 의료정보를 고속의 네트워킹 올 통해 전송할 수 있는 PACS(Picture Archiving and Communications System)의 개발을 가능하게 하였다. 본 논문에서는 무선네트워크의 발달과 PDA(Personal Digital Assistants)의 보편화에 따라 PDA를 이용하여 HIS(Hospital Information System)/RIS(Radiology Information System)/PACS의 자료를 검색 및 갱신할 수 있도록 설계하고, 현재 병원의 데이터베이스와 PDA의 연동이 가능하도록 병원관리 에이전트를 분석 및 설계하였다.
