통풍환자에 대한 간호사 주도 환자교육의 효과 (Effects of Nurse-led Patient Education for Gout Patients)

  • 통풍은 관절염뿐만 아니라 심혈관질환 등을 유발하는 만성염증질환이다. 하지만 대부분의 환자들이 단순히 관절질환으로 인식하고 있어 치료에 장애로 작용한다. 이 연구를 통해 간호사 주도의 통풍환자 대상 교육이 환자의 통풍에 대한 인식 개선 및 질병 경과에 미치는 영향을 확인하고자 하였다. 교육전문 간호사가 상급종합병원 류마티스내과 외래에 내원한 환자를 대상으로 통풍에 대한 일대일 교육을 시행하였고 환자의 만족도 및 복약순응도, 혈중 요산치의 변화 정도를 측정하였다. 교육을 받은 환자군의 진료만족도는 대조군에 비해 유의하게 높았으며 두 번째와 세 번째 외래 방문 시에 측정한 복약순응도와 혈중 요산치는 대조군에 비해 교육군에서 유의하게 개선이 되었다. 결론적으로 간호사 주도 환자교육은 진료 만족도 개선 및 질환 경과의 호전에도 도움이 되는 것으로 확인되었다.

보건행정 전공 대학생들의 병원실습 만족도에 영향을 미치는 요인분석 (Factors Analysis Related to Health Administration Students' Satisfaction on Hospital Practice)

  • Purpose .This study aims to assess the influence factors on the level of satisfaction with hospital training and to provide basic data for more efficient hospital practice of students' public health administration . Methods . We have conducted a survey on students satisfaction with their hands on practice against 200 students in Busan, Kyeongnam and Kyungbook area, who have completed their practice in public health administration. We have identified the general characteristics of the target respondents, the current training status of the department for the hospital practice and training venue; also analyzed the characteristics composed of the apprentice student's attitude before the practice, satisfaction with the department training program and hospital for practice, as well as their practice at hospital. The general characteristics of the target respondents and clinic practice status at the hospital were measured in percentage and frequency analysis; the average and standard deviations of hospital size were also measured; and the correlations between satisfaction with the department, attitude towards the practice and satisfaction with the hospital were analyzed. Results . Most public health administration students experienced their practice at the hospital after the first semester on the $2^{nd}$year and they have received the foundation training for the practice. The survey allowed multiple responses and the results are as follows: medical terminology 49.5%, medical coordinator 36.7%, Hospital administration 26.5%, Health insurance 17.3%, and medical recording and practice 13.8% respectively. In terms of each individual student's attitude towards the practice at the hospital, the respondents answered as follows: I have completed the obligatory subjects for the practice before the practice 89.5%; I have been fully informed on the hospital for my practice. 74.5%; I responsibly practiced and worked with professionalism and sense of belonging to the hospital as a member of staff 90%; I have actively learned and adopted 95%; My knowledge and skills from the practice was relevant to the theory and practice from my course 83.5%. The findings of satisfaction by size of hospital were in order of a small clinic($3.24{\pm}0.63$), medium sized clinic($3.27{\pm}0.65$), hospital ($3.20{\pm}0.61$), and large sized comprehensive hospital ($2.93{\pm}0.74$). Consequently the satisfaction rate shows no significant difference by size of hospital. In relations between practice attitude and satisfaction with subject, the matter of acquiring information and fully understanding of the hospital for practice before practice(p=0.04), the matter of a sense of belonging and responsibility at hospital during practice(p=0.33). the matter of active adaptation attitude during practice (p=0.42), and the matter of correlations with school curriculum during practice(p=0.00) showed significant results. In relations between practice attitude and satisfaction with the hospital for practice, the matter of acquiring information and fully understanding of the hospital for practice before practice(p=0.01), the matter of a sense of belonging and responsibility at hospital during practice(p=0.04), and the matter of correlations with school curriculum during practice(p=0.00) showed significant results. Conclusion . The most essential subject for the practice in public health administration is medical terminology and there is no significance in satisfaction with the practice by size of hospital. Students were content with the hospital where they can experience as much as possible. Students content with their major show positive attitude towards the practice and so do those content with the hospital. Those with the positive attitude towards the practice show the high correlation of satisfaction with both the major and hospital. As a result, the satisfaction with the major is the significant attribute to the practice in the hospital.

호주 일 지역의 병원 자원봉사활동 실태와 만족도 (Study of the Actual Condition and Satisfaction of Volunteer Activity in Australian Hospital)

  • 목적: 호주 일 지역의 종합병원을 중심으로 이루어지고 있는 호스피스 자원봉사자들의 특성을 파악하고 이들이 실제 환자들에게 시행하고 있는 활동내용과 그 만족도를 파악하기 위하여 시도하였다. 방법: 101명의 자가보고 질문지에 의해 자료수집되었으며, 자료의 분석은 SPSS/WIN 12.0 프로그램을 이용하여 다음과 같이 빈도와 백분율, 평균과 표준편차로 분석하였다. 결과: 1. 병원에서의 봉사활동경력은 $5{\sim}10$년이 32.7%, 10년 이상이 30.7%, $2{\sim}3$년이 11.9%, $3{\sim}5$년이 10.9%의 순이었다. 주요 봉사활동 형태는 신체적 간호가 32.7%, 신체 및 정서적 간호가 14.9%, 기타가 18.8%의 순이었다. 봉사업무할당 방법은 봉사활동 조정자에 의해서가 55.7%, 봉사자의 뜻에 따라서와 봉사자와 조정자의 합의에 의해서가 각각 20.5%의 순이었다. 봉사활동을 하는 주요 이유는 아픈 사람을 돕고 싶어서가 61.4%로 가장 많았으며, 다음은 여가시간을 선용하기 위해서가 22.8%였다. 봉사활동을 시작하게 된 경로는 자신의 조사에 의해서가 43.4%로 가장 많았으며, 다음은 다른 봉사자로부터 듣고서가 30.7%, 대중매체로부터가 13.1%의 순이었다. 봉사활동관련 교육을 받은 여부는 받았다가 80.2%였다. 봉사활동업무가 자신의 기술과 기능에 맞는 정도는 아주 잘 맞는다가 74.0%였고, 다음은 대체로 맞는다가 18.0%로 대체로 잘 맞는 것으로 나타났다. 봉사활동에 대해 받는 보상은 토큰이나 점심 혹은 집단 소풍이 31.7%로 가장 많았고, 다음은 토큰과 점심이나 집단 소풍이 각각 19.8%였다. 봉사활동에 대한 평가빈도는 이따금이 37.2%, 자주가 30.9%, 항상이 17.0%, 전혀 안 함이 14.9%의 순이었다. 봉사활동조정자와 관계는 매우 좋다가 85.0%로 대부분을 차지하였으며, 다른 봉사자와의 관계는 매우 좋다가 81.2%로 대부분을 차지하였고, 병원직원과의 관계는 매우 좋다가 69.7%였고, 다음은 대체로 좋다가 21.2%의 순이었다. 봉사활동에 대해 가족이나 친구의 지지는 어떠한가는 매우 좋다가 83.2%로 대부분을 차지하였다. 2. 대상자의 자원봉사활동 만족도는 평점 $3.09{\pm}0.49$(도구범위 $1{\sim}4$점)로 중간정도이었다. 영역별로 살펴보았을 때 만족도가 가장 높았던 영역은 사회적 접촉영역($3.48{\pm}0.61$)이었고, 다음은 성취영역($3.43{\pm}0.53$), 사회적 인정영역($3.35{\pm}0.70$)의 순이었다. 만족도가 가장 낮았던 영역은 사회적 교환영역($1.65{\pm}0.63$)이었다. 3. 대상자의 인구사회학적 특성에 따른 봉사활동 만족도를 분석한 결과 성별(t=2.038, P=0.044), 결혼상태(F=3.806, P=0.013)에 따라 유의한 차이를 보였다. 4. 대상자의 자원봉사활동 실태에 따른 봉사활동 만족도를 분석한 결과병원봉사활동기간(F=3.326, P=0.008), 봉사활동을 하는 주된 이유(F=2.707, P=0.035), 봉사활동을 위한 교육을 받은 여부(t=-1.982, P=0.050), 봉사활동의 평가 빈도(F=7.877, P=0.000), 봉사활동이 자신의 기술이나 능력에 적합도(F=2.712, P=0.049), 관리자와의 관계(t=-2.517, P=0.013), 다른 병원직원과의 관계(F=5.202, P=0.007), 자원봉사자로서의 활동에 대해 가족이나 친지로부터의 지지(t=-3.394, P=0.001)에 따라 봉사활동 만족도가 유의하게 차이가 있는 것으로 나타났다. 결론: 자원봉사활동 만족도는 중간정도이었고, 봉사활동 만족도는 대상자의 인구사회학적 특성에 따라서는 성별(t=2.038, P=0.044), 결혼상태(F=3.806, P=0.013)에 따라 유의한 차이를 보였고, 자원봉사활동 실태에 따라서는 병원봉사활동기간(F=3.326, P=0.008), 봉사활동을 하는 주된 이유(F=2.707, P=0.035), 봉사활동을 위한 교육을 받은 여부(t=-1.982, P=0.030), 봉사활동의 평가 빈도(F=7.877, P=0.000), 봉사활동이 자신의 기술이나 능력에 적합도(F=2.712, P=0.049), 관리자와의 관계(t=-2.517, P=0.013), 다른 병원직원과의 관계(F=5.202, P=0.007), 자원봉사자로서의 활동에 대해 가족이나 친지로부터의 지지(t=-3.394, P=0.001)에 따라 유의한 차이가 있는 것으로 나타났다. 따라서 자원봉사자를 관리할 때에 위의 요인들을 고려할 것이 요구된다고 볼 수 있다.

치과의사의 치과 코디네이터 업무 및 인식에 관한 조사연구 (A Study on the Outlook of Dentists on Dental Coordinators and Their Job)

  • The purpose of this study was to examine how dentists perceived dental coordinators including their education, hiring criteria, working condition and job. It's basically attempted to help define the job and role of 5 and to suggest how they should be nurtured. The subjects in this study were dentists at dental hospitals and clinics where dental coordinators were employed among approximately 200 dental institutions in Seoul, Cyeonggi province and Incheon. After a survey was conducted in June 2005, answer sheets from 99 respondents were analyzed. The findings of the study were as follows: 1. Regarding education for dental coordinators, 99.9% of the dentists investigated felt the need for separate education programs for dental coordinators, 42.4% knew what would-be dental coordinators learned about, and 81.8% considered it necessary for them to take intermediate or higher courses. An organization affiliated with the Korea Dental Hygienists Association was viewed as the best institute to educate dental coordinators, and educational institutes that included a department of dental hygiene was looked upon as the second best one. 68.7% believed that dental coordinators should take an official examination to test their qualifications, and concerning educational subsidy, the largest group of the dentists thought that a certain amount of subsidy should be provided. 2. As for coordinator hiring, the top priority was the impression(look) of applicants(55%), followed by adjustability to existing employees(24.5%) and professional competency(17.3%). As to the route of hiring, 41.4 percent, the largest group, reeducated some of existing employees, and dental hygienists were regarded as the best personnels to serve as a coordinator. Concerning job performance, they put the most emphasis on interpersonal relationship, which was followed by executive ability, impression and career, 58.6% the largest group, believed that dental coordinators should have a three-year or higher career to work at a dental institute. 3. As to working conditions, 75.7%, the largest group, paid dental coordinators based on their job performance, and 23.2%, the second largest group, had their pay equal to that of dental hygienists, 88.9% allowed them to determine their own retirement age. 4. In regard to their perception of dental coordinators, the largest number of the dentists considered it necessary for them to keep receiving education(4.29), and the second largest group felt that they served to enhance the image of dental institutes(4.18). The third largest group thought that they contributed to letting patients more satisfied with the quality of dental services. But they tended not to agree that their turnover rate was low(3.04), and they didn't find them to receive appropriate education, either(3.10). 5. The current major job of coordinators associated with customer services was handling appointments with customers(91.9%), treating unsatisfied customers(85.9%), and controling waiting time(84.8%). Regarding self-management, coordinators directed their energy into having good manners(89.9%), acquiring fundamental dental knowledge(84.8%), and learning how to treat customers(83.8%). Concerning hospital affairs handling, they were most responsible for information desk(87.9%), which was followed by receiving(86.9%). As a result of investigating what type of job the dentists hoped dental coordinators to fulfill in consideration of their career, their age and type of investment, the dentists wanted them the most to speak a foreign language, which belonged to the field of self-management.

호스피스간호사의 역할수행 정도와 직무만족도 (Role accomplishment and job satisfaction of hospice nurse)

  • 목적: 본 연구의 목적은 호스피스간호사의 역할수행정도와 직무만족도를 파악하고, 역할수행정도와 직무만족도에 영향을 미치는 요인을 분석하는데 있다. 방법: 자료수집은 전국 53개 기관에서 6개월 이상 근무하고 있는 189명의 호스피스 간호사를 대상으로 구조화된 설문지를 사용하여 자기기입식으로 2007년 10월부터 11월 사이에 시행하였다. 역할수행정도 측정도구는 Riehl의 간호역할을 기초로 유은광(1979)이 고안한 도구와 최화숙 등(2005)의 연구를 바탕으로 8개의 영역으로 구성하여 만들었으며, 이 도구의 신뢰도는 Cronbach's $\alpha$=.963 이었다. 직무만족도는 Slavitt et al.(1978)의 도구와 박정순(1994)의 연구도구를 수정·보완하여 6개의 영역으로 구성하였으며, 이 도구의 신뢰도는 Cronbach's $\alpha$=.881이었다. 수집된 자료의 분석을 위해 SPSS WIN 12.0 프로그램을 이용하였으며, 실수와 백분율, 평균과 표준편차, t-test, ANOVA, Pearson Correlation Coefficient 등으로 분석하였다. 결과: 본 연구의 대상자는 모두 여자였으며 연령은 20~29세군이 33.3%로 많았고 평균 나이는 36.7세이었다. 87.8%가 종교를 갖고 있었으며 59.3%가 병동형에서 근무하고 있었다. 대부분 근무기관에 호스피스 팀 구성이 되어 있었으며, 호스피스·완화간호 경력은 1년 이상~5년 미만이 57.7%로 많았고, 호스피스·완화교육은 호스피스간호 교육이수(12개월 이내)가 58.7%이었다. 대상자의 근무 중 가장 큰 어려움으로는 환자와 가족의 간호 28.8%, 의료제도 등의 환경장애 23.6%, 낮은 보수 15.3%, 역할의 모호성 13.1% 등 이었다. 호스피스간호사의 직무만족을 높이기 위해 개선할 사항에 대한 의견으로는 전문가적 위치 및 존중 33.1%, 업무여건 개선 23.2%, 보수 인상 22.4%, 행정적 지원 11.8% 등 이었다. 대상자의 역할수행정도와 직무만족도는 각각 3.53점, 3.39점이었다. 호스피스간호사의 역할수행은 옹호자, 실무전문가, 조정자(협동), 교육자, 질 관리자, 상담자, 행정가, 연구자 순으로 수행하고 있었다. 직무만족도는 전문가적 위치에 대한 만족, 상호작용에 대한 만족, 직무자체에 대한 만족, 행정적인 측면에서 만족, 자율성에 대한 만족 순이었고, 보수에 대한 만족이 가장 낮았다. 대상자의 역할수행정도는 연령, 기혼여부, 학력과 직위에 따라, 조정자 역할 여부, 호스피스 전담 여부, 호스피스완화간호 경력과 호스피스·완화간호교육 정도에 따라 유의하게 차이가 있었다(p<.05). 직무만족도는 연령 증가, 호스피스 팀 구성 직종 수에 따라, 조정자 역할 여부, 호스피스 전담 여부, 호스피스·완화 간호교육 정도, 근무를 가능한 오래하고 싶어 하는 대상자에서 유의하게 차이가 있었다(p<.05). 대상자의 역할수행정도와 직무만족도는 통계적으로 유의한 순상관관계를 나타내었다(r=.541, p<.01). 결론: 이상의 결과를 종합하여 볼 때, 호스피스간호사 개인과 전문직의 발전을 위해서는 연구하며 일하는 자세와 모든 역할수행에서 적극적이고 창조적인 노력이 요구된다. 그리고 보수에 대한 낮은 만족도에도 불구하고 전체적인 만족도가 높은 것은 업무수행에 있어서의 만족과 호스피스.완화의료서비스에 대한 가치에서 많은 만족을 얻고 있는 것으로 보인다. 이는 향후 호스피스간호사의 보수에 대한 개선이 시급함을 보여주는 결과라고 하겠다.

암성통증에 대한 SBVP 효과 - 무작위대조시험, 이중맹검 (The Effect of Sweet Bee Venom Pharmacopuncture(SBVP) on Cancer-Related Pain : A Randomized Controlled Trial and Double Blinded - Pilot study)

  • Objectives : To investigate the therapeutic effects of SBVP in the treatment of patients with cancer-related pain. Design : A prospective randomized, double-blind, placebo-controlled study of SBVP. Setting : The study was conducted at the East West Cancer Center of Daejeon University Dunsan Oriental Hospital from March 1, 2007 to June 20, 2007. Patients : 11 patients diagnosed with cancer-related pain of over 3rd degree on the Numeric Rating Scale(NRS)(0, no pain at all, 10, worst pain imaginable) were entered into a double-blind, placebo-controlled trial of SBVP. They were randomized into Groups A and B(SBVP and control group, respectively) using the table of random sampling numbers and never informed of their affiliation by the coordinator. 5 of 6 patients in Group A and 4 of 5 patients in Group B completed the clinical trial. Intervention : SBVP(1ml/day) for group A and Normal Saline Placebo(1ml/day) for group B was injected into the abdomen acupoint, Zhong Wan(CV 12). The treatment was administered daily for five days. Outcome Measures : Degree of cancer-related pain was measured using the Numeric Rating Scale(NRS) before and after each treatment for "Pain right now" and "Average pain in last 24 hours". Statistical Analysis : Analysis regarding variations in NRS was carried out by applying t-tests(independent sample t-test and paired sample t-test) and Wilcoxon signed rank test with level of significance at 5%. Results : Differences in NRS of "Pain right now" for the two groups were statistically significant. The mean improvement point of SBVP was significantly higher than the control group($2.48^{\circ}{\pm}1.52$ vs $0.97^{\circ}{\pm}1.88$, p<0.05). Differences in average pain score before and after treatment in SBVP group were also significant($5.13^{\circ}{\pm}1.77$ vs $2.65^{\circ}{\pm}0.67$, p<0.05) compared with control group. The two groups showed no significant differences for long term effects in "Average pain in last 24 hours." Conclusion : Although further study will be needed on the large scale, SBVP shows potential as an effective treatment for immediate relief of cancer-related pain.

외상환자의 치료에서 외상 전문간호사 도입 결과 (Outcomes for Employment of a Trauma Clinical Nurse Specialist in the Treatment of Trauma Patients)

  • Purpose: Ongoing treatment and care, as well as initial stabilization, are required for trauma patients. With increasing number of sickest trauma patients and shortage of surgeons, the need for advanced practice nurse to provide and coordinate trauma care has been greater. The purpose of this study is to analyze the effect of hiring a trauma clinical nurse specialist and its influence on the treatment of trauma patients. Methods: Based on the employment of the clinical nurse specialist in December 2010, the patients were divided into two groups: patients admitted from January 1, 2010 to November 30, 2010 and patients admitted from December 1, 2010 to December 31, 2011. Retrospectively, data were collected using electronic medical records. The general characteristics, clinical courses, and ICU re-admission rates, collaboration (transfers to other departments and collaborative surgery) were compared. Results: To have a clinical nurse specialist on the trauma team resulted in a statistically significant reductions in the length of general ward hospital stay (p<0.05), the ICU re-admission rate, (p<0.03), the lead-time before transfer to other departments (p<0.05). Conclusion: The clinical nurse specialist, as a professional practitioner, improved the quality of treatment through early detection and management of problems. In addition, as a coordinator, the clinical nurse specialist maintained a cooperative relationship with multi-disciplinary medical personnel. The trauma clinical nurse specialist contributed to the treatment of trauma patients positively through a decrease in ICU re-admission rate and length of hospital stay.

Optimal Phase of Dynamic Computed Tomography for Reliable Size Measurement of Metastatic Neuroendocrine Tumors of the Liver: Comparison between Pre- and Post-Contrast Phases

  • Objective: The reliability of size measurements of liver metastases from neuroendocrine tumors (NETs) on contrast-enhanced computed tomography (CT) phases made by different readers may be hampered due to transient, variable rim enhancement in arterial phase (AP) or portal venous phase (PVP) images. We aimed to assess the reliability of tumor size measurements in pre- and post-contrast scans. Materials and Methods: The study coordinator selected target lesions according to Response Evaluation Criteria in Solid Tumors 1.1 guidelines in 44 consecutive patients with pathologically confirmed NET liver metastases. Two blinded readers measured the longest diameters of target lesions on pre-contrast, AP, and PVP images twice with a 4-week interval. Inter- and intra-observer agreements were evaluated using Bland-Altman plots and 95% limit of agreement (LOA) calculations. Results: Of the 79 target lesions (approximate mean size of 3 cm), 45 showed rim enhancement. Inter-observer agreement assessed based on LOA was highest in pre-contrast CT images (-6.1-5.7 mm), followed by PVP (-7.9-7.1 mm) and AP (-8.5-7.4 mm) images. Intra-observer agreement showed the same trend: -2.8-2.9 mm and -2.9-2.9 mm for readers 1 and 2, respectively, on pre-contrast CT, -2.8-2.9 mm and -3.0-3.2 mm, respectively, on PVP, and -3.2-4.2 mm and -3.4-3.2 mm, respectively, on AP images. Mean tumor diameters differed significantly among the phases in the following increasing order: pre-contrast CT, PVP, and AP images. Conclusion: There was better inter- and intra-observer agreement in size measurements of NET liver metastases on pre-contrast scans than on AP and PVP scans. Pre-contrast CT may be the optimal for measuring NET liver metastases if its accuracy is proven.

보건소내 물리치료실 운영실태와 물리치료사의 배치방안 (A Study on the Operation and Use of Physiotherapy room at Public Health Service Center)

  • The public health service center and its branch are, as one of public institution, responsible for providing non-profitable medical service to local residents, which can't be addressed by private medical institutes. In addition, as a planning coordinator, they need to manage local medical resources effectively to make sure local residents can be offered good-quality health service fairly and impartially. The purpose of this study was to highlight the importance of physical therapy in public health service center's successful role performance, one of treatment to which local residents had a direct access. For attaining the purpose, an investigation was made into the actual facilities of public health service center and its branches in North Cholla province, the extent of use of physicaltherapy room, and the general characteristics of physical therapists. And literatures concerned were referred to. As a result, the dominant opinion was that the current physicaltherapy room shoulder be more reinforced(38.9%) or that it should he kept as it was(46.5%). Besides, the physical therapists investigated still were serving in a poor working condition. To elevate the quality and quantity of medical service, the current way to employ expert personnels temporarily or not to guarantee the position of physiotherapists should be removed immediately. Instead, physicaltherapists should be employed as regular public official. As, in home-visit project, physiotherapist are expected to play a bigger role in rehabilitation care or the management of patients with chronic disease, the physicaltherapy room should be increased and more physicaltherapists should be provided to health service institutions.

웃음요법이 방사선요법을 받는 유방암 여성의 우울, 불안, 스트레스에 미치는 효과 (The Effect of Laughter Therapy on Depression, Anxiety, and Stress in Patients with Breast Cancer Undergoing Radiotherapy)

  • Purpose: The purpose of this study was to investigate the of laughter therapy on depression, anxiety, and stress among patients underwent radiotherapy. Methods: Participants in the study were comprised of 60 breast cancer outpatients who received radiotherapy. Thirty-one of the patients were assigned in the experimental group and the other 29 patients made up the control group. Laughter therapy was consisted of the delivery of information and active motion resulting in laughter. We provided laughter therapy 4 times for 2 weeks. Each session lasted 60 min. The experimental group received laughter therapy intervention as a group. An average attendance frequency per person was 2.7. To evaluate the effect of laughter therapy, the degrees of depression and anxiety were measured by Hospital Anxiety and Depression (HAD) and stress was measured by the Brief Encounter Psychosocial Instrument (BEPSI). Data analysis included a 2-test, t-test, and Fisher's exact test using SPSS 12.0 and SAS 8.0. Results: There was significant decrease in the degrees of depression, anxiety, and stress in the experimental group compared to those in the control group (p<.01, p=.04, and p<.01, respectively). Conclusion: This study provides evidence that laughter therapy is an effective intervention in improving depression, anxiety, and stress in breast cancer patients.
