간호사의 의료기관인증제 인식, 감염관리 인지도 및 수행도 (Nurses' Perception of Accreditation, Awareness and Performance of Infection Control in an Accredited Healthcare System)

  • Purpose: This study was conducted to provide basic data necessary to develop a program to improve infection control by examining nurses' perception of accreditation and by identifying its relationship with awareness and performance of infection control. Methods: This cross-sectional study was performed using questionnaires. Data were collected from 210 nurses who were working at one hospital between May 14 and May 19, 2015. Data were analyzed using SPAW. Results: The mean score for nurses' perception of accreditation was 3.10 points out of 5 points. Score for awareness of infection control was $4.63{\pm}0.39$ points and for performance of infection control, $4.39{\pm}0.39$ points. There were significant positive correlation among perception of accreditation, awareness of infection control, and performance of infection control. In the regression analysis, performance of infection control was influenced by awareness of infection control which accounted for 42.6% of the variance. It also showed additional improvement of 1.4% of the variance by when perception of accreditation was added. Conclusion: The result of this study show that nurses' perception of accreditation is relatively positive and that performance of infection control is highly enhanced according to nurses' positive recognition of accreditation as well as attaching importance to accreditation.

의료기관인증제 도입에 따른 환자안전과 의료의 질이 병원경영활동에 미치는 복합영향 (Complex impact of Patient Safety and Medical Quality on Hospital Management Activities due to Healthcare Accreditation Adoption)

  • 본 연구는 의료기관인증제도 도입에 따른 환자안전과 의료서비스의 질이 병원경영활동에 미치는 복합영향을 확인하고 조직문화의 매개효과를 파악하고자 하였다. 이를 위해, 2014년 6월말까지 의료기관인증을 획득한 대구 경북지역에 위치하는 300병상 미만과 300병상 이상, 6개 의료기관 종사자를 대상으로 2014년 9월 22일부터 4주간 설문조사 하였다. 해당 병원에서 1년 이상 근무한 377명을 연구대상으로 하였다. 연구결과, 300병상 미만 중소병원과 300병상 이상 병원 및 종합병원 모두, 환자안전과 의료의 질, 조직문화와 병원경영활동은 변수 간 정(+)의 상관관계가 있는 것으로 나타났다. 환자안전과 의료의 질, 병원경영활동 사이에 조직문화가 부분 매개효과가 있는 것으로 나타났으며 인과효과가 높았다. 따라서 의료기관인증제도 도입에 따른 환자안전과 의료의 질 향상 활동은 병원경영활동을 증진시키는데 효과가 있다 여겨진다.

정신의료기관 인증여부에 따른 임상영양사의 임상영양 관리업무 수행도와 중요도 분석 (Analysis of Clinical Nutrition Management Performance and Importance of Clinical Nutritionists According to the Accreditation of Mental Medical Institutions)

  • Purpose: The purpose of this study was to determine the factors that influence the performance and importance of clinical nutritionists. Methods: Ninety-nine nutritionists working in mental health institutions were assessed. Results: Factors affecting the performance of clinical nutritionists in certified hospitals were shown to have a significant positive effect on the description of treatment diet, status of clinical nutrition management, and presence of a nutrition counselor. The factors affecting importance were the number of permission beds and total work experience. Conclusions: In order to improve the accreditation system of mental hospitals, it is necessary to expand the scope of this study to include hospitals with fewer beds, and clinical nutrition management should be mandatory.

간호사의 국제의료기관평가에 대한 인식, 간호업무성과, 자아개념 및 재직의도 간의 관계 (Relationship of Nurses' Perception of the Joint Commission International Hospital Accreditation, Nursing Performance, Self-concept and Retention Intention)

  • Purpose: This study was done to identify the relationships of perception of JCI (Joint Commission International) hospital accreditation, nursing performance, self-concept and retention intention in nurses. Methods: Self-administered questionnaires were used to collect data from 199 JCI-accredited nurses in general hospitals located in Gyeonggi Province. Data collection was done in May 2015 and analyzed using t-test, ANOVA, $Scheff{\acute{e}}^{\prime}s$ test, and Pearson correlation coefficients. Results: The scores for perception, and nursing performance were (on a five-point Likert scale) 3.34, 3.78 respectively, and self-concept and retention intention were (on a eight-point Likert scale) 5.40 and 5.21 respectively. Nurses' perception, nursing performance, self-concept, and retention intention showed significant differences depending on age, marital status, education, department, work experience, position, Korean accreditation, and JCI accreditation. A positive correlation was observed between perception, nursing performance, self-concept, and retention intention. Conclusion: The results of this study indicate that enhancing positive perception and good nursing performance can produce good self-concept and retention intention.

지역사회 중·소규모 치과의료기관 인증 평가지표 개발 (Development of Evaluation Indicators for the Accreditation of Small-Sized Dental Hospitals within Local Communities)

  • Objectives : The aim of this research was to develop evaluation indicators that ensure safe services and enhance the quality of small-sized dental hospitals within local communities to prepare for the dental hospital accreditation system. Methods : Materials and data from a total of 460 respondents were analyzed after quality improvement indicators were formed by surveying experts and studying documents in Korea and abroad, and by conducting a survey from August 21, 2013 to September 30, 2013 for validation purposes. Results : Based on the results of factor analysis. all measured items appeared to be statistically significant with a factor loading score of at least 0.5, and the correlation between the factors all appeared to be high as well. The goodness-of-fit for the model also appeared to be within the recommended acceptance level. Thus, the proposed evaluation indicators for dental hospital accreditation developed in this study consisted of a total of seven major category factors and a total of 91 detailed items. Conclusions : Furthermore, the results suggest that the evaluation indicators can be used to improve the quality and to ensure the safe services of dental hospitals in local communities.

요양병원 의료기관인증제 시행 후 환자안전, 의료의 질, 조직문화 및 병원경영성과의 변화 (Change in patient safety, quality of medical care, organization culture and hospital management performance after healthcare accreditation system for convalescent hospital)

  • 이 연구는 요양병원 의료기관인증제 시행 후 요양병원의 환자안전, 의료의 질, 조직문화 및 경영성과에 미친 영향을 파악함에 목적이 있다. 2015년 9월 말까지 요양병원 의료기관인증을 획득한 대구지역의 36개 요양병원 중 10개 기관을 임의로 선정하였으며 2016년 2월 15일부터 2016년 3월 25일까지 요양병원 종사자들에게 설문조사를 하여 253명을 최종분석 대상자로 선정하였다. 설문조사 내용으로는 일반적 특성, 인증이후 환자안전과 의료의 질 향상, 조직문화, 경영성과의 변화가 포함되어 있다. 연구결과 환자안전과 의료의 질 향상의 변화정도에서는 진료지원체계가 3.93점으로 가장 높았으며, 조직문화의 변화정도에서는 인적자원문화가 3.78점으로 가장 높았고, 요양병원의 경영성과 부분에서는 내부프로세스 관점, 학습 및 성장관점이 3.71점으로 가장 높게 나타났다. 병원경영성과에 영향을 미치는 요인을 파악한 결과 직종이 행정직보다 간호직, 직급이 국장보다 사원이거나 과장, 환자안전과 질 향상에서 기본가치체계가 개선되었다고 생각하는 경우, 조직문화에서 인적자원문화, 개방체계문화, 위계서열문화, 생산중심문화가 개선되었다고 생각하는 경우 병원경영성과를 긍정적으로 변화시키는 영향 요인이라 응답하였다. 요양병원 의료기관인증제 시행 후 환자안전과 의료의 질 향상에는 높은 변화를 보여 긍정적 영향을 미쳤으나, 조직문화의 4가지 유형 중 위계서열문화와 생산중심문화는 보통수준의 변화를 보였으며 병원경영성과 중 재무관점은 다른 관점에 비해 상대적으로 낮은 변화를 보였다.

의료기관인증이 병원 근로자가 지각하는 환자안전과 질, 직무만족 및 조직몰입에 미치는 효과 -일개 종합병원을 대상으로- (The impact of healthcare accreditation on patient safety and quality, job satisfaction and organizational commitment: about a general hospital)

  • 본 연구는 의료기관인증이 병원 근로자가 지각하는 환자안전과 질, 직무만족 및 조직몰입에 미치는 효과를 평가하기 위한 단일집단 사전 사후 검사 설계연구이다. 대상자는 서울시내 일개 종합병원에서 1년 이상 근무한 405명을 대상으로 하였다. 시험처치는 2012년 4월부터 준비를 시작하여 2012년 11월 의료기관평가인증원으로부터 조사를 받은 의료기관인증이며, 자료 분석은 SPSS(version 18.0) 프로그램을 사용하여 paired t-test와 One-Way ANOVA로 검정하였다. 연구결과, 연구결과, 의료기관인증 후 병원근로자가 지각하는 환자안전과 질(t=7.28, p=.000), 직무만족(t=7.13, p=.000) 및 조직몰입(t=3.44, p=.001)은 통계적으로 유의하게 증가하였다. 따라서 의료기관인증은 환자안전과질, 직무만족 및 조직몰입을 증진시키는 데 효과가 있다고 볼 수 있다.

요양병원인증제 전·후의 요양병원 임상영양관리 현황 비교 (Clinical Nutrition Management Status in Convalescent Hospitals Before and After Healthcare Accreditation Process)

  • The increasing elderly population has created an urgent need for well-managed convalescent hospitals, which should provide appropriate clinical nutrition services. The new accreditation policy requiring participation of all convalescent hospitals since 2013 may promote improvement of clinical nutrition services. This study examined whether or not the accreditation policy has increased practice level and dietitians' perception of the importance of clinical nutrition management. Of the 177 convalescent hospitals accredited by January 30, 2014, dietitians from 73 hospitals (41.2%) completed the survey questionnaire. The pre-tested questionnaire surveyed general characteristics of the hospital and dietitians, current status of clinical nutrition management, and changes in the perception and practice levels of various aspects of food and clinical nutrition management. In average, dietitians with more than 5 years of work experience (68.1%) provided food and clinical nutrition services (71.2%). After accreditation, dietitians' perception of the importance and practice level of clinical nutrition service increased (P<0.001). Level of perception, however, was significantly (P<0.001) higher than practice level before and after accreditation. During perception and practice level of initial nutrition assessment, a compulsory accreditation item, notably and significantly (P<0.001) improved after accreditation. The significant difference between perception and practice level disappeared after accreditation. In conclusion, the accreditation process had positive effects on clinical nutrition management in terms of dietitians' perception and practice levels. Making more accreditation items compulsory and providing motivation and professional education to dietitians in convalescent hospitals could lead to additional improvements.

간호학과 인증평가에 따른 간호교육의 질 향상 및 질 확보 노력에 대한 교수 인식 (Faculty Perceptions of the Improvement and Effort for the Achievement of Quality in Nursing Education through Accreditation in Baccalaureate Nursing Programs)

  • This study investigated faculty perceptions of the improvement and effort for achievement of the quality of nursing education through accreditation in baccalaureate nursing programs in 2006. Survey data from 190 professors of 33 accredited nursing schools were statistically analyzed using SPSS ver. 19.0 software with descriptive statistics, t-tests, and ANOVA. Through accreditation, about two-thirds of subjects perceived that the quality of education had 'improved' and the purposes of the accreditation were moderately successfully achieved (score 3.50). They also perceived their schools made a strong attempt (score 4.24) to assure quality. The perception scores of efforts to assure the quality of education varied significantly according to the characteristics of the subjects, the existence of a university hospital, the type of college, and the year of school founding. There were significant differences between the level of improvement in education and the extent of effort to assure the quality of education. Accreditation contributed to improved quality of nursing education except for some issues which need more consideration. To maintain high quality, the accreditation of baccalaureate nursing programs should be performed with continuity and on a regular basis.
