Consumer Impairment and Its Improvement on Longterm Care Insurance Service : Focused on the Current Conditions and the Analysis of Counseling Cases in Korea and Japan (노인장기요양보험 서비스에 관한 소비자피해와 개선방안 : 한일 서비스이용현황 및 상담사례 분석을 중심으로)

  • This study looked into the current conditions in Korean and Japanese care management for the elderly, cases of consumer counseling in these countries, and the supporting institutions for Japanese users regarding the use of care management for the elderly. The number of recipients of care management for the elderly in both Korea and Japan is growing every year, and more Japanese users receive various services compared to Korean users. The results of an analysis on consumer counseling regarding the use of Korean and Japanese care management revealed, two types of complaints: counseling for the improvement of the institution and complaints related to the procurement of service. Regarding the insurance system, the complaints were mostly related to affirmation of a rating and the burden incurred by cost-sharing. Regarding the use of service, such key impairment cases were related to in-service medical accidents, illegal acts including caregiver contract violations and forced retirement, careless service by workers, and human rights violations of the elderly. Japan has developed governmental and, related-administrative and business services as well as a local governmental system to redress customer impairment issues as this pertains to service for better quality care management. In addition, they have enlarged the locally closed service, provided exact information about the services offered, and improved service appraisal techniques. The Japanese cases will serve as a good reference to improve the Korean system. In particular, the construction of a system that reduces customer losses and the availability of more counseling information are urgently required to improve the system in terms of customer experiences when they seek care management.

Current Status of Home-Based Cancer Patients Management in Jeju (제주지역에서의 호스피스.완화의료기관과 보건소의 재가암 관리 현황)

  • Purpose: As a part of analysis of home-based cancer patients management of public health centers, regional cancer center, and hospice institution in Jeju, this study was undertaken to establish their role. Methods: We investigated current status of hospice palliative care, especially home-based cancer patients management and summary demand of public health centers. Results: Services provided through the home-based cancer patients management project included physical, emotional, spiritual and education/informative services, even though there was little difference between them. The result showed that in the view of patients there was little relationship between public health centers, regional cancer center and hospice palliative institution. Conclusion: The relationship between home-based cancer patients management and institutions should be reinforced. Patients in acute state and difficulty care of patients should be referred to regional cancer center whereas maintenance state of patients should be referred to public health center.

  • As the world's aging population increases, the aging problem in China, which has a huge population, has reached a serious situation. Therefore, this study intends to present the CCRC model of smart home care. Seven application factors of CCRC of smart home care and problems of CCRC of smart home care in China were derived through theoretical review and case analysis. Based on this, this study presented a value proposition for the 7 application elements of smart home care CCRC tailored to China's aging status and future technology demand. First, it improves the environmental atmosphere and increases the comfort of the spatial environment. Second, we value the physical and environmental safety of the elderly. Third, online convenience services are provided. Fourth, we guarantee the health and psychological health of the elderly. Fifth, it increases the efficiency of responding to unexpected situations. Sixth, predict the physical condition and surrounding environment of the elderly. Seventh, add more recreational rides. Through this, we expect to improve the quality of life by providing a safe and comfortable smart service environment for the elderly.

  • The purpose of this study is to introduce ubiquitous home services for the aged. In comparison with general ubiquitous homes, ubiquitous homes for the aged have two major characteristics. First, the service contents should be expanded and strengthened. Second, the services should be customized to each individuals. The primary objective of a ubiquitous home for the aged is health care. The three affordances for this objective are 1)prevention of sickness and reducing the speed of aging process, 2)technical assistance for disability, 3)recovering and enhancing the physical and cognitive function. For this purpose this paper analyses the characteristics of the aged and current ubiquitous technology and ubiquitous home service, thus providing an outline for ubiquitous services necessary for aged people.

The Analysis of Expenditure for Private Education and Child-Care Service of Preschool and Elementary Children (아동기 자녀를 둔 가정의 사교육비 및 위탁양육비 분석)

  • The purpose of this study were to identify the income allocation structure of private education and child care and to analyze contributing factors income allocation structure of private education and child care for two children among Korean married couples. For these purposes total sample of 760 married couples those having two children of both pre-school aged and elementary school aged wee selected and total sample was divided into two groups by first child's school aged; those were pre-school aged(375) and elementary school aged (385) Statistics were frequencies means percentile and Tobit and OLS analysis. The results were as follows. First 268 households among the households those having the first child of pre-school aged spent 11% of income for private education while 29 households spent 7% of income for child care. 348 households among the households having the first child of elementary school aged spent 11% of income for private education. Second pre=-school aged' contri uting factors to income allocation struture of private education and child care were household family type family size home ownership and financial asset amount and elementary school-aged factors were satisfaction of family relationship and financial asset.

Expenditures on Market Substitutes for Housework: Dual-Income and Single-Income Households (맞벌이가계와 비맞벌이가계의 가사노동 시장대체지출비 분석)

  • The purpose of this study is to investigate the effects of wife's employment status on the household expenditures of timesaving market substitutes for houseworks. Seven expenditure categories were considered such as food away from home, convenient/prepared food, housing care service, clothing care service, childcare, supplemental education, and domestic services. The data were taken from 1999 Family Expenditure Survey by National Statistical Office. The sample consisted of 29,963 households with 33.2% dual-income households. The average monthly expenditure for food away from home was 127,795 won for dual-income households, while 103.100 won for single-income households. The expenditure for childcare of dual-income households was over six times of single-income households'. Dual-income households spent over ten times of single-income households for domestic services. For most expenditure categories, households with wife working at white-color jobs spent more than other dual-income households. After being other household characteristics to be constant, wife's occupation had found to be related with the household expenditures for most market substitutes. For the expenditures on both food away from home and childcare, employed-wife households with any kind of jobs were found to have higher possibility to spend and to be spent more than non-employed-wife households. The households with wife employed at white-color jobs spent more on clothing care service and domestic services than the households with the not working. Employed-wife households had higher possibility to spend on supplemental education, but they did not spend more on the expenditure, compared to nonemployed-wife households.

  • Mankind has been attempting to live a happy and safe family life in a residential space. Due to the advent of the mobile phone in the 1990s and the smart phone in the 2000s, when the information and communication age came, human life has been innovatively changed. The revolution of human civilization led to the Neolithic Revolution and the Iron Age, followed by a smart phone revolutionizing human life, and the revolution faces with the era of info-communication, smart phones became a daily life and the fourth industrial revolution. The fourth industrial revolution is an era of info-communication technology (ICT), creating a new paradigm across human life through technological developments such as artificial intelligence (AI), IoT, big data, mobile, and cloud. The smart home is actively researched in a direction to support the overall human life as a representative future residential culture paradigm. However, the study considering the needs according to the lifestyle, functional characteristics of each living space and human lifestyle of the Phono Sapiens era where smart phones live like daily life was relatively insufficient. In addition, research on smart home service design should be considered from the apartment residential space planning stage. Therefore, this study has significance in suggesting the direction of research on human-centered smart home service design considering the characteristics of each living space and resident's life-style in the smart phone era.

Effectiveness of Home Health Care Service for Elders after Spinal Surgery

  • Purpose: This study was done to evaluate effectiveness of home healthcare services (HHCS) specialized for elders who received spinal surgeries. Methods: A non-equivalent control group pre-post test quasi-experimental study was performed. HHCS was developed based on the Rice model of dynamic self-determination for self-care. For data collection, a control group (n=23) and an experimental group (n=23) were selected by matching age, BMI, pain, general characteristics and type of spine surgery. Measurement tools to evaluate uncertainty and knowledge were developed by the authors. The Numeric Rating Scale (NRS) and Japanese Orthopedic Association Back Pain Evaluation Questionnaire (JOABPEQ) were used to evaluate pain levels. Muscular strength in the legs was measured using a digital muscle tester and tape ruler. Questionnaires were used to evaluate disability in performing ADL and psychological distress levels. Results: The experimental group showed significant decrease in uncertainty (p=.028), increased knowledge (p=.038), and partially decreased pain (p=.003-.331). Partial muscle strength increased significantly (p=.021-.644). Disability in performing ADL and psychological distress in the experimental group decreased significantly compared to control group (p=.002, p=.004). Conclusion: Results indicate HHCS is an efficient home care nursing program for these elders. Further experimental studies with larger samples are required to confirm effects of HHCS.

A Study on the Market Substitutes for Housework in the United States (미국내 가사노동의 시장대체실태와 관련변수와 관한연구)

  • The purposes of this study are as follows: 1) To estimate the amounts of differences in service expenditures resulting from the effects of mother's marital and employment status. 2) To find out the relationship of socio-economic variables to expenditures for time-saving durables and services Data were taken from the 1988-1989 Consumer Expenditure Survey. The sample consisted of 2,216 families with 334 single-mother families and 1,792 married-mother families. Tobit regres-sion analyses were used to test of variables related to expenditures for (a) food away from home. (b) clothing care (c) child care (d) domestic services and (e) total services. The results of this study were as follows; 1) The percentage difference from non-employed married-mother families was higher for employed single-mother families than for other types of families indicating the positive effect on expenditures on market substitutes of mother's marital and employment status. 2) The father's wage rate was associated only with expenditures for domestic services. A positive relationship was found between family nonlabor income and domestic services Total family income was positively associated with expenditues for all dependent variables. There was a negative relationship between expenditures for child care and age of mother squared. Mother's education was associated with expenditures in all categories. Families of nonwhite spent less on time-saving durables. food away from home, and total services and spent more on apparel services than families of white mothers. The presence of young child was positively related to total services and child care services and negatively related to food away from home.

A Study to Assess the Need of Customized Visiting Health Care Services for Children according to Socioeconomic Status in a Province (사회계층에 따른 영유아 맞춤형 방문건강관리사업 요구도)

  • Purpose: This study was conducted to survey children's health status and need of customized visiting health care services in one province. Methods: The participants in this study were 237 caregivers of infants and preschoolers. Data were collected at the participant's home or public health center. Results: Many of the children did not receive developmental screening tests or dental examinations. In the beneficiary group, the prenatal checkup rate and children's vaccination rate were lower, and caregivers had more health problems than the other groups. On the assessment of home safety, unsafe conditions were more frequently found in the beneficiary group. The caregivers in the beneficiary group showed lower child rearing confidence than the other groups, and wanted customized visiting health care services most in the areas of developmental screening, regular health check-up and counseling, nutritional supplementation, and oral health care. Conclusion: These results indicate that it is necessary for children and parents in poverty to be provided with professional home visiting interventions for the promotion of child health and prevention of developmental problems and diseases. These findings can be used for developing future customized visiting health care service programs for infants and children in this community area.
