Home Nursing and Quality of Life in Parents of Children with Atopic Dermatitis (아토피피부염 환아 부모의 가정관리와 삶의 질)

  • Purpose: This study was done to assess the home nursing and quality of life of parents caring for preschool children with atopic dermatitis and to identify the factors which influence quality of life. Methods: Participants in the study were 149 parents of preschool children with atopic dermatitis attending a kindergarten and a nursery school in S city. Parent completed a set of questionnaires (assessing demographic factors, home nursing, quality of life). Descriptive analysis, t-test, ANOVA and multiple regression were used to analyze the data. Results: The mean score for home nursing was $3.06{\pm}0.38$ and for quality of life, $4.18{\pm}0.74$. The lowest scored question in the 13 question on quality of life was that of frustration related to itching sensation. Significant factors that affect quality of life were severity and home care. These variables explained 21% of variance in quality of life (F=21.17, p<.001). Conclusion: The results indicate that severity of childhood atopic dermatitis and home nursing impact on the quality of life of parents. Using the above findings, health professionals who work with children with atopic dermatitis and their parents need to develop specific nursing interventions that will promote health of children with atopic dermatitis and quality of life in their parents.

Importance of Education Contents and Competencies on Home Care Nursing of the Hospital and Public Health Nurses in Korea (임상간호사와 보건간호사가 지각한 가정간호 교육내용의 중요도 및 업무수행능력)

  • This study was conducted during the time period of September 1997 to February 1999. in order to provide data concerning important facets of horne care nursing and the degree to which practicing hospital and public health nurses believe themselves to be competent in each area. The study subjects consisted of 610 hospital nurses, 158 public health nurses and 21 other nurses from Seoul and the province of Kyunggi. Korea. Data was collected through self-reporting questionnaires which was used by Kim et al.(1999) to evaluate the status of home health nursing and the varying ideas of self-competence that practicing nurses have. The results of the study were as follows: 1) The mean scores of perceived important components and competencies on home health nursing measured on the Likert 4 point scale were $3.15{\pm}0.36$ for importance. and $2.56{\pm}0.36$ for competency. Of the four categories regarding nursing services, the 'Nursing skill' factor had the highest importance and competency. 2) There were significantly higher scores for hospital nurses' importance components and competencies of home care nursing as compared to those scores for public health nurses. 3) The significant factors of the importance component of home care nursing listed in order of priority were 'general infection control', 'bed sore care skill', 'rehabilitation care', 'keeping maintenance of the client's confidentiality', 'malpractice', 'diabetes patient care', 'ability for problem solving' respectively. In contrast, of low priority in the importance components of home care nursing were 'referring nursing as a job to others'. 'record keeping', 'family dynamics', 'medical equipment', 'economic problems', 'environmental assessment', 'suture removal', 'multidisciplinary coordination' respectively. 4) The high priority of the degree to which practicing nurses believe themselves to be competent in home health nursing were the 'blood sugar test', 'enema skill', 'injection skill', 'skin care', 'bed sore care skill', etc. In contrast, lower competence for home care nursing as perceived by nurses were 'AIDS care', 'family violence care', 'substance abuse care', 'mental health care' respectively.

Correlates of Social Support, Family Function, and Conditions of Home Care Nursing in Family Caregivers (가정간호대상자 가족의 사회적 지지와 가족기능, 가정간호 특성 간의 관계)

  • Purpose: The purpose of this study was to determine the factors related to the social support, family and friend support as well as home care nurse support, in family caregivers of patients with home care service. Method: The participants were 111 family caregivers of patient, who were receiving home care services from home health care centers of 3 different general hospitals located in Seoul. The data was collected, using self-administered questionnaires. Result: The level of family and friend support varied significantly according to patients' mental status, period of home care nursing, frequency of home care service, caregiver's education level, family income, and family functioning level. On the other hand, home care nurse support varied significantly according to patients' mental status, caregiver's age, existence of interchangeable family caregivers, and family functioning level. There was a significantly positive correlation between the social support the family caregivers' perceived and family function while there was a negative correlation between family and friend support and the period of home care service. Conclusion: Thus, the establishment of nursing interventional program, with understanding of their social support, is needed for both patients and their caregivers.

Factors Affecting Job Satisfaction of In-Home Child Care Helpers: Focusing on Child-Care Efficacy, Job Efficacy, and Health Promoting Lifestyle (아이돌보미의 직무만족에 영향을 미치는 요인: 보육효능감, 직무효능감, 건강증진 생활양식을 중심으로)

  • Purpose: The purpose of this study was to investigate the influence of child-care efficacy, job efficacy, and health promoting lifestyle on the job satisfaction of in-home child care helpers who are called Idolbomi. Methods: Participants for this survey were 153 in-home child care helpers who worked in Seoul. Data were collected from May 23 to October 17, 2014 using self-report structured questionnaires. Data were analyzed using IBM/SPSS 18.0 program. Results: There were significantly positive correlations between child-care efficacy, job efficacy, health promoting lifestyle, and job satisfaction. Stepwise multiple regression analysis revealed that the predictors of job satisfaction were spiritual growth, education level, job efficacy, effect of income. Conclusion: Therefore, we need to develop strategies to enhance the job efficacy and health promoting lifestyle of in-home child care helpers to improve their job satisfaction.

Developing a Visiting Health Care Program at the Public Health Center in Korea (한국의 보건소 방문건강관리시스템 발전 과제)

  • This paper provides the guidelines from which to develop a visiting health care program at the Public Health Center in Korea and involves an expanded payment compensation system of preventive services based on the new long-term health insurance system in Japan. The function and management methods to achieve the goals practiced in a community contact center for elderly support which have recently been established will guide the specific directions and strategies that the Public Health Center should pursue. That is to say, comprehensive and continuous efforts will be put forth in preventive home visiting care targeting the elderly in certain jurisdictions. At this point in time in which the visiting care nursing program has not yet started, visiting health care provided by the Public Health Center oversees chronic diseases of a vulnerable population. But after it has been developed nationwide, the visiting health care system at the Public Health Center will be distinctive and focused on health promotion and prevention.

Development of the Home-Based Prenatal Care System via Information Superhighway (초고속 통신망을 이용한 재택산전간호관리 시스템 개발)

  • Due to the rapid socioeconomic development and the introduction of the national health insurance system the general population's need for health care and utilization of health care services have increased dramatically. As a result of this change. Korea is experiencing a shortage of health care facilities and health manpower, and this leads long wailing line at doctor's offices. One of the solutions of this problem could be home health care system for those who have minor health related problems. With this background, this study was conducted to look at the feasibility of a home-based prenatal care system using information superhighway and nursing informatics specialists. With the home-based prenatal care system, the pregnant woman checks her blood pressure, tests her urine for sugar and protein, and measures her body weight at home and sends the information to a hospital computer via the information networks such as public telephone line and information superhighway. Nursing informatics specialist at the hospital will go through each patient record and screen those who have abnormal values and notify them to see a doctor as soon as possible. Besides telemonitoring features, the proposed system will include tole-education capabilities for the patients so that patient can learn whatever they need to know ragarding the prenatal care via information networks. If this system develops and operates, patient can save time in terms of travel to and from the hospital and waiting time in the hospital. And the health care institute can utilize its resource more efficiently.

Customized Home Visiting Nurses' Experiences in Telephone Visiting (맞춤형방문간호사의 전화 방문 경험)

  • Purpose: The purpose of this study was to explore and understand customized home visiting nurses' experiences in telephone visiting. Methods: Qualitative data were collected from several discussions of 4 focus groups consisting of 20 customized home visiting nurses. All interviews were recorded and transcribed according to thematic content analysis processes. Results: The five main themes on customized home visiting nurses' experiences of telephone visiting were 'greeting', 'counseling', 'using strategies', 'maintaining attitude for enhancing relations' and 'enduring difficulties'. The category of 'counseling' was composed of confirming, educating, supporting, adjusting, and indicating. And, the category of 'using strategies' included complimenting, scolding, persuading, compensating, and ignoring. Conclusion: Developing a standardized manual on telephone visiting is needed to guide an effective counseling. In order for a customized home visiting nurse to be a good telephone counselor, training of motivational interviewing is recommended.

Quality of Life of Older Adult with Home Health Services - Applied PRECEDE Model - (취약계층 노인의 삶의 질에 영향을 미치는 요인 - PRECEDE 모형 적용 -)

  • Purpose: The purpose of this study was to determine factors that affect poor quality of life (QOL) of older adults who received home health service. Method: The sample 492 older adults participated in the study. The QOL was measured using the scale of QOL of Ware and other data were collected through face-to-face interviews from September to August, 2009. Results: The level of QOL was moderate (Mean 24.4, SD 7.4). The QOL was poorer in older adults (p<.05) and in those living alone (p<.01) compared to older adults and those living with couple or family respectively. The QOL was positively correlated with a sense of mastery (r=.213, p<.05), connection of health (r=.160, p<.05) and a cognitive function (r=.119, p<.01), and negatively correlated with Activities of Daily Living (r=-.266, p<.01) and Instrumental Activities of Daily Living (r=-.339, p<.01). Sense of mastery, connection of health & welfare, and IADL were significant predictors of QOL. Conclusion: Finding suggest that home health service program should incorporate strategies for increasing sense of mastery and capability of performing IADL, and strengthening connection of health service that may improve QOL of older home health service recipients.

A Study for the Evaluation of Students on a Practical Performance of the Home Health Care (일 가정간호 교육기관 교육생의 임상수행능력 평가에 관한 연구)

  • This study was conducted to investigate the evaluation of students on a practical performance of the home health care and to provide data for practical education on the home care nurse institution. Research was performed from March to December. 2002. 40 Respondents participated in the evaluation four times. The data were analysed by using the SPSS/PC + version 10. 0. The results of this study were as followings: 1. The average age of the subjects was 32. Majority of the subjects were interested in home health care. Majority of the subjects were staff nurse(57.5%) and workers at general hospitals(50%). 2. According to the result of the comparison of practical performances. performance evaluations of 20 items were higher when they admitted special practice than when they admitted any other practice. There was an positive increase in the evaluation of health assessment. nursing documentation. explanation. coping with emergency status. coordination and self assertion. In addition to health assessment. CPR. oxygenation. education & consultation. developmental assessment of child. nursing documentation. mastering nursing practice and consideration of client were significantly different. 3. The subjects were useful for practical circumference where they are working at. From these research findings. the following suggestions have been drawn: it is needed to manage practical curriculum variously and to include not only clinical aspect but also organic or administrational aspect and to reflect on students' needs and to choose to be reasonable items and to develop instruments for evaluation capability of home care nurse.

Effects of Home Visiting Care Program for Patients with Diabetes Mellitus Provided by Public Health Center (보건소에서 실시한 당뇨병 환자 방문간호 프로그램의 효과)

  • Purpose: The aim of this study was to identify the effects of home visiting care program provided by public health center in control of blood sugar for patients with diabetes mellitus who were cared for at home. Method: The subjects were 50 randomly selected diabetics registered at S-Gu Public Health Center. The program comprised in-person education and home visitations; and telephone contact to educate, monitor diet, exercise, blood sugar, and provide consultation. The control group was provided home visitation by only home calls nurses. The program ran from July 7. 2008 to September 12. 2008. Result: After the program, glycated hemoglobin was decreased in the experimental and control groups; the difference in those receiving home care was not statistically significant. Changes on blood cholesterol in experimental group and control groups were not statistically different. Self-efficacy and self-care performance were increased in those receiving home care. Conclusion: While not statistically significant in this small-scale study, home care for diabetes mellitus patients may promote an increased patient responsibility for self-care that is important in their long-term health.
