간호자 구강간호 교육프로그램이 요양시설 노인의 구취와 영양소 섭취량에 미치는 효과 (The Effects of Oral Health Care Education for Nursing Staff on Halitosis and Nutrient Intake of the Elderly in a Long-term Care Facility)

  • The purpose of this study was to investigate the effects of an oral health care education program for care providers on the nutritional status of the elderly in a long-term care facility. This study was conducted at a long-term care facility located in K metropolitan city using a nonequivalent control group non-synchronized design. Fifty-four nursing home residents aged 65 years or older were assigned either to the intervention group (n=27) or the control group (n=24). The intervention group received oral health care from the care providers' intervention group, who provided oral health care for 6 weeks after 6 weeks of oral health care education. Data were collected from the control group and intervention group at the baseline 6 and 12 weeks after oral care education and were analyzed using SPSS windows 16.0. The halitosis was lower in the intervention group than the control group at 12 weeks (P<0.01). Body mass index of the intervention group at 12 weeks was higher than that at 6 weeks. Iron intake of the intervention group at 12 weeks was lower than that at baseline. At baseline, the intakes of riboflavin, folate, and potassium were lower than 75% of dietary reference intakes. In conclusion, an oral care education program for care providers was effective in improving the oral hygiene of nursing home residents, and dietary plans are needed to improve the nutritional status of them.

노인 장기요양보험 이용실태 및 서비스내용에 대한 인식 (Awareness and using status on long-term care insurance and insurance benefits)

  • Objectives: The purpose of the study is to investigate the awareness toward use and service contents of long term care for the elderly. Methods: A self-reported questionnaire was completed by 296 adults from August 13 to October 20, 2014. The questionnaire consisted of general characteristics of the subjects, awareness toward long term care insurance for the elderly, awareness toward long term oral health care services, use of long term care service and use intention for the long term care insurance. Data were analyzed by SPSS 18.0 program. Results: Those who were aware of the long term care insurance accounted for 55.4 percent. Approximately 50 percent of the respondents recognized long term care service items, home visit care, home visit bathing, and home visit nursing. Most of the respondents had information of long term care services by way of mass media and direct contact. Only 13.4 percent of the respondents were aware of the oral health service in the long term care insurance. The subjects were aware of denture cleaning, oral cleaning and oral health education out of oral health service in order; and oral health services that needed to be offered were denture cleaning, oral health education and professional toothbrushing. They reported that dental hygienists were the most important manpower that offered the efficient oral health care services. They answered that professional manpower and financial support are required for oral health services. The positive thinking to long term care insurance accounted for 89.2 percent and 91.3 percent had use intention for oral health services. Conclusions: Many elderly people have mastication or dysphagic problems due to systemic diseases. Therefore, it is necessary to announce the long term care insurance and long term care services for the elderly people.

  • Objectives: This case - study was conducted to assess the oral health status change of the elderly through visiting oral care interventions based on a community health care project. Methods: Professional dental hygiene treatment and oral health education, including brushing using interdental toothbrushes and sponge brushes, were performed on three senior citizens who received home visiting oral health care benefit. Results: The subject's periodontal conditions improved including gingival inflammation and bleeding. The gingival color tured pink by controlling the dental plaque. Conclusions: Visiting oral health care contributed to the improvement of oral health of the elderly. Therefore, based on the characteristics of the elderly with various systemic diseases, it is necessary to discover various cases that can perform professional and customized visiting oral health care programs.

  • In the welfare law for the disabled in Korea, the disabled is defined as "the persons sho have significant limitations of daily or social life due to physical or mental disabilities over a long period." The number of registered domestic disabled person in 2008 survey was approximately 2.1 million. 90% of the disabled was caused by the aposteriori illness or accidents. So, in this aging society, the number of people with disabilities will increase even more. In general, people with physical and mental disabilities tend to have more oral diseases, but, are difficult to get treated. Therefore, a thorough oral care through individualized prevention is more important than ever. In this article, we review the oral care methods in dental office and home and in the hospital for the elderly and finally focus on a denture care method. According to this article, the dental professionals can be able to select and apply the oral care method in agreement with the characteristics of the disabilities. Particularly, it is important to prevent the aspiration pneumonia in the elderly in hospital.

취약계층 노인의 구강건강상태에 영향을 미치는 요인 (Factors Affecting the Oral Health Status in Vulnerable Elders)

  • Propose: This study was to identify the factors affecting the oral health status in vulnerable elders receiving home visiting health care service. Methods: A total of 444 elderly people over the age of 65 at 33 public health centers in Daegu and Gyungbuk areas were included in this survey. The data were collected by personal interview with nurses using oral health knowledge, behavior, and status questionnaires from the $10^{th}$ to $22^{nd}$ of July, 2012. The data were analyzed with t-test, one-way ANOVA, $Scheff{\acute{e}}$ test, and stepwise multiple regression, using SPSS program. Results: Factors affecting the oral health status were chewing satisfaction with denture 14.5% (${\beta}=.391$, p<.001), denture care (wash with water) 3.2% (${\beta}=-.187$, p<.01), and types of health insurance (medical aid type1, 2) 1% (${\beta}=-.111$, p<.05). The factors explained 18.7% of variance in the oral health status of vulnerable elderly subjects. Conclusion: The results of this study indicate that in order to improve the oral health status of vulnerable elders, it is necessary to provide oral health education that includes the proper usage of denture to elders who receive home-visiting care.

일 지역 의료인의 가정간호사업 및 서비스 제공 가능성에 대한 인식 조사 (Recognition of Home Care Services for Nurses and Physicians)

  • The purpose of this study is to determine the level of recognition of home care services and to provide basic data for implementation of home care services. Data collection was carried out between December 2004 and January 2005 by surveying 88 nurses and 40 physicians working at a general hospital and 28 physicians working at different clinics in the Seoul metropolitan city area. The results of this study were as follows: 1. Home care services were recognized by $94.2\%$ of nurses, $77.5\%$ of physicians and $92.9\%$ of clinic physicians. The main sources of information for most of them were medical and nursing journals. 2. The percentages of staff regarding find home care services as necessary for the institutions were $88.6\%$ of nurses, $74.4\%$ of physicians and $57.1\%$ of clinic physicians. All of them anticipated that home care services would maintain 'continuous care' and 'long-term patient care'. 3. The percentages of staff willing to refer their patients to home care were $95.5\%$ of nurses, $100\%$ of physicians and $87.1\%$of clinic physicians. However, only $7.1\%$ of clinic physicians were willing to refer actively. 4. Most nurses and physicians replied that a majority of test-related services is suitable for home care services. However, among medication-related services, intravenous injections were not suitable for home care services. Among treatment-related services, most nurses and physicians replied that Levin tube feeding, oral and nasal suction, simple dressing, perineal care, and enema were suitable for home care services, but incision and drainage, and tracheostomy tube change were not suitable for home care services. In conclusion, for the implementation of hospital-based home care services, it is necessary to educate nurses and physicians on the present condition and precedent at other hospitals.

노인장기요양보험 재가급여 수급자의 구강위생서비스 요구도와 비용지불의사 (Demand and willing to pay for oral hygiene service in long-term care insurance of elderly)

  • Objectives: This study was conducted to identify the demand and willingness to pay for oral hygiene services among elderly people with long-term care insurance. Methods: Our study was a cross-sectional analysis. Subjects comprised 126 elderly individuals from long-term home-care centers. A total of 28 centers were selected through convenience sampling from among 78 centers in ${\bigcirc}{\bigcirc}$. For analysis, semi-structured questionnaires that required about 20-30 minutes to complete were used. Analysis was performed using SPSS 23.0 software. Results: The overall demand for oral hygiene services was 44.4%, and willingness to pay was 31.0%. Thirty-three people (58.9%) of elderly those who have demand for an oral hygiene service were willing to pay for the service, and 64 people (91.4%) who did not have a demand were not willing to pay for it. Among those with partial dependence on brushing, 65.6% had demand for oral hygiene services and 50.0% were willing to pay costs. Among basic livelihood beneficiaries, 69.6% were willing to pay for oral hygiene services; general subjects and relievers were less willing to pay. Conclusions: The overall demand for oral hygiene services among elderly people was 44.4%, and the willingness to pay was as low as 31.0%.

Responsibilities and Difficulties of Caregivers of Cancer Patients in Home Care

  • Background: Having been known as a virulent disease in 1970s, cancer is now onsidered a chronic disease and 64% of cancer patients live for five years after diagnosis. Home care has gradually gained more importance and it is a great burden on the shoulders of caregivers. Caregivers have to undertake the responsibility of the cancer patient's home management, and organize care and arrange health care services according to the ever-changing condition of patients. Caregivers should be prepared for home care so they can provide accurate and complete care to patients. This descriptive study aims to investigate challenges that caregivers encounter in the home care of patients and the reasons for these challenges. Materials and Methods: The research group consisted of caregivers of outpatients in a daily treatment center in a university hospital. The research sampling consisted of 137 voluntary caregivers of patients who attended the Daily Treatment Center for control, chemotherapy or other supportive cares services between January-June, 2011. Data were collected with face-to-face interviews in the Daily Treatment Center. Ethics Committee approval was taken university hospital; caregivers and their patients were informed about the research and their approval was taken as well. Results: It was found that 54.01% of caregivers help patient's nutrition, 50.36% help medicine use, 26.28% help oral hygiene, 26.28% help to meet urinary needs and 51.82% help to change clothes, 69.34% of caregivers help to change bed sheets, 38.69% help the patient to communicate with their environment and 71.53% help to bring the patient to hospital or outside. Conclusions: This study, it was found that caregivers experience challenges due to following factors: patient nutrition, medicine use, oral and body hygiene, colostomy maintenance and stomach tube feeding, concern of dropping the patient, feeling incompetency in body temperature and fever control, fatigue, and lack of personal time.

노인요양시설 내 의료서비스 발생빈도와 병원중심 가정간호 요구도 조사 (Incidence of Medical Services and Needs for Hospital-based Home Care Nursing in Elder Care Institutions)

  • Purpose: This study aimed to clarify the needs for hospital-based home care nursing medical services in elder care institutions by analyzing the details and frequency of medical services provided by, and the needs for, hospital based home care nursing in select institutions in Korea. Methods: Seventy-seven staffs at elderly care institutions located throughout the country completed self-report questionnaires between February 1 and May 31, 2009. SPSS ver. 14.0 was used for data analysis regarding frequency and percentage, mean and standard deviation. Results: Forty-eight hospital-based home care nursing medical services in eight domains were identified as being needed in elderly care institutions. The most commonly used medical services were providing instruction in oral drug administration, checking drug beneficial/adverse effects, and administering blood glucose test, while the most needed medical services requiring hospital based home care nursing were complex pressure ulcer care, followed by diabetic foot ulcer management and nutrient injection. Conclusion: The present results should provide fundamental data for better healthcare services with hospital based home care nursing at elderly care institutions as part of a 'win-win' strategy through which medical expenses are reduced, insurance costs are kept stable, and safe and high-quality medical services are provided for residents of elder care institutions. Political decisions intended to promote visits by hospital based home care nurses to elder care institutions would be a prudent course.

Seoul, Gyeonggi, Gyeongsang, Jeolla, Chungcheong located in 228 people in the ICU nurses using a questionnaire on oral health awareness and management behavior were investigated. Using oral hygiene aids to living in a residential home, according to the respondents from the two was significant association (p<0.05), oral hygiene, tooth brushing behavior and the use of aids in the relevance of the educational experience, depending on how brushing brushing, depending on how experienced oral there was significant association with the use products, dental products, depending on the path of tooth brushing training methods used were relevant and significant, depending on the state of your oral health, dental supplies used was associated with a significant (p<0.05).
