일 종합병원 의사, 간호사 및 입원환자의 가정간호사업에 대한 인식 조사 (A Study on the Recognition of Home Care Services by nurses, doctors and patients.)

  • Purpose: To provide the data for improving home health care through investigating the recognition of home care services and the possibility of providing it by nurses, doctors and patients. Method: The subjects were 167 nurses and 71 doctors who were working at a general hospital in Seoul and 72 patients who were admitted to that hospital. Data were collected through questionnaire surveys from November 29 to December 17 in 2004. Result: Home care services were recognized by 70.7% of nurses, 64.8% of doctors and 58.4% of patients. The percentages of agreement regarding the necessity of home care services were 70.7% of nurses, 47.9% of doctors and 86.1% of patients. The percentages of agreement to refer the patients to home care when they only want to receive it, were 58.1% of nurses and 57.7% of doctors. There were significantly higher in nurses than doctors between the differences of possible or impossible percentages in agreements about each items in the test-related services, medication-related services and treatment-related services. Conclusion: The above findings indicated that more specific and continuous educations and advertisements are needed to enlarge home care services in general hospitals.

국외 커뮤니티 케어의 방문간호서비스 사례 분석 (Community Care and Visiting Nursing Services in Denmark, Sweden, the United States, and Japan)

  • Purpose: This study compared visiting nursing services of Denmark, Sweden, Japan, and the United States to provide baseline data for the development of models for community care nursing services in South Korea. Methods: A review of the literature was performed that include journal articles, government reports, institutional reports, and national/international statistics. Site visits were performed to explore the visiting nursing services of Denmark. Results: Government centered visiting nursing services were provided in Denmark and Sweden mostly by public organizations, while private services prevailed in the United States and Japan. Nursing services included case management or care coordination services, while nurse practitioners or nurse specialists provided visiting nursing services in all of the four countries and the services were provided 24 hour a day. Conclusions: Based on the review of visiting nursing services in foreign countries, the development of models is needed to provide integrated visiting nursing services in Korea that encompass home care nursing, visiting nursing, and visiting health care services.

의료기관 가정간호의 현황(2007-2012): 가정간호 급여청구자료 분석 (The Current State of Hospital-based Home Care Services in Korea: Analysis of Data on Insurance Claims for Home Care from 2007 to 2012)

  • Purpose: The objectives of this study were to analyze the state of hospital-based home care (HC) services annually and to provide basic information for research and policy regarding home care. Methods: This study is a secondary analysis of the yearly state of HC services from the Health Insurance Review & Assessment Services from 2007 to 2012. Results: The decreased by 34.6%, from 214 agencies in 2007 to 140 in 2012. The annual average number of active home care nurses was 440, which included 6.7% of the licensed home care nurses until 2012. The annual average number of HC patients were 32,000, and this number decreased by 21% in 2012, compared to that of 2008. Of the HC patients, about 70% were over 60 years of age. The chronic diseases among HC patients have been decreasing steadily since 2007. Seventy to eighty percent of the home visits were made in general hospitals or higher level hospitals. The total medical cost for HC services was 21 billion won in 2007, which consisted of 0.06% of the national medical costs, and it was 22 billion won and 0.03% in 2012. Conclusion: Based on the results of this study, further research on HC services is necessary to frame policies for the expansion of HC agencies.

노인요양시설 건강지원서비스 현황과 장애요인: 포커스 집단 면접 (Current Status and Barriers to Health Care Services for Nursing Home Residents: Perspectives of Staffs in Korean Nursing Homes)

  • Purpose: Focus group interviews were conducted to explore the current status and barriers of health care services in nursing home from the viewpoint of staffs taking care of nursing home residents. Methods: A qualitative thematic analysis using the focus group interview method was used. A total of 32 health care professionals (19 nurses and 13 social workers) from thirteen nursing homes in South Korea attended (5 focus groups) in 2014. Results: The two main themes were identified: 'minimal health care services that left personal care needs unmet' which has three subthemes of 'stereotyped and fragmented care by types of care providers', 'medically-oriented health care services' and 'health care services mixed with social or recreational programs'; and 'barriers to proper and timely care in nursing homes' with four subthemes including 'unmet care needs due to cognitive dysfunction or lack of expression', 'care guides or tools not suitable for long-term care facilities', 'health care needs that are beyond the facility's care boundary', and 'care delay due to lack of understanding on the older adult's status'. Conclusion: The findings from this study should help health care policy makers to recognize the factors that influence health care services and provide direction for nurses and other staffs involved in supporting health care services for nursing home residents.

우리나라 방문건강관리사업의 과거, 현재와 미래 (Past, Present, and Future of Home Visiting Healthcare Services based on Public Health Centers in Korea)

  • Purpose: We present improvements to the Korean home visiting healthcare service based on analysis of Korean home visiting healthcare services considering recent sociodemographic changes and demands for healthcare services. Methods: This is a review study in which the results are derived through a literature review and data analysis. We collected data through a search of electronic databases, Google Scholar, and governmental websites. Results: Changes in Korean home visiting healthcare services are classified into four stages: 'introduction (1990-2000)', 'pilot project (2003-2006)', 'nationwide expansion (2007-2012)', 'various types (2013-2018)'. Korean home visiting healthcare service based on public health centers has achieved outcomes such as improved health behavior and health management, increased health management ability, and establishment of comprehensive healthcare infrastructure. Conclusion: In the future, the demand for home visiting healthcare service will increase steadily because of deepening social polarization, rapid aging of the population, and increases in chronic diseases. To improve health management and health equity, we suggest that Korean home visiting healthcare service will expand to all the people as a core public health service. It is necessary to establish a management team for various types of home visiting healthcare service in the public health center.

통합가정방문급여 평가지표 개발 (Development of Evaluation Indicators for Integrated Home Care)

  • Purpose: The purpose of this study was to develop integrated evaluation indicators of home care services in the hope that the increasing group of long-term home care patients could receive quality care services. Methods: The development involves a methodological study on a development phase and a verification phase. The main survey at a verification stage was conducted by the staffs at 146 institutions who agreed to participate on this study. Results: The evaluation index for the integrated home visit care consisted of five categories and 57 indicators including Managing Institution (12), Environment and Safety (3), Right and Responsibility (7), Process of Care (31) and Results of Care (4). The criterion-related validity was verified in regard to the participation in the 2010 evaluation of long-term home-care institutions by the National Health Insurance Corporation. Conclusion: The evaluation index of the integrated home visiting care developed in this study is considered suitable to utilize as evaluating indicators in managing and evaluating the way of how institutions integrate and provide home visit care services as well as home nursing care services.

노인인구의 간호요양원(Nursing Home) 필요예측에 관한 연구 (Estimation of nursing home needs in elderly people)

  • The purpose of this study is to estimate the population requiring nursing home services among elderly people in Korea. This study identifies the need of nursing home services determined by health care professionals and estimates the proportion of elderly people requiring nursing home service according to the admission criteria. Surveys were conducted on health care professionals including medical doctors, home care nurses, and nurse practitioners. They were asked to assess nursing home need based on four content areas: Physical function (Activities of Daily Living), chronic disease, Physical symptoms (incontinence), mobility, eating, and sensory function. Based on the professionally determined need criteria the proportion of elderly people requiring nursing home services was estimated using secondary data from the 1994 Survey on the Living Status of the Korean Elderly. The number of study subjects to estimate nursing home need who were 60 and older totaled 2,058. The most important factor contributing to the admission eligibility criteria was the elderly living alone. Other factors related were the elderly being unable or having difficulty carrying out activities, and having insufficient help from other our activities, and having insufficient help from other members of the household. Using only physical function, the proportion of elderly people requiring nursing home was $8{\sim}9%$. When only chronic disease was used, proportions varied widely; for the doctor's group, the proportion was over 30%. Using all areas, the proportions of elderly people requiring nursing home were between 13% and 38%. The estimate using chronic disease and physical function was similar to the on using all areas.

가정호스피스 간호중재 표준서에 의한 임상간호활동 분석 (Analysis of Clinical Nursing Activities Using Home Hospice Nursing Intervention Standards)

  • Purpose: The purpose of this study was to validate the nursing intervention standards of home care to home care setting for a guide in giving quality care to home hospice patients. Methods: The developed nursing standards were applied to 79 home hospice patients at K hospital, C university. Data were collected through the developed nursing intervention standards of home care with 19 selected nursing interventions and 418 associated nursing activities from January to June in 2006. Results: The performance frequency was 509 times for total nursing interventions and 7,815 times for total associated nursing activities. The most frequently used nursing activity was 722 times(9.24%) on teaching prescribed medication, followed by surveillance(718 times, 9.18%), and vital signs & neurologic monitoring nursing intervention(701 times, 8.97%). The highest mean nursing activity performance rate showed on Intravenous therapy(82%), followed by dying care(81%), and vital signs & neurologic monitoring (80%). Among 418 nursing activities, there were three classifications: 168 as core activities, 165 as major, and 85 as accessory activities. Conclusion: The final validated nursing intervention standards can guide home care nurses to perform quality care and contribute to computerized nursing services and request of nursing fees.

당뇨병성 족부궤양 환자의 치료형태 및 가정간호 연계 (An Analysis of Treatment Types and Home Care Services Referral for Patients with Diabetic Foot)

  • Purpose: The purpose of this study was to investigate treatment types and the status of referral to home care services for patients with diabetic foot. Methods: A retrospective survey was conducted by reviewing medical records from January to December in 2008 at a university hospital. The subjects were 76 patients at the age of 20 years or older who were admitted, had home care services, or received outpatient care for diabetic foot. The data were analyzed using descriptive statistics. Results: Among the total of 9,317 patients diagnosed with diabetes, 5.03% (n=469) had diabetic foot. Admission (81.6%) was the most frequently used treatment type followed by outpatient care only (7.9%) and hemodialysis only (10.5%). Of the 76 admission cases, 44.9% received post-discharge care at outpatient clinics, 20.5% had both outpatient and home care services, and 16.7% were transferred to other hospitals. Readmission rate after discharge was 15.6% for one year. Conclusion: This study suggests referral to home care services should be encouraged to provide effective follow-up care to patients with diabetic foot after discharge from a hospital.

노인대상 의료기관 가정간호사업의 운영실태 (An Analysis of the Elderly Care and Management in Hospital-Based Home Care Agencies)

  • Purpose: To analyze the home care services provided to the elderly aged 65 and older by a hospital-based home care agencies and to investigate the effects of long-term care insurance for the elderly. Method: The subjects were the home care service recipients aged 65 and older in 172 hospital-based, home care agencies registered in Health Insurance Review & Assessment Service in January, 2007. The data were collected using a questionnaire from March 16 to April 15, 2007. The questionnaire return rate was 43.8%. Result: The hospital-based home care agencies were able to visit 66.5% of the national administrative districts. Of the home care service recipients, over 50% were 65 years old and older. About 43% of the agencies reported that over 50% of their patients would be subject to the long-term care insurance. They expressed concern that home care services would be withdrawn once the insurance system is initiated. Conclusion: This study suggests that hospital-based home care agencies need to manage home care services with long-term care insurance. It also recommends developing guidelines for the use of services and referrals.
