Included is a brief overview of its nature, providers of Home Care Services, recent history of Home Care Services, and the impact of the national movement toward cost containment in health care. The data used in this study are obtained from the Elderly Program of the Medical Services and other data on the Home Care Services in Japan. With the growing elderly population in Japan, it is to be expected that the medical care expenditure for this sector will continue to increase. With the aim of keeping expenditure for medical care within reasonable bounds, it is essential that this increasing expenditure on the elderly be used effectively. With the Health and Medical Services Law for the Aged was enforced, therefore, remuneration for medical treatment of the elderly and what is known as the staff placement standard at hospital for the elderly were rationalized. For this it is required that Home Care Services, such as health services like visiting guidance by public health nurse in hospital of Health Center. • In Japan, a lot of elderly housing types have been developed in order to meet various needs of the older person and the change of social situations. Elderly housings can be divided into three categories elderly housings for healthy older persons, elderly care homes for the healthy and elderly care facilities for the unhealthy. Elderly housings include public and private rental housings. Sometimes they can be designated only for the elderly. Elderly care homes for the healthy elderly include full fee charging elderly housing, elderly homes, low fee charging elderly homes and care houses. Elderly care facilities for the unhealthy elderly consist of full fee charging elderly care homes, group homes for the dementia, elderly health facilities, nursing homes, elderly hospitals, and so on. However "elderly care facilities" have been proved not to be efficient for the delivery of elderly welfare services nor satisfactory to the frail older person. Therefore, based on the concept of the "Normalization", daily services have been provided for the elderly in order that they can live at their own home in the community for themselves. As a result, Japan aims not only to reduce elderly welfare expenses but also to increase elderly users' satisfaction. Emphasis on non-institutionalization and in-home services, regional characterization, harmony between Hard and Soft, user oriented services, substantiality, universal design and so on are sought for the sake of those goals.

  • There are 3,931,822 elderly who receives long-term care at home and 1,704,469 elderly who stay at long-term care facilities in those of 34,610,000 of +65 years old population in Japan; thus, the elderly people who need home care are 2.3 times more than facilities (2016).In the near future, the number of elderly people with a degree of daily life independence and dementia category II or more will increase to 4.7 million (12.8% of the elderly population). Therefore, the use of home-care robots is essential for the disaster evacuation of the overwhelming majority of those elderly who need home care and for of vitals checks to keep family, caregivers, and medical professionals informed to reduce the risk in chronic health conditions in welfare evacuation centers. This also helps to care for the caregivers who are caring for the elderly, and the introduction of home-robots to evacuation centers should receive prompt attention from our care-friendly society.

This study identifies the need of nursing home services determined by health care professionals and estimates the proportion of elderly people requiring nursing home service according to the admission criteria. Surveys were conducted on health care professionals including medical doctors, home care nurses, and nurse practitioners. They were asked to assess nursing home need based on four content areas: Physical function (Activities of Daily Living), chronic disease, Physical symptoms (incontinence), mobility, eating, and sensory function. Based on the professionally determined need criteria the proportion of elderly people requiring nursing home services was estimated using secondary data from the 1994 Survey on the Living Status of the Korean Elderly. The number of study subjects to estimate nursing home need who were 60 and older totaled 2,058. The most important factor contributing to the admission eligibility criteria was the elderly living alone. Other factors related were the elderly being unable or having difficulty carrying out activities, and having insufficient help from other our activities, and having insufficient help from other members of the household. Using only physical function, the proportion of elderly people requiring nursing home was $8{\sim}9%$. When only chronic disease was used, proportions varied widely; for the doctor's group, the proportion was over 30%. Using all areas, the proportions of elderly people requiring nursing home were between 13% and 38%. The estimate using chronic disease and physical function was similar to the on using all areas.

This study's purpose is to provide the reserch group-home for the elderly in Japan rural area. And this paper provided planning of group-home for the elderly in Korea rural area. For this study, respective 5senior housing of Korea and Japan were chosen as respresenting two nations, and every manager was interviewed on housing pattern. This study suggests the planning material as following. First, the effectiveness of the group-home are big. Especially, the elderly was far healthier. Design point is public space(living room, small living room, etc). Maybe, healthy group-home concept is that would be design by Korean traditional housing. Also important public space design is that would be inner courtyard-house, wood floor, Deachong, etc. Two group-homes will construct elderly welfare facilities in between. Otherwise that is need layout of a big public space. In the case of vacant house can used by considering of scale, structure, site condition. Closed school can used through so easy remodeling too. More often than not complex type(hospital, community heath center) was appeared in Japan rural area. In the case of Korea, recommend complex type in community health center and public building. In this case, considering communication, access, contact, group-home keeps on ground floor. In this paper, we hope to take a lesson from group-home planning source for the elderly in Korea rural area.

  • Korean society is experiencing rapid changes which are certain to shape the lives of the older people and their families. So, the purpose of this study was to improve the quality of the elderly welfare service in Korea. Even though the interest about the elderly welfare in Korea is rapidly on the increase, the reality of Korean Society still leaves much to be desired, quantitatively as well as qualitatively. Pay attent to this point, now going to grasp the situation of not the handicapped elder people but the general elder people actual life condition and demand for welfare. So this study focused on the demand of not out-of home service but in-home care service. It was based on the data from by Korea National Statistical Office, having observed 6,139 elderly people. The results were as follows. They hope to get the elderly welfare services about health examination service, nursing survice, supporting service for household. That was, the most of them wanted in-home care service than out-of home care service. To ensure effective care to the elderly, it needed development and settlement of welfare service in face of their daily living. And, even though they recognized that they should cover the elderly life expenses themselves, the rate was hit that indicated their adult children as a parents supporter. This means that we should consider not only to the elder people but also to the family which contained the old people as the elderly welfare service. As a remedy, we can find the cooperation between elder welfare service at the social welfare organization and counsel, education at the family strengthen center. To improve the quality of the elderly welfare service in Korea, the elderly welfare service should focus not only on the physical and spiritual health maintenance but also active understanding living environment and growing ability to arbitrate between individual and living.
