This study aims to review various efforts required by medical institutions to prevent medical accidents in advance and to suggest the necessity of introducing liability insurance for medical accidents based on cases abroad and compulsory professional indemnity insurance at home. Over the past five years between 2013 and 2017, the number of inquiries regarding medical accidents and medical disputes has increased by 11.1 percent from 36,099 to 54,929, and the number of mediation and arbitration for medical disputes has increased by 14.3 percent from 1,304 to 2,225. Since some medical accidents even cause social problems, a compulsory insurance system for the liability of medical institutions for damages need to be introduced to promptly compensate the victims of medical accidents and to ensure compensation by medical personnel. In Korea, a system is in place to provide compensation for a client who suffers an accidental damage after receiving professional services, regardless of whether or not the professional service provider can provide compensation. In major foreign countries, a medical liability system is in place that is applied either by the principle of liability with fault, or the principle of liability without fault. In this study, the cases of compulsory insurance and semi-compulsory insurance in the US and Japan to which the principle of liability with fault is applied, as well as the case of New Zealand to which the principle of liability without fault is applied, were examined. It is necessary to urgently introduce the compulsory insurance system for the liability of compensation to prevent medical disputes and to compensate for the life and physical damages of the victims of medical accidents in domestic medical institutions. Doing so is expected to ensure fair compensation for the victims of medical malpractice and compensation by medical personnel, thereby improving medical practice.

Home healthcare system has lot of things to do for the integration with medical instruments in the sense of standardization. We will design and implement mobile healthcare system based on smart gateway thru adaptation of ISO/IEEE 11073, and HL7 standard. And also we are willing to research heart rate monitor, pulse-oxygen meter and ECG monitor in order to develop mobile healthcare system for medical appliance. We studied the connection of standard medical devices, i.e, continua health alliance under the Android 4.x Bluetooth HDP(Health Device Profile) environments. We also tried to implement healthcare system which can support diagnosis for healthcare provider and user based on HL7 standard.
The purpose of this study is to develop a management model to effectively provide hospice service of nursing home. In a method of study to achieve this purpose, a positive research and literature study have been performed at the same time. Concerning contents to be examined as a definition of notions and patterns about hospice for the aged, foreign hospice and structural elements of hospice management, a literature study to support a theoretical background has presented leading studies as an analysis form of this study through analyzing domestic and foreign literature, thesis, journal and so on. In order to attain the purpose of this study, the positive research is to carry out a survey of demand and desire of people who have already got hospice service in the first place as a fact-finding survey related to the subject of hospice service, investigate organizations to operate nursing home and hospice and relevant experts in the second place as a survey of supply system to provide hospice service, and develop an appropriate management model to connect the demand and supply of nursing home through it. Therefore, this study has developed a delivery system and a chart of the practical trend to be done minutely, presenting a model to manage a hospice of nursing home based on the above results. The management model externally has connected medical team of hospital with hospice team of home based on a community, and internally is composed centering on five elements such as administrative organization, manpower, program and contents of service, finances. In practical model of hospice management of nursing home, in order to plan a program, provide service and give a high-quality service to the aged, this study has presented a trend chart of service management on the basis of five structural elements, an analysis form of this study. In conclusion, this study could present data to develop a hospice in the field of nursing home for the aged by showing a model concerning a hospice management of nursing home, but in my opinion, a following study to be detailed and developed even more than this study should be continued.
Purpose: The study was aimed at qualitatively enhancing and promoting a home visiting nursing program established in Korea on July 1, 2008, as part of the Long-Term Care Insurance for the Elderly program. Methods: Structural, procedural and consequential aspects of home visiting nursing care wereclassified on the horizontal axis by applying the standard notions for the evaluation of medical care (Donabedian, 1998). At the same time, the home visiting nursing care service support system and the service provision system weredivided on the vertical axis with reference to the accreditation standards for home visiting nursing care organizations suggested by the Joint Commission on the Accreditation of Healthcare Organizations (JCAHO, 2008). The data were collected from June 4, 2008 to October 27, 2008, and were analyzed using SPSS ver. 15.0. Results: Twenty-two (proposed) standards, centered on the standard elements under the conceptual framework of the study, were developed, and comprised structural aspects (n=10), procedural aspects (n=6) and consequential aspects (n=6). Those criteria and indicators underwent two content validity surveys among groups of home visiting nursing care research and training experts. The research produced 22 proposed standards, 50 proposed criteria and 166 proposed indicators. Conclusion: The home visiting nursing care standards developed pursuant to the Long-Term Care Insurance for the Elderly Act and the applicability of these standards need to be verified by home visiting nurses. These proposed standards should prove useful in developing an assessment tool to encourage the qualitative enhancement of visiting nursing care in Korea.

Recently there are some trends to construct smart home system infrastructure depending upon the development of Information and Communication Technology. Also requirements of the Ubiquitous Healthcare Systems at home which can monitor the status of health continuously are increased rapidly comparing with hospitals. Healthcare service can be divided into two categories. The first one is Alarm Service that can be used for the emergency status and the other one is Remote Support Service which can monitor the patient including home environments and give those diagnosis information to medical office or to his family. Generally wired networks and fixed healthcare measuring system have some limits to transmit reliable realtime based information for both categories described above comparing with portable monitoring system. Getting over the inefficiency we will design and implement portable healthcare system under the wireless Zigbee network environments.

Purpose : This study compared the cost-effectiveness ratio of physical therapy in health centers and home physical therapy, two physical therapy methods for home-bound stroke patients, and clarified the economic validity regarding the effect of home physical therapy. Methods : To measure and compare the cost and effectiveness of the two physical therapy methods for stroke patients, subjects were recruited based on in-hospital and home physical therapy. Among the entire data collected, 82 and 90 participants were selected for in-hospital and home physical therapy, respectively. To measure costs, regarding both in-hospital and home physical therapy, direct cost and indirect cost for patients, family, medical institutes, and the government were measured. In addition, activities of daily living were measured in both methods to measure their effectiveness. Through collected data, the cost-effectiveness and incremental cost-effectiveness ratios were analyzed. Results : Based on the analysis of cost-effectiveness, home physical therapy showed lower cost-effectiveness than in-hospital physical therapy. Furthermore, the incremental cost-effectiveness ratio also showed a difference, which implied home physical therapy could have high effectiveness compared to cost. Conclusion : Based on these results, home physical therapy could be considered as an alternativeto other methods of physical therapy, for home-bound stroke patients. In addition, the result of thisstudy contribute by providing evidence that home physical therapy offers economic benefits and canbe more effective in treating home-bound patients when policy decisions are made to establish a home physical therapy system.
This study was carried out during the month of September 1976 to analyse and compare the health complaints of two groups of the university women, those who lived at home and those who lived in the university dormitory. The purpose of the study was to provide basic data required by the university health program for planning related to the health need of women students. The study sample consisted of 434 students living in the dormitory and 381 students living at home enrolled for the fall semester 1976 in a womens university in Seoul. The instrument used for the collection of data was an abridged version of a modified Cornell Medical Index. The questionarre included 35 items related to physical health complaint and 22 items related to mental health complaints. The data was treated by a computer (SPSS) using one way analysis, the Fishers' ratio and Chi-Square test at the 5% level were used for the test for statistical significance. The interpretation of this study is limited due to the sample which was restricted to one university and not randomly selected. To guide the direction of the study, it was hypothesized that the rate of expressed health problems of students living in the dormitory would be Venter than that of students living at home. The hypothesis was tested and rejected. The following is a summary of the findings; 1. Total health (physical and mental) complaints a. There was no statistically significant difference between the home and dormitory groups with regard to total health complaints expressed. b. The rate of total complaints expressed by the home group significantly higher than dormitory group only among third year students. c. There was no statistically significant between the home and dormitory groups in their satisfaction with their economic situation. d. The home group showed a significantly higher rate of complaints related to the Nervous System compared to that of the dormitory group. 2. Physical health complaints a. Students living at home showed a significantly higher rate of physical complaints than the dormitory group. b. When the year variable was controlled, the third year was the only group which showed a different rate between home and dormitory groups; the home group presented a higher rate. c. Since the year variable seemed to affect the physical complaints those data were further analysed to see whether the specific system areas were operating as variables in each year. The results were as follow: Among the home group, First year students showed a higher rate in Family History of Disease, while the third year students more Nervous System and Cardiovascular System complaints. Among the dormitory group, only fourth year students showed a higher rate in the Skeletal-Muscular System. This was the only area the dormitory group though only for the fourth year students supported the hypothesis. d. When the economic satisfaction variable was controlled, the satisfied group was the only group which showed a different rate between home and dormitory groups; the home group presented higher rate. e. Since the economic satisfaction variable seemed to affect the physical complaints those data were further analyzed to see whether the specific system areas were operating as variables in each economic satisfaction level, but there was no statistically significant difference between home and dormitory groups. 3. Mental health complaints a. There was no significantly difference between home and dormitory groups with regard to mental health complaints expressed. b. When the year variable was controlled the third year group was the only group which showed a different rate between home and dormitory groups; the home group presented a higher rate. c. Since the year variable seemed to affect the mental complaints, those data were further analyzed to see whether the specific system areas were operatings variables in each economic satisfaction level. The result were as follows: Among the home group, the third year students showed higher rates in Inadequacy and Anxiety. d. When the economic satisfaction variable was controlled, the very satisfied group was the only group which showed a different rate between home and dormitory groups: the home group presented a higher rate. Since the economic satisfaction variable seemed to affect the mental complaints, those data were further analysed to see whether the specific system areas were operating as variables in each economic satisfaction level, but there was no statistical significant difference between the home and dormitory groups. Although the social environment of dormitory life differs from family life, there was no difference in the rate of total health problem complaints between the home and dormitory groups but the home group showed a higher rate of physical health complaints than the dormitory group. Possible positive factors influencing dormitory life and negative factors influencing family life affecting health complaints must be explored in order to relate to the health needs of the university health program. This study could not define the causes for the fewer physical complaints of dormitory students living at home. Further study of such causal factors recommended in order to provide the data needed to contribute to a more effective health program.

This study was done to examine the home care service provided by home care nurses and the level of client satisfaction. Data were collected from 110 clients who received the home care service at the P-hospital in Pusan from 23th April, 2001 to 30th November, 2002. The tool for measurement of satisfaction was composed of 16 items and was 4 score scale. Data was analyzed by using SPSS/WIN 10.5 program, the results of this study are summarized as follows; 1) 77.3% among 110 clients were over sixty years old. By the disease pattern. subjects were distributed into cancer(52.7%), cerebro-vascular disease(22.7%) and the others. 36.4% of clients were received home visits from 10 to 20 times by home care nurse. 2) The total number of home care services was 20,828. And most of the clients received the health education and training. 3) The mean score of satisfaction on provided home care services was $3.36\pm0.45$, out of 4. among 16 items. 'The home care nurses were kind enough' was highest ($3.59\pm0.49$), the total level of satisfaction of home care services was very high. with total mean score $53.84\pm7.16$. As mentioned above. the satisfaction level of home care services was very high. so we can except that the prospect of home care services is very challengeable. Therefore. we should try to expand the service recipients with promotional education to the home care clients under the cooperation with medical staff and make more efforts to develop the management system about clients' information. to improve the quality of the service, to assure close ties with the public health centers for the continuous home care service linking.

Purpose: The purpose of this study was to analyze the services and cost of CVA patients in hospital-based home health care and compare the differences of home health care cost by hospital types. Methods: The subjects of this study were 5,756 home care patients with cerebrovascular disease. Data were collected by using home health care medical expense claims from 127 hospitals in 2004. Results: The home care service 'indewelling catheterization' was the highest(19.28%), and then 'nasogastric tube insertion and change(16.72%)', 'bladder irrigation(15.98)', 'wound management(simple dressing)(10.42%)' followed. Average home health care cost per visit was 39,943 won, and the highest 46,058 won in general hospitals and the lowest 33,922 won in tertiary hospitals, so there were statistically significant among the types of hospitals(F=1112.47, p<0.0001). Conclusions: The number of home health care patients has been rapidly growing with the increase of aged population and demand for home care services is rising. So, it could be urgent to develop a reasonable cost reimbursement system for home health services and to expend scopes of the roles of home care specialist nurses. Amid the demand of more detail understanding the present status of home care, our study can be contributed to provide fundamental information of home care in Korea.
