The purpose of this study is to analyze the demand for telemedicine and telehealthmanagement services, which are key elements of home based u-health. The conjoint analysis, which is a conventional method for demand analysis for newly introduced products, is employed, utilizing the survey data on 500 seoul citizens. Further, multivariate probit model is used to estimate the demand. The result shows that the demand for telemedicine services is greater than that of telehealthmanagement services. Further, home-based u-health services will play a role as a complementary for face-to-face medical treatments, rather than a substitute. Meanwhile, the demand for home-based u-health services is found to be very sensitive to price.

Purpose: The purpose of this study is to identify factors affecting the urinary incontinence (UI) prevalence and the quality of life among elderly adult women who are subject to home visiting healthcare services. Methods: A cross-sectional study was conducted with 400 women aged 60 or over who were registered for home health care services at one health center in Gangwon-do. Data were collected using a structured questionnaire consisting of UI severity, UI type, and the quality of life. The UI severity was assessed using International Consultation of Incontinence Questionnaire-Short Form (ICIQ-SF), the quality of life using Incontinence Quality of Life (I-QOL), and type of UI using Questionnaire for Urinary Incontinence Diagnosis (QUID). The data were analyzed by using x2 test, independent t-test, one-way ANOVA, Pearson's Correlation, binary logistic, and multiple lineal regression. Results: The prevalence of UI is 51.7%. The mean score of ICIQ-SF was 9.70±4.04 for women with UI and 0.04±0.51 for women without UI (t=-33.67, p<.001). As the frequency of day time urination (OR=1.34), the subjective health status (OR=1.45), and the educational status (OR=0.90) were higher, the risk of UI prevalence was the higher. The factors affecting I-QOL were ICIQ-SF score, mixed UI, subjective health status, and nighttime urination (adjusted R2=.61). Conclusion: The results of this study showed that UI severity, mixed UI, subjective health status, and day time urination affected the quality of life. It suggests that the assessment for the severity and type of UI be needed to prevent UI or improve the quality of life of UI vulnerable elderly adult women.

BACKGROUD/OBJECTIVES: Registered dietitian nutritionists (RDN) are providers of medical nutrition therapy (MNT) to address health and chronic disease. Traditionally, RDNs have provided care in healthcare facilities including hospitals and private care facilities. The purpose of this study was to determine how RDN individualized MNT in the home impacted nutrition, physical activity, and food security. SUBJECTS/METHODS: This is a secondary data analysis. The mean age of the participants (n = 1,007) was 51.6 years old with a mean body mass index (BMI) of 34.1 kg/m2. Individualized MNT visits were delivered by an RDN in the home setting from January to December 2019. Participants were referred by healthcare professionals or self-referred. Participants had MNT benefits covered by their health insurance plan (43.3% Medicaid; 39.8% private insurance; 7.9% Medicare, 9% other). Health outcomes related to nutrition care were measured. Outcomes included self-reported consumption of nutrition factors and physical activity. Our secondary outcome focused on food security. The changes in weight, BMI, physical activity, and nutrition factors were analyzed by a linear regression model or linear mixed model, adjusting for age, sex, baseline value, and number of appointments. Food security was summarized in a 2 by 2 contingency table. RESULTS: Baseline values had significantly negative impacts for all changes and number of appointments was significant in the changes for weight and BMI. Increases in physical activity were significant for both female and male participants, 10.4 and 12.6 minutes per day, respectively, while the changes in weight and BMI were not. Regarding dietary factors, the consumption total servings per day of vegetables (0.13) and water (3.35) significantly increased, while the consumption of total servings of whole grain (-0.27), fruit (-0.32), dairy (-0.80) and fish (-0.81) significantly decreased. About 24% (of overall population) and 45% (of Medicaid population) reported improvements in food security. CONCLUSIONS: This study found that home visits were a useful setting for MNT delivered by RDNs. There is a strong need for individualized counseling to meet the participants' needs and personal goals.

IOT has carried out important function in converting the traditional fitness care corporation. With developing call for in population, traditional healthcare structures have reached their outmost functionality in presenting sufficient and as plenty as mark offerings. The worldwide is handling devastating developing antique population disaster and the right want for assisted-dwelling environments is turning into inevitable for senior citizens. There furthermore a determination by means of the use of way of countrywide healthcare organizations to increase crucial manual for individualized, right blanketed care to prevent and manipulate excessive coronial situations. Many tech orientated packages related to Health Monitoring have been delivered these days as taking advantage of net boom everywhere on globe, manner to improvements in cellular and in IOT generation. Such as optimized indoor networks insurance, community shape, and fairly-low device fee performances, advanced tool reliability, low device energy consumption, and hundreds higher unusual common usual performance in network safety and privacy. Studies have highlighted fantastic advantages of integrating IOT with health care location and as era is improving the rate also cannot be that terrific of a problem. However, many challenges in this new paradigm shift notwithstanding the fact that exist, that need to be addressed. So the out most purpose of this research paper is 3 essential departments: First, evaluation of key elements that drove the adoption and boom of the Internet of factors based totally domestic some distance off monitoring; Second, present fashionable improvement of IOT in home a long manner off monitoring shape and key building gadgets; Third, communicate future very last effects and distinct guidelines of such type a long way off monitoring packages going ahead. This study investigated the current oral health conditions of the elderly at home and welfare facilities in their age over 65 years around some rural areas in Gangwon province, which would expect the fewer medical benefits even with lower interest than urban areas, despite of relatively high ratio of elder populations, so that it could prepare a basic document necessary to determine certain planned quantification for the benefit of elder's oral healthcare. As of the end of December 2004 both 50 elders at home and 50 elders at welfare facilities were randomly sampled in their age over 65 years in Samcheok city. As a result of this study, it was found that the elders at welfare facilities scored 15 pts. in DMFT index level typical of oral health conditions, which was higher than the elders at home. In addition, the elders at welfare facilities scored 26.0% in the coexistence of immobile bridge and partial denture higher than the elders at home with regard to the presence of intraoral prosthetic appliance. The results of analyzing the difference in the one-year dental visiting experience of respondents hereof showed that the elders at home were relatively more in ratio(62.0%) than those at welfare facilities, while many of the former group(38.0%) had relatively more handicap in masticatory movement than the latter one with regard to the conditions of dental prosthesis in use. Besides, many of the elders at facilities(30% or more) felt subjective symptoms of periodontal disease including bleeding or swelling, which indicates higher ratio than the elders at home. Finally, the elders at home used to brush their teeth at more frequency on a daily basis than those at facilities, while the latter group suffered general body disease more than the former group. Summing up, it is concluded that a formulated oral healthcare system will become more needed in near future than now for the benefit of the elderly living in welfare facilities, while nationwide policy-level supports would be urgent for them in the aspect of national welfare.

The tremendous growth of the Internet of things is unbelievable. Many IoT devices have emerged on the market over the last decade. This has made our everyday life easier inside our homes. The technology used at home has changed significantly over the past several decades, leading to what is known today as the smart home. However, this growth has also brought new challenges to our home security and privacy. With the smart home becoming more mainstream, cybersecurity issues have become a fundamental concern. The smart home is an environment where heterogeneous devices and appliances are interconnected through the Internet of Things (IoT) to provide smart services to residents. These services include home climate control, energy management, video on demand, music on-demand, remote healthcare, remote control, and other similar services in a ubiquitous manner. Smart home devices can be controlled via the Internet using smartphones. However, connecting smart home appliances to wireless networks and the Internet makes individuals vulnerable to malicious attacks. Remote access within the same environment or over the Internet requires an effective access control mechanism. This paper intends to shed light on how smart home devices are working as well as the type of security and privacy threats of the smart home. It also illustrated the types of authentication methods that can be used with smart home devices. In addition, a comparison of Smart home IoT-based security protocols was presented along with a security countermeasure that can be used in a smart home environment. Finally, a few open problems were mentioned as future research directions for researchers.

The coefficient of parameters with frailty change was serviced year (β=0.29, p<.001), age (70~79 to 60~69; β=0.98, p=.018) and sex (female to male; β=2.55, p<.001). Conclusion: This study showed that the home visiting health service needs to take attention to aged 70s and over, female. The home health care of public health center need to be extended more practical and effective services in terms of 'community care'and 'ageing in place'.
Purpose: This study aimed to explore the difficulties and coping that advanced practice nurses (APNs) experience in home health nursing field. Methods: The participants were 12 APNs who had experience in home health nursing for over 5 years. Data were collected through two focus group interviews between August and September, 2017. The results were analyzed using a qualitative content analysis. Results: Out of 10 sub-categories and 29 codes, 4 categories emerged: "crushed by the responsibilities as the only healthcare professional", "worn out by family who interfere with work", "anxiety about my safety being threatened" and "using self-learned know-hows". Conclusion: APNs face difficulties due to the environmental characteristics of the homes, the characteristics of chronically critically ill (CCI) patients and their families, and legal or systematical limitations when dealing with problems based on their personal competence. In order to ensure that professional nursing care is more readily provided to CCI patients through home health nursing services in local communities, it is imperative that support systems at the organizational and institutional levels be established in a systematic way to resolve the issues faced by APNs.
